LAWSON HANDLEY Abstract We constructed a microsatellite library from four Crocidura russula Y chromosome-specific bacterial artificial chromosome (BAC) clones. Only one of eight microsatellites was male-specific, despite genome walking to obtain more flanking sequence and testing of 93 primer combinations. Potential reasons for this low success are discussed. The male-specific locus, CRY3, was genotyped in 90 males, including C. russula from across the species range and two related species. The large difference in CRY3 allele size between eastern and western lineages supports earlier reports of high divergence between them. Despite polymorphism of CRY3 in Morocco, only one allele was found throughout the whole of Europe, consistent with previous studies that suggest recent colonization of Europe from a small number of Moroccan founders. Ming Chan MD Abstract The primary function of skeletal muscle is to produce force for postural control and movement. Although the contractile properties of the whole muscle are useful functional indicators, they do not accurately reflect the heterogeneity of the constituent motor units (MUs) and their changes in health and disease. However, data on the contractile properties of human MUs, in comparison to other animal species, are relatively sparse. This, in part, is due to greater methodological challenges of in vivo studies of MUs in the human. The purpose of this review is to critically appraise the methods used in humans; to describe the normative data from different muscle groups; to discuss differences between data from healthy humans and other animal species; and, last, to characterize changes of the MU contractile properties in aging, disease, and in response to intervention. Because the spike-triggered averaging technique can only be used to study the twitch properties, other methods were subsequently developed to measure a wider range of contractile properties. Although there is general agreement between human data and those from other animal species, major differences do exist. Potential reasons for these discrepancies include true biological differences, but differences in the techniques used may also be responsible. Although limited, measurement of MU contractile properties in humans has provided insight into the changes associated with aging and motoneuronal diseases and provides a means of gauging their adaptive capacity for training and immobilization. © 2001 John Wiley & Sons, Inc. [source] Congruence of individual cranial bone morphology and neutral molecular affinity patterns in modern humansAMERICAN JOURNAL OF PHYSICAL ANTHROPOLOGY, Issue 2 2009Noreen von Cramon-Taubadel Abstract Recent studies have demonstrated that the shape of the human temporal bone is particularly strongly correlated with neutral genetic expectation, when compared against other cranial regions, such as the vault, face, and basicranium. In turn, this has led to suggestions that the temporal bone is particularly reliable in analyses of primate phylogeny and human population history. While several reasons have been suggested to explain the temporal bone's strong fit with neutral expectation, the temporal bone has never systematically been compared against other individual cranial bones defined using the same biological criteria. Therefore, it is currently unknown whether the shapes of all cranial bones possess reliable information regarding neutral genetic evolution, or whether the temporal bone is unique in this respect. This study tests the hypothesis that the human temporal bone is more congruent with neutral expectation than six other individual cranial bones by correlating population affinity matrices generated using neutral genetic and 3D craniometric data. The results demonstrate that while the temporal bone shows the absolute strongest correlation with neutral genetic data compared with all other bones, it is not statistically differentiated from the sphenoid, frontal, and parietal bones in this regard. Potential reasons for the temporal bone's consistently strong fit with neutral expectation, such as its overall anatomical complexity and/or its contribution to the architecture of the basicranium, are examined. The results suggest that future phylogenetic and taxonomic studies would benefit from considering the shape of the entire cranium minus those regions that deviate most from neutrality. [source] Effects of soy vs. casein protein on body weight and glycemic control in female monkeys and their offspringAMERICAN JOURNAL OF PRIMATOLOGY, Issue 9 2009Janice D. Wagner Abstract Nutritional interventions are important for reducing obesity and related conditions. Soy is a good source of protein and also contains isoflavones that may affect plasma lipids, body weight, and insulin action. Described here are data from a monkey breeding colony in which monkeys were initially fed a standard chow diet that is low fat with protein derived from soy. Monkeys were then randomized to a defined diet with a fat content similar to the typical American diet (TAD) containing either protein derived from soy (TAD soy) or casein,lactalbumin (TAD casein). The colony was followed for over two years to assess body weight, and carbohydrate and lipid measures in adult females (n=19) and their offspring (n=25). Serum isoflavone concentrations were higher with TAD soy than TAD casein, but not as high as when monkey chow was fed. Offspring consuming TAD soy had higher serum isoflavone concentrations than adults consuming TAD soy. Female monkeys consuming TAD soy had better glycemic control, as determined by fructosamine concentrations, but no differences in lipids or body weight compared with those consuming diets with TAD casein. Offspring born to dams consuming TAD soy had similar body weights at birth but over a two-year period weighed significantly less, had significantly lower triglyceride concentrations, and like adult females, had significantly lower fructosamine concentrations compared to TAD casein. Glucose tolerance tests in adult females were not significantly different with diet, but offspring eating TAD soy had increased glucose disappearance with overall lower glucose and insulin responses to the glucose challenge compared with TAD casein. Potential reasons for the additional benefits of TAD soy observed in offspring but not in adults may be related to higher serum isoflavone concentrations in offspring, presence of the diet differences throughout more of their lifespan (including gestation), or different tissue susceptibilities in younger animals. Wiernsperger Abstract The implication of oxidative stress (OS) in diabetes is a major concern for the development of therapeutics aimed at improving the metabolic and/or vascular dysfunctions of this burdening disease. Ample evidence is available suggesting that OS is present in essentially all tissues and can even be observed in prediabetic states. This raises the question of the origin of OS and suggests that, although hyperglycemia is largely linked with free radical production, its role may mainly be the aggravation of a preexisting state. Indeed other factors are also causally linked to OS, such as hormones and lipids. The main debate is about the pertinence of antioxidant therapy since the large scale clinical trials performed recently have essentially failed to show any significant improvement in metabolic or vascular disturbances of diabetic patients. However this conclusion must be tempered by the fact that they have mainly been using vitamin E +/-C; indeed many arguments suggest that either the choice or the application modalities of these substances may have been inadequate. Potential reasons for the actual failure of antioxidant therapy in diabetes are discussed; the indisputable involvement of OS in this disease still leaves hope for alternative therapeutic approaches. Chambers Abstract Objective, Debate exists as to the benefits of performing mucosectomy as part of pouch surgery for ulcerative colitis (UC) and familial adenomatous polyposis (FAP). Whilst mucosectomy results in a more complete removal of diseased mucosa, this benefit may be at the price of poorer function. We examined these issues. Method, Using Medline, Embase, Ovid and Cochrane database searches papers were identified relating to the outcome following pouch surgery with and without mucosectomy. Potential reasons for functional problems were investigated, as were rates of ,cuffitis', dysplasia, polyposis and cancer in the ileal pouch and anal canal. Results, The available evidence suggests that performing a mucosectomy leads to a worse functional outcome. Meta-analysis suggested that nighttime seepage of stool and resting and squeeze pressure were worse after mucosectomy. The most likely reason for functional impairment following pouch surgery was the degree of anal manipulation. Mucosectomy does seem to confer benefit in terms of disease control but this benefit does not reach statistical significance. Conclusion, Stapled anastomosis avoiding mucosectomy is the approach of choice for ileal pouch anal anastomosis because this leads to superior functional outcome. Performing mucosectomy results in some clinical benefits in terms of lower rates of inflammation and dysplasia in the retained mucosa in UC patients and lower rates of cuff polyposis in FAP patients. However, on the basis of available evidence mucosectomy is only indicated in those cases where the patient is at a high risk of disease in the retained rectal cuff. [source] LOW-INCOME HOMEOWNERSHIP: DOES IT NECESSARILY MEAN SACRIFICING NEIGHBORHOOD QUALITY TO BUY A HOME?JOURNAL OF URBAN AFFAIRS, Issue 2 2010ANNA M. SANTIAGO ABSTRACT:,Questions have been raised about the wisdom of low-income homeownership policies for many reasons. One potential reason to be skeptical: low-income homebuyers perhaps may be constrained to purchase homes in disadvantaged neighborhoods. This is a potential problem because home purchases in such neighborhoods: (1) may limit appreciation; (2) may reduce quality of life for adults; and (3) may militate against reputed advantages of homeownership for children. Our study examines the neighborhood conditions of a group of 126 low-income homebuyers who purchased their first home with assistance from the Home Ownership Program (HOP) operated by the Denver Housing Authority. Our approach is distinguished by its use of a comprehensive set of objective and subjective indicators measuring the neighborhood quality of pre-move and post-move neighborhoods. Do low-income homebuyers sacrifice neighborhood quality to buy their homes? Our results suggest that the answer to this question is more complex than it might at first appear. On the one hand, HOP homebuyers purchased in a wide variety of city and suburban neighborhoods. Nonetheless, a variety of neighborhood quality indicators suggest that these neighborhoods, on average, were indeed inferior to those of Denver homeowners overall and to those in the same ethnic group. However, our analyses also revealed that their post-move neighborhoods were superior to the ones they lived in prior to homeownership. Moreover, very few HOP destination neighborhoods evinced severe physical, environmental, infrastructural, or socioeconomic problems, as measured by a wide variety of objective indicators or by the homebuyers' own perceptions. Indeed, only 10% of HOP homebuyers perceived that their new neighborhoods were worse than their prior ones, and only 8% held pessimistic expectations about their new neighborhoods' quality of life. Finally, we found that Black homebuyers fared less well than their Latino counterparts, on average, in both objective and subjective measures. [source] A cognitive aid for neonatal resuscitation: a randomized controlled trialPEDIATRIC ANESTHESIA, Issue 7 2009M.D. Bould Introduction:, Anaesthetists are among several health care practitioners responsible for neonatal resuscitation in Canada. The Neonatal resuscitation program (NRP) courses are the North American educational standard. NRP has been shown to be an effective way of learning skills and knowledge but retention has been found to be problematic [1]. The use of cognitive aids is mandatory in industries such as aviation, to avoid dependence on memory when decision making in critical situations. Visual cognitive aids have been studied retrospectively in resuscitation and performance was found to correlate to the frequency of use of the aid [2]. Cognitive aids have been found to be of benefit in an unblinded prospective study [3]. We aimed to conduct the first blinded study on the effect of a cognitive aid on the performance of simulated resuscitation. Methods:, We conducted a single-blind randomized controlled trial to investigate whether the presence of a cognitive aid improved performance in a simulated neonatal resuscitation. After ethics board approval we recruited 32 anaesthesia residents who had previously passed the NRP. Subjects were randomized to an intervention group that had a poster detailing the NRP algorithm and a control group without the poster. The cognitive aid was positioned so that it could not be seen on the video recordings of the simulation that was used to assess performance. The scenario was piloted to confirm adequate blinding. Both groups had their performance in a simulated neonatal resuscitation recorded and subsequently analyzed by a peer, an expert anaesthetist and an expert neonatologist, using a previously validated checklist. A further rater observed the scenario in real time to examine frequency of use of the cognitive aid. Results:, The inter-rater reliability of the checklist was excellent with an intraclass correlation coefficient of 0.88. Consequently the mean of the scores assigned by all three raters was used for analysis. The median checklist score in the control group 18.2 [15.0,20.5 (10.7,25.3)] was not significantly different from that in the intervention group 20.3 [18.3,21.3 (15.0,24.3)] (P = 0.08). Retention of NRP skills and knowledge of was poor: when evaluated by the neonatologist none of the subjects correctly performed all life-saving interventions necessary to pass the checklist. Although only one subject in the intervention group did not use the aid at all, only 26.7% used the aid frequently and none used it extensively. Discussion:, Retention of skills after NRP training was poor. Our study confirms previous findings of poor retention of skills after NRP training: Kaczorowski et al. investigated family medicine trainees and found that none of 44 residents that were retested 6,8 months after an NRP course would have passed the course due to errors in life-saving interventions [1]. Previous research has shown that the presence of a cognitive aid can improve performance in the simulated management of a rare, high stakes scenario: malignant hyperthermia [3]. Our negative findings contrast with this and another previous study [2]. A potential reason for this discrepancy is that the raters in the previous studies were not blinded to group allocation, nor were the rating scales used validated. The infrequent use of the cognitive aid may be the reason that it did not improve performance in. Further research is required to investigate whether cognitive aids can be useful if their use is incorporated into NRP training. Conclusion:, A randomized single-blinded trial found that a cognitive aid did not improve performance at simulated resuscitation, in contrast to previous retrospective and unblended studies. Retention of skills and knowledge after resuscitation training remains an ongoing challenge for medical educators. [source] In situ Mixing of Organic Matter Decreases Hydraulic Conductivity of Denitrification Walls in Sand AquifersGROUND WATER MONITORING & REMEDIATION, Issue 1 2008Gregory F. Barkle In a previous study, a denitrification wall was constructed in a sand aquifer using sawdust as the carbon substrate. Ground water bypassed around this sawdust wall due to reduced hydraulic conductivity. We investigated potential reasons for this by testing two new walls and conducting laboratory studies. The first wall was constructed by mixing aquifer material in situ without substrate addition to investigate the effects of the construction technique (mixed wall). A second, biochip wall, was constructed using coarse wood chips to determine the effect of size of the particles in the amendment on hydraulic conductivity. The aquifer hydraulic conductivity was 35.4 m/d, while in the mixed wall it was 2.8 m/d and in the biochip wall 3.4 m/d. This indicated that the mixing of the aquifer sands below the water table allowed the particles to re-sort themselves into a matrix with a significantly lower hydraulic conductivity than the process that originally formed the aquifer. The addition of a coarser substrate in the biochip wall significantly increased total porosity and decreased bulk density, but hydraulic conductivity remained low compared to the aquifer. Laboratory cores of aquifer sand mixed under dry and wet conditions mimicked the reduction in hydraulic conductivity observed in the field within the mixed wall. The addition of sawdust to the laboratory cores resulted in a significantly higher hydraulic conductivity when mixed dry compared to cores mixed wet. This reduction in the hydraulic conductivity of the sand/sawdust cores mixed under saturated conditions repeated what occurred in the field in the original sawdust wall. This indicated that laboratory investigations can be a useful tool to highlight potential reductions in field hydraulic conductivities that may occur when differing materials are mixed under field conditions. A cross-sectional survey of Australian firms is used to examine the influence of seven technological and organizational factors on firms' initial interest in ABC and their decision to adopt it or not. The organizational factors of top management support, the support of an internal champion, and organizational size were shown to be associated with initial interest in ABC. The decision to adopt or reject ABC had one organizational factor associated with it, the support of an internal champion. Crow MD Abstract Objective: To review selected issues regarding the development of drug treatments for anorexia nervosa (AN). Method: The existing pharmacotherapy literature for AN is reviewed, and the theoretical and practical considerations are discussed. Results: A very wide variety of drugs have been examined in AN, generally with negative results. There are a number of potential reasons for this finding, including compliance, nutritional deficits, selection of the wrong targets or the wrong outcome measures, use of monotherapy, lack of animal models, or factors intrinsic to AN. Conclusion: Pharmacotherapy provides little benefit in the treatment of AN at present. Brenneman Abstract Giraffe were historically free-ranging across most of sub-Saharan Africa but are now most often confined to national parks, conservation areas, or private ranches. Five viable populations of Rothschild's giraffe (Giraffa camelopardalis rothschildi) remain in protected areas in Kenya and Uganda. The viable population in Uganda is Murchison Falls National Park and the four populations in Kenya are Lake Nakuru National Park (LNNP), Ruma National Park, Giraffe Manor, and Mwea Natural Reserve. The Kenya Wildlife Service queried a rapid decline in LNNP giraffe numbers falling from 153 individuals (1995) to 62 individuals (2002) and the failure of that population to recruit young in those years. Significantly reduced browse options, inbreeding depression and preferential lion predation were considered as potential reasons for this trend. Population genetic parameter estimates derived from multilocus genotype analyses suggest that the LNNP population was in good genetic health with respect to the likelihood of inbreeding depression. The population decline coincided with the drought attributed to the 1994 El Niño. Possible dietary complications from highly concentrated tannin levels because of forced over consumption of the park's declining acacia trees may have compromised young giraffe, making them easy and opportunistic prey for the park's lion population. Smith MPH Abstract Background Evidence regarding the unequal burden of occupational injuries between workers employed by temporary agencies and those in standard employment arrangements is unclear. Studies range from no significant differences in risk to substantial increased risk for temporary workers. The purpose of this study is to compare the workers' compensation experience of a large cohort of temporary agency employed workers with those in standard forms of employment. Methods Washington State Fund workers' compensation data were obtained for claims with injury dates from January 1, 2003 to June 30, 2006, resulting in 342,540 accepted claims. General descriptive statistics, injury rates (per 10,000 FTE), and rate ratios (temp agency/standard employer) were computed by injury type and industry. Results Temporary agency employed workers had higher rates of injury for all injury types, and higher median time loss (40 vs. 27 days) but lower time loss costs (median $1,224 vs. $1,914, P,<,0.001) and lower medical costs ($3,026 vs. $4,087, P,<,0.001) than standard arrangement workers. Temporary agency workers had substantially higher rates for "caught in" and "struck by" injuries in the construction (IRR 4.93; 95% CI 2.80,8.08) and manufacturing (IRR 4.05; 95% CI 3.25, 5.00) industry sectors. Conclusion Temporary agency employed workers have higher claims incidence rates than those in standard employment arrangements. The rate ratios are twofold higher in the construction and manufacturing industry sectors. More research is needed to explore potential reasons for this disparity in occupational injuries. Industry or some measure of job exposure should be included when comparing injury rates in different types of employment in order to better identify areas for prevention. Am. J. Ind. The narrow view asserts that the ideal of public reason must regulate questions of constitutional essentials and matters of basic justice, but should not apply beyond this limited domain. The broad view claims that the ideal of public reason ought to be applied, whenever possible, to all political decisions where citizens exercise coercive power over one another. The paper questions whether there are any good grounds for accepting the narrow view. I survey and reject three potential reasons. The priority argument for the narrow view claims that constitutional essentials and matters of basic justice are the only proper subjects of public reason because they have a special moral priority for our reasoning about justice. The basic interests argument supports the narrow view by arguing that public reasons only exist at the level of constitutional essentials and matters of basic justice. Finally, the completeness argument defends the narrow view on the grounds that public reason can only be complete if it abstains from most legislative questions. I conclude that there are no good reasons for accepting the narrow view of the scope of public reason, whereas there are several reasons to prefer the broad view. [source] Immune activation and inflammation in HIV-1 infection: causes and consequences,THE JOURNAL OF PATHOLOGY, Issue 2 2008V Appay Abstract Thorough research on HIV is progressively enabling us to understand the intricate mechanisms that link HIV-1 infection to the onset of immunodeficiency. The infection and depletion of CD4+ T cells represent the most fundamental events in HIV-1 infection. However, in recent years, the role played by chronic immune activation and inflammation in HIV pathogenesis has become increasingly apparent: quite paradoxically, immune activation levels are directly associated with HIV-1 disease progression. In addition, HIV-1-infected patients present intriguing similarities with individuals of old age: their immune systems are characterized by a loss of regenerative capacity and an accumulation of ageing T cells. In this review, we discuss the potential reasons for the establishment of sustained immune activation and inflammation from the early stages of HIV-1 infection, as well as the long-term consequences of this process on the host immune system and health. A simplified model of HIV pathogenesis is proposed, which links together the three major facets of HIV-1 infection: the massive depletion of CD4+ T cells, the paradoxical immune activation and the exhaustion of regenerative capacity. [source] When Outcomes Prompt Criticism of Procedures: An Analysis of the Rodney King CaseANALYSES OF SOCIAL ISSUES & PUBLIC POLICY, Issue 1 2006Elizabeth Mullen A content analysis of newspaper editorials about the trial of the four officers accused of beating Rodney King investigated when people would become concerned with procedural propriety in the case. Consistent with research demonstrating that people's moral convictions are important determinants of their perceptions of fairness and reactions to outcomes, results revealed that people were more critical of the procedures used in the case after learning the "unjust" verdict than before. Specifically, editorials only mentioned aspects of procedures after the verdict was announced, despite potential reasons for preverdict procedural concern. Editorials also contained more mentions of racism post- than preverdict suggesting that the "unjust" verdict also prompted concerns with institutionalized procedural problems. Beecham Summary Data suggests that the P86L polymorphism (rs2986017) in the calcium homeostasis modulator 1 (CALHM1) gene interferes with CALHM1 functionality, increases A, levels, and is associated with late-onset Alzheimer's disease (LOAD). Previous studies have demonstrated association with P86L and LOAD in three of five case-control cohorts, and a joint analysis of all datasets showed association with a p-value of 2 × 10,10 and an allele-specific odds ratio of 1.44 (2,043 cases, 1,361 controls total). In this short communication we attempt to replicate these results in our case-control cohort (510 cases, 524 controls). [source] Risk factors for breast cancer in East Asian women relative to women in the WestASIA-PACIFIC JOURNAL OF CLINICAL ONCOLOGY, Issue 4 2009Christine S PERRY Abstract The incidence of breast cancer in women of East Asian ancestry (Chinese, Japanese and Korean) is lower than in women of European ancestry but is currently rising. This review explores potential reasons for this inter-ethnic difference in incidence by profiling breast cancer risk factors reported for East Asian and Western women. Factors such as endogenous hormone exposure, lifestyle choices, diet and genetic predisposition are associated with breast cancer risk in both East Asian and Western women. However, the relative exposure to these risk factors may vary according to a woman's geographical ancestry and culture. For example, age at menarche and menopause, parity, breast-feeding history, low fat and high soy consumption as well as the prevalence of high risk genetic alleles may vary with a woman's geographical ancestry and/or culture. Differences in exposure to these risk factors in East Asian and Western women are consistent with the inter-ethnic differences in breast cancer incidence observed. Understanding the underlying factors contributing to differences in the profile of breast cancer across populations is important when considering screening and prevention programs for East Asian women resident in the East or the West. In this process, the structures of inhibitors or other ligands of interest complexed with their macromolecular target are solved and the structural information is used iteratively to design new molecules. The ability to form cocrystal complexes between a target protein and a ligand is essential to this process and therefore is of considerable interest to anyone practicing in this field. In the course of obtaining the necessary ligand,protein crystals, even with crystallization conditions well established for a protein of interest, obtaining co-structures with inhibitors either through cocrystallization or soaking is too often not successful. There are numerous potential reasons for this lack of success and this article outlines a number of possible factors that may be involved and discusses considerations that should be taken into account when designing successful experiments to obtain iterative costructures. FULLER Rare male mating advantage (a form of negative frequency dependence) is frequently proposed as a mechanism for the maintenance of genetic variation within populations. This hypothesis is attractive for systems with pronounced male colour polymorphism because it can maintain particularly high levels of variation. We tested for negative frequency-dependent mating success between yellow and red male colour patterns in bluefin killifish, Lucania goodei. Lucania goodei populations harbour substantial colour pattern polymorphism, and a large proportion of this variation has a genetic basis. We established outdoor mesocosms with red and yellow males in three different ratios: yellow rare (one yellow , : five red ,), even (three yellow , : three red ,), and red rare (five yellow , : one red ,). We obtained eggs and used microsatellites to determine paternity. By contrast to expectations, we found no support for a rare male mating advantage. Red males had slightly higher spawning success than yellow males, particularly in replicates with large clutches and when red males were rare. However, yellow males did not have higher mating success when rare. [source] The experience of EMAS in three European countries: a cultural and competitive analysisBUSINESS STRATEGY AND THE ENVIRONMENT, Issue 1 2002Professor Ulrich Steger EMAS, the European Eco-Management and Audit Scheme, has been open for participation for six years now. Looking at registrations per country, a concentration of registrations in northern European countries is evident. The first part of this article investigates potential reasons by applying two popular models. Geert Hofstede's four cultural dimensions are used to explain favourable or unfavourable conditions for EMAS in a country. Michael E. Porter's national diamond is used to investigate determinants for national competitiveness and their influence on environmental management. These two frameworks are applied to Germany as a benchmark and France and Spain as representatives of southern European countries. In the second part of the article conclusions are drawn from this investigation on the influence of national culture, conditions and incentives for EMAS in these countries. Lastly actions for increased participation in EMAS of southern European countries are presented. This investigation explores potential reasons for these differential findings among female victims. Methods:, This cross-sectional analysis examined data from consecutive female sexual assault victims, at least 12 years old, who agreed to a forensic exam between November 1, 2002, and November 30, 2006. Exams utilized colposcopy, anoscopy, macrodigital imaging, and toluidine blue dye to delineate anogenital injury (AGI), which was defined as the presence of recorded anogenital abrasions, tears, or ecchymosis. Demographic variables of the victim, including sexual experience and reproductive parity, and assault characteristics were recorded in the database for bivariate and multivariate analysis with AGI. Results:, Forty-nine percent of the initial 3,356 patients displayed AGI. Of this total, 2,879 cases included complete data for all variables and were included in the multivariate logistic regression model. A statistically significant increased risk for AGI was noted with: educational status (odds ratio [OR] 1.53, 95% CI = 1.25 to 1.87); vaginal or attempted penetration using penis (OR 2.29, 95% CI = 1.74 to 3.01), finger (OR 1.61, 95% CI = 1.88 to 1.94), or object (OR 3.19, 95% CI = 1.52 to 6.68); anal,penile penetration (OR 2.00, 95% CI = 1.57 to 2.54); alcohol involvement (OR 1.25, 95% CI = 1.04 to 1.50); and virgin status of victim (OR 1.38, 95% CI = 1.11 to 1.71). Victims were less likely to display AGI with a longer postcoital interval (OR 0.50, 95% CI = 0.39 to 0.65) and increased parity (OR 0.76, 95% CI = 0.57 to 0.99). Conclusions:, Approximately half the patients displayed AGI. This rate is higher than earlier studies, but consistent with current investigations utilizing similar injury detection methods. The correlates of injury found reinforce the findings of prior studies, while prompting questions for future study. [source] Simultaneous voiding cystourethrography and voiding urosonography reveals utility of sonographic diagnosis of vesicoureteral reflux in childrenACTA PAEDIATRICA, Issue 12 2003M Nakamura Aim: To evaluate the diagnostic potential of voiding urosonography (VUS) compared with fluoroscopic voiding cystourethrography (VCUG) under identical conditions and to evaluate potential reasons for false-negative VUS results, particularly regarding bladder concentrations of the US contrast agent, Levovist. Methods: Fifty-six paediatric patients (M/F 34/22, mean age 2.3 y, age range 1 mo-14 y) underwent simultaneous VUS and VCUG under identical conditions. The bladder was filled by simultaneous administration of Levovist and the X-ray contrast medium, DIP Conray. Levovist concentrations in bladders were calculated using amounts of Levovist injected and total DIP Conray infused when reflux was first observed in either procedure. Results: Sensitivities of VUS and VCUG for detection of vesicoureteral reflux (VUR) were both 86%, assuming that VUR detected by either method represented a true-positive, and no reflux by either method represented a true-negative. Patients under 24-mo of age displayed a better VUS sensitivity, of 94%. Levovist concentrations in bladders ranged from 1.8% to 23%, with older children tending to demonstrate increased bladder capacity and lower concentration. All VUS false-negative units displayed Levovist bladder concentrations of less than 5%. Conclusion: The present simultaneous study suggests that: 1) the two techniques demonstrate similar sensitivity for detection of reflux; 2) sustained Levovist bladder concentrations of below 5% may not allow detection of reflux on VUS; and 3) VUS represents a suitable technique, particularly for small children whose bladder capacity is not so large.
