Pilot randomised controlled trial of community pharmacy administration of buprenorphine versus methadone

Objectives The established regime for opiate substitute prescribing for drug misusers is daily methadone administered under supervision in community pharmacies. Buprenorphine has recently been introduced as an alternative. However there is a lack of evidence of the effectiveness of buprenorphine maintenance therapy (BMT) in the UK treatment setting. This study aimed to assess methods for a randomised controlled trial (RCT) and the feasibility of pharmacy-based supervised self-administration (SSA) of buprenorphine compared to methadone. Setting Specialist substance misuse service, general practices and community pharmacies in Aberdeen, Scotland. Method The design was a pilot RCT. Opiate-dependent drug misusers, newly referred for maintenance treatment were randomised to receive BMT or methadone maintenance therapy (MMT). Clients and pharmacists were interviewed at baseline and at the end of a 12-week intervention period. Clients completed the quality of life measure EQ-5D. Pharmacy activities were timed. Key findings Twenty-one opiate-dependent clients were recruited (BMT = 11, MMT = 10). Recruitment levels improved as the trial progressed. Clients' treatment preferences were evident. Withdrawals occurred early with BMT. Clients found SSA of buprenorphine acceptable, but found daily administration more manageable than three times weekly. Pharmacists found the dispensing of buprenorphine to be an acceptable role, but felt less certain of ensuring against diversion with buprenorphine than they were with methadone. Pharmacy activities associated with buprenorphine took longer than those associated with methadone (mean = 7 min 25 s versus mean = 3 min 27 s, respectively). Conclusion Recruitment to a trial comparing MMT to BMT for opiate-dependent clients within a UK treatment setting is feasible. Clients and pharmacists found buprenorphine acceptable. [source]


Lateral movement of common carp (Cyprinus carpio L.) in a large lowland river and floodplain

Abstract,,, Common carp (Cyprinus carpio L.) are a major freshwater invader and knowledge of their movements is important for planning control efforts. To investigate the movement patterns of common carp, radio-tags were implanted into 46 adult fish; 37 near a large floodplain wetland, the Barmah-Millewa forest, and 9 in the Murray River approximately 175 km upstream. Tagged fish were located every second week between August 1999 and March 2001. Common carp occupied total linear ranges (TLR) between 0.4 and 238 km (mean 30 ± 61 km), with 25 fish (62.5%) occupying a TLR < 10 km. Two fish made large distance movements approximately 650 km downstream. Fish sex, the number of locations, time at large, or tagging location explained little variability (P > 0.05) in TLR. Monthly distance from release varied from 0.04 to 238 km (mean 15 ± 44 km), and was not significantly related to river discharge and water temperature, but 29 of 31 (93.5%) fish tagged at Barmah moved from the Murray River into adjacent floodplain habitats upon flooding. Five fish (12.5%) moved large distances (>127 km) upstream of the Barmah-Millewa forest. Fourteen fish (35%) showed site fidelity to within 20 m and usually occupied one or two home sites. Twenty-six fish (65%) showed site fidelity to within 100 m occupying up to five sites during the study period. Movement patterns of common carp were complex, and individuals exhibited different strategies, which is typical of invasive species. Efforts to control and potentially reduce common carp populations in regulated river-floodplain environments should target key floodplain access points and over-wintering habitats to reduce adult biomass, spawning and recruitment levels. Population dynamics in Digitalis purpurea: the interaction of disturbance and seed bank dynamics

Summary 1Plant ecologists have long since realized that the persistence of many facultative biennial plants depends upon disturbance. However, we still have a limited knowledge of the population-level effects of disturbance, and the connection between adult and seed bank dynamics. 2Using data from a 3-year demographic study combined with experimental gap-opening in a large population of Digitalis purpurea, we parameterized stochastic transition matrix models in ,disturbed' vs. ,undisturbed' areas. We simulated different gap sizes (fraction of population that was disturbed) and temporal disturbance patterns (constant, random, regular and irregular return intervals) and evaluated the effects on population growth rate and seed bank dynamics. To explore seed bank importance we used two alternatives for seed bank survival rate (0.75/0.35) and three alternatives for seed bank recruitment fraction (0.9/0.5/0.1). 3Observed background recruitment levels were insufficient to ensure a positive population growth rate. Increased amounts of gap-opening led to higher growth rates, and population persistence was predicted at moderate disturbance levels if seed bank survival was high (0.75). 4Temporal disturbance pattern affected model results; random and interval scenarios resulted in lower population growth rates and higher extinction risks than constant scenarios of the same average disturbance level. Small and frequent disturbances led to considerably higher growth rates than large and rare disturbances. 5Stochastic elasticity analyses identified the seed bank as the most important life cycle stage with respect to population growth and persistence in most scenarios, and its relative impact was positively related to seed bank survival rate and negatively related to disturbance level. Variation in the recruitment fraction from seed bank vs. seed rain affected both population growth rate and elasticity patterns, indicating the large impact of spatial variation in seed bank density. 6Synthesis: Despite the existence of a large seed bank, our data suggest that recruitment may be locally seed-limited due to a patchy seed bank structure. Local population development may consequently differ widely from gap to gap. These results illustrate how spatial structures in both seed bank, adult population and gap formation interact to shape plant population dynamics, as well as the occurrence of microsite- vs. seed-limitation. Seed Dispersal of the Palm Syagrus romanzoffiana by Tapirs in the Semi-deciduous Atlantic Forest of Argentina

ABSTRACT Vertebrates play a fundamental role in the dispersal of Neotropical trees, generating different seed shadows according to their physical and behavioral features. Tapirs are capable of consuming great quantities of large fruits, and they defecate seeds far from parent trees. For instance, intact seeds of the palm Syagrus romanzoffiana have been found in tapir dung piles in the Atlantic Forest of southeastern Brazil, suggesting that tapirs effectively disperse this species. However, recruitment was not examined therein. We studied tapir endozoochory of large and medium seeds in the semi-deciduous Atlantic Forest of Argentina by examining dung piles found within Iguazu National Park. We recorded dung-pile positions to evaluate the spatial distribution. We also counted the number of juveniles in 2 × 2 m quadrats placed on old dung piles in latrines, beneath adults and in random sites to estimate recruitment levels. Syagrus romanzoffiana seeds were present in 98 percent of dung piles, averaging >200 seeds/dung pile, indicating that this species constitutes the main fruit component in the tapir's diet. Dung piles showed a clumped deposition pattern reflecting the use of latrines. Juvenile recruitment in latrines was 21 times higher than that of under-adult sites and 500 times greater than in random sites, and correlated with the frequency of use of the areas. We concluded that the lowland tapir is a major disperser of S. romanzoffiana. Given that this palm can be considered a keystone species, a disruption of this interaction might affect the entire community of frugivores in the long term. [source]
