[source] Building the Powerful 10-Minute Office Visit: Part II.THE LARYNGOSCOPE, Issue 1 2001Beginning a Critical Literature Review Abstract Objective This is the second part in a series of sequential Tutorials in Clinical Research. The objective of this tutorial is to introduce methods of searching the vast stores of information now available, to review some of the computer resources available, to reintroduce the concept of an a priori design for the search, and to reveal the need for assessment of the clinical importance and validity of each pertinent article found. Study Design Tutorial. Methods An open working group has been formed with the specific aim of surveying and translating the large volume of complex information on research design and statistics into easily understood, useable, and non-threatening tutorials for the busy practitioner. The hypotheses under which this work is conducted are highly intelligent, but extremely busy, surgeons are interested in evidence-based medicine and will increase personal participation in critical reading of the literature, pending an expanded familiarity with clinical research design and statistics. Results Available resources for literature searching, methods of quick personal overviews, and quick question-specific reviews are discussed. Additionally, the methods, with examples, of beginning a critical literature review are presented. Conclusions Rapid, personal, critical literature review requires succinct formulation of the question, efficient search for the best available evidence, and critical appraisal of the pertinent individual articles to determine if sufficient evidence exists to support a clinical contention. [source] Resource distribution and the trade-off between seed number and seed weight: a comparison across crop speciesANNALS OF APPLIED BIOLOGY, Issue 1 2010B.L. Gambín In grain crops, total sink capacity is usually analysed in terms of two components, seed number and individual seed weight. Seed number and potential individual seed weight are established at a similar timing, around the flowering period, and seed weight at maturity is highly correlated with the potential established earlier. It is known that, within a species, available resources during the seed set period are distributed between both yield components, resulting in a trade-off between seed number and seed weight. Here we tested if this concept could apply for interspecific comparisons, where combinations of numbers and size across species could be related to the total available resources being either allocated to more seed or larger potential individual seed weight during the seed set period. Based on this, species differences in seed weight should be related to resource availability per seed around the period when seed number is determined. Resource availability per seed was estimated as the rate of increase in aboveground biomass per seed around the period of seed set. Data from 15 crop species differing in plant growth, seed number, seed weight and seed composition were analysed from available literature. Because species differed in seed composition, seed weight was analysed following an energy requirement approach. There was an interspecific trade-off relationship between seed number per unit of land area and seed weight (r = 0.92; F(1, 13) = 32.9; n = 15; P < 0.001). Seed weight of different species was positively correlated (r = 0.90; F(1, 13) = 52.9; n = 15; P < 0.001) with resource availability per seed around the seed set period. This correlation included contrasting species like quinoa (Chenopodium quinoa; ,100000 seeds m,2, ,4 mg equivalent-glucose seed,1) or peanut (Arachis hypogaea; ,800 seeds m,2, ,1000 mg equivalent-glucose seed,1). Seed number and individual seed weight combinations across species were related and could be explained considering resource availability when plants are adjusting their seed number to the growth environment and seeds are establishing their storage capacity. Available resources around the seed set period are proportionally allocated to produce either many small seeds or few larger seeds depending on the particular species. [source] A peer-to-peer decentralized strategy for resource management in computational GridsCONCURRENCY AND COMPUTATION: PRACTICE & EXPERIENCE, Issue 9 2007Antonella Di Stefano Abstract This paper presents a peer-to-peer (P2P) approach for the management, in a computational Grid, of those resources that are featured by numerical quantity and thus characterized by a coefficient of utilization, such as percentage of CPU time, disk space, memory space, etc. The proposed approach exploits spatial computing concepts and models a Grid by means of a flat P2P architecture consisting of nodes connected by an overlay network; such a network topology, together with the quantity of resource available in each node, forms a three-dimensional surface, where valleys correspond to nodes with a large quantity of available resource. In this scenario, this paper proposes an algorithm for resource discovery that is based on navigating such a surface, in search of the deepest valley (global minimum, that is, the best node). The algorithm, which aims at fairly distributing among nodes the quantity of leased resource, is based on some heuristics that mimic the laws of kinematics. [source] The Contribution of Bioenergy to a New Energy ParadigmEUROCHOICES, Issue 3 2005Daniel De La Torre Ugarte Biomass is a widely available resource that is receiving increased consideration as a renewable substitute for fossil fuels. Developed sustainably and used efficiently, it can induce growth in developing countries, reduce oil demand, and address environmental problems. The potential benefits include: reduction of greenhouse gases, recuperation of soil productivity and degraded land, economic benefits from adding value to agricultural activities and improving access to and quality of energy services. The production of bioenergy involves a range of technologies, including solid combustion, gasification, and fermentation. These technologies produce energy from a diverse set of biological resources - traditional crops, crop residues, energy-dedicated crops, dung, and the organic component of urban waste. The results are bioenergy products that provide multiple energy services: cooking fuel, heat, electricity and transportation fuels. It is this very diversity that holds the potential of a win-win-win for the environment, social and economic development. Bioenergy has to be viewed not as a replacement for oil, but as an element of a portfolio of renewable sources of energy. Coherent and mutually supportive environmental and economic policies may be needed to encourage the emergence of a globally dispersed bioenergy industry that will pursue a path of sustainable development. The system can help a user to remember names of people whom he/she has met before and find useful information, such as names and research interests, about people whom he/she is interested in talking to. The system uses a captured face image as the lookup index to retrieve information from some available resource such as departmental directory, web sites, personal homepages, etc. We describe the development of a PDA-based face recognition system and introduce algorithms for image preprocessing to enhance the quality of the image by sharpening focus and normalizing both lighting condition and head rotation. We use a unified LDA/PCA algorithm for face recognition. We address design issues of the interface to assist in visualization and comprehension of retrieved information. Manfredi Abstract This paper is concerned with the analysis, design and validation of a reliable management strategy for industrial monitoring and control over wireless sensor network (WSN). First, we investigate the interactions between contention resolution and congestion control mechanisms in Wireless Industrial Sensor Network (briefly WISN). An extensive set of simulations are performed in order to quantify the impacts of several network parameters (i.e. buffer, sensors reporting rate) on the overall network performance (i.e. reliability, packet losses). This calls for cross-layer mechanisms for efficient data delivery over WISN. Second, a reliable sink resource allocation strategy based on log-utility fairness criteria is proposed. It is shown that the resource sink manager can plan strategies to better allocate the available resource among competing sensors. Finally, the analysis, design and validation of a reliable sinks cooperative control for WISN are introduced. A sufficient condition for wireless network stability in presence of multiple sinks and heterogeneous sensors with different time delays is given and it is used for network parameters design. The stability condition and the resulting cooperative control performance in terms of fairness, link utilization, packet losses, reliability and latency are validated by Matlab/Simulink-based simulator TrueTime, which facilitates co-simulation of controller task execution in real-time kernels and in the wireless network environment. [source] Middle School Mathematics Teachers' Beliefs About Inclusion of Students with Learning DisabilitiesLEARNING DISABILITIES RESEARCH & PRACTICE, Issue 2 2006Janet R. DeSimone The purpose of this descriptive study was to investigate middle school general education mathematics teachers' beliefs and self-perceived knowledge regarding teaching students with learning disabilities (LD) in inclusive classrooms. Teacher beliefs regarding administrative support and higher education teacher preparation were also examined. The Survey on Teaching Mathematics to Students With Learning Disabilities in Middle School was completed by 228 sixth-, seventh-, and eighth-grade general education mathematics inclusion teachers from 19 states. In addition, telephone interviews were conducted with a subset of 26 survey respondents. Frequency analyses were performed on the survey data, with ,2 tests comparing teachers on demographic variables. Follow-up interview responses were summarized to elaborate on the major research questions. The findings revealed three central issues: (1) teachers had a limited understanding of the mathematics learning needs of students with LD, (2) teacher collaboration was judged to be the most beneficial and available resource by general educators teaching students with LD in inclusive mathematics classrooms, and (3) teachers did not feel that teacher education programs at the preservice level and professional development at the inservice level were adequate in preparing them for teaching students with LD in inclusive mathematics classrooms. Implications and recommendations for teacher preparation and program implementation are provided. [source] The Biomolecular Crystallization Database Version 4: expanded content and new featuresACTA CRYSTALLOGRAPHICA SECTION D, Issue 1 2009Michael Tung The Biological Macromolecular Crystallization Database (BMCD) has been a publicly available resource since 1988, providing a curated archive of information on crystal growth for proteins and other biological macromolecules. The BMCD content has recently been expanded to include 14,372 crystal entries. The resource continues to be freely available at http://xpdb.nist.gov:8060/BMCD4. In addition, the software has been adapted to support the Java-based Lucene query language, enabling detailed searching over specific parameters, and explicit search of parameter ranges is offered for five numeric variables. Extensive tools have been developed for import and handling of data from the RCSB Protein Data Bank. The updated BMCD is called version 4.02 or BMCD4. BMCD4 entries have been expanded to include macromolecule sequence, enabling more elaborate analysis of relations among protein properties, crystal-growth conditions and the geometric and diffraction properties of the crystals. The BMCD version 4.02 contains greatly expanded content and enhanced search capabilities to facilitate scientific analysis and design of crystal-growth strategies. [source] Operative Platform Applied to Building AutomationCOMPUTER-AIDED CIVIL AND INFRASTRUCTURE ENGINEERING, Issue 1 2009João Figueiredo This structure is composed by three interrelated levels: the Operational level,where the field equipment is controlled, the Inter-Active level,where inhabitants communicate the building their preferences regarding control variables (lights, temperature, etc.), and the higher-level control, the Overall Building Well-Being Model, which manages the global building, taking into account the optimization of the inhabitants preferences, constrained by the available resources. At this third level, the inter-building communication is available. Each building has the capability to communicate with its neighbors, informing about fires, floods, security problems, power consumption expectations, and so on. This article implements one of the three above-referred interrelated control levels: the Operational-level control. This operative platform is structured over a cascade hierarchical control architecture where inner loops are performed by local PLCs (Programmable Logic Controllers), and the outer loop is managed by a centralized SCADA system (Supervisory Control and Data Acquisition) that interacts with the entire PLC network. The lower-level control loop assures high processing velocity tasks, the upper-level control loop updates the local references, knowing the complete system state. This operative model is tested on two prototypes, where all instrumentation in place is controlled by the industrial PLC network. Both prototypes worked perfectly showing the huge potential of communication systems between distributed processes. These communication systems allow intelligent centralized algorithms to manage decision-making problems in real-time environments. The system presented in this article combines several technologies (local PLCs, SCADA systems, and network communications) to reach the goal of efficient management of intelligent buildings. [source] Adaptive workflow processing and execution in PegasusCONCURRENCY AND COMPUTATION: PRACTICE & EXPERIENCE, Issue 16 2009Kevin Lee Abstract Workflows are widely used in applications that require coordinated use of computational resources. Workflow definition languages typically abstract over some aspects of the way in which a workflow is to be executed, such as the level of parallelism to be used or the physical resources to be deployed. As a result, a workflow management system has the responsibility of establishing how best to execute a workflow given the available resources. The Pegasus workflow management system compiles abstract workflows into concrete execution plans, and has been widely used in large-scale e-Science applications. This paper describes an extension to Pegasus whereby resource allocation decisions are revised during workflow evaluation, in the light of feedback on the performance of jobs at runtime. The contributions of this paper include: (i) a description of how adaptive processing has been retrofitted to an existing workflow management system; (ii) a scheduling algorithm that allocates resources based on runtime performance; and (iii) an experimental evaluation of the resulting infrastructure using grid middleware over clusters. Jarvis Abstract In this paper we present a predictive performance model for a key biomedical imaging application found as part of the U.K. e-Science Information eXtraction from Images (IXI) project. This code represents a significant challenge for our existing performance prediction tools as it has internal structures that exhibit highly variable runtimes depending on qualities in the input data provided. Since the runtime can vary by more than an order of magnitude, it has been difficult to apply meaningful quality of service criteria to workflows that use this code. The model developed here is used in the context of an interactive scheduling system which provides rapid feedback to the users, allowing them to tailor their workloads to available resources or to allocate extra resources to scheduled workloads. Barbisch RN As economic forces have reduced immediately available resources, the need to surge to meet patient care needs that exceed expectations has become an increasing challenge to the health care community. The potential patient care needs projected by pandemic influenza and bioterrorism catapulted medical surge to a critical capability in the list of national priorities, making it front-page news. Proposals to improve surge capacity are abundant; however, surge capacity is poorly defined and there is little evidence-based comprehensive planning. There are no validated measures of effectiveness to assess the efficacy of interventions. Before implementing programs and processes to manage surge capacity, it is imperative to validate assumptions and define the underlying components of surge. The functional components of health care and what is needed to rapidly increase capacity must be identified by all involved. Appropriate resources must be put into place to support planning factors. Using well-grounded scientific principles, the health care community can develop comprehensive programs to prioritize activities and link the necessary resources. Building seamless surge capacity will minimize loss and optimize outcomes regardless of the degree to which patient care needs exceed capability. [source] A comparison of invasive and non-invasive dayflowers (Commelinaceae) across experimental nutrient and water gradientsDIVERSITY AND DISTRIBUTIONS, Issue 5-6 2004Jean H. Burns ABSTRACT Little is known about the traits and mechanisms that determine whether or not a species will be invasive. Invasive species are those that establish and spread after being introduced to a novel habitat. A number of previous studies have attempted to correlate specific plant traits with invasiveness. However, many such studies may be flawed because they fail to account for shared evolutionary history or fail to measure performance directly. It is also clear that performance is context dependent. Thus, an approach that corrects for relatedness and incorporates multiple experimental conditions will provide additional information on performance traits of invasive species. I use this approach with two or three pairs of invasive and closely related non-invasive species of Commelinaceae grown over experimental gradients of nutrient and water availability. Invasive species have been introduced, established, and spread outside their native range; non-invasive species have been introduced, possibly (but not necessarily) established, but are not known to have spread outside their native range. The invasive species had higher relative growth rates (RGR) than non-invasive congeners at high nutrient availabilities, but did not differ from non-invasive species at low nutrient availabilities. This is consistent with a strategy where these particular invasive species are able to rapidly use available resources. Relative growth rates were also higher for two out of three invasive species across a water availability gradient, but RGR did not differ in plasticity between the invasive and non-invasive species. This suggests that nutrient addition, but not changes in water availability, might favour invasion of dayflowers. This approach is novel in comparing multiple pairs of invasive and non-invasive congeners across multiple experimental conditions and allows evaluation of the robustness of performance differences. It also controls for some of the effects of relatedness that might confound multispecies comparisons. CAMPBELL This article compares chronologies reconstructed from historical records of prices, wages, grain harvests, and population with corresponding chronologies of growing conditions and climatic variations derived from dendrochronology and Greenland ice-cores. It demonstrates that in pre-industrial, and especially late medieval, England, short-term environmental shocks and more enduring shifts in environmental conditions (sometimes acting in concert with biological agencies) exercised a powerful influence upon the balance struck between population and available resources via their effects upon the reproduction, health and life expectancy of humans, crops, and livestock. Prevailing socio-economic conditions and institutions, in turn, shaped society's susceptibility to these environmental shocks and shifts. [source] Epidemiology of unarmed threats in the emergency departmentEMERGENCY MEDICINE AUSTRALASIA, Issue 4 2005Jonathan C Knott Abstract Objective:, To evaluate the precipitants, subject characteristics, nature and outcomes of unarmed threats in the ED. Methods:, A 12 month prospective survey of security codes precipitated by an unarmed threat (Code Grey). Results:, Data were collected on 151 subjects. The Code Grey rate was 3.2/1000 ED presentations. They were most frequent on Saturday and in the late evening/early morning. There were verbal or physical threats of violence made to staff on 104 occasions (69%, 95% confidence interval [CI] 61,76) and a perceived threat of patient self-harm on 114 occasions (76%, 95% CI 68,82). Median time to be seen by a doctor was 8 min (interquartile range [IQR]: 2,21 min) and median time from presentation to Code was 59 min (IQR: 5,222 min). Sixteen subjects (11%, 95% CI 6,17) had a history of violence, 45 (30%, 95% CI 23,38) were affected by alcohol, 25 (17%, 95% CI 11,24) had used illicit drugs and 79 (52%, 95% CI 44,60) had a significant mental illness contributing to the Code Grey. Seventy-one patients (47%, 95% CI 39,55) required psychiatric admission, 49 (79%, 95% CI 66,88) involuntarily. Conclusion:, Acutely agitated subjects pose a threat to themselves and the staff caring for them. The reason for the agitation is multifactorial and the majority arrive in a behaviourally disturbed state requiring early intervention. The times most likely to result in a Code Grey coincide with least available resources: ED and hospital risk management policies must account for this. A coherent approach by ED to this population is required to optimize patient and staff outcomes. [source] Interspecific competition among urban cockroach speciesENTOMOLOGIA EXPERIMENTALIS ET APPLICATA, Issue 1 2004Stéphane Boyer Abstract The aim of this study was to quantify and to compare the effects of intraspecific and interspecific competition among pairs of urban cockroaches [Blatta orientalis L., Periplaneta americana (L.), and Periplaneta australasiae Fabricius (Dictyoptera: Blattodea)] in relation to the limitation of resources such as shelter or food. Our approach was to assess whether the presence of one species affected the resource exploitation of another. A reduction in access to shelters or to food for one species revealed dominant/subordination relationships among species and induced spatial segregation in shelters and temporal segregation during food exploitation. The fragmentation of available resources facilitated spatial segregation and the access of more individuals of the subordinate species. Individuals of each species were aware of the presence of non-conspecifics. The outcome of interspecific competition under laboratory conditions should help us to understand how segregated spatial distributions occur under natural conditions in urban areas. Asadi-Pooya Summary Epilepsy surgery has been proved to be efficacious, safe and cost-effective in developing countries. However, the success of epilepsy surgery depends on selecting suitable candidates based on the available resources and technologies. Some of the challenges to provide appropriate surgical treatment for epilepsy patients in developing countries include providing human and technological resources and developing realistic presurgical protocols. Detection of ideal candidates for epilepsy surgery is possible for well-trained epileptologists with the help of basic investigative technologies, for example, magnetic resonance imaging (MRI) and electroencephalography (EEG). Patients with potentially epileptogenic, well-circumscribed lesions on MRI and patients with mesial temporal lobe epilepsy (MTLE) are reasonable candidates for surgery. Palliative epilepsy surgeries include corpus callosotomy and other disconnections. These operations are feasible in developing countries with a knowledgeable team consisting of an epileptologist, neurosurgeon and technicians and with using MRI and EEG as basic investigative technologies. [source] When do the Costs of Spermatogenesis Constrain Sperm Expenditure?ETHOLOGY, Issue 1 2009Remarks on the Pattern of the Spermatogenic Cycle The costs of spermatogenesis constrain sperm expenditure when sperm production per day is limited. Thus, males are challenged to allocate available resources to sperm production and other life history functions. However, this prevailing assumption is not applicable to species in which spermatogenesis becomes quiescent during the breeding season. Males of these species prepare large quantities of sperm before the breeding season. Among these species, constraints on ejaculates have been intensively investigated in salamanders that deposit spermatophores. Although it is predicted that sperm expenditure should not be limited because of abundantly prepared sperm, spermatophore deposition is often limited during the breeding season when vas deferens are full of sperm. We tested a hypothesis regarding limited spermatophore deposition by measuring sperm quantity and volume of spermatophores sequentially deposited by male eastern newts Notophthalmus viridescens. A male newt rarely deposits more than three spermatophores per mating. If depletion of non-sperm components of spermatophores limits spermatophore deposition, we predicted that spermatophore volume decreases while sperm quantity remains constant as a male deposits more spermatophores. Alternatively, some regulative mechanisms allow a limited portion of available sperm to be expended per mating, in which sperm quantity is predicted to decrease while the spermatophore volume remains constant. Finally, depletion of non-sperm components may regulate sperm expenditure, which predicted that both spermatophore volume and sperm quantity decrease. We found that both sperm quantity and the spermatophore volume decreased as a male deposited more spermatophores during a single mating. Sperm expenditure was constrained without the costs involved in active spermatogenesis, and depletion of non-sperm components likely regulate sperm quantity loaded in spermatophores. In dissociated spermatogenesis, constrained sperm expenditure do not mean that costly spermatogenesis is directly limiting male mating capacity but rather suggest that the evolution of physiological mechanisms regulating sperm expenditure per mating maximizes male reproductive success. [source] Internal Capital Markets and Capital Structure: Bank Versus Internal DebtEUROPEAN FINANCIAL MANAGEMENT, Issue 3 2010Nico Dewaelheyns G32; G21 Abstract We argue that domestic business groups are able to actively optimise the internal/external debt mix across their subsidiaries. Novel to the literature, we use bi-level data (i.e. data from both individual subsidiary financial statements and consolidated group level financial statements) to model the bank and internal debt concentration of non-financial Belgian private business group affiliates. As a benchmark, we construct a size and industry matched sample of non-group affiliated (stand-alone) companies. We find support for a pecking order of internal debt over bank debt at the subsidiary level which leads to a substantially lower bank debt concentration for group affiliates as compared to stand-alone companies. The internal debt concentration of a subsidiary is mainly driven by the characteristics of the group's internal capital market. The larger its available resources, the more intra-group debt is used while bank debt financing at the subsidiary level decreases. However, as the group's overall debt level mounts, groups increasingly locate bank borrowing in subsidiaries with low costs of external financing (i.e. large subsidiaries with important collateral assets) to limit moral hazard and dissipative costs. Overall, our results are consistent with the existence of a complex group wide optimisation process of financing costs. Shanley Abstract., The disposable soma theory suggests that aging occurs because natural selection favors a strategy in which fewer resources are invested in somatic maintenance than are necessary for indefinite survival. However, laboratory rodents on calorie-restricted diets have extended life spans and retarded aging. One hypothesis is that this is an adaptive response involving a shift of resources during short periods of famine away from reproduction and toward increased somatic maintenance. The potential benefit is that the animal gains an increased chance of survival with a reduced intrinsic rate of senescence, thereby permitting reproductive value to be preserved for when the famine is over. We describe a mathematical life-history model of dynamic resource allocation that tests this idea. Senescence is modeled as a change in state that depends on the resources allocated to maintenance. Individuals are assumed to allocate the available resources to maximize the total number of descendants. The model shows that the evolutionary hypothesis is plausible and identifies two factors, both likely to exist, that favor this conclusion. These factors are that survival of juveniles is reduced during periods of famine and that the organism needs to pay an energetic "overhead" before any litter of offspring can be produced. If neither of these conditions holds, there is no evolutionary advantage to be gained from switching extra resources to maintenance. The model provides a basis to evaluate whether the life-extending effects of calorie-restriction might apply in other species, including humans. Faced with widespread degradation of riverine ecosystems, stream restoration has greatly increased. Such restoration is rarely planned and executed with inputs from ecological theory. In this paper, we seek to identify principles from ecological theory that have been, or could be, used to guide stream restoration. 2. In attempts to re-establish populations, knowledge of the species' life history, habitat template and spatio-temporal scope is critical. In many cases dispersal will be a critical process in maintaining viable populations at the landscape scale, and special attention should be given to the unique geometry of stream systems 3. One way by which organisms survive natural disturbances is by the use of refugia, many forms of which may have been lost with degradation. Restoring refugia may therefore be critical to survival of target populations, particularly in facilitating resilience to ongoing anthropogenic disturbance regimes. 4. Restoring connectivity, especially longitudinal connectivity, has been a major restoration goal. In restoring lateral connectivity there has been an increasing awareness of the riparian zone as a critical transition zone between streams and their catchments. 5. Increased knowledge of food web structure , bottom-up versus top-down control, trophic cascades and subsidies , are yet to be applied to stream restoration efforts. 6. In restoration, species are drawn from the regional species pool. Having overcome dispersal and environmental constraints (filters), species persistence may be governed by local internal dynamics, which are referred to as assembly rules. 7. While restoration projects often define goals and endpoints, the succession pathways and mechanisms (e.g. facilitation) by which these may be achieved are rarely considered. This occurs in spite of a large of body of general theory on which to draw. 8. Stream restoration has neglected ecosystem processes. The concept that increasing biodiversity increases ecosystem functioning is very relevant to stream restoration. Whether biodiversity affects ecosystem processes, such as decomposition, in streams is equivocal. 9. Considering the spatial scale of restoration projects is critical to success. Success is more likely with large-scale projects, but they will often be infeasible in terms of the available resources and conflicts of interest. Small-scale restoration may remedy specific problems. In general, restoration should occur at the appropriate spatial scale such that restoration is not reversed by the prevailing disturbance regime. 10. The effectiveness and predictability of stream ecosystem restoration will improve with an increased understanding of the processes by which ecosystems develop and are maintained. Ideas from general ecological theory can clearly be better incorporated into stream restoration projects. This will provide a twofold benefit in providing an opportunity both to improve restoration outcomes and to test ecological theory. [source] Research and development at the health and social care interface in primary care: a scoping exercise in one National Health Service regionHEALTH & SOCIAL CARE IN THE COMMUNITY, Issue 6 2002Jo Cooke MA Abstract The present project aimed to identify research activity at the health and social care interface in primary care within one National Health Service region, and to determine levels of research capacity and support within social services. The study was commissioned by a primary care research network (PCRN) in order to assess opportunities to increase research capacity within social services. Data were collected in two phases from 61 managers, team leaders and senior practitioners in social care, and six public health representatives in health authorities, using telephone interviews and focus groups. The findings highlighted a lack of infrastructure and support for research and development in social care. However, many social care respondents wanted opportunities to develop research skills with healthcare colleagues. Despite poor support, many small-scale projects were described, and many respondents showed an enthusiasm for engaging with research. Methods in use included surveys, action research, needs analysis and evaluation of service developments. Many examples of user involvement were given. Interface projects were usually instigated by interagency forums and funded from multiple sources. Most project work was motivated by service improvement or development, rather than aiming to produce generalisable knowledge. Barriers to conducting research included lack of confidence, research skills and time, as well as workload demands, lack of cover to release staff for research and lack of supervision. Research was not seen as legitimate work in some social care environments or as part of a career path. Existing joint working initiatives (such as the National Service Frameworks) were highlighted as flashpoints for potential research and evaluation activity. The findings suggest clear opportunities for PCRNs to develop research capacity at the interface with social care; for example, by signposting available resources, providing training grants and secondments for social care staff, and supporting interagency networks with a focus on evaluation. In turn, experience in promoting user involvement in social services could add value to research expertise at the primary care,social care interface. A questionnaire survey of 114 nurses working on five acute wards in a large inner city teaching hospital investigated their general use of computers and the three databases, cinahl, medline and the Cochrane Library. Eighty-two qualified nurses responded (response rate 72%). The results show limited confidence and low frequency in using the databases. Thirty-four per cent expressed low confidence using cinahl. Twenty-seven per cent used cinahl on a regular basis. Twenty-two per cent never used it. Eighteen per cent were unaware that it was available locally. Knowledge and use of medline was even lower with only 18% using it regularly. Knowledge of the Cochrane Library was extremely limited, with 75% unaware of its existence. Use of a home computer and higher education were associated with higher frequency of use of cinahl and medline. If nurses are to make use of electronic resources to contribute to evidence-based practice, effort needs to be put into ensuring that already qualified nurses have basic computer skills and specific knowledge of available resources. This article shows that the existing picture of the English preparations is flawed in several ways and that they were better organized, more efficient and more willing than has been recognized. The privy council was called upon to deploy limited forces to defend a long coastline against an unpredictable attacker, and the evidence shows that they contrived to maximize the effectiveness of the available resources whilst balancing the calls of military practicality, financial necessity and political constraints. To increase the effectiveness of counter systems, improved methods of planning and designing such systems are prerequisite. This paper describes briefly a methodological approach for predicting debris flow characteristics, and proposes techniques for evaluating and improving the mitigative effectiveness of check dams against debris flows in steep mountain torrents. Additionally, a non-dimensional parameter, namely potential storage volume, is introduced to generalize the evaluation processes. As an example, the 1999 debris-flow event in the San Julian River, Venezuela, is chosen for discussion. The paper also proposes a method of evaluating the control function of a series of check dams as well as the criteria for the selection of their sizes, numbers and locations. It is hoped that this work will help to determine which combinations of check dams will fit best together for the optimal control of debris flows and available resources in any river basin. During this time, ethnic identity is central to the migrant's self-esteem. With completion of the mourning processes, the migrant is psychically freed up to also identify with the new country. Over time, the new hybrid identity is consolidated, so that self-esteem is no longer determined by cultural or national identities. The current study empirically tested this postulation in three groups of Chinese migrants who varied in available resources in completing their psychic restructuring. As hypothesized, the function of Chinese and American identities in their self-esteem varied significantly at the time of the study and reflected differential degrees of post-migration identity consolidation. This has stimulated global interest in educating and empowering consumers. The UK government has made a very ambitious commitment to ensure that the framework for consumer empowerment and support is at the level of the best in the world by 2008. The government, many consumer organizations and regulators believe that empowered consumers are key to the success of competitive markets. Two national strategies to co-ordinate activities in the UK have been developed by the Office of Fair Trading (OFT) and the Financial Services Authority (FSA). The OFT consumer education strategy aims to deliver targeted, effective consumer education by increasing co-ordination and making the best use of available resources. The FSA is leading a financial capability strategy designed to deliver change to improve the UK's financial capability. Both strategies share a vision of educated and confident consumers making informed choices about the products and services they buy, and both aim to empower vulnerable consumers. Given the global interest and the development of national strategies, it is useful to consider what is meant by the term consumer empowerment. Is there a shared view of consumer empowerment internationally? Does the education of consumers result in empowered consumers? To what extent do the national strategies address the empowerment of vulnerable, disadvantaged, excluded or susceptible consumers? These questions will be addressed in this article which reviews the global context for the consumer education and empowerment agenda and considers key UK developments, with particular reference to the needs of vulnerable consumers. In this paper, we present a network management tool for resource-partition based L1-VPNs. A Transaction Language One (TL1) proxy is designed to achieve resource partitioning at the network element level. Building on top of a TL1 proxy, we implemented a User-Controlled LightPath (UCLP) system to support physical network brokers to assign and allocate virtually dedicated resources to customers, and to enable customers to directly manage their resources. With such a capability, customers are able to create wide area networks based on their traffic pattern, and to adjust their traffic pattern based on available resources.
