Aslanyan We sought to simultaneously confirm that substantial recovery at day 1 and day 7 after acute ischaemic stroke onset is associated with subsequent neurological deterioration in patients of the Acute Stroke Therapy by Inhibition of Neutrophils randomized clinical trial. Substantial recovery was assessed by improvement in the National Institutes of Health Stroke Score (NIHSS). Neurological deterioration was defined as any stroke event or NIHSS worsening from recovery assessment to day 90. After adjusting for age, t-PA and day 1 NIHSS, there was a non-significant tendency of substantial (pre-specified as 75%) recovery at day 1 to be associated with later deterioration [odds ratio (OR) 2.47; 95% CI, 0.95,6.50]. The corresponding OR for substantial (pre-defined as 65%) recovery at day 7 was 1.84 (0.85,3.96). Other thresholds for recovery were significantly associated with later deterioration: >50%, 80%, 90% and 100% for day 1 and >50%, 60%, 70%, 90% and 100% for day 7. The effect of recovery at day 1 was more important than that of later recovery. This study confirms the association between recovery and subsequent neurological deterioration and is the first to indicate the greater importance of acute recovery at day 1 in comparison with later recovery. [source] Influence of duration of exposure to the pyrethroid fenvalerate on sublethal responses and recovery of Daphnia magna strausENVIRONMENTAL TOXICOLOGY & CHEMISTRY, Issue 5 2005Sebastián Reynaldi Abstract This study compares lethal and sublethal responses of Daphnia magna Straus exposed to fenvalerate continuously (21 d) and as a pulse (24 h). Survival was reduced more severely in the continuous- than in the pulse-exposure regime. Complete mortality occurred at 1 ,g/L for continuous exposure and at 3.2 ,g/L for pulse exposure. Regarding reproductive endpoints, fenvalerate delayed the age at first reproduction. At the beginning of the reproductive phase (day 10), this delay resulted in a reduction of the neonates per living female at similar concentrations in both exposure regimes (0.3 and 0.1 ,g/L for continuous and pulse exposure, respectively). The population growth rate was inhibited in continuous and pulse exposure at 0.3 and 0.6 ,g/L, respectively. However, the effects of fenvalerate in the pulse exposure were transient. After 21 d, a recovery to values close to the controls occurred with respect to the total neonates per female and the population growth rate over a broad range of concentrations from 0.1 up to 1 ,g/L. In contrast, no substantial recovery occurred in the continuous-exposure regime. [source] PRECLINICAL STUDY: Electroacupuncture treatment reverses morphine-induced physiological changes in dopaminergic neurons within the ventral tegmental areaADDICTION BIOLOGY, Issue 4 2009Ling Hu ABSTRACT Chronic morphine administration decreases the size of dopamine (DA) neurons in the ventral tegmental area (VTA). These transient morphological changes are accompanied by a reduced sensitivity of morphine-induced conditioned place preference (CPP) after chronic exposure to the drug. In this study we examined alterations in the firing rate of DAergic neurons by means of extracellular recording following chronic morphine exposure and applied 100 Hz electroacupuncture (EA) treatment to reverse the reduced firing rate of these neurons. In the first set of experiments we show that in rats, which received chronic morphine treatment for 14 days, a small dose of morphine was not able to induce a CPP response anymore. However, the sensitivity to morphine was reinstated by consecutive EA treatment for 10 days. The electrophysiological response of VTA DA neurons to morphine was markedly reduced in chronic morphine-treated rats compared to saline-treated controls. A substantial recovery of the reactivity of VTA DA neurons to morphine was observed in rats that received 100 Hz EA for 10 days. Our findings suggest that 100 Hz EA is a potential therapy for the treatment of opiate addiction by normalizing the activity of VTA DA neurons. [source] Anti,N-methyl-D-aspartate receptor (NMDAR) encephalitis in children and adolescents,ANNALS OF NEUROLOGY, Issue 1 2009Nicole R. Florance MD Objective To report the clinical features of anti,N-methyl-D-aspartate receptor (NMDAR) encephalitis in patients , 18 years old. Methods Information was obtained by the authors or referring physicians. Antibodies were determined by immunocytochemistry and enzyme-linked immunosorbent assay (ELISA) using HEK293 cells ectopically expressing NR1. Results Over an 8-month period, 81 patients (12 male) with anti-NMDAR encephalitis were identified. Thirty-two (40%) were ,18 years old (youngest 23 months, median 14 years); 6 were male. The frequency of ovarian teratomas was 56% in women >18 years old, 31% in girls ,18 years old (p = 0.05), and 9% in girls ,14 years old (p = 0.008). None of the male patients had tumors. Of 32 patients ,18 years old, 87.5% presented with behavioral or personality change, sometimes associated with seizures and frequent sleep dysfunction; 9.5% with dyskinesias or dystonia; and 3% with speech reduction. On admission, 53% had severe speech deficits. Eventually, 77% developed seizures, 84% stereotyped movements, 86% autonomic instability, and 23% hypoventilation. Responses to immunotherapy were slow and variable. Overall, 74% had full or substantial recovery after immunotherapy or tumor removal. Neurological relapses occurred in 25%. At the last follow-up, full recovery occurred more frequently in patients who had a teratoma that was removed (5/8) than in those without a teratoma (4/23; p = 0.03). Interpretation Anti-NMDAR encephalitis is increasingly recognized in children, comprising 40% of all cases. Younger patients are less likely to have tumors. Behavioral and speech problems, seizures, and abnormal movements are common early symptoms. The phenotype resembles that of the adults, although dysautonomia and hypoventilation are less frequent or severe in children. Ann Neurol 2009;66:11,18 [source] |
