Oral health in preschool children with cerebral palsy: a case,control community-based study

International Journal of Paediatric Dentistry 2010; 20: 330,335 Objectives., To assess and compare the oral health status of preschool children with and without cerebral palsy (CP). Methods., Preschool children with CP (72) were recruited from 23 Special Child Care Centers in Hong Kong. An age (±3 months) and gender matched sample of preschool children from mainstream preschools were recruited as the control group. Dental caries status, gingival health status, tooth wear, developmental defect of enamel, malocclusion, dental trauma and oral mucosal health were assessed and compared between the two groups. Results., Significant differences in gingival health status were found between children with and without CP (mean plaque index scores, P = 0.001 and mean gingival index scores, P < 0.05). Tooth wear involving dentine was more prevalent among CP children (P < 0.001), as were evidence of anterior open-bite (P < 0.001) and oral mucosal lesions (P < 0.05). Children with and without CP had similar caries experiences (P > 0.05), prevalence of enamel defects (P > 0.05) and dental trauma (P > 0.05). Conclusions., Differences of oral health status exist among preschool children with and without CP. Preschool children fare worse in terms of gingival health, tooth wear, oral mucosal health and malocclusion. [source]


Investigation of matrix metalloproteinase-1 ,1607 1G/2G polymorphism in a Turkish population with periodontitis

Abstract Aim: Matrix metalloproteinase-1 (MMP-1) is a proteolytic enzyme that degrades extracellular matrix and plays a fundamental role during destruction of periodontal tissues. The aim of this study was to examine the association between MMP-1 ,1607 1G/2G polymorphism and chronic periodontitis susceptibility in a Turkish population. Material and Methods: A total of 180 subjects were enrolled in this study. All the subjects received a periodontal examination including full-mouth clinical attachment loss measurements, probing depths, plaque index scores, gingival index scores and radiographic bone loss ratios. Three groups formed according to periodontal conditions were healthy, moderate periodontitis and severe periodontitis groups. MMP-1 ,1607 1G/2G gene promoter polymorphism was genotyped using a polymerase chain reaction-restriction fragment length polymorphism method. Results: Analysis of the polymorphism showed no differences in distribution of the MMP-1 ,1607 1G/2G polymorphism among healthy, moderate periodontitis and severe periodontitis groups (p>0.05). When the groups were further stratified by smoking status, we found no significant differences in genotype distributions, allele frequencies and carriage rates among any groups either (p>0.05). Conclusions: On the basis of the results, no significant association is found for the MMP-1 ,1607 1G/2G polymorphism with susceptibility to periodontitis. Moreover, smoking status did not seem to affect this result. [source]


The changes in T lymphocyte subsets following periodontal treatment in patients with chronic periodontitis

Objective:, The aim of this study was to determine whether there was any change in T-lymphocyte subsets in patients with chronic periodontitis after applying different periodontal treatment methods. Patients and methods:, Twenty-four patients with chronic periodontitis were included in the study. In every phase of the treatment (pretreatment, initial treatment, curettage and flap operations) the biopsy samples were taken from the gingival tissues at sites of chronic periodontitis. Then CD4+ and CD8+ lymphocyte and CD4+/CD8+ ratio values were determined using flow cytometry in the biopsy samples. At the same time, gingival pocket depth, Löe,Silness gingival index, and Silness,Löe plaque index scores were recorded to assess the periodontal status in patients. To determine the correlation between the clinical measurements and the laboratory results obtained before the treatment, after initial treatment, after curettage and after flap operations, we conducted an analysis using a paired t -test. Results:, Flow cytometry findings in the patients with chronic periodontitis showed that CD4+ and CD8+ lymphocyte values before treatment were under the normal value and the CD4+/CD8+ ratio was within the normal distribution interval. The CD4+/CD8+ ratio decreased postcurettage and postflap operation. This decrease was statistically significant (p < 0.001). The CD4+ and CD8+ lymphocyte values were increased postcurettage and postflap operation. This increase was also statistically significant (p < 0.001). Conclusions:, These findings suggest that local immune response was poor in the patients with chronic periodontitis. CD4+ and CD8+ T-lymphocytes could play a significant role in chronic periodontitis pathobiology. [source]


Short-term clinical effects of commercially available gel containing Acacia arabica: a randomized controlled clinical trial

Abstract Background:, Certain plants used in folk medicine serve as a source of therapeutic agent by having antimicrobial and other multi-potential effects. This prospective, randomized, placebo and positively controlled clinical trial was designed to evaluate the short-term clinical effects of a commercially available gel containing Acacia arabica in the reduction of plaque and gingival inflammation in subjects with gingivitis. Methods:, Ninety subjects diagnosed with chronic generalized gingivitis were selected and randomly divided into three groups: Group I , placebo gel, Group II , gumtone gel and Group III , 1% chlorhexidine gel. Clinical evaluation was undertaken using the gingival index of Loe and Silness and the plaque index at baseline, 2 weeks, 4 weeks and 6 weeks. A subjective evaluation was undertaken by questionnaire. Results:, Gumtone gel showed significant clinical improvement in gingival and plaque index scores as compared to a placebo gel. This improvement was comparable to 1% chlorhexidine gel. Unlike chlorhexidine gel, gumtone gel was not associated with any discolouration of teeth or unpleasant taste. Conclusions:, Gumtone gel may be a useful herbal formulation for chemical plaque control agent and improvement in plaque and gingival status. [source]
