Limitations of previously published systematic reviews evaluating the outcome of endodontic treatment

Abstract The aim of this work was to identify the limitations of previously published systematic reviews evaluating the outcome of root canal treatment. Traditionally, periapical radiography has been used to assess the outcome of root canal treatment with the absence of a periapical radiolucency being considered a confirmation of a healthy periapex. However, a high percentage of cases confirmed as healthy by radiographs revealed apical periodontitis on cone beam computed tomography (CBCT) and by histology. In teeth, where reduced size of the existing radiolucency was diagnosed by radiographs and considered to represent periapical healing, enlargement of the lesion was frequently confirmed by CBCT. In clinical studies, two additional factors may have further contributed to the overestimation of successful outcomes after root canal treatment: (i) extractions and re-treatments were rarely recorded as failures; and (ii) the recall rate was often lower than 50%. The periapical index (PAI), frequently used for determination of success, was based on radiographic and histological findings in the periapical region of maxillary incisors. The validity of using PAI for all tooth positions might be questionable, as the thickness of the cortical bone and the position of the root tip in relation with the cortex vary with tooth position. In conclusion, the serious limitations of longitudinal clinical studies restrict the correct interpretation of root canal treatment outcomes. Systematic reviews reporting the success rates of root canal treatment without referring to these limitations may mislead readers. The outcomes of root canal treatment should be re-evaluated in long-term longitudinal studies using CBCT and stricter evaluation criteria. [source]


Classification of leaf images

Abstract There are tremendous content-based retrieval systems. Most of them are applied to general image databases. Some were proposed for specified databases such as texture databases, ancient paintings, document image databases, digital mammography, face image databases, etc. However, there are fewer for plant databases. Plants are used in various fields such as in foodstuff, medicine, and industry. Recently, plant is important for environment protection. On the other hand, the problem of plant destruction becomes worse in the few years. We should train people to know about plants, in turn, to treasure and protect them. In addition to the limited number of expert botanists, the convenient content-based retrieval system for plant is necessary and useful, since it can retrieve related information and knowledge from plant database for the query leaf so as to facilitate fast learning of plants. In this study, a leaf database is constructed and a classification method for leaves is proposed. Most approaches for leaf classification in literature used contour-based features. The proposed method tries to use region-based features. The reasons are that region-based features are more robust than contour-based features since significant curvature points are hard to find. Those features adopted include aspect ratio, compactness, centroid, and horizontal/vertical projections. The effectiveness of the proposed method has been demonstrated by various experiments. On the average, our method has the classification accuracy for 1-NN rule as 82.33% and the recall rate for 10 returned images as 48.2%, while the contour-based method has 37.6% and 21.7%, respectively. © 2006 Wiley Periodicals, Inc. A Prospective Ten-Year Clinical Trial of Porcelain Veneers

abstract Objective:, The purpose of this prospective clinical study was to evaluate the performance of porcelain veneers after 5 and 10 years of clinical service. Materials and Methods:, A single experienced clinician placed 87 porcelain veneers in 25 patients in 1990 and 1991. The teeth included maxillary central incisors to first premolars. As described in the 5-year report of this study, preparations included a chamfer margin, 0.3 to 0.7 mm labial enamel reduction, and incisal edge coverage. A single laboratory technician fabricated the veneers using feldspathic porcelain on refractory dies. Internal surfaces were etched with 5% hydrofluoric acid and silanated. Teeth were isolated with a rubber dam prior to veneer placement. All veneers were bonded with a light-activated resin cement. Patients were recalled at 5 to 6 years and at 10 years after initial veneer placement. Two evaluators examined each veneer for retention, fractures, color match, surface roughness, marginal adaptation, leakage, recurrent caries, pulp vitality, and patient satisfaction. Marginal adaptation was assessed further using scanning electron microscopy to examine epoxy replicas. Results:, Five years after placement, all 87 veneers remained in place and had "perfect" color match and surface smoothness. Four veneers had fractures, but only one of those required repair. Ninety-nine percent of the veneers had clinically acceptable marginal adaptation, although just 14% of the veneers had "perfect" marginal adaptation at all margins. One had clinically unacceptable staining from leakage. Recurrent caries was present at the proximal margin of two veneers. At the 10-year evaluation, which had a 93% recall rate, color match and surface roughness remained optimal. Thirteen of 22 patients were very satisfied with the esthetic result, whereas 7 complained of minor esthetic problems. The fracture rate increased substantially, to 34% at the 10-year recall. However, only 11% of the fractures were clinically unacceptable. None of the veneers had debonded, but the percentage of veneers with "perfect" marginal adaptation had declined to only 4%. Leakage was now evident around two-thirds of the veneers, and eight restorations had recurrent caries. Conclusion:, Porcelain veneers are a reliable and effective means for conservative esthetic treatment of anterior teeth in the long term. After 10 years of clinical service, esthetic results remained good, patient satisfaction was high, and the retention rate was excellent. The number of irreparable fractures was low. Appropriate preparation design, occlusion, and use of adhesive materials contribute to the ultimate outcome. [source]


CoagMDB: a database analysis of missense mutations within four conserved domains in five vitamin K,dependent coagulation serine proteases using a text-mining tool,

Abstract Central repositories of mutations that combine structural, sequence, and phenotypic information in related proteins will facilitate the diagnosis and molecular understanding of diseases associated with them. Coagulation involves the sequential activation of serine proteases and regulators in order to yield stable blood clots while maintaining hemostasis. Five coagulation serine proteases,factor VII (F7), factor IX (F9), factor X (F10), protein C (PROC), and thrombin (F2),exhibit high sequence similarities and all require vitamin K. All five of these were incorporated into an interactive database of mutations named CoagMDB (http://www.coagMDB.org; last accessed: 9 August 2007). The large number of mutations involved (especially for factor IX) and the increasing problem of out-of-date databases required the development of new database management tools. A text mining tool automatically scans full-length references to identify and extract mutations. High recall rates between 96 and 99% and precision rates of 87 to 93% were achieved. Text mining significantly reduces the time and expertise required to maintain the databases and offers a solution to the problem of locus-specific database management and upkeep. A total of 875 mutations were extracted from 1,279 literature sources. Of these, 116 correspond to Gla domains, 86 to the N-terminal EGF domain, 73 to the C-terminal EGF domain, and 477 to the serine protease domain. The combination of text mining and consensus domain structures enables mutations to be correlated with experimentally-measurable phenotypes based on either low protein levels (Type I) or reduced functional activities (Type II), respectively. Automatic appearance-based loop detection from three-dimensional laser data using the normal distributions transform

We propose a new approach to appearance-based loop detection for mobile robots, using three-dimensional (3D) laser scans. Loop detection is an important problem in the simultaneous localization and mapping (SLAM) domain, and, because it can be seen as the problem of recognizing previously visited places, it is an example of the data association problem. Without a flat-floor assumption, two-dimensional laser-based approaches are bound to fail in many cases. Two of the problems with 3D approaches that we address in this paper are how to handle the greatly increased amount of data and how to efficiently obtain invariance to 3D rotations. We present a compact representation of 3D point clouds that is still discriminative enough to detect loop closures without false positives (i.e., detecting loop closure where there is none). A low false-positive rate is very important because wrong data association could have disastrous consequences in a SLAM algorithm. Our approach uses only the appearance of 3D point clouds to detect loops and requires no pose information. We exploit the normal distributions transform surface representation to create feature histograms based on surface orientation and smoothness. The surface shape histograms compress the input data by two to three orders of magnitude. Because of the high compression rate, the histograms can be matched efficiently to compare the appearance of two scans. Rotation invariance is achieved by aligning scans with respect to dominant surface orientations. We also propose to use expectation maximization to fit a gamma mixture model to the output similarity measures in order to automatically determine the threshold that separates scans at loop closures from nonoverlapping ones. We discuss the problem of determining ground truth in the context of loop detection and the difficulties in comparing the results of the few available methods based on range information. Furthermore, we present quantitative performance evaluations using three real-world data sets, one of which is highly self-similar, showing that the proposed method achieves high recall rates (percentage of correctly identified loop closures) at low false-positive rates in environments with different characteristics. © 2009 Wiley Periodicals, Inc. Does adaptive training work?

People intuitively alter the allocation of study time between items of varying difficulty, and such adaptive learning methods are widely used in education and in commercially available memory training programs. We investigated the effectiveness of a computer-based adaptive learning system that utilises spacing and repetition effects by presenting difficult items with short gaps to establish fast learning, and easy items with long intervals to optimise long-term retention. The immediate and delayed effects of adaptive training on cued recall were investigated relative to a control condition of non-adaptive, random spacing. Adaptive training produced significantly higher immediate and delayed recall rates than random spacing. These results encourage the use of adaptive training in education and rehabilitation.
