Bagwell Abstract Developing a reliable and quantitative assessment of the potential virulence of a malignancy has been a long-standing goal in clinical cytometry. DNA histogram analysis provides valuable information on the cycling activity of a tumor population through S-phase estimates; it also identifies nondiploid populations, a possible indicator of genetic instability and subsequent predisposition to metastasis. Because of conflicting studies in the literature, the clinical relevance of both of these potential prognostic markers has been questioned for the management of breast cancer patients. The purposes of this study are to present a set of 10 adjustments derived from a single large study that optimizes the prognostic strength of both DNA ploidy and S-phase and to test the validity of this approach on two other large multicenter studies. Ten adjustments to both DNA ploidy and S-phase were developed from a single node-negative breast cancer database from Baylor College (n = 961 cases). Seven of the adjustments were used to reclassify histograms into low-risk and high-risk ploidy patterns based on aneuploid fraction and DNA index optimum thresholds resulting in prognostic P values changing from little (P < 0.02) or no significance to P < 0.000005. Other databases from Sweden (n = 210 cases) and France (n = 220 cases) demonstrated similar improvement of DNA ploidy prognostic significance, P < 0.02 to P < 0.0009 and P < 0.12 to P < 0.002, respectively. Three other adjustments were applied to diploid and aneuploid S-phases. These adjustments eliminated a spurious correlation between DNA ploidy and S-phase and enabled them to combine independently into a powerful prognostic model capable of stratifying patients into low, intermediate, and high-risk groups (P < 0.000005). When the Baylor prognostic model was applied to the Sweden and French databases, similar significant patient stratifications were observed (P < 0.0003 and P < 0.00001, respectively). The successful transference of the Baylor prognostic model to other studies suggests that the proposed adjustments may play an important role in standardizing this test and provide valuable prognostic information to those involved in the management of breast cancer patients. [source] Nonlinear determinism in river flow: prediction as a possible indicatorEARTH SURFACE PROCESSES AND LANDFORMS, Issue 7 2007Bellie SivakumarArticle first published online: 6 DEC 200 Abstract Whether or not river flow exhibits nonlinear determinism remains an unresolved question. While studies on the use of nonlinear deterministic methods for modeling and prediction of river flow series are on the rise and the outcomes are encouraging, suspicions and criticisms of such studies continue to exist as well. An important reason for this situation is that the correlation dimension method, used as a nonlinear determinism identification tool in most of those studies, may possess certain limitations when applied to real river flow series, which are always finite and often short and also contaminated with noise (e.g. measurement error). In view of this, the present study addresses the issue of nonlinear determinism in river flow series using prediction as a possible indicator. This is done by (1) reviewing studies that have employed nonlinear deterministic methods (coupling phase-space reconstruction and local approximation techniques) for river flow predictions and (2) identifying nonlinear determinism (or linear stochasticity) based on the level of prediction accuracy in general, and on the prediction accuracy against the phase-space reconstruction parameters in particular (termed as the ,inverse approach'). The results not only provide possible indications to the presence of nonlinear determinism in the river flow series studied, but also support, both qualitatively and quantitatively, the low correlation dimensions reported for such. Therefore, nonlinear deterministic methods are a viable complement to linear stochastic ones for studying river flow dynamics, if sufficient caution is exercised in their applications and in interpreting the outcomes. Bergstrom Abstract Assessment of geographical patterns in fluctuating asymmetry (small, random differences between sides of bilateral characters) among populations shows promise as a tool to resolve the relative biomechanical importance of traits, in addition to being a possible indicator of habitat quality. We used 115 endemic freshwater populations of threespine stickleback (Gasterosteus aculeatus) from Haida Gwaii (Queen Charlotte Islands), British Columbia, Canada, to explore the degree of concordance between geographical variation of asymmetry in a predator defense structure (bony lateral plates) and geographical variation in several indirect measures of predation regime as well as several abiotic habitat variables. We found a geographical cline in the population frequency of lateral plate asymmetries, with reduced asymmetry in the southern clear-water regions of the archipelago characterized by long reaction distances and greater chance of capture by predators, and elevated asymmetry in the northern stained-water regions with poor visibility and low chances of capture. Lateral plate asymmetry was strongly correlated with expression of several defensive armor traits, including total plate numbers among populations, mean cross-sectional diameter of stickleback with the dorsal and pelvic spines erect, and mean degree of overlap between the plates and spine supports. There were no significant correlations between frequency of asymmetric fish and any of our abiotic habitat variables. Stickleback with structural plate asymmetries had fewer trout-induced scars than symmetric fish in the significant majority of populations, and there was a decrease in structural plate asymmetry with age in stained-water habitats, suggesting that trout predators may be selectively removing asymmetric fish in some lakes. This study provides evidence that geographical variation in developmental stability of threespine stickleback, as seen in the frequencies of asymmetry, reflects differences among populations in the importance of structural defenses to fitness rather than differences in habitat quality, and that asymmetry may be a target of selection by predators in wild populations. Quantification of increased T2-weighted MRI signal that is associated with hippocampal sclerosis (HS) can be performed through (1) mean of hippocampal signal in single-echo T2 MRI and (2) hippocampal T2 relaxometry. It is not clear whether these two techniques are equivalent. In this study, we compare the hippocampal signal, detected by single-echo T2 quantification and by T2 relaxometry, in patients with medial temporal lobe epilepsy (MTLE). Methods. We studied magnetic resonance images from 50 MTLE patients and 15 healthy subjects. We compared the quantification of a T2 signal from single echo images to T2 relaxometry, both obtained from a manually traced region of interest (ROI) in coronal slices involving the whole hippocampus. Repeated measures ANOVA was used to evaluate the differences in the distribution of the Z -scores from single-echo T2 quantification and T2 relaxometry within subjects. Results. We observed a significant difference between the measurements obtained from single-echo T2 quantification and T2 relaxometry (P < .001). Measurements from head, body, and tail of the hippocampus were different (P=.04), with a significant interaction between anatomic location and type of measurement used (P= .008). Post hoc paired comparisons revealed that T2 relaxometry yielded greater Z -scores for the body (P= .002) and tail (P < .0001). Conclusions. For each subject with MTLE, T2 relaxometry was able to detect a higher signal in the body and tail of the hippocampus compared to single-echo T2. This is a possible indicator that T2 relaxometry is more sensitive in detecting T2 abnormalities within the body and tail of the hippocampus in patients with MTLE. Abstract Background:, For the detection of allergen-specific IgE in sera, solid-phase IgE-binding assays like the CAP test are commonly used. Although such immunochemical methods are very sensitive, they frequently produce false positives. Degranulation of the human IgE receptor (Fc,RI)-transfected rat mast cell (RBL) lines seems to be a possible indicator for human IgE, but spontaneous mediator release from these cells in the presence of human sera is not negligible. Methods:, The nuclear factor of activated T-cells (NFAT)-responsive luciferase reporter gene was stably transfected into human Fc,RI-expressing RBL-SX38 cells. One established clone (RS-ATL8) was sensitized with 1 : 100 dilution of sera from patients with egg white allergy and then stimulated with purified or a crude extract of egg white allergen. Results:, Sensitization with 15 pg/ml IgE was sufficient to detect IgE crosslinking,induced luciferase expression (EXiLE) by anti-IgE stimulation. Allergen-specific EXiLE was elicited by as little as 1 fg/ml of egg white protein without cytotoxicity. There was a good correlation between results with EXiLE and oral food challenge tests on patients with egg allergy (P = 0.001687, Fisher's exact test). The measured values of EXiLE and the CAP test also correlated well (R = 0.9127, Spearman's test). Conclusion:, The EXiLE test using RS-ATL8 cells is a promising in vitro IgE test to evaluate the biological activity of the binding between IgE and allergens. Methods:, Children aged 10,12 years from two schools in the Singapore Cohort study Of the Risk factors for Myopia (SCORM) were included. Results of a Year 4 standard nation-wide examination were obtained. Cycloplegic autorefraction and A-scan ultrasound biometry measurements were performed in the schools. Results:, The odds ratio for myopia (defined as right eye spherical equivalent at least ,0.5 D) was 2.5 (95% confidence interval 1.4,4.5) for children with average school examination scores in the fourth quartile compared with the first, after adjusting for confounders including reading in books per week and IQ test scores. Similar significant associations were found for English language (p = 0.001) and native language school examination scores (p < 0.001), while the associations with mathematics school examination scores were of borderline significance (p = 0.055). Conclusion:, School grades, a possible indicator of either cumulative engagement in near work activity or intelligence, were positively associated with myopia in Singapore children. Dichter MSW Abstract Objectives:, The purpose of this study was to evaluate the utility of asking female emergency department (ED) patients about police calls for service as a possible indicator of intimate partner violence (IPV). Methods:, Trained research assistants screened female, adult, English-speaking patients presenting to an urban university ED 7am to midnight, 7 days per week during the 2006,2007 academic year. Patients were asked two commonly used IPV screening questions regarding past-year experience with physical violence or threat by an intimate partner and whether or not the police had been called due to a fight between themselves and a male partner. Results:, Of the 4,984 patients screened, 3.9% screened positive for an IPV-related police call in the past 12 months; more than one-third (37.8%) of those screened negative for IPV on the traditional screening questions. The question about an IPV-related police call for service identified an additional 74 cases of possible IPV, representing 1.5% of the overall sample or a 30.8% increase over those identified with the traditional IPV screening questions. Conclusions:, Adding an additional question regarding police calls to standard IPV screening could alert healthcare providers to possible IPV risk. [source] Developing indicators for measuring Research Capacity Development in primary care organizations: a consensus approach using a nominal group techniqueHEALTH & SOCIAL CARE IN THE COMMUNITY, Issue 3 2009Gill Sarre LCST Abstract Research Capacity Development (RCD) in the National Health Service supports the production of evidence for decision-making in policy and practice. This study aimed to establish a level of consensus on a range of indicators to measure research capacity in primary care organizations. Indicators were developed in a two-stage process using workshops and modified nominal group technique. In 2005, workshops were used to generate possible indicators from a wide range of research active and research-interested people. A theoretical framework of six principles of RCD was used to explore and identify indicators. Data were thematically coded, and a 129-item, 9-point Likert scale questionnaire was developed. A purposive sample of nine experts in developing research capacity in primary care agreed to take part in a nominal group in April 2006. The questionnaire was circulated prior to the meeting, and analysis of the responses formed the basis for structured discussion. Participants were then asked to rescore the questionnaire. Only seven participants were able to take part in the discussion and rescore stages. Data were analysed in two ways: level of relevance attributed to each indicator as a measure of organizational RCD, represented by median responses (medians of 7,9 defined strong support, 4,6 indicated moderate support and 1,3 indicated weak support), and level of consensus reached by the group. Consensus was reached if 85% of the group rated an indicator within the same band. Eighty-nine (68%) indicators were ranked as strongly relevant, and for seventy-three of these indicators, a consensus was reached. The study was successful in generating a set of agreed indicators considered relevant for measuring RCD in primary care organizations. These will form the basis of a pilot tool kit to assist primary care organizations to develop research capacity. Further work will explore the applicability of the indicators in practice. [source] Metrics or Peer Review?POLITICAL STUDIES REVIEW, Issue 1 2009Evaluating the 2001 UK Research Assessment Exercise in Political Science Evaluations of research quality in universities are now widely used in the advanced economies. The UK's Research Assessment Exercise (RAE) is the most highly developed of these research evaluations. This article uses the results from the 2001 RAE in political science to assess the utility of citations as a measure of outcome, relative to other possible indicators. The data come from the 4,400 submissions to the RAE political science panel. The 28,128 citations analysed relate not only to journal articles, but to all submitted publications , including authored and edited books and book chapters. The results show that citations are the most important predictor of the RAE outcome, followed by whether or not a department had a representative on the RAE panel. The results highlight the need to develop robust quantitative indicators to evaluate research quality which would obviate the need for a peer evaluation based on a large committee. Bibliometrics should form the main component of such a portfolio of quantitative indicators. [source] Biodiversity recovery during rainforest reforestation as indicated by rapid assessment of epigaeic ants in tropical and subtropical AustraliaAUSTRAL ECOLOGY, Issue 4 2009SCOTT D. PIPER Abstract There is growing interest in the potential for reforestation to assist the recovery of rainforest biodiversity. There is also a need to identify taxonomically tractable groups for use as cost-effective indicators when monitoring the status of biodiversity within reforested sites. Insects are an important component of terrestrial biodiversity but often require considerable resources to sample at species level. Ant genera and generic-based functional groups have been suggested as possible indicators of environmental disturbance. Here we ask to what extent the development of biodiversity is indicated by epigaeic ant genera and functional groups, across different types of reforestation in tropical and subtropical Australia. In each region, we used pitfall traps to sample the ants in replicate sites of: unmanaged regrowth, monoculture and mixed species plantations and ,ecological restoration' plantings, together with reference sites in pasture and rainforest. We recorded 35 epigaeic ant genera (and 4623 individuals) from 50 tropical sites, and 39 genera (and 9904 individuals) from 54 subtropical sites, with 47 genera overall. Community composition of both genera and functional groups differed between pasture and rainforest, although many genera were widespread in both. Reforested sites were intermediate between pasture and rainforest in both regions, and showed a gradient associated with decreasing grass and increasing tree and litter cover. Older monoculture plantations and ecological restoration plantings had the most rainforest-like ant assemblages, and mixed-species cabinet timber plots the least, of the reforested sites. We conclude that ground-active ant genera and functional groups sampled in rapid surveys by pitfall-trapping showed only a modest ability to discriminate among different types of reforestation. Species-level identification, perhaps together with expanded sampling effort, could be more informative, but would require resourcing beyond the scope of rapid assessments. [source] Final height after combined growth hormone and GnRH analogue treatment in adopted girls with early pubertyACTA PAEDIATRICA, Issue 11 2004T Tuvemo Background: Girls adopted from developing countries often have early or precocious puberty, requiring treatment with gonadotrophin-releasing hormone (GnRH) analogues. During such treatment, decreased growth velocity is frequent. Aim: To study whether the addition of growth hormone (GH) to GnRH analogue treatment improves final height in girls with early or precocious puberty. Methods: Forty-six girls with early or precocious puberty (age ± 9.5y) adopted from developing countries were randomized for treatment for 2,4 y with GnRH analogue, or with a combination of GH and GnRH analogue. Results: During treatment, the mean growth velocity in the GH/GnRH analogue group was significantly higher compared to the control group. Combined GH/GnRH analogue treatment resulted in a higher final height: 158.9 cm compared to 155.8 cm in the GnRH analogue-treated group. Three out of 24 girls (13%) in the combined group and nine of the 22 girls (41%) treated with GnRH analogue alone attained a final height below ,2 standard deviation scores (SDS). Conclusion: The difference between the two groups is statistically significant, and possibly of clinical importance. A future challenge is to identify a subgroup with clinically significant advantage of GH addition to GnRH analogue treatment. Being very short on arrival in Sweden and being short and young at start of treatment are possible indicators. [source] |
