The Ages & Stages Questionnaire: Social,Emotional: A validation study of a mother-report questionnaire on a clinical mother,infant sample,

Mother-report questionnaires of infant socioemotional functioning are increasingly used to screen for clinical referral to infant mental health services. The validity of the Ages & Stages Questionnaire: Social Emotional (ASQ:SE; J. Squires, D. Bricker, K. Heo, & E. Twombly, 2002) was investigated in a sample of help-seeking mothers with young infants. It was compared with independent observer-rated dyadic interactions, and the quality of dyadic relationships was rated by expert clinicians. The ASQ:SE ratings also were compared with questionnaires on maternal psychological stress and distress. The ASQ:SE did not correlate significantly with either external ratings of dyadic interaction or clinically assessed relationship qualities, though the latter two were strongly associated with each other. In contrast, ASQ:SE scores were associated with questionnaires relating to maternal psychological distress. This was especially true for mothers classified as depressed. Furthermore, reports on the ASQ:SE were strongly predicted by maternal stress. The study points to some problems with the concurrent validity of the ASQ:SE in clinical samples. It also demonstrates a close link between mothers' psychological distress and their ratings of infant social and emotional functioning. Further research should investigate the extent to which the ASQ:SE specifically measures infant functioning or maternal distress, and how it functions in clinical versus nonclinical samples. Los cuestionarios en que las madres reportan el funcionamiento socio-emocional del infante se están usando más para determinar la necesidad de referir a los servicios de salud mental infantil. Se investigó la validez del Cuestionario de Edades y Niveles: Socio-emocional (ASQ:SE) en un grupo muestra de madres con infantes pequeños las cuales buscaban ayuda. Al grupo se le comparó con las interacciones de las díadas evaluadas por un observador independiente, y con la calidad de las relaciones de las díadas evaluadas por clínicos expertos. Los puntajes del ASQ:SE también fueron comparados con cuestionarios sobre el estrés y la ansiedad maternales sicológicas. Resultados: el ASQ:SE no se correlacionó significativamente ni con los puntajes externos de la interacción de la díada ni con las cualidades de la relación evaluadas clínicamente, aunque ambas fueron asociadas fuertemente una con la otra. En contraste, los puntajes de ASQ:SE fueron asociados con cuestionarios que se referían a la ansiedad mental psicológica maternal. La validité du questionnaire "âge et étapes: social et émotionnel" (en anglais Ages and Stages Questionnaire: Social et Emotionnel, abrégé ASQ:SE en anglais) a été examinée chez un échantillon de mères de jeunes bébés cherchant de l'aide. Elle a été comparée à des interactions dyadiques évaluées par un observateur indépendant, la qualité des relations dyadiques étant évaluée par des cliniciens experts. Les évaluations ASQ:SE ont aussi été comparées aux questionnaires sur la détresse et le stress psychologique maternel. Résultats: Le questionnaire ASQ:SE n'a pas été fortement mis en corrélation avec soit les évaluations externes d'interaction dyadique soit les qualités de la relation évaluées cliniquement, bien que ces deux dernières aient été fortement liées les unes aux autres. Par contre, les scores ASQ:SE étaient liés aux questionnaires qui portaient sur la détresse psychologique maternelle. Ceci s'est surtout avéré vrai pour les mères classifiées comme déprimées. De plus, les rapports sur le ASQ:SE étaient frotement prédits par le stress maternel. Cette étude met en lumière certains problèmes avec la validité simultanée du questionnaire ASQ:SE dans les échantillons cliniques. Elle démontre aussi un lien étroit entre la détresse psychologique des mères et leurs évaluations du fonctionnement social et émotionnel du nourrisson. Des recherches plus approfondies devraient porter sur la mesure dans laquelle le questionnaire ASQ:SE mesure spécifiquement le fonctionnement du nourrisson ou la détresse maternelle, et comment il fonctionne dans des échantillons cliniques par rapport à non-cliniques. Fragebögen der sozial-emotionalen Funktionsweisen, die auf Elternurteilen beruhen, werden zunehmend zur Klärung der Frage eingesetzt, ob eine ärztliche Überweisung im Rahmen von psychischer Gesundheit im Kleinkindalter induziert ist. Die Gültigkeit des Fragebogens zum Alter und zu den Entwicklungsstufen "Sozial Emotional" (ASQ: SE) wurde von einer Gruppe Hilfe-suchender Mütter mit Kleinkindern entwickelt. Die Fragebögen wurden von unabhängigern Beobachter innerhalb dyadischer Interaktionen validiert. Zusätzlich überprüften erfahrene Kliniker die Qualität der dyadischen Beziehungen. Die Bewertungen mittels ASQ: SE wurden darüber hinaus mit Fragebogen zu mütterlichen psychologischem Stress und Ängste verglichen. Ergebnisse: Die ASQ: SE zeigte keine signifikant Korrelation mit den externen Ergebnissen der dyadischen Interaktion oder den klinisch beurteilten Beziehungsqualitäten, obwohl die beiden letzteren trotzdem in Zusammenhang standen. Im Gegensatz dazu zeigten die Ergebnisse des ASQ: SE einen Zusammenhang mit den Fragebögen zur mütterlichen psychischen Belastung. Dies galt vor allem für Mütter die als depressiv eingestuft wurden. Außerdem machten die Ergebnisse des ASQ: SE eindeutige Vorhersagen was den mütterlicher Stress anbelangt. Die Studie weist auf einige Probleme der übereinstimmenden Gültigkeit des ASQ: SE in klinischen Stichproben hin. Es zeigt aber auch eine enge Verbindung zwischen mütterlicher psychischer Belastung und ihren Bewertungen der sozialen und emotionalen Funktionsfähigkeit ihrer Säuglinge hin. Weitere Forschung sollte untersuchen, inwieweit der ASQ: SE gezielt Maßnahmen zur Steigerung der Funktionalität von Säuglingen oder mütterliche Not misst und in wie weit der Fragebogen im klinischen bzw. nicht-klinischen Setting Gültigkeit behält. Use of the Ages and Stages Questionnaire to predict outcome after hypoxic-ischaemic encephalopathy in the neonate

Background: Infants who suffer hypoxic-ischaemic encephalopathy (HIE) at birth are at increased risk of developmental disability. In this at-risk population, reliable, inexpensive and early identification of those children who are likely to require formal developmental assessment and intervention is needed. Aim: To evaluate the ability of the Ages and Stages Questionnaire (ASQ) to detect developmentally delayed children in an Australian population of infants who suffered HIE at birth. Methods: Fifty-five children who survived HIE were followed until 12,14 months of age. Test characteristics were calculated to examine the ability of the ASQ to appropriately identify developmentally delayed infants against this study's ,gold standard': the Bayley Scales of Infant Development II. Results: Comparing the ASQ with the Bayley Scales of Infant Development II, the questionnaire had the following test characteristics: sensitivity 92%, specificity 95%, positive predictive value 92%, negative predictive value 95% when used to detect severe developmental delay; and sensitivity 67%, specificity 93%, positive predictive value 92%, negative predictive value 68% when used to detect both severe and mild developmental delay. However, the ASQ used at standard cut-offs failed to detect any of the children with mild delay. Conclusions: The ASQ is extremely effective for the detection of severe developmental delay in children who have suffered HIE at birth. Its capacity to identify those with milder delay is limited. The ability of the test to detect only those with severe developmental delay means that the ASQ is of little value as a screening tool in this population. [source]


Characteristics, head shape measurements and developmental delay in 287 consecutive infants attending a plagiocephaly clinic

Abstract Aim:, To describe the characteristics, developmental status and severity of head shape deformation in infants presenting to a plagiocephaly clinic. Methods:, Head shape was measured and neck function assessed in 287 consecutive infants presenting to a plagiocephaly outpatient clinic. Information was obtained on demographic and obstetric factors, plagiocephaly history and current positioning strategies. Development was assessed by the Ages and Stages Questionnaire (ASQ). Results:, After clinical examination, craniosynostosis was suspected in seven infants (2%) and a skull computed tomography (CT) scan was performed; five showed suture abnormalities. Fifty-eight percent of cases had a history of limitation of neck function; this was more likely in plagiocephalic infants than brachycephalic infants. Males, firstborn infants, instrument-delivered infants, supine sleep position and right-sided flattening were predominant. One or more delays on the ASQ were seen in 36% of infants. Conclusion: Physical examination of infants with head shape deformities is essential in order to rule out craniosynostosis. Infants with deformational plagiocephaly frequently have neck muscle dysfunction. We postulate that the higher than expected number of developmental delays may be related to the effects of supine sleep position, low or variable tone, lower activity levels, male gender and neck muscle dysfunction. Successfully Translating Language and Culture when Adapting Assessment Measures

Abstract A need exists for culturally valid and reliable developmental assessment tools for children with disabilities that are able to accommodate multiple languages. One way in which this goal can be achieved is through test translations. The purpose of this preliminary study was to examine the use of translations of select developmental assessment instruments from English to Afrikaans and from one cultural context to another (Western to South African). Specifically, we examined children's performance on two measures of development: the Mullen Scales of Early Learning and the Ages and Stages Questionnaires (ASQ). Both measures were completed for 47 typically developing South African preschool children between 3 and 6 years of age. The Mullen was completed by a speech and language therapist and the ASQ by a parent. Both of the measures used yielded similar results, and compared favorably with the existing norms. The procedures provide a framework for expanding such adaptations in other applications. [source]


Evaluation of the Ages and Stages Questionnaires in identifying children with neurosensory disability in the Magpie Trial follow-up study

Abstract Aim: To evaluate performance of the Ages and Stages Questionnaires (full ASQ), and a shortened version (short ASQ), in detecting children with severe neurosensory disability in the Magpie Trial follow-up study. Methods: All children, born to women in the Magpie Trial and selected for follow-up, with a completed full 30 items and/or short 9-items ASQ were included in this analysis. Sensitivity and specificity, corrected for verification bias, were computed to assess detection ability. Results: Of the 2046 children who completed a full ASQ, 406 (19.8%) failed the assessment, 54 of whom had confirmed neurosensory disability. Adjusted sensitivity and specificity (95% confidence intervals) were 87.4% (62.9,96.6%), and 82.3% (80.5,83.9%), respectively. Two of the five domains in the full ASQ (Fine Motor and Problem Solving) contributed little to detection ability. Sensitivity and specificity for the short ASQ were 69.2% and 95.7%, respectively. Conclusions: Sensitivity of the full ASQ for severe neurosensory disability is generally good, and does not appear to be much reduced by restricting questions to three out of the five domains. The short ASQ reported here reduced performance, although this might be improved by a different choice of questions or scoring system. [source]


Parent-completed developmental screening in a Norwegian population sample: a comparison with US normative data

Aim: To compare normative data of a Norwegian translation of the Ages and Stages Questionnaires with original US normative data. Methods: Norwegian-born mothers randomly selected from the population register completed Norwegian translations of the Ages and Stages Questionnaires, a series of 19 age-specific child development screening questionnaires each made up of 30 items in five domains: Communication, Gross Motor, Fine Motor, Problem Solving, and Personal-Social. Domain score group differences with original US normative data on 10 age-specific questionnaires (for ages 4, 8, 12, 16, 20, 24, 30, 36, 48, and 60 mo) were investigated. The Norwegian data consisted of 1341 children, varying between 82 and 176 per age interval. Results: On the whole, parents' reports of their children's development were very similar in the two data sets. Only five out of 50 mean comparisons revealed a mean difference either greater than a Cohen's d of 0.5 or greater than the smallest increment on a domain score. The variation in scores tended to be somewhat smaller in the Norwegian sample. Conclusion: It seems reasonable to expect that domain scores on the Ages and Stages Questionnaires may be interpreted in the same way in Norway and the United States, and these results may also generalize to other Western settings. These findings from a true random sample also increase the confidence in the original normative data. [source]
