Catheterization,Doppler Discrepancies in Nonsimultaneous Evaluations of Aortic Stenosis

Prior validation studies have established that simultaneously measured catheter (cath) and Doppler mean pressure gradients (MPG) correlate closely in evaluation of aortic stenosis (AS). In clinical practice, however, cath and Doppler are rarely performed simultaneously; which may lead to discrepant results. Accordingly, our aim was to ascertain agreement between these methods and investigate factors associated with discrepant results. We reviewed findings in 100 consecutive evaluations for AS performed in 97 patients (mean age 72 ± 10 yr) in which cath and Doppler were performed within 6 weeks. We recorded MPG, aortic valve area (AVA), cardiac output, and ejection fraction (EF) by both methods. Aortic root diameter, left ventricular end-diastolic dimension (LVIDd) and posterior wall thickness (PWT) were measured by echocardiography and gender, heart rate, and heart rhythm were also recorded. An MPG discrepancy was defined as an intrapatient difference > 10 mmHg. Mean pressure gradients by cath and Doppler were 36 ± 22 mmHg and 37 ± 20 mmHg, respectively (P = 0.73). Linear regression showed good correlation (r = 0.82) between the techniques. An MPG discrepancy was found in 36 (36%) of 100 evaluations; in 19 (53%) of 36 evaluations MPG by Doppler was higher than cath, and in 17 (47%) of 36, it was lower. In 33 evaluations, EF differed by >10% between techniques. Linear regression analyses revealed that EF difference between studies was a significant predictor of MPG discrepancy (P = 0.004). Women had significantly higher MPG than men by both cath and Doppler (43 ± 25 mmHg versus 29 ± 15 mmHg [P = 0.001]; 42 ± 23 mmHg versus 32 ± 15 mmHg [P = 0.014], respectively). Women exhibited discrepant results in 23 (47%) of 49 evaluations versus 13 (25%) of 51 evaluations in men (P = 0.037). After adjustment for women's higher MPG, there was no statistically significant difference in MPG discrepancy between genders (P = 0.22). No significant interactions between MPG and aortic root diameter, relative wall thickness (RWT), heart rate, heart rhythm, cardiac output, and time interval between studies were found. In clinical practice, significant discrepancies in MPG were common when cath and Doppler are performed nonsimultaneously. No systematic bias was observed and Doppler results were as likely yield lower as higher MPGs than cath. EF difference was a significant predictor of discrepant MPG. Aortic root diameter, relative wall thickness, heart rate, heart rhythm, cardiac output, presence or severity of coronary artery disease, and time interval between studies were not predictors of discrepant results. Effects of tractor wheeling on root morphology and yield of lucerne (Medicago sativa L.)

Summary The purpose of this study was to determine the effect of soil compaction on the herbage yield and root growth of lucerne (Medicago sativa L.). A field experiment was conducted on a silty loam Mollic Fluvisols soil in 2003,2006. Herbage yield and root morphology, in terms of root length density, mean root diameter, specific root length and distribution of dry matter (DM) in roots, were measured. Four compaction treatments were applied three times annually by tractor using the following number of passes: control without experimental traffic, two passes, four passes and six passes. The tractor traffic changed the physical properties of the soil by increasing bulk density and penetration resistance. Soil compaction also improved its water retention properties. These changes were associated with changes in root morphology and distribution of the DM in roots. Soil compaction resulted in higher proportions of the DM in roots, especially in the upper, 0,10 cm, soil horizon. Decreases in the root length density were observed in a root diameter range of 0·1,1·0 mm. It was also found that roots in a more compacted soil were significantly thicker. An effect of the root system of lucerne on soil compaction was observed. The root system of lucerne decreased the effects of soil compaction that had been recorded in the first and the second year of the experiment. An increase in the number of passes resulted in a decrease in the DM yield of herbage in the second and third harvests each year. [source]


CASIROZ: Root Parameters and Types of Ectomycorrhiza of Young Beech Plants Exposed to Different Ozone and Light Regimes

Abstract: Tropospheric ozone (O3) triggers physiological changes in leaves that affect carbon source strength leading to decreased carbon allocation below-ground, thus affecting roots and root symbionts. The effects of O3 depend on the maturity-related physiological state of the plant, therefore adult and young forest trees might react differently. To test the applicability of young beech plants for studying the effects of O3 on forest trees and forest stands, beech seedlings were planted in containers and exposed for two years in the Kranzberg forest FACOS experiment (Free-Air Canopy O3 Exposure System, http:www.casiroz.de) to enhanced ozone concentration regime (ambient [control] and double ambient concentration, not exceeding 150 ppb) under different light conditions (sun and shade). After two growing seasons the biomass of the above- and below-ground parts, beech roots (using WinRhizo programme), anatomical and molecular (ITS-RFLP and sequencing) identification of ectomycorrhizal types and nutrient concentrations were assessed. The mycorrhization of beech seedlings was very low (ca. 5 % in shade, 10 % in sun-grown plants), no trends were observed in mycorrhization (%) due to ozone treatment. The number of Cenococcum geophilum type of ectomycorrhiza, as an indicator of stress in the forest stands, was not significantly different under different ozone treatments. It was predominantly occurring in sun-exposed plants, while its majority share was replaced by Genea hispidula in shade-grown plants. Different light regimes significantly influenced all parameters except shoot/root ratio and number of ectomycorrhizal types. In the ozone fumigated plants the number of types, number of root tips per length of 1 to 2 mm root diameter, root length density per volume of soil and concentration of Mg were significantly lower than in control plants. Trends to a decrease were found in root, shoot, leaf, and total dry weights, total number of root tips, number of vital mycorrhizal root tips, fine root (mass) density, root tip density per surface, root area index, concentration of Zn, and Ca/Al ratio. Due to the general reduction in root growth indices and nutrient cycling in ozone-fumigated plants, alterations in soil carbon pools could be predicted. [source]


Growth after ventricular septal defect repair: does defect size matter?

Abstract Aim:, To evaluate whether the ventricular septal defect (VSD) size, along with the degree of preoperative growth impairment and age at repair, may influence postoperative growth, and if VSD size can be useful to identify children at risk for preoperative failure to thrive. Methods:, Sixty-eight children submitted to VSD repair in a Brazilian tertiary-care institution were evaluated. Weight and height measurements were converted to Z -scores. Ventricular septal defect size was normalized by dividing it by the aortic root diameter (VSD/Ao ratio). Results:, Twenty-six patients (38%) had significantly low weight-for-height, 10 patients (15%) had significantly low height-for-age and 13 patients (19%) had both conditions at repair. Catch-up growth occurred in 82% of patients for weight-for-age, in 75% of patients for height-for-age and in 89% of patients for weight-for-height. Weight-for-height Z -scores at surgery were significantly lower in patients who underwent repair before 9 months of age. The VSD/Ao ratio did not associate with any other data. On multivariate analysis, weight-for-age Z -scores and age at surgery were independent predictors of long-term weight and height respectively. Conclusion:, The VSD/Ao ratio was not a good predictor of preoperative failure to thrive. Most patients had preoperative growth impairment and presented catch-up growth after repair. Preoperative growth status and age at surgery influenced long-term growth. [source]


Mitral Valve Prolapse in Marfan Syndrome: An Old Topic Revisited

Background: The echocardiographic features of mitral valve prolapse (MVP) in Marfan syndrome have been well described, and the incidence of MVP in Marfan syndrome is reported to be 40,80%. However, most of the original research was performed in the late 1980s and early 1990s, when the diagnostic criteria for MVP were less specific. Our goal was to investigate the characteristics of MVP associated with Marfan syndrome using currently accepted diagnostic criteria for MVP. Methods: Between January 1990 and March 2004, 90 patients with definitive diagnosis of Marfan syndrome (based on standardized criteria with or without genetic testing) were referred to Massachusetts General Hospital for transthoracic echocardiography. Patients' gender, age, weight, height, and body surface area at initial examination were recorded. Mitral valve thickness and motion, the degree of mitral regurgitation and aortic regurgitation, and aortic dimensions were quantified blinded to patients' clinical information. Results: There were 25 patients (28%) with MVP, among whom 80% had symmetrical bileaflet MVP. Patients with MVP had thicker mitral leaflets (5.0 ± 1.0 mm vs. 1.8 ± 0.5 mm, P < 0.001), more mitral regurgitation (using a scale of 1,4, 2.2 ± 1.0 vs. 0.90 ± 0.60, P < 0.0001), larger LVEDD, and larger dimensions of sinus of Valsalva, sinotubular junction, aortic arch, and descending aorta indexed to square root body surface area, when compared with those without MVP. When echocardiographic features of patients younger than 18 years of age and those of patients older than 18 were compared, adult Marfan patients had larger LA dimension (indexed to square root body surface area), larger sinotubular junction (indexed to square root body surface area), and more mitral regurgitation and aortic regurgitation. Conclusions: The prevalence of MVP in Marfan syndrome is lower than previously reported. The large majority of patients with MVP have bileaflet involvement, and those with MVP have significantly larger aortic root diameters, suggesting a diffuse disease process. [source]


Effects of decreasing soil water content on seminal lateral roots of young maize plants

Abstract Soil micropores that contain water at or below field capacity cannot be invaded by seminal or first-order lateral roots of maize plants because their root diameters are larger than 10 ,m. Hence, at soil-water levels below field capacity plant roots must establish a new pore system by displacement of soil particles in order to access soil water. We investigated how decreasing soil water content (SWC) influences growth and morphology of the root system of young maize plants. Plants were grown in rhizotrons 40,cm wide, 50,cm high, and approximately 0.7,cm thick. Five SWC treatments were established by addition of increasing amounts of water to soil and thorough mixing before filling the rhizotrons. No water was added to treatments 1,4 throughout the experiment. Treatment 5 was watered frequently throughout the experiment to serve as a control. Seminal-root length and SWC in soil layers 0,10, 10,20, 20,30, 30,40, and 40,50,cm were measured at intervals of 2,3 d on scanner images by image analysis. At 15 d after planting, for treatments 1,4 shoot dry weight and total root length were directly related to the amount of water added to the soil, and for treatments 4 and 5, total root length and shoot dry weights were similar. Length of seminal roots visible at the transparent surface of the rhizotron for all treatments was highest in the uppermost soil layer and decreased with distance from the soil surface. For all layers, seminal-root elongation rate was at maximum above a SWC of 0.17,cm3,cm,3, corresponding to a matric potential of ,30 kPa. With decreasing SWC, elongation rate decreased, and 20% of maximum seminal root elongation rate was observed below SWC of 0.05,cm3,cm,3. After destructive harvest for treatment 1,4, number of (root-) tips per unit length of seminal root was found uninfluenced over the range of initial SWC from 0.10 to 0.26,cm3,cm,3. However, initial SWC close to the permanent wilting point strongly increased number of tips. Average root length of first-order lateral (FOL) roots increased as initial SWC increased, and the highest length was found for the frequently watered treatment 5. The results of the study suggest that the ability to produce new FOL roots across a wide range of SWC may give maize an adaptive advantage, because FOL root growth can rapidly adapt to changing soil moisture conditions. Developmental shifts in watermelon growth and reproduction caused by the squash bug, Anasa tristis

Summary ,,Compared with leaf-feeding herbivores, little is known about how sap-feeding herbivores affect plant growth, morphology and reproduction. This study examines effects of the sap-feeding squash bug ( Anasa tristis ) on watermelon ( Citrullus lanatus ) root, leaf and reproductive structures. ,,Plants at the four-leaf stage were exposed to different densities of caged squash bugs for 67 d (to plant maturity). ,,Initial effects were on roots. Herbivory was associated with a significant reduction in mean total root length, root surface area and number of root tips, as well as an increase in root diameters. Herbivore-exposed plants had significantly more leaves, although leaf lifespan was decreased. Both total plant dry mass and fruit dry mass per unit leaf area were significantly greater in controls. Significant effects of herbivores on plant reproductive traits included delayed flower formation (by 7,12 d), change in floral sex ratio (in favor of femaleness), increased fruit abortion, and smaller fruit size. ,,Developmental consequences were related to changes in plant water relations, including decreased water-use efficiency. Water use in treated plants was three to nine times greater per unit of plant dry mass than in controls. [source]
