    Effectiveness of Supportive Educative Learning programme on the level of strain experienced by caregivers of stroke patients in Thailand

    Abstract In Thailand, the crude death rate from stroke is 10.9/100 000 population and increasing. Unlike Western countries where community rehabilitation programmes have been established to provide services following the acute stage of stroke recovery, there is no stroke rehabilitation team in the community in Thailand. Therefore, family caregivers are the primary source for ongoing care and support. While family members accompany patients during their hospitalisation, they receive little information about how to assist their relatives, and as a result feel inadequately trained, poorly informed and dissatisfied with the support that is available after discharge. Family caregivers report that they suffer both physically and psychologically and find themselves overwhelmed with strain, experiencing burden and exhaustion. This study aimed to develop and implement a nurse-led Supportive Educative Learning programme for family caregivers (SELF) of stroke survivors in Thailand and to evaluate the effect of the SELF programme on family caregiver's strain and quality of life. This was a non-randomised comparative study with concurrent controls, using a two-group pre-test and post-test design. A total of 140 stroke survivors and 140 family caregivers were recruited; 70 patients/caregiver pair in each group. Caregivers of patients admitted to the intervention hospital following an acute stroke received the intervention, while caregivers of patients admitted to the comparison hospital received the usual care provided at the hospital. The data were collected prior to discharge of the patients and after 3 months. The family caregivers in the intervention group had a significantly better quality of life than the comparison group (GHQ-28 at discharge t = 2.82, d.f. = 138, P = 0.006; and at 3 months t = 6.80, d.f. = 135, P < 0.001) and they also reported less strain (Caregiver Strain Index at discharge t = 6.73, d.f. = 138, P < 0.001; and at 3 months t = 7.67, d.f. = 135, P < 0.001). This research demonstrated that providing education and support to the family caregiver of stroke survivors can reduce caregiver strain and enhance their quality of life. [source]

    Older people specific health status and quality of life: a structured review of self-assessed instruments

    Abstract Objectives, To review evidence relating to the measurement properties of older people specific self-assessed, multi-dimensional measures of health status. Design, Systematic literature searches to identify instruments. Pre-defined criteria relating to reliability, validity, responsiveness, precision and acceptability. Results, A total of 46 articles relating to 18 instruments met the inclusion criteria. Most evidence was found for the OARS Multidimensional Functional Assessment Questionnaire (OMFAQ), CARE, Functional Assessment Inventory (FAI) and Quality of Life Profile , Seniors Version (QOLPSV). Most instruments have been evaluated in single studies. Four instruments have evidence of internal consistency and test,retest reliability , LEIPAD, Philadelphia Geriatrics Centre Multilevel Assessment Inventory, Perceived Well-being Scale, Wellness Index (WI). Two instruments lack evidence of reliability , Brief Screening Questionnaire, Geriatric Quality of Life Questionnaire (GQLQ). Older people contributed to the content of the GQLQ, QOLPSV and WI. Most instruments were assessed for validity through comparisons with other instruments, global judgements of health, or clinical and socio-demographic variables. Limited evidence of responsiveness was found for five instruments , GQLQ, OMFAQ, PGCMAI, QOLPSV, Self-Evaluation of Life Scale (SELF). Conclusion, Although most evidence was found for the OMFAQ this was largely for the ADL domain; evidence for reliability and responsiveness is limited. Limited evidence of reliability, validity and responsiveness was found for the PGCMAI, QOLPSV and SELF. The lack of evidence for measurement properties restricts instrument recommendation. Instrument content should be assessed for relevance before application and the concurrent evaluation of specific and widely used generic instruments is recommended. Several instruments, including the BSQ and EASY-Care, were developed recently and further evidence of instrument performance is required. [source]


    DEVELOPMENT ENGAGEMENT WITHIN AND FOLLOWING DEVELOPMENTAL ASSESSMENT CENTERS: CONSIDERING FEEDBACK FAVORABILITY AND SELF,ASSESSOR AGREEMENT

    This study sought to understand employees' level of behavioral engagement in response to feedback received in developmental assessment center (DAC) programs. Hypotheses were drawn from theories of self-enhancement and self-consistency and from findings in the multisource feedback and assessment center literatures regarding recipients' perceptions of feedback. Data were gathered from 172 U.S. middle managers participating in a DAC program. Results suggested that more favorable feedback was related to higher behavioral engagement. When discrepancies between self- and assessor ratings were examined, overraters (participants whose overall self-ratings were higher than their assessor ratings) tended to show less engagement in the program compared to underraters. However, pattern agreement on the participant's dimension profile did not significantly correlate with behavioral engagement. Based on these findings, avenues for future research are presented and practical implications are discussed. [source]


    SELF- VERSUS OTHERS' RATINGS AS PREDICTORS OF ASSESSMENT CENTER RATINGS: VALIDATION EVIDENCE FOR 360-DEGREE FEEDBACK PROGRAMS

    Although 360-degree feedback programs are rapidly increasing in popularity, few studies have examined how well ratings from these programs predict an independent criterion. This study had 2 main aims: First, to examine the validity of ratings from a 360-degree feedback program using assessment center ratings as an independent criterion and to determine which source (i.e., self, supervisor, peers, or subordinates) provided the most valid predictor of the criterion measure of competency. Second, to better understand the relationship between self-observer discrepancies and an independent criterion. The average of supervisor, peer, and subordinate ratings predicted performance on the assessment center, as did the supervisor ratings alone. The self-ratings were negatively and nonlinearly related to performance with some of those who gave themselves the highest ratings having the lowest performance on the assessment center. Supervisor ratings successfully discriminated between overestimators but were not as successful at discriminating underestimators, suggesting that more modest feedback recipients might be underrated by their supervisors. Peers overestimated performance for poor performers. Explanations of the results and the implications for the use of self-ratings in evaluations, the design of feedback reports, and the use of 360-degree feedback programs for involving and empowering staff are discussed.     COMMUNITY, CONTEXT, AND THE PRESENTATION OF SELF IN DISTRIBUTED WORKPLACE INTERACTION

    Instantaneous communications technology has made it possible for distant coworkers to be interconnected to an unprecedented degree. Despite this, distributed workers often feel deeply disconnected from the production and performance of conventional workplace relationships and workplace culture. As the knowledge economy workforce trends toward ever-greater distribution and globalization, this raises important questions about the practice and experience of creative coengagement by colleagues who are not proximate to each other in time and space. How are shared understandings of workers' behavioral norms disseminated and practiced when workers are physically isolated from the collective workspace? How are relationships of collegiality and hierarchy constructed and performed through increasingly narrow channels of social interaction? How do workers signal their energy and commitment to a collective creative enterprise when their actual productive activity is largely invisible to others with whom and for whom they work? This article draws on my research with distributed knowledge workers, informal observations of colleagues, and personal experiences working as an independent consultant in distributed settings. It focuses on the challenges these workers face in defining their workplace community and effectively representing their professional selfhood when working at a distance. In this article I suggest that one key to alleviating these challenges is to extend the attributes of "placehood" to distant work spaces. [source]

    Exact and Robust (Self-)Intersections for Polygonal Meshes

    Abstract We present a new technique to implement operators that modify the topology of polygonal meshes at intersections and self-intersections. Depending on the modification strategy, this effectively results in operators for Boolean combinations or for the construction of outer hulls that are suited for mesh repair tasks and accurate mesh-based front tracking of deformable materials that split and merge. By combining an adaptive octree with nested binary space partitions (BSP), we can guarantee exactness (= correctness) and robustness (= completeness) of the algorithm while still achieving higher performance and less memory consumption than previous approaches. The efficiency and scalability in terms of runtime and memory is obtained by an operation localization scheme. We restrict the essential computations to those cells in the adaptive octree where intersections actually occur. Within those critical cells, we convert the input geometry into a plane-based BSP-representation which allows us to perform all computations exactly even with fixed precision arithmetics. We carefully analyze the precision requirements of the involved geometric data and predicates in order to guarantee correctness and show how minimal input mesh quantization can be used to safely rely on computations with standard floating point numbers. We properly evaluate our method with respect to precision, robustness, and efficiency. [source]


    Schooling the Possible Self

    ABSTRACT From a social perspective, one's identity is entirely the product of interaction with others. As children participate in the vast range of social situations, they collect impressions of themselves that coalesce to form a sense of who they are, as well as a narrative framework that helps explain the world and their place within it. These insights create a dynamic identity that is stimulated by one's sense of potential and possibility. The social perspective provides a way to understand how school situations offer the substance from which children develop a sense of self. Literacy is a particularly powerful conduit for the development of self. An understanding of language and literacy, and how these processes are taken up by the child as means to shape his or her social connections and, by extension, his or her social reality, demands an understanding of self and how it evolves through interaction in a range of contexts. The purpose of this article is to describe how "self" plays out through literacy situations at home and school. Borrowing from social and cultural descriptions of the development of self, this article illustrates how these situations provide contexts for the expression and development of self, and offers implications for curriculum and classroom practice.     Writing as Inquiry: Storying the Teaching Self in Writing Workshops

    Recent research demonstrates that the process of telling and writing personal stories is a powerful means of fostering teachers' professional growth (Connelly & Clandinin, 1995; Conle, 1996; Diamond, 1994; Heikkinen, 1998; Kelchtermans, 1993). This article aims to further understanding of writing in the development of teachers' narratives of practice, and to critically examine the potential of the writing workshop as a space where diverse voices can find expression. I take up a narrative perspective, seeing the practice of teaching as constructed when teachers tell and live out particular stories. I examine the autobiographic writing of teachers who participated in a graduate course on autobiography and professional development, drawing on phenomenological (Van Manen, 1990) and narrative methods (Mishler, 1986) and attending to issues of voice (Raymond, Butt, & Townsend, 1992, Brown & Gilligan, 1992) and "restorying" (Clandinin & Connelly, 1996, 1998). The main questions addressed are how do teachers narratively construct their own development and how does the university context, usually construed as a locus of knowledge transmission, function as a framework for the processes of storytelling, reflection, and restorying of experience and for the elaboration by teachers of an internally persuasive discourse (Bakhtin, 1981)? The article describes the experience of the course and the various uses to which participants put autobiographic writing; the range of voices used in the writing is indicated. Three "moments" in the writing process are discussed: describing, storying, and questioning, moments that, taken together, are seen to make up the restorying process. The conclusions point to limitations and possibilities of writing in the academic setting, in particular the place of theory in helping to draw out teachers' voices. [source]


    Symptomatic candidiasis: Using self sampled vaginal smears to establish the presence of Candida, lactobacilli, and Gardnerella vaginalis

    Abstract In a prospective cohort study, 10 symptomatic women with recurrent vulvovaginal candidiasis were taught how to prepare vaginal smears of their own vaginal fluids on days 7, 14, 21, and 28. The 40 smears were stained with the PAS-method and examined by three different cytopathologists for presence of Candida. Thereafter, the smears were restained with Giemsa-stain to determine presence of lactobacilli, Gardnerella vaginalis ("clue cells") and neutrophils. All three cytopathologists unequivocally established Candida blastospores and (pseudo)hyphae in 27 out of the 40 PAS-stained vaginal smears, whereas in the remaining 13 smears Candida was not found. All 10 patients had Candida in their smears during the second half of their menstrual cycle. Self sampled smears prove to be reliable for establishing the presence of Candida in symptomatic patients with candidiasis. Candida is associated with a lactobacillus -predominated vaginal flora, but with the absence of Gardnerella vaginalis. Further studies may be directed towards the interaction between the various members of the vaginal flora. It fails, I suggest, primarily because it consists of mutually supporting but erroneous conceptualisations of knowledge and the human subject.     Children's Sense of Self in Relation to Clinical Processes: Portraits of Pharmaceutical Transformation

    This article presents in-depth accounts of pharmaceutical transformation from the perspective of two children diagnosed with behavioral and emotional disorders. These portraits provide the basis for an examination of the complex interrelation between self and clinical processes. Narrative data were collected in the context of a 13-month anthropological study of the lived experiences of children diagnosed with behavioral and emotional disorders and their families living in the northeastern United States. Participating families (N=20) were from diverse racial/ethnic (African American, Euro-American, and Latino) and socioeconomic backgrounds. Psychiatric diagnoses and pharmaceuticals present tangible constraints in the lives of children that call attention to otherwise fluid and ephemeral self processes. These accounts suggest that psychiatric diagnoses and psychotropic medications present dilemmas for children's developing sense of self, revealing limitations to biopsychiatric "pharmaceutical promises."     Sitting in Silence: Self, Emotion, and Tradition in the Genesis of a Charismatic Ministry

    David Willson was the charismatic leader of a small Utopian Quaker sect, the Children of Peace (1812,89), who prophesied a millenarian transformation of the British empire. This article examines the confluence of social forces and historical conditions that made this charismatic ministry possible. Following Csordas, the emphasis is placed on the means by which followership is created, rather than on the personality of the leader. I argue that Willson's charismatic leadership was predicated upon inculcating a distinctive habitus, on shaping and molding cultural conceptions of self and of emotion, which create the distinctive disposition to obey infollowers. A "theology of mind" was critical to Willson's ministry, and the culturally and historically distinctive emotions and dispositions it described were inculcated in the communal ritual practice of "sitting in silence."     Getting better byte by byte: a pilot randomised controlled trial of email therapy for bulimia nervosa and binge eating disorder,

    Abstract One hundred and ten people in an university population responded to emailed eating disorder questionnaires. Ninty-seven fulfilling criteria for eating disorders (bulimia nervosa (BN), binge eating disorder (BED), EDNOS) were randomised to therapist administered email bulimia therapy (eBT), unsupported Self directed writing (SDW) or Waiting list control (WLC). Measures were repeated at 3 months. Diagnosis, Beck depression inventory (BDI) and Bulimia investigatory test (BITE) scores were recorded. Follow-up rate was 63% and results must be interpreted cautiously. However significantly fewer participants who had received eBT or SDW fulfilled criteria for eating disorders at follow up compared to WLC. There was no significant difference between eBT and SDW in the analysis of variance (ANOVA), although in separate analyses, eBT was significantly superior to WLC (p,<,0.02) and the difference for SDW approached significance (p,=,0.06). BDI and BITE scores showed no significant change. For eBT participants there was a significant positive correlation between words written and improvement in BITE severity score.     Pramipexole and pergolide in the treatment of depression in Parkinson's disease: a national multicentre prospective randomized study

    An 8-month multicentre prospective randomized study aimed at comparing the effects of dopamine receptor agonists pramipexole (PPX; Mirapexin®) and pergolide (PRG; Permax®) as add-on to L-dopa therapy on depression [Montgomery and Asberg Depression Rating Scale (MADRS)] in 41 non-demented patients (25 men, 16 women) suffering from both mild or moderate depression and advanced Parkinson's disease (PD). The assessment was performed by a blinded independent observer. Motor symptoms (UPDRS III), motor complications (UPDRS IV), activities of daily living (UPDRS II and VI) and depressive symptoms as measured by Self , Rating Depression Scale by Zung were evaluated in an open-label design. The average value of Zung scores decreased significantly in both groups with no statistical difference between both groups. A significant decrease in the average value of MADRS scores was present only in the PPX group. The average UPDRS scores decreased significantly with no statistical difference between both groups at the comparable average total daily dose of both preparations. In both cases, the total daily dose of L-dopa decreased significantly but the decrease was statistically more pronounced in the PRG group. Our results demonstrate the antidepressant effect of PPX in patients with PD while we can't make any conclusions with regard to antidepressant effect of PRG. [source]


    Self and partner personality in intimate relationships

    Two studies were conducted to examine the relations between both partners' personality and marital quality in married or cohabiting heterosexual couples. In Study 1 (N,=,1380, or 690 couples), personality was assessed by means of the Dutch Personality Questionnaire, whereas in Study 2 (N,=,564, or 282 couples) personality was assessed by means of the Five-Factor Personality Inventory. We expected neuroticism to relate negatively, and extraversion positively, to marital quality. Furthermore, we expected that spouses would only marginally resemble each other with regard to personality, and that differences in personality would not affect marital quality, when controlling for the individual's levels of personality.     Taxonomy and structure of Croatian personality-descriptive adjectives

    This paper describes the development of a comprehensive taxonomy of Croatian personality-descriptive terms, organized in three studies. In the first study three judges searched through a standard dictionary of the Croatian language for person-descriptive terms. In the second study, personality-descriptive adjectives were classified by seven judges into 13 different categories of descriptors. In the third study, the 483 adjectives that the majority of judges in the second study classified as dispositions were rated for self-descriptions by 515 University of Zagreb students and for peer-descriptions by 513 students' best acquaintances. Self- and peer ratings were factor analysed separately and the Croatian emic lexical factors from both data sets were interpreted to be similar to the Big-Five factors: Agreeableness, Extraversion, Conscientiousness, Intellect, and Emotional Stability. The inspection of factor content of the Croatian emic factors and their relation to imported Big-Five measures revealed high correspondences for all five Croatian factors although the relation between the Croatian and the imported factors of Emotional Stability and Agreeableness was somewhat more complex.     Bodily Awareness, Imagination and the Self

    Common wisdom tells us that we have five senses: sight, hearing, smell, taste and touch. These senses provide us with a means of gaining information concerning objects in the world around us, including our own bodies. But in addition to these five senses, each of us is aware of our own body in ways in which we are aware of no other thing. These ways include our awareness of the position, orientation, movement, and size of our limbs (proprioception and kinaesthesia), our sense of balance, and our awareness of bodily sensations such as pains, tickles, and sensations of pressure or temperature. We can group these together under the title ,bodily awareness'. The legitimacy of grouping together these ways of gaining information is shown by the fact that they are unified phenomenologically; they provide the subject with an awareness of his or her body ,from the inside'. Bodily awareness is an awareness of our own bodies from within. This perspective on our own bodies does not, cannot, vary. As Merleau-Ponty writes, ,my own body,is always presented to me from the same angle' (1962: 90). It has recently been claimed by a number of philosophers that, in bodily awareness, one is not simply aware of one's body as one's body, but one is aware of one's body as oneself. That is, when I attend to the object of bodily awareness I am presented not just with my body, but with my ,bodily self'. The contention of the present paper is that such a view is misguided. In the first section I clarify just what is at issue here. In the remainder of the paper I present an argument, based on two claims about the nature of the imagination, against the view that the bodily self is presented in bodily awareness. Section two defends the dependency thesis; a claim about the relation between perception and sensory imagination. Section three defends a certain view about our capacity to imagine being other people. Section four presents the main argument against the bodily self awareness view and section five addresses some objections. [source]


    Effects of mortality salience aroused by threats against human identity on intergroup bias

    An experimental study on the effects of mortality salience aroused by threats against different components of the Self (personal, social and human identities) on intergroup bias is presented. It is hypothesised and found that the mortality salience per se does not inevitably led to increments of intergroup bias.     Self, in-group, and out-group evaluation: bond or breach?

    A number of studies have looked at causes of in-group bias, but few studies have actually investigated whether the two components of in-group bias, i.e. in-group and out-group evaluation, are related to each other and whether they have similar or different predictors. In the Fiji Islands, self-, in-group, and out-group evaluations were obtained using within-subject correlations from a sample of 336 indigenous and Indian Fijians. Self-evaluation was positively related to in-group evaluation, and both were positively related to out-group evaluation, supporting a spillover model. After controlling for background variables and the other evaluation variables, regression analyses showed that in-group identification was positively related to in-group evaluation, and social distance and political ethnocentrism were negatively related to out-group evaluation.     Determinants of Institutional Responses to Self,Tender Offers

    I examine how institutional investors respond to self,tender offers for common shares. I find that institutions sell more shares in larger offers and with higher proration factors. Institutions also sell more shares when officer and director holdings are not at risk in the offers. Banks, investment advisors, and other managers respond similarly, selling more shares in larger offers. Although institutions as a group do not respond differently by offer type, insurance companies and investment advisors sell more shares in fixed,price offers. Mutual funds, which differ from other types of institutions, sell more shares for firms with greater increases in leverage. [source]
