Effectiveness of the Takeda Three Colors Combination Test as a screening test for dementia

Abstract Background:, The aged population is increasing worldwide and it is expected that dementia, for which aging is a risk factor, will increase as well. A critical issue then becomes a community's capacity for the early detection of dementia among its senior citizens. In the present paper, we report on the development and potential use of a screening test for dementia that can be administered and assessed easily in a short period of time by non-specialist clinicians and represents no burden for those undergoing the screening. Methods:, Three hundred and sixty senior citizens, over 60 years of age, participated in the study. Of these, 126 had Alzheimer's disease (AD), 60 had vascular dementia (VaD), 41 had mild cognitive impairment (MCI), and 133 were healthy volunteers (control group). A screening test for dementia, which consisted of a colored cards configuration memory task (the Takeda Three Colors Combination Test; TTCC) was examined for sensitivity, specificity, reliability and criterion-related validity. Results:, Sufficient sensitivity and specificity were demonstrated for each clinical group (AD, VaD) and the control group. The sensitivity of the TTCC was 0.94, 0.82, and 0.71 for the AD, VaD, and MCI groups, respectively; specificity was 0.83. In addition, sufficient reliability and validity were established. Administration of the TTCCand assessment procedures required only 1 or 2 min. Conclusion:, Satisfactory sensitivity and specificity were indicated for both the AD and VaD groups, with sufficient reliability and validity also indicated. Thus, the TTCC is an effective dementia screening test. [source]


The Use of Silver Solid Amalgam Working Electrode for Determination of Nitrophenols by HPLC with Electrochemical Detection

Abstract The use of a silver solid amalgam working electrode for HPLC with electrochemical detection has been investigated. The thin-layer and wall-jet detectors based on this electrode were constructed and applied for the determination of a mixture of nitrophenols. The optimal separation and detection conditions for the determination of 2-nitrophenol, 4-nitrophenol, 2,4-dinitrophenol and 2-methoxy-5-nitrophenol in mixture were found using RP-HPLC at Nova-Pack C18 column and amperometric detection with the above mentioned detectors. It has been proved that the silver solid amalgam electrode is a suitable working electrode for HPLC-ED and provides sufficient sensitivity for determination of tested nitrophenols. [source]


A Contactless Impedance Probe for Simple and Rapid Determination of the Ratio of Liquids with Different Permittivities in Binary Mixtures

Abstract Simple contactless cells with planar or tubular electrodes have been designed for measurement of the permittivity of solutions. The cells, connected to an integrated circuit of astable multivibrator, respond primarily to the capacitance component of the cell impedance, the multivibrator frequency depends in a defined manner on the solution permittivity and is readily used as the analytical signal in determinations of the ratios of components in binary liquid mixtures; water solution of methanol, ethanol and dioxane have been tested. The response of the cell with planar electrodes satisfies well the simple theoretical model and both the cells provide results with a sufficient sensitivity, a low LOD value (units of %vol) and a good precision (around 1%rel). The cell simplicity, small dimensions, long-term stability and the possibility of powering them from a battery make them suitable for hand-held meters. As an example of application in practice, the content of ethanol was determined in the car fuel petrol. [source]


Determination of gaseous and particulate carbonyls in air by gradient-elution micellar electrokinetic capillary chromatography

Abstract A new continuous-flow gradient-elution micellar electrokinetic capillary chromatography method is developed for the determination of airborne carbonyls after derivatization with 2,4-dinitrophenylhydrazine. A total of 16 carbonyls can be determined with detection limits ranging from 0.94 to 8.50,mg/L, working range from 4.72 to 346,mg/L, and repeatabilities (relative standard deviation, n=5) from 1.23 to 4.6% or 3.93 to 7.6% for migration time and peak area, respectively. Coupling with denuder-filter sampling, a preliminary survey has been conducted to determine gaseous and particulate carbonyls from air sampled at a roadside station. The method is shown to have sufficient sensitivity for 1-h sampling of ambient carbonyls with detection limits ranging from 0.045 to 1.2,,g/m3 and working range from 0.11 to 43.3,,g/m3 at a flow rate of 10,Lpm. The method requires minimal modification of commercially available capillary electrophoresis equipment and can differentiate gaseous and particulate carbonyls to provide essential information and objective data for adopting effective measures to combat the discharge of carbonyl compounds to the atmosphere. Prospective Validation of a Modified Thrombolysis In Myocardial Infarction Risk Score in Emergency Department Patients With Chest Pain and Possible Acute Coronary Syndrome

Abstract Objectives:, This study attempted to prospectively validate a modified Thrombolysis In Myocardial Infarction (TIMI) risk score that classifies patients with either ST-segment deviation or cardiac troponin elevation as high risk. The objectives were to determine the ability of the modified score to risk-stratify emergency department (ED) patients with chest pain and to identify patients safe for early discharge. Methods:, This was a prospective cohort study in an urban academic ED over a 9-month period. Patients over 24 years of age with a primary complaint of chest pain were enrolled. On-duty physicians completed standardized data collection forms prior to diagnostic testing. Cardiac troponin T-values of >99th percentile (,0.01 ng/mL) were considered elevated. The primary outcome was acute myocardial infarction (AMI), revascularization, or death within 30 days. The overall diagnostic accuracy of the risk scores was compared by generating receiver operating characteristic (ROC) curves and comparing the area under the curve. The performance of the risk scores at potential decision thresholds was assessed by calculating the sensitivity and specificity at each potential cut-point. Results:, The study enrolled 1,017 patients with the following characteristics: mean (±SD) age 59.3 (±13.8) years, 60.6% male, 17.9% with a history of diabetes, and 22.4% with a history of myocardial infarction. A total of 117 (11.5%) experienced a cardiac event within 30 days (6.6% AMI, 8.9% revascularization, 0.2% death of cardiac or unknown cause). The modified TIMI risk score outperformed the original with regard to overall diagnostic accuracy (area under the ROC curve = 0.83 vs. 0.79; p = 0.030; absolute difference 0.037; 95% confidence interval [CI] = 0.004 to 0.071). The specificity of the modified score was lower at all cut-points of >0. Sensitivity and specificity at potential decision thresholds were: >0 = sensitivity 96.6%, specificity 23.7%; >1 = sensitivity 91.5%, specificity 54.2%; and >2 = sensitivity 80.3%, specificity 73.4%. The lowest cut-point (TIMI/modified TIMI >0) was the only cut-point to predict cardiac events with sufficient sensitivity to consider early discharge. The sensitivity and specificity of the modified and original TIMI risk scores at this cut-point were identical. Conclusions:, The modified TIMI risk score outperformed the original with regard to overall diagnostic accuracy. However, it had lower specificity at all cut-points of >0, suggesting suboptimal risk stratification in high-risk patients. It also lacked sufficient sensitivity and specificity to safely guide patient disposition. Both scores are insufficiently sensitive and specific to recommend as the sole means of determining disposition in ED chest pain patients. Genetic and epigenetic aspects of bladder cancer

Abstract Transitional cell carcinoma of the urinary bladder has a diverse collection of biologic and functional characteristics. This is reflected in differing clinical courses. The diagnosis of bladder cancer is based on the information provided by cystoscopy, the gold standard in combination with urinary cytology findings. Many tumor markers have been evaluated for detecting and monitoring the disease in serum, bladder washes, and urinary specimens. However, none of these biomarkers reported to date has shown sufficient sensitivity and specificity for the detection of the whole spectrum of bladder cancer diseases in routine clinical practice. The limited value of established prognostic markers requires the analysis of new molecular parameters of interest in predicting the prognosis of bladder cancer patients; in particular, the high-risk patient groups at risk of progression and recurrence. Over the past decade, there has been major progress elucidating of the molecular genetic and epigenetic changes leading to the development of transitional cell carcinoma. This review focuses on the recent advances of genetic and epigenetic aspects in bladder cancer, and emphasizes how molecular biology would be likely to affect the future therapies. © 2005 Wiley-Liss, Inc. Analysis of 5-fluorouracil in saliva using surface-enhanced Raman spectroscopy

Abstract The ability of surface-enhanced Raman spectroscopy (SERS) to measure the chemotherapy drug 5-fluorouracil in saliva is presented. A silver-doped sol,gel provided SERS and also some chemical selectivity. 5-Fluorouracil and physiological thiocyanate produced SERS, whereas large biochemicals, such as enzymes and proteins, did not, supporting the expectation that the larger molecules do not diffuse through the sol,gel to any appreciable extent. In addition, 5-fluorouracil samples of 2 µg ml,1 were easily measured, and an estimated limit of detection of 150 ng ml,1 in 5 min should provide sufficient sensitivity to perform pharmacokinetic studies and to monitor and regulate patient dosage. This would fill a critical need for this highly used drug, since genetic-based variations in its metabolism can range by as much as five-fold from one patient to another. Membrane protected conductive polymer as micro-SPE device for the determination of triazine herbicides in aquatic media

Abstract A micro-SPE technique was developed by fabricating a rather small package including a polypropylene membrane shield containing the appropriate sorbent. The package was used for the extraction of some triazine herbicides from aqueous samples. Solvent desorption was subsequently performed in a microvial and an aliquot of extractant was injected into GC-MS. Various sorbents including aniline- ortho -phenylene diamine copolymer, newly synthesized, polypyrrole, multiwall carbon nanotube, C18 and charcoal were examined as extracting media. Among them, conductive polymers exhibited better performance. Influential parameters including extraction and desorption time, desorption solvent and the ionic strength were optimized. The developed method proved to be rather convenient and offers sufficient sensitivity and good reproducibility. The detection limits of the method under optimized conditions were in the range of 0.01,0.04,ng/mL. The RSDs at a concentration level of 0.1,ng/mL were obtained between 4.5 and 9.3% (n=5). The calibration curves of analytes showed linearity in the range of 0.05,10,ng/mL. The developed method was successfully applied to the extraction of selected triazines from real water samples. The whole procedure showed to be conveniently applicable and quite easy to manipulate. [source]


Determination of oxalate in beer by zone electrophoresis on a chip with conductivity detection

Abstract The use of a poly(methylmethacrylate) capillary electrophoresis chip, provided with a high sample load capacity separation system (a 8500 nL separation channel combined with a 500 nL sample injection channel) and a pair of on-chip conductivity detectors, for zone electrophoresis (ZE) determination of oxalate in beer was studied. Hydrodynamic and electroosmotic flows of the solution in the separation compartment of the chip were suppressed and electrophoresis was a dominant transport process in the separations performed on the chip. A low pH of the carrier electrolyte (3.8), implemented by aspartic acid and bis-tris propane, provided an adequate selectivity in the separation of oxalate from anionic beer constituents and, at the same time, also a sufficient sensitivity in its conductivity detection. Under our working conditions, this anion could be detected at a 0.5 ,mol/L concentration also in samples containing chloride (a major anionic constituent of beer) at a 1800 higher concentration. Such a favorable analyte/matrix concentration ratio made possible accurate and reproducible [typically, 2,5% relative standard deviation (RSD) values of the peak areas of the analyte in dependence on its concentration in the sample] determination of oxalate in 500 nL volumes of 20,50-fold diluted beer samples. Short analysis times (about 200 s), minimum sample preparation, and reproducible migration times of this analyte (0.5,1.0% RSD values) were characteristic for ZE on the chip. [source]


Modification of the 21-cm power spectrum by X-rays during the epoch of reionization

ABSTRACT We incorporate a contribution to reionization from X-rays within analytic and seminumerical simulations of the 21-cm signal arising from neutral hydrogen during the epoch of reionization. The relatively long X-ray mean free path (MFP) means that ionizations due to X-rays are not subject to the same density bias as UV ionizations, resulting in a substantive modification to the statistics of the 21-cm signal. We explore the impact that X-ray ionizations have on the power spectrum (PS) of 21-cm fluctuations by varying both the average X-ray MFP and the fractional contribution of X-rays to reionization. In general, prior to the epoch when the intergalactic medium (IGM) is dominated by ionized regions (H ii regions), X-ray-induced ionization enhances fluctuations on spatial scales smaller than the X-ray MFP, provided that X-ray heating does not strongly suppress galaxy formation. Conversely, at later times when H ii regions dominate, small-scale fluctuations in the 21-cm signal are suppressed by X-ray ionization. Our modelling also shows that the modification of the 21-cm signal due to the presence of X-rays is sensitive to the relative scales of the X-ray MFP and the characteristic size of H ii regions. We therefore find that X-rays imprint an epoch and scale-dependent signature on the 21-cm PS, whose prominence depends on fractional X-ray contribution. The degree of X-ray heating of the IGM also determines the extent to which these features can be discerned. We further show that the presence of X-rays smoothes out the shoulder-like signature of H ii regions in the 21-cm PS. For example, a 10 per cent contribution to reionization from X-rays translates to a 20,30 per cent modulation in the 21-cm PS across the scale of H ii regions. We show that the Murchison Widefield Array will have sufficient sensitivity to detect this modification of the PS, so long as the X-ray photon MFP falls within the range of scales over which the array is most sensitive (,0.1 Mpc,1). In cases in which this MFP takes a much smaller value, an array with larger collecting area would be required. As a result, an X-ray contribution to reionization has the potential to substantially complicate analysis of the 21-cm PS. On the other hand, a combination of precision measurements and modelling of the 21-cm PS promises to provide an avenue for investigating the role and contribution of X-rays during reionization. Quantitative ATP synthesis in human liver measured by localized 31P spectroscopy using the magnetization transfer experiment

Abstract The liver plays a central role in intermediate metabolism. Accumulation of liver fat (steatosis) predisposes to various liver diseases. Steatosis and abnormal muscle energy metabolism are found in insulin-resistant and type-2 diabetic states. To examine hepatic energy metabolism, we measured hepatocellular lipid content, using proton MRS, and rates of hepatic ATP synthesis in vivo, using the 31P magnetization transfer experiment. A suitable localization scheme was developed and applied to the measurements of longitudinal relaxation times (T1) in six healthy volunteers and the ATP-synthesis experiment in nine healthy volunteers. Liver 31P spectra were modelled and quantified successfully using a time domain fit and the AMARES (advanced method for accurate, robust and efficient spectral fitting of MRS data with use of prior knowledge) algorithm describing the essential components of the dataset. The measured T1 relaxation times are comparable to values reported previously at lower field strengths. All nine subjects in whom saturation transfer was measured had low hepatocellular lipid content (1.5,±,0.2% MR signal; mean,±,SEM). The exchange rate constant (k) obtained was 0.30,±,0.02,s,1, and the rate of ATP synthesis was 29.5,±,1.8,mM/min. The measured rate of ATP synthesis is about three times higher than in human skeletal muscle and human visual cortex, but only about half of that measured in perfused rat liver. In conclusion, 31P MRS at 3,T provides sufficient sensitivity to detect magnetization transfer effects and can therefore be used to assess ATP synthesis in human liver. A multicenter survey of rituximab therapy for refractory antineutrophil cytoplasmic antibody,associated vasculitis

Objective B cell depletion with rituximab has allowed remissions in relapsing or refractory antineutrophil cytoplasmic antibody (ANCA),associated vasculitis in small studies. The aim of this study was to determine the efficacy and safety of rituximab for ANCA-associated vasculitis in a larger multicenter cohort. This permitted comparison of rituximab dosing regimens, the value of continuing immunosuppression, and investigation of ANCA and B cell levels as re-treatment biomarkers. Methods Retrospective, standardized data collection from 65 sequential patients receiving rituximab for refractory ANCA-associated vasculitis at 4 centers in the UK was used. Results All patients achieved B cell depletion. Complete remission occurred in 49 of the 65 patients (75%), partial remission in 15 (23%), and no response in 1 (2%). The prednisolone dosage was reduced from 12.5 mg/day (median) to 9.0 mg/day at 6 months (P = 0.0006). Immunosuppressive therapy was withdrawn in 37 of 60 patients (62%). Twenty-eight of 49 patients who achieved full remission (57%) experienced relapse (median 11.5 months). B cell return preceded relapse in 14 of 27 patients (52%). Although ANCA levels fell after rituximab therapy, relapse was not associated with ANCA positivity or a rise in ANCA levels. Neither the initial rituximab regimen (4 infusions of 375 mg/m2 each given 1 week apart or 2 infusions of 1 gm each given 2 weeks apart) nor withdrawal of immunosuppressive therapy (37 of 60 patients [62%]) influenced the timing of relapse. Thirty-eight patients received ,2 courses of rituximab, and complete remission was induced or maintained in 32 of them (84%). IgM levels fell, although IgG levels remained stable. Forty-six serious adverse events occurred, including 2 episodes of late-onset neutropenia, which were attributed to rituximab. Conclusion Rituximab was effective remission induction therapy for refractory ANCA-associated vasculitis in this study. There was no difference in efficacy between the 2 main treatment regimens. Continuing immunosuppression did not reduce relapses. Relapses occurred, but re-treatment was effective and safe. There was no clear influence of rituximab on the frequency of serious adverse events. ANCA and B cell levels lacked sufficient sensitivity to guide the timing of re-treatment. [source]


Antenna performance analysis for decameter solar radio observations

Abstract Decameter wavelength radio emission is finely structured in solar bursts. For their research it is very important to use a sufficient sensitivity of antenna systems. In this paper we study an influence of the radiotelescope-antenna effective area on the results of decameter solar radio observations. For this purpose we compared the solar bursts received by the array of 720 ground-based dipoles and the single dipole of the radiotelescope UTR-2. It is shown that a larger effective area of the ground-based antenna allows us to measure a weaker solar emission and to distinguish a fine structure of strong solar events. Topiramate in painful diabetic polyneuropathy: findings from three double-blind placebo-controlled trials

Objectives , To evaluate the efficacy and tolerability of topiramate in patients with painful diabetic polyneuropathy. Materials and methods , Patients with moderate to extreme pain (0,4 Categorical Pain Scale score , 2) were randomized to placebo or topiramate (100, 200, or 400 mg/day) in three similar double-blind trials. The primary efficacy analysis was pain reduction from final visit to baseline in the 100-mm Visual Analog Scale (VAS) for the intent-to-treat populations. Results , After 18,22 weeks of double-blind treatment, pain reductions were numerically greater with topiramate in two studies but differences between topiramate and placebo in VAS scores or in the secondary efficacy endpoints did not reach statistical significance in any of the three studies. Across all studies, 24% of topiramate-treated patients and 8% of placebo-treated patients discontinued due to adverse events; groups did not differ in the occurrence of serious adverse events. Conclusion , These studies did not find topiramate to be significantly more effective than placebo in reducing pain scores in patients with painful diabetic polyneuropathy. Several design features may have precluded the studies from having sufficient sensitivity to differentiate effective and ineffective treatments. The study design and results are instructive for other investigators designing future clinical studies in neuropathic pain. [source]
