Protospathidium serpens (Kahl, 1930) is frequent in semiterrestrial and terrestrial habitats worldwide. Conventionally, all populations are considered as conspecific because they have very similar overall morphologies and morphometrics. We studied in detail not only the morphology of the vegetative cells but also the resting cysts using live observation, protargol impregnation, and scanning electron microscopy. These revealed a cryptic diversity and biogeographic pattern in details of the dorsal brush and cyst wall morphology. The cyst wall is spiny in the Austrian specimens, while smooth in the South African and Antarctic populations. Accordingly, P. serpens consists of at least two species: P. serpens (with spiny cyst wall) and P. fraterculum n. sp. (with smooth cyst wall); the latter is probably composed of two distinct taxa differing by the absence (South African)/presence (Antarctic) of a monokinetidal bristle tail in brush row 3, the number of dikinetids comprising brush row 1 (seven versus three), and the total number of brush dikinetids (29 versus 17). Protospathidium serpens is neotypified with the new population from Austria. The significance of resting cyst morphology is discussed with respect to alpha-taxonomy and overall ciliate diversity. [source] P-25 DOES THE PRESENCE OF KOILOCYTES IN A BNA SMEAR AFFECT CLINICAL OUTCOMES?CYTOPATHOLOGY, Issue 2006L. Alexander Introduction:, There is ongoing debate about the terminology used in the classification of dyskaryosis, including whether BNA smears should be classified according to whether koilocytes are present or not. We explored the effect of koilocytosis in the management experiences and clinical outcomes of women with a single BNA smear. Methods:, This study includes 410 women aged 20,59 years, resident in Tayside who had an ,baseline' BNA smear between 31/10/1999 and 31/10/2002 who were eligible for the TOMBOLA trial but who did not participate. Recommended follow-up for these women was a repeat smear in six months. Up to three-years follow-up data was collected on subsequent cytological smears, colposcopy examinations and any related histology. The baseline smear was re-read for the presence or absence of koilocytes. Women were classified according to their clinical outcomes during their three years. Results:, 47% (192/410) of women were classified as having koilocytosis at baseline. The mean age was lower among these women (25 years) compared to those without koilocytosis (35 years). 55% (105/192) of women with koilocytosis at baseline had one or more negative smears and no further abnormal smears, compared to 63% (137/218) of those without koilocytosis, the difference was not statistically significant. 28% (53/192) of women with koilocytosis at baseline were referred to colposcopy after subsequent low-grade or high-grade smears compared to 19% (42/218) of those without koilocytosis; the difference was statistically significant. 14% (27/192) of women with koilocytosis at baseline were diagnosed with CIN2/3 or worse during follow-up compared to 10% (21/218) of women without koilocytosis; the difference was not statistically significant. Discussion:, Our results suggest that the presence or absence of koilocytosis has little impact on the clinical outcomes of women with a BNA smear. [source] 2. PRESENCE ACHIEVED IN LANGUAGE (WITH SPECIAL ATTENTION GIVEN TO THE PRESENCE OF THE PAST)HISTORY AND THEORY, Issue 3 2006HANS ULRICH GUMBRECHT ABSTRACT The aim of this essay is to ask whether what it calls the "presence" of things, including things of the past, can be rendered in language, including the language of historians. In Part I the essay adumbrates what it means by presence (the spatio-temporally located existence of physical objects and events). It also proposes two ideal types: meaning-cultures (in which the interpretation of meaning is of paramount concern, so much so that the thinghood of things is often obscured), and presence-cultures (in which capturing the tangibility of things is of utmost importance). In the modern period, linguistic utterance has typically come to be used for, and to be interpreted as, the way by which meaning rather than presence is expressed, thereby creating a gap between language and presence. Thus, in Part II the essay explores ways that this gap might be bridged, examining seven instances in which presence can be "amalgamated" with language. These range from instances in which the physical dimensions of language itself are made manifest, to those through which the physicality of the things to which language refers is supposed to be made evident. Of particular note for theorists of history are those instances in which things can be made present by employing the deictic, poetic, and incantatory potential of linguistic expression. The essay concludes in Part III with a reflection on Heidegger's idea that language is the "house of being," now interpreted as the idea that language can be the medium through which the separation of humans and the (physical) things of their environment may be overcome. The hope of achieving presence in language is no less than a reconciliation of humans with their world, including,and of most interest to historians,the things and events of their past. [source] 3. ANKERSMIT ABSTRACT There are no dictionary meanings or authoritative discussions of "presence" that fix the significance of this word in a way that ought to be accepted by anybody using it. So we are in the welcome possession of great freedom to maneuver when using the term. In fact, the only feasible requirement for its use is that it should maximally contribute to our understanding of the humanities. When trying to satisfy this requirement I shall relate "presence" to representation. Then I focus on a variant of representation in which the past is allowed to travel to the present as a kind of "stowaway" (Runia), so that the past is literally "present" in historical representation. I appeal to Runia's notion of so-called "parallel processes" for an analysis of this variant of historical representation. [source] 4. THE MATERIAL PRESENCE OF THE PASTHISTORY AND THEORY, Issue 3 2006EWA DOMANSKA ABSTRACT This article deals with the material presence of the past and the recent call in the human sciences for a "return to things." This renewed interest in things signals a rejection of constructivism and textualism and the longing for what is "real," where "regaining" the object is conceived as a means for re-establishing contact with reality. In the context of this turn, we might wish to reconsider the (ontological) status of relics of the past and their function in mediating relations between the organic and the inorganic, between people and things, and among various kinds of things themselves for reconceptualizing the study of the past. I argue that the future will depend on whether and how various scholars interested in the past manage to modify their understanding of the material remnants of the past, that is, things as well as human, animal, and plant remains. In discussing this problem I will refer to Martin heidegger's distinction between an object and a thing, to bruno latour's idea of the agency of things and object-oriented democracy, and to Don Ihde's material hermeneutics. To illustrate my argument I will focus on some examples of the ambivalent status of the disappeared person (dead or alive) in argentina, which resists the oppositional structure of present versus absent. In this context, the disappeared body is a paradigm of the past itself, which is both continuous with the present and discontinuous from it, which simultaneously is and is not. Since there are no adequate terms to analyze the "contradictory" or anomalous status of the present-absent dichotomy, I look for them outside the binary oppositions conventionally used to conceptualize the present-absent relationship in our thinking about the past. for this purpose I employ Algirdas Julien Greimas's semiotic square. In this study, the ability of a Microcystis aeruginosa Kütz. strain to produce free dissolved enzymes was documented. We have observed that this highly toxic strain releases alkaline phosphatase, leucine aminopeptidase, and ,-glucosidase into the ambient environment. Additionally, the inhibitory activity of selected phenols produced by aquatic plants on the activity of these enzymes was analyzed. The investigated compounds, pyrogallol and, to a lesser degree, hydroquinone, decreased the activity of extracellular enzymes produced by M. aeruginosa, with leucine aminopeptidase being the most sensitive to the inhibitors. The noncompetitive character of enzymatic inhibition suggests that the polyphenols produced by aquatic plants are able to influence the activity of different extracellular or membrane-bound enzymes. [source] PRESENCE OF SPOROPHYLLS IN FLOATING KELP RAFTS OF MACROCYSTIS SPP. (PHAEOPHYCEAE) ALONG THE CHILEAN PACIFIC COAST,JOURNAL OF PHYCOLOGY, Issue 5 2005Erasmo C. Macaya Some species of macroalgae continue to live for extended periods of time after detachment and may even maintain reproductive structures, yet very little is known about this process. Here, we describe the presence of sporophylls (with sporogenous tissues) on floating kelp rafts of Macrocystis spp. along the coast of Chile. Surveys were conducted at nine sites (18,50° S) during austral summer 2002, and floating kelp rafts were seen and collected at seven of these nine sites (between 22 and 50° S). Fifteen (26.8%) of the 56 samples had sporophylls, indicating maintenance of sporophylls after detachment. Some of the kelp sporophytes with reproductive blades showed signs of having been afloat for long periods (indicated by the large size of attached stalked barnacles). Additionally, experiments showed that floating kelps released viable zoospores. To understand the reproductive dynamics of floating kelps, we compared these results with information from attached populations of Macrocystis spp. at nearby coastal sites. In general, attached kelp had higher proportions of sporophylls than floating rafts, suggesting that detachment may negatively affect reproductive status. Nevertheless, floating kelps remained functionally reproductive, suggesting that zoospores may be dispersed via floating rafts. Published reports on other macroalgae indicate that some species (Lessoniaceae, Fucaceae, and Sargassaceae) are fertile and probably release zoospores or zygotes while floating or drifting in ocean currents. Because dispersal distances achieved by spores of most macroalgae are relatively short, release of spores from floating algae may be an alternative mechanism of long-distance dispersal. [source] RELATIONSHIP BETWEEN PRESENCE OF A MOTHER CELL WALL AND SPECIATION IN THE UNICELLULAR MICROALGA NANNOCHLORIS (CHLOROPHYTA),JOURNAL OF PHYCOLOGY, Issue 1 2003Maki Yamamoto The cell division mechanisms of seven strains from six species of Nannochloris Naumann were analyzed and compared with those of three species of Chlorella Beijerinck and Trebouxia erici Ahmadjian using differential interference microscopy and fluorescence microscopy. Nannochloris bacillaris Naumann divides by binary fission and N. coccoides Naumann divides by budding. Distinct triangular spaces or mother cell walls were found in the dividing autosporangia of the other five strains from four species of Nannochloris, three species of Chlorella, and T. erici. In an attempt to infer an evolutionary relationship between nonautosporic and autosporic species of Nannochloris, we constructed a phylogenetic tree of the actin genes using seven strains from six species of Nannochloris, three species of Chlorella, and T. erici. Nannochloris species were polyphyletic in the Trebouxiophyceae group. Two nonautosporic species of N. bacillaris and N. coccoides were monophyletic and positioned distally. Moreover, to determine their phylogenetic position within the Trebouxiophyceae, we constructed phylogenetic tree of 18S rRNA genes adding other species of Trebouxiophyceae. Nannochloris species were polyphyletic in the Trebouxiophyceae and appeared in two different lineages, a Chlorella,Nannochloris group and a Trebouxia,Choricystis group. The nonautosporic species, N. bacillaris and N. coccoides, and three autosporic species of Nannochloris belonged to the Chlorella,Nannochloris group. Nannochloris bacillaris and N. coccoides were also monophyletic and positioned distally in the phylogenetic tree of 18S rRNA genes. These results suggest that autosporulation is the ancestral mode of cell division in Nannochloris and that nonautosporulative mechanisms, such as binary fission and budding, evolved secondarily. Pinto ABSTRACT The paper analyzes the effect of fiscal competition when local governments choose the level of public goods that generate spillover effects elsewhere. For instance, law enforcement activities affect both the crime level in the jurisdiction providing the good and in neighboring communities. The model shows that when local governments rely on capital taxation to finance these expenditures the spillover effects may not lead to an inefficient provision of public goods as predicted by the tax competition literature. In the model, capital is costlessly mobile and offenders relocate responding to differential criminal opportunities and differential local law enforcement efforts. [source] SUCROSE DETECTION AND THE STABILITY OF THE 2-AFC PROCEDURE IN THE PRESENCE OF A CONFOUNDING VARIABLEJOURNAL OF SENSORY STUDIES, Issue 5 2008DANIEL SHEPHERD ABSTRACT This study attempted to measure absolute thresholds for sucrose in aqueous solution for 51 experienced judges. Two experiments utilizing the two-alternative forced-choice (2-AFC) procedure generated 6-point psychometric functions plotting percentage correct as a function of sucrose concentration. In both experiments, the judges were divided into two groups and tested in either purpose-built sensory booths or on open tables situated in a laboratory. In the first experiment, the influence of a confounding variable was apparent, with nonmonotonic psychometric functions being obtained. In experiment II, the confounding variable was eliminated, permitting the estimation of absolute thresholds. In both experiments, there was no main effect of gender or session, though there was an effect of testing locality (P < 0.05). Data are reported to emphasize the importance of controlling extraneous variables and to demonstrate the robustness of the 2-AFC procedure. PRACTICAL APPLICATIONS This research contributes to an otherwise impoverished database on the detection of sucrose in a solution. The uses of the research include estimates of sucrose detection thresholds for comparative purposes; confirmation of the stability of the two-alternative forced-choice procedure; the utility of using formal testing areas as opposed to ad hoc testing stations; and the dangers of utilizing substandard experimental equipment while conducting research of this nature. [source] MUTUAL FUND PORTFOLIO CHOICE IN THE PRESENCE OF DYNAMIC FLOWSMATHEMATICAL FINANCE, Issue 2 2010Julien Hugonnier We analyze the implications of dynamic flows on a mutual fund's portfolio decisions. In our model, myopic investors dynamically allocate capital between a riskless asset and an actively managed fund which charges fraction-of-fund fees. The presence of dynamic flows induces "flow hedging" portfolio distortions on the part of the fund, even though investors are myopic. Our model predicts a positive relationship between a fund's proportional fee rate and its volatility. This is a consequence of higher-fee funds holding more extreme equity positions. Although both the fund portfolio and investors' trading strategies depend on the proportional fee rate, the equilibrium value functions do not. Finally, we show that our results hold even if investors are allowed to directly trade some of the risky securities. [source] CONSTANT PROPORTION PORTFOLIO INSURANCE IN THE PRESENCE OF JUMPS IN ASSET PRICESMATHEMATICAL FINANCE, Issue 3 2009Rama Cont Constant proportion portfolio insurance (CPPI) allows an investor to limit downside risk while retaining some upside potential by maintaining an exposure to risky assets equal to a constant multiple of the cushion, the difference between the current portfolio value and the guaranteed amount. Whereas in diffusion models with continuous trading, this strategy has no downside risk, in real markets this risk is nonnegligible and grows with the multiplier value. We study the behavior of CPPI strategies in models where the price of the underlying portfolio may experience downward jumps. Our framework leads to analytically tractable expressions for the probability of hitting the floor, the expected loss, and the distribution of losses. This allows to measure the gap risk but also leads to a criterion for adjusting the multiplier based on the investor's risk aversion. Finally, we study the problem of hedging the downside risk of a CPPI strategy using options. The results are applied to a jump-diffusion model with parameters estimated from returns series of various assets and indices. If the characteristics of interest of the excluded units differ from those of the rest of the population, the use of naďve estimators may result in highly biased estimates. In this paper, we discuss the use of auxiliary information to reduce the bias by means of calibration and balanced sampling. We show that the use of the available auxiliary information related to both the variable of interest and the probability of being excluded enables us to reduce the potential bias. This paper investigates the importance of instability in models of realised volatility and their corresponding forecasts. Testing for model instability is conducted with a subsampling method. We show that removing structurally unstable data of a short duration has a negligible impact on the accuracy of conditional mean forecasts of volatility. In contrast, it does provide a substantial improvement in a model's forecast density of volatility. In addition, the forecasting performance improves, often dramatically, when we evaluate models on structurally stable data. These gradients describe how the incident radiance field changes with respect to translation on surfaces. Previous techniques for computing gradients ignore the effects of volume scattering and attenuation and assume that radiance is constant along rays connecting surfaces. We present a novel gradient formulation that correctly captures the influence of participating media. Our formulation accurately accounts for changes of occlusion, including the effect of surfaces occluding scattering media. We show how the proposed gradients can be used within an irradiance caching framework to more accurately handle scenes with participating media, providing significant improvements in interpolation quality. [source] Presence of Biventricular Dysfunction in Patients With Type II Diabetes MellitusCONGESTIVE HEART FAILURE, Issue 2 2007Mohammad-Reza Movahed MD Diabetes mellitus (DM) has been found to be associated with depressed left ventricular (LV) function. Right ventricular (RV) function in DM patients, however, has not been well studied. The goal of this study was to evaluate the occurrence of LV and RV dysfunction in patients with DM. A series of 157 patients underwent simultaneous measurement of LV ejection fraction (LVEF) and RV ejection fraction (RVEF). Four of 26 DM patients had RVEF <30% (15.4%) vs 4 of 126 controls (3.2%) (P=.01). Eleven of 27 (40.7%) patients with DM had LVEF <30% vs 9 of 128 controls (7%) (P<.0001). Using multivariate analysis, DM remained independently associated with severely decreased biventricular function (RVEF <30%; odds ratio, 5.7; confidence interval, 1.3,25.4 [P=.02] and LVEF <30%; odds ratio, 12.9; confidence interval, 3.8,43.7 [P<.0001]). These results suggest that diabetic cardiomyopathy involves both ventricles as an independent pathologic process. Nersisyan Abstract The collisionless interaction of an expanding high,energy plasma cloud with a magnetized background plasma in the presence of a dipole magnetic field is examined in the framework of a 2D3V hybrid (kinetic ions and massless fluid electrons) model. The retardation of the plasma cloud and the dynamics of the perturbed electromagnetic fields and the background plasma are studied for high Alfvén,Mach numbers using the particle,in,cellmethod. It is shown that the plasma cloud expands excluding the ambient magnetic field and the background plasma to form a diamagnetic cavity which is accompanied by the generation of a collisionless shock wave. Raghavan Abstract Nonlinear optical (NLO) material of bis (dimethyl sulfoxide) manganese mercury thiocyanate (MMTD) was synthesized by two step reaction method. The solubility and metastable zonewidth were experimentally determined in order to optimize the growth parameters. Bulk crystals of MMTD were grown by slow cooling and slow evaporation methods. The structure of the grown crystal was confirmed by X-ray diffraction analysis. Presence of functional groups and the coordination of Lewis base ligand of dimethyl sulfoxide (DMSO) were confirmed by FT-IR analysis. Optical transparency of the grown crystals was studied by UV-Vis spectroscopy. Nonlinear optical property of the grown crystal was confirmed by Kurtz powder method. Over the last 15 years, a way of thinking has reemerged that strives to reposition students in educational research and reform. Best documented in Australia, Canada, England, and the United States, this way of thinking is premised on the following convictions: that young people have unique perspectives on learning, teaching, and schooling; that their insights warrant not only the attention but also the responses of adults; and that they should be afforded opportunities to actively shape their education. Although these convictions mean different things to different people and take different forms in practice, a single term has emerged to capture a range of activities that strive to reposition students in educational research and reform: "student voice." In this discussion the author explores the emergence of the term "student voice," identifies underlying premises signaled by two particular words associated with the term, "rights" and "respect," and explores the many meanings of a word that surfaces repeatedly across discussions of student voice efforts but refers to a wide range of practices: "listening." The author offers this discussion not as an exhaustive or definitive analysis but rather with the goal of looking across discussions of work that advocates, enacts, and critically analyzes the term "student voice." Bobes Objective:, Aggressive behaviour has been related to schizophrenia both in in-patient and out-patient samples. In this study, we aimed to assess the prevalence and factors associated with aggressive behaviour in out-patient compliant with their prescribed medication. Method:, Eight hundred and ninety-five patients were interviewed at Community-Based Mental Health Services about aggressive,violent behaviour within the week prior to the study visit. Adult patients diagnosed with schizophrenia and receiving stable pharmacological treatment were enrolled. Presence of aggressive episodes, including type of aggression, severity and frequency, was assessed with the Modified Overt Aggression Scale (MOAS). Violence was defined as a score of 3 or more in any of the MOAS subscores. Results:, Prevalence of recent aggressive behaviour was 5.07%, (95% CI 5.04,5.10), where 47% (43 behaviours 91) reached the violent threshold. Among the 91 violent episodes rated, most episodes were verbal (44%), followed by physical violence towards objects (29%), violence towards others (19%) and self-directed violence (8%). Recent episodes of any severity were more likely among patients with a history of violence and also with relapses within the previous year and with low treatment satisfaction. Conclusion:, Five per cent of the studied cases showed aggressive behaviour in the week prior to assessment, despite having been compliant with their medication. Most aggressive behaviour was verbal rather than physical. Lang Abstract It is currently unclear whether retinal ganglion cell (RGC) axon regeneration depends on down-regulation of axon growth-inhibitory proteins, and to what extent outgrowth-promoting substrates contribute to RGC axon regeneration in reptiles. We performed an immunohistochemical study of the regulation of the axon growth-inhibiting extracellular matrix molecules tenascin-R and chondroitin sulphate proteoglycan (CSPG), the axon outgrowth-promoting extracellular matrix proteins fibronectin and laminin, and the axonal tenascin-R receptor protein F3/contactin during RGC axon regeneration in the lizard, Gallotia galloti. Tenascin-R and CSPG were expressed in an extracellular matrix-, oligodendrocyte/myelin- and neuron-associated pattern and up-regulated in the regenerating optic pathway. The expression pattern of tenascin-R was not indicative of a role in channeling or restriction of re-growing RGC axons. Up-regulation of fibronectin, laminin, and F3/contactin occurred in spatiotemporal patterns corresponding to tenascin-R expression. Moreover, we analyzed the influence of substrates containing tenascin-R, fibronectin, and laminin on outgrowth of regenerating lizard RGC axons. In vitro regeneration of RGC axons was not inhibited by tenascin-R, and further improved on mixed substrates containing tenascin-R together with fibronectin or laminin. These results indicate that RGC axon regeneration in Gallotia galloti does not require down-regulation of tenascin-R or CSPG. Presence of tenascin-R is insufficient to prevent RGC axon growth, and concomitant up-regulation of axon growth-promoting molecules like fibronectin and laminin may override the effects of neurite growth inhibitors on RGC axon regeneration. However, it is uncertain how often these features are present in different types of cytologic specimens, and which type of preparation is optimal for this diagnosis. We therefore reviewed the cytologic features of CCA in three types of specimens, including 15 scrape cytology specimens, 7 fine-needle aspiration (FNA) specimens, and 15 peritoneal cytology specimens, with emphasis on the features observed in DQ-stained smears. The cell morphology in scrape cytology specimens and FNA specimens was comparable, whereas in peritoneal cytology specimens, the cytoplasm was better preserved. Most tumor cells had fragile cytoplasm containing variable amounts of fine vacuoles, and round nuclei with distinct or prominent nucleoli. Hyaline stroma was present in 93% of scrape cytology specimens, 71% of FNA specimens, and 80% of peritoneal cytology specimens. Tigroid background was observed in 47% of scrape cytology specimens, 43% of FNA specimens, but in none of the peritoneal cytology specimens. Formation of a tigroid background may be prevented by the abundant fluid content in peritoneal cytology specimens. Hyaline stroma and tigroid background were uncommonly seen in scrape smears from other types of primary ovarian tumors, mainly juvenile granulosa cell tumor and yolk sac tumor. However, the additional presence of papillary structures allows CCA to be readily distinguished from these other tumors. [source] Is the Presence of Mitral Annular Calcification Associated with Poor Left Atrial Function?ECHOCARDIOGRAPHY, Issue 8 2009Vignendra Ariyarajah M.D. Introduction: Mitral annular calcification (MAC) is characterized by calcium and lipid deposition in the annular fibrosa of the mitral valve. MAC is associated with cardiovascular events but little is known of its association with left atrial (LA) function. Methods: We prospectively obtained 12-lead electrocardiograms (ECGs) and transthoracic echocardiograms (TTE) on patients scheduled for nonemergent echocardiographic assessment at a tertiary care hospital. MAC was graded as 0 = none, 1 = mild, 2 = moderate, 3 = severe. LA linear and volume measurements (stroke volume, LA passive emptying fraction, LA active emptying fraction and LA kinetic energy) were done specifically in addition to commonly measured TTE parameters. Results: From the 124 considered for the study, 72 patients remained (aged 68±18 years; 44% male) after excluding those with poor ECG tracings and/or poor TTE images. Eighteen patients had MAC; mild MAC = 14, moderate MAC = 3, severe MAC = 1. When patients with MAC were compared to those without MAC, no significant difference was noted, except for LA linear dimension index (2.1±0.4 vs. 1.9±0.3 cm/m2; P = 0.03). For those with mild and moderate MAC, a trend was noted toward lower LA function with increasing MAC severity. In addition, significant differences were noted between those with and without interatrial conduction delay, where those with such delay had significantly impaired LA stroke volume (9.8±3 vs. 19.93±4 ml; P < 0.0001), LA active emptying fraction (18.83±8 vs. 65.71±9%; P < 0.0001) and LA total/reservoir fraction (39.54±6 vs. 75.1±6%; P < 0.0001). Conclusions: MAC is associated with increase in LA linear dimension on TTE and may be equally represented with lower overall LA function. Further study in a much larger cohort is warranted to delineate these and other potential associations of MAC. [source] Stress-Induced Wall Motion Abnormalities with Low-Dose Dobutamine Infusion Indicate the Presence of Severe Disease and Vulnerable MyocardiumECHOCARDIOGRAPHY, Issue 7 2007Stephen G. Sawada M.D. Background: Patients with left ventricular (LV) systolic dysfunction due to coronary artery disease (CAD) may develop stress-induced wall motion abnormalities (SWMA) with low-dose (10 ,g/kg/min) dobutamine infusion. The clinical significance of low-dose SWMA is unknown. Objective: We investigated the clinical, hemodynamic and angiographic correlates of low-dose SWMA in patients with chronic ischemic LV systolic dysfunction. Methods: Seventy patients with chronic ischemic LV systolic dysfunction who had dobutamine stress echocardiography were studied. Clinical, hemodynamic, and angiographic parameters at rest and low-dose were compared between 38 patients (mean ejection fraction (EF) of 30 ± 8%) with low-dose SWMA and 32 patients (EF 30 ± 11%) without low-dose SWMA. Results: Multivariate analysis showed that the number of coronary territories with severe disease (stenosis ,70%)(P = 0.001, RR = 6.3) was an independent predictor of low-dose SWMA. An increasing number of collateral vessels protected patients from low-dose SWMA (P = 0.011, RR = 0.25). A higher resting heart rate was a negative predictor of low-dose SWMA (P = 0.015, RR = 0.92) but no other hemodynamic variables were predictors. In the patients with low-dose SMA, regions with low-dose SWMA were more likely to be supplied by vessels with severe disease than regions without low-dose SWMA (92% vs 58%, P < 0.001). Conclusion: In patients with ischemic LV systolic dysfunction, the extent of severe disease and a lower numbers of collaterals predict the occurrence of low-dose SWMA. Low-dose SWMA is a highly specific marker for severe disease. [source] |
