[source] Are stock assessment methods too complicated?FISH AND FISHERIES, Issue 3 2004A J R Cotter Abstract This critical review argues that several methods for the estimation and prediction of numbers-at-age, fishing mortality coefficients F, and recruitment for a stock of fish are too hard to explain to customers (the fishing industry, managers, etc.) and do not pay enough attention to weaknesses in the supporting data, assumptions and theory. The review is linked to North Sea demersal stocks. First, weaknesses in the various types of data used in North Sea assessments are summarized, i.e. total landings, discards, commercial and research vessel abundance indices, age-length keys and natural mortality (M). A list of features that an ideal assessment should have is put forward as a basis for comparing different methods. The importance of independence and weighting when combining different types of data in an assessment is stressed. Assessment methods considered are Virtual Population Analysis, ad hoc tuning, extended survivors analysis (XSA), year-class curves, catch-at-age modelling, and state-space models fitted by Kalman filter or Bayesian methods. Year-class curves (not to be confused with ,catch-curves') are the favoured method because of their applicability to data sets separately, their visual appeal, simple statistical basis, minimal assumptions, the availability of confidence limits, and the ease with which estimates can be combined from different data sets after separate analyses. They do not estimate absolute stock numbers or F but neither do other methods unless M is accurately known, as is seldom true. [source] Gender differences in jail inmates' symptoms of mental illness, treatment history and treatment seeking,CRIMINAL BEHAVIOUR AND MENTAL HEALTH, Issue 3 2009Amy L. Drapalski Background,Rates of mental illness among prisoners are substantial, but little is known about the unique mental health needs of women in jail, those under pre-trial custodial remand or serving short sentences. Aims,To compare male and female jail inmates along a wide range of symptoms of mental illness using identical assessment methods, and to examine gender differences in treatment seeking before and during incarceration. Methods,Soon after incarceration in a county jail, 360 male and 154 female pre-trial and post-trial inmates completed the Personality Assessment Inventory, a wide-ranging measure of psychiatric symptoms. Treatment seeking information was taken from official jail records. Results,Women were more likely to report clinically significant symptoms of anxiety, borderline personality features, somatic concerns and trauma-related symptoms; however, trauma-related symptoms and borderline features were also common among male inmates. Although both men and women reported high rates of drug-related problems, alcohol-related problems were twice as prevalent among male inmates. Female inmates were more likely to seek and be enrolled in jail-based treatment; there were no differences in reported help seeking prior to incarceration. Conclusions,Female jail inmates are especially in need of mental health services. Effective interventions for post-traumatic stress disorder and borderline personality disorder are needed in jail settings for both male and female inmates during incarceration and upon release. [source] Neuropsychology of academic and behavioural limitations in school-age survivors of bacterial meningitisDEVELOPMENTAL MEDICINE & CHILD NEUROLOGY, Issue 11 2004Irene Koomen MD Neuropsychological impairments possibly underlying academic and/or behavioural limitations were studied in 149 school-age survivors of bacterial meningitis, 68 with and 81 without academic and/or behavioural limitations. Academic limitations affected mathematics, reading, and writing. Behavioural limitations were inferred from scores in the clinical range on the Child Behaviour Checklist. These children had been selected from a cohort of 674 children (57% males) who had recovered from non- Haemophilus influenzae type B bacterial meningitis and who had a mean age at infection of 2 years 4 months (range 1mo to 9y 5mo). They had neither,complex onset'meningitis, prior cognitive or behavioural problems, nor severe disease sequelae. They were assessed with standardized assessment methods a mean of 7.8 years (range 4 to 10.4) after meningitis. Children with limitations (32% of the cohort) performed generically poorly on measures of cognitive functioning, speed, and motor steadiness, rather than having impairments in specific neuropsychological domains. The presence of two or more minor neurological signs was more frequent in the group with than in the group without limitations (30% versus 9%); this may explain the relatively poor speed and motor steadiness of the group with limitations. [source] Development and application of a species sensitivity distribution for temperature-induced mortality in the aquatic environmentENVIRONMENTAL TOXICOLOGY & CHEMISTRY, Issue 12 2008Pepijn de Vries Abstract Current European legislation has static water quality objectives for temperature effects, based on the most sensitive species. In the present study a species sensitivity distribution (SSD) for elevated temperatures is developed on the basis of temperature sensitivity data (mortality) of 50 aquatic species. The SSD applies to risk assessment of heat discharges that are localized in space or time. As collected median lethal temperatures (LT50 values) for different species depend on the acclimation temperature, the SSD is also a function of the acclimation temperature. Data from a thermal discharge in The Netherlands are used to show the applicability of the developed SSD in environmental risk assessment. Although restrictions exist in the application of the developed SSD, it is concluded that the SSD approach can be applied to assess the effects of elevated temperature. Application of the concept of SSD to temperature changes allows harmonization of environmental risk assessment for stressors in the aquatic environment. When a synchronization of the assessment methods is achieved, the steps to integration of risks from toxic and nontoxic stressors can be made. [source] Sediment chemical contamination and toxicity associated with a coastal golf course complex,ENVIRONMENTAL TOXICOLOGY & CHEMISTRY, Issue 7 2001Michael A. Lewis Abstract The increasing density of golf courses represents a potential source of sediment contamination to nearby coastal areas, the chemical and biological magnitude of which is almost unknown. The objective of this study was to determine the concentrations of contaminants and toxicities of sediments impacted by a coastal golf course complex. Sediment contaminant concentrations were determined at least twice during the two-year study period at 14 sampling stations. In addition, a combination of acute and chronic bioassays were conducted exposing four invertebrate test species to whole sediments and associated pore waters. Overall, the Florida, USA, golf course complex had a measurable impact on sediment chemical quality, particularly in near-field areas. Higher concentrations of several trace metals and organochlorine pesticides were detected in many golf course-associated sediments compared with reference areas; however, concentrations decreased seaward and only a few, primarily chlorinated pesticides, exceeded proposed sediment quality guidelines. Chromium, zinc, and mercury were detected more frequently than other trace metals. The DDT and associated metabolites, dieldrin and chlordane, were the more commonly detected organic contaminants. Acute toxicity was uncommon and occurred consistently for sediment collected from one coastal location. In contrast, chronic toxicity occurred at several study sites based on the response of Mysidopsis bahia. It was concluded that the impact of golf course runoff on sediment quality may be subtle and sensitive biological assessment methods, such as chronic toxicity tests, will be needed to detect adverse effects. [source] A simulation tool for designing nutrient monitoring programmes for eutrophication assessments,ENVIRONMETRICS, Issue 1 2010Janet Heffernan Abstract This paper describes a simulation tool to aid the design of nutrient monitoring programmes in coastal waters. The tool is developed by using time series of water quality data from a Smart Buoy, an in situ monitoring device. The tool models the seasonality and temporal dependence in the data and then filters out these features to leave a white noise series. New data sets are then simulated by sampling from the white noise series and re-introducing the modelled seasonality and temporal dependence. Simulating many independent realisations allows us to study the performance of different monitoring designs and assessment methods. We illustrate the approach using total oxidised nitrogen (TOxN) and chlorophyll data from Liverpool Bay, U.K. We consider assessments of whether the underlying mean concentrations of these water quality variables are sufficiently low; i.e. below specified assessment concentrations. We show that for TOxN, even when mean concentrations are at background, daily data from a Smart Buoy or multi-annual sampling from a research vessel would be needed to obtain adequate power. Waterhouse Abstract The aim of this work was to develop the first north-east based primary dental care outreach (PDCO) course for clinical dental undergraduate students at Newcastle University. The process of course design will be described and involved review of the existing Bachelor of Dental Surgery (BDS) degree course in relation to previously published learning outcomes. Areas were identified where the existing BDS course did not meet fully these outcomes. This was followed by setting the PDCO course aims and objectives, intended learning outcomes, curriculum and structure. The educational strategy and methods of teaching and learning were subsequently developed together with a strategy for overall quality control of the teaching and learning experience. The newly developed curriculum was aligned with appropriate student assessment methods, including summative, formative and ipsative elements. Härtel A detailed assessment of clinical outcomes after ischemic stroke in childhood is necessary to evaluate prognostic factors. Previous studies are difficult to compare because of differences in test instruments, study design, heterogeneity of cohorts and number of included cases. Depending on neurodevelopmental assessment methods, major and subtle/minor disabilities, especially in infants, may not have been detected. Most outcome studies reveal only limited information about behavioral changes and quality of life in children with ischemic stroke. Thus the assumption that children make a better recovery from stroke than adults due to the immature brain's capacity to reorganize function is not evidence-based. We systematically review the current literature with regard to the neurological and psychosocial development of affected children as well as their quality of life. Implications for future research strategies follow the review to encourage further clinical study of the neurobehavioral trajectory of childhood stroke. We wholeheartedly agree with this goal. In our comment we argue for more specific and ambitious requirements for behavioural personality research. Specifically, we show why behaviour should be observed directly. Moreover, we illustratively describe potentially interesting approaches in behavioural personality research: lens model analyses, the observation of multiple behaviours in diverse experimentally created situations and the observation of behaviour in real life. Insufficiently considered, however, is the clarity of the categories being used for the coding of behaviour. Evidence is reported showing that interjudge agreement for retrospective and even concurrent codings of behaviour does not execeed interjudge agreement for personality traits if the categories being used for the coding of behaviour are not clearly defined. Whilst agreeing with most of his points, I remain unclear as to how behaviour (as specifically defined by Furr) relates to other forms of psychological data (e.g. cognitive task performance). In addition, it is not clear how the functions of behaviour are to be decided: different behaviours may serve the same function; and identical behaviours may serve different functions. On the Difference Between Experience-Sampling Self-Reports and Other Self-Reports WILLIAM FLEESON Department of Psychology, Wake Forest University, Winston-Salem, NC, USA fleesonW@wfu.edu Furr's fair but evaluative consideration of the strengths and weaknesses of behavioural assessment methods is a great service to the field. As part of his consideration, Furr makes a subtle and sophisticated distinction between different self-report methods. It is easy to dismiss all self-reports as poor measures, because some are poor. In contrast, Furr points out that the immediacy of the self-reports of behaviour in experience-sampling make experience-sampling one of the three strongest methods for assessing behaviour. This comment supports his conclusion, by arguing that ESM greatly diminishes one the three major problems afflicting self-reports,lack of knowledge,and because direct observations also suffer from the other two major problems afflicting self-reports. Naturalistic observation with a method such as the EAR can inform researchers' understanding of personality in its relationship to daily behaviour in two important ways. It can help calibrate personality effects against act-frequencies of real-world behaviour and provide ecological, behavioural personality criteria that are independent of self-report. In the selection of a method for assessing behaviour, there should be a careful analysis of the definition of the behaviour and the purpose of assessment. This commentary clarifies and expands upon some points concerning the suitability of experience sampling measures, referred to as Intensive Repeated Measurements in Naturalistic Settings (IRM-NS). IRM-NS measures are particularly useful for constructing measures of differing levels of specificity or generality, for providing individual difference measures which can be associated with multiple layers of contextual variables, and for providing measures capable of reflecting variability and distributional features of behaviour. I provide some historical and theoretical background on the study of behaviour in psychology and biology, from which I conclude that a general definition of behaviour might be out of reach. However, psychological research can gain from adding a functional perspective on behaviour in the tradition of Tinbergens's four questions, which takes long-term outcomes and fitness consequences of behaviours into account. The present commentary instead suggests that personality psychology has more value as an integrative science rather than one that narrowly pursues a behavioural agenda. Cognition, emotion, motivation, the self-concept and the structure of personality are important topics regardless of their possible links to behaviour. Indeed, the ultimate goal of personality psychology is to understanding individual difference functioning broadly considered rather than behaviour narrowly considered. First, I argue that the relevance of behaviour for our field depends on how we define personality. Second, I propose that every taxonomy of behaviour should be grounded in theory. The quality of behavioural data does not only depend on the validity of the measures we use. It also depends on how well behavioural data reflect theoretical assumptions on the causal factors and mechanisms that shape behaviour. Third, I suggest that the quality of personality theories, personality research and behavioural data will profit from ideas about the psychological processes and mechanisms that link personality and behaviour. FUNDER Department of Psychology, University of California, Riverside, CA, USA funder@ucr.edu It is often presumed that objective measures of behaviour (e.g. counts of the number of smiles) are more scientific than more subjective measures of behaviour (e.g. ratings of the degree to which a person behaved in a cheerful manner). We contend that the apparent objectivity of any behavioural measure is illusory. First, the reliability of more subjective measures of behaviour is often strikingly similar to the reliabilities of so-called objective measures. [source] Challenges in developing fish-based ecological assessment methods for large floodplain riversFISHERIES MANAGEMENT & ECOLOGY, Issue 6 2007J. J. DE LEEUW Abstract, Large European floodplain rivers have a great diversity in habitats and fish fauna, but tend to be heavily modified. The complexity of these river systems and their multiple human impacts pose considerable challenges for assessment of their ecological status. This paper discusses: (1) the application of historical information on fish fauna and habitat availability to describe reference conditions; (2) responses of fish assemblages to human disturbance by comparing various rivers and river segments with different impacts and/or time series within rivers; (3) the role of floodplain water bodies in ecological assessment; and (4) monitoring of large rivers using different gears and sampling designs for main channels and floodplain habitats. The challenge for the future is to standardise and calibrate sampling methods and data to enhance the potential for ecological assessment of large rivers. [source] Shift in birch leaf metabolome and carbon allocation during long-term open-field ozone exposureGLOBAL CHANGE BIOLOGY, Issue 5 2007SARI KONTUNEN-SOPPELA Abstract Current and future ozone concentrations have the potential to reduce plant growth and increase carbon demand for defence and repair processes, which may result in reduced carbon sink strength of forest trees in long-term. Still, there is limited understanding regarding the alterations in plant metabolism and variation in ozone tolerance among tree species and genotypes. Therefore, this paper aims to study changes in birch leaf metabolome due to long-term realistic ozone stress and to relate these shifts in the metabolism with growth responses. Two European white birch (Betula pendula Roth) genotypes showing different ozone sensitivity were growing under 1.4,1.7 × ambient ozone in open-field conditions in Central Finland. After seven growing seasons, the trees were analysed for changes in leaf metabolite profiling, based on 339 low molecular weight compounds (including phenolics, polar and lipophilic compounds, and pigments) and related whole-tree growth responses. Genotype caused most of the variance of metabolite concentrations, while ozone concentration was the second principal component explaining the metabolome profiling. The main ozone caused changes included increases in quercetin-phenolic compounds and compounds related to leaf cuticular wax layer, whereas several compounds related to carbohydrate metabolism and function of chloroplast membranes and pigments (such as chlorophyll-related phytol derivatives) were decreasing. Some candidate compounds such as surface wax-related squalene, 1-dotriacontanol, and dotriacontane, providing growth-related tolerance against ozone were demonstrated. This study indicated that current growth-based ozone risk assessment methods are inadequate, because they ignore ecophysiological impacts due to alterations in leaf chemistry. EFRIM BORITZ ABSTRACT In recent years many professional accounting associations have become interested in establishing competency-based professional requirements and assessment methods for certifying accounting professionals. A competency-based approach to qualification specifies expectations in terms of outcomes, or what an individual can accomplish, rather than in terms of an individual's knowledge or capabilities. This idea has an obvious appeal to many practitioners and administrators of professional qualification programs. However, there is limited knowledge about competency-based approaches in the accounting profession and among accounting academics, which is constraining discussion about the value of these approaches and about the strengths and weaknesses of the different competency models that have sprung up in various jurisdictions. In this paper we review and synthesize the literature on competency-based approaches. We identify a number of theoretical benefits of competency-based approaches. However, we also find many alternative definitions and philosophies underlying competency-based approaches, and a variety of visions of how competencies should be determined and assessed. We note that there is limited evidence supporting many competency-based approaches and we identify 14 research questions that could be used to help policy makers to more effectively address policy matters related to competency-based education and assessment. Striegel-Moore Abstract Objective First described over 50 years ago, binge eating disorder (BED) only recently has become the focus of epidemiologic studies. This article provides a comprehensive review of these studies. Method Relevant studies were examined and summarized in the form of a narrative review. Results Similar to the early studies of bulimia nervosa (BN), the first generation of epidemiologic studies of BED is limited in scope or methodology. They focus on prevalence rates and provide only basic demographic characteristics and often use less than optimal sampling or assessment methods. Discussion Results suggest that the demographic profile of BED may be more diverse than that of BN. Aim., This paper is a report of a review of the quantitative evidence for medium to high fidelity simulation using manikins in nursing, in comparison to other educational strategies. Background., Human simulation is an educational process that can replicate clinical practices in a safe environment. Although endorsed in nursing curricula, its effectiveness is largely unknown. Review methods., A systematic review of quantitative studies published between 1999 and January 2009 was undertaken using the following databases: CINAHL Plus, ERIC, Embase, Medline, SCOPUS, ProQuest and ProQuest Dissertation and Theses Database. The primary search terms were ,simulation' and ,human simulation'. Reference lists from relevant papers and the websites of relevant nursing organizations were also searched. The quality of the included studies was appraised using the Critical Appraisal Skills Programme criteria. Results. Twelve studies were included in the review. These used experimental or quasi-experimental designs. All reported simulation as a valid teaching/learning strategy. Six of the studies showed additional gains in knowledge, critical thinking ability, satisfaction or confidence compared with a control group (range 7,11%). The validity and reliability of the studies varied due to differences in design and assessment methods. Conclusion. Medium and/or high fidelity simulation using manikins is an effective teaching and learning method when best practice guidelines are adhered to. Simulation may have some advantage over other teaching methods, depending on the context, topic and method. Further exploration is needed to determine the effect of team size on learning and to develop a universal method of outcome measurement. Aim., This paper is a report of the item selection process and evaluation of the content validity of the Critical-Care Pain Observation Tool for non-verbal critically ill adults. Background., Critically ill patients experience moderate to severe pain in the intensive care unit. While critical care clinicians strive to obtain the patient's self-report of pain, many factors compromise the patient's ability to communicate verbally. Pain assessment methods often need to match the communication capabilities of the patient. In non-verbal patients, observable behavioural and physiological indicators become important indices for pain assessment. Method., A mixed method study design was used for the development of the Critical-Care Pain Observation Tool in 2002,2003. More specifically, a four-step process was undertaken: (1) literature review, (2) review of 52 patients' medical files, (3) focus groups with 48 critical care nurses, and interviews with 12 physicians, and (4) evaluation of content validity with 17 clinicians using a self-administered questionnaire. Results., Item selection was derived from different sources of information which were convergent and complementary in their content. An initial version of the Critical-Care Pain Observation Tool was developed including both behavioural and physiological indicators. Because physiological indicators received more criticism than support, only the four behaviours with content validity indices >0·80 were included in the Critical-Care Pain Observation Tool: facial expression, body movements, muscle tension and compliance with the ventilator. Conclusion., Item selection and expert opinions are relevant aspects of tool development. While further evaluation is planned, the Critical-Care Pain Observation Tool appears as a useful instrument to assess pain in critically ill patients. Competencies and skills for remote and rural maternity care: a review of the literature Aim., This paper reports a review of the literature on skills, competencies and continuing professional development necessary for sustainable remote and rural maternity care. Background., There is a general sense that maternity care providers in rural areas need specific skills and competencies. However, how these differ from generic skills and competencies is often unclear. Methods., Approaches used to access the research studies included a comprehensive search in relevant electronic databases using relevant keywords (e.g. ,remote', ,midwifery', ,obstetrics', ,nurse,midwives', education', ,hospitals', ,skills', ,competencies', etc.). Experts were approached for (un-)published literature, and books and journals known to the authors were also used. Key journals were hand searched and references were followed up. The original search was conducted in 2004 and updated in 2006. Findings., Little published literature exists on professional education, training or continuous professional development in maternity care in remote and rural settings. Although we found a large literature on competency, little was specific to competencies for rural practice or for maternity care. ,Hands-on' skills courses such as Advanced Life Support in Obstetrics and the Neonatal Resuscitation Programme increase confidence in practice, but no published evidence of effectiveness of such courses exists. Conclusion., Educators need to be aware of the barriers facing rural practitioners, and there is potential for increasing distant learning facilitated by videoconferencing or Internet access. They should also consider other assessment methods than portfolios. More research is needed on the levels of skills and competencies required for maternity care professionals practising in remote and rural areas. [source] Dynamics of black spot sea bream (Pagellus bogaraveo) mean length: evaluating the influence of life history parameters, recruitment, size selectivity and exploitation ratesJOURNAL OF APPLIED ICHTHYOLOGY, Issue 3 2006K. Erzini Summary Stochastic simulations were used to evaluate the influence of recruitment pattern (log-normal, decreasing), size selectivity (normal, logistic model) and fishing mortality pattern (abrupt, continuous increase in fishing mortality) on the evolution of mean length and the dispersion of mean length for a relatively long-lived deep-water species, the black spot sea bream (Pagellus bogaraveo). An abrupt increase in fishing mortality resulted in mean size decreasing and stabilizing at a lower level while a steady increase in fishing mortality caused the continuous decrease in mean size that has been reported for many long-lived species. Decrease in mean size was greatest for logistic model simulations and for cases where fish were susceptible to capture at a small size. Logistic selectivity, with decreasing recruitment and increasing fishing mortality over time, resulted in mean length and variability in mean length trends similar to that observed for the Strait of Gibraltar fishery. Furthermore, it was found with the declining recruitment that moderate increases in fishing mortality can result in significant decreases in mean length. Given the importance of mean size as an indicator of the state of a resource, these simulations are a useful alternative or complement to standard fisheries assessment methods, helping to provide information on exploitation patterns and rates that can be used for conservation and management. [source] Acoustic estimation of wildlife abundance: methodology for vocal mammals in forested habitatsAFRICAN JOURNAL OF ECOLOGY, Issue 3 2010Mya E. Thompson Abstract Habitat loss and hunting pressure threaten mammal populations worldwide, generating critical time constraints on trend assessment. This study introduces a new survey method that samples continuously and non-invasively over long time periods, obtaining estimates of abundance from vocalization rates. We present feasibility assessment methods for acoustic surveys and develop equations for estimating population size. As an illustration, we demonstrate the feasibility of acoustic surveys for African forest elephants (Loxodonta africana cyclotis). Visual surveys and vocalizations from a forest clearing in the Central African Republic were used to establish that low-frequency elephant calling rate is a useful index of elephant numbers (linear regression P < 0.001, radj.2 = 0.58). The effective sampling area was 3.22 km2 per acoustic sensor, a dramatic increase in coverage over dung survey transects. These results support the use of acoustic surveys for estimating elephant abundance over large remote areas and in diverse habitats, using a distributed network of acoustic sensors. The abundance estimation methods presented can be applied in surveys of any species for which an acoustic abundance index and detection function have been established. This acoustic survey technique provides an opportunity to improve management and conservation of many acoustically-active taxa whose populations are currently under-monitored. Cette étude présente une nouvelle méthode de recherche qui échantillonne sur de longues périodes de façon constante et non intrusive et qui permet d'obtenir des estimations d'abondance à partir des taux de vocalisation. Nous présentons les méthodes d'évaluation de la faisabilité d'études acoustiques et nous développons des équations pour estimer la taille des populations. Pour illustrer ceci, nous montrons la faisabilité d'études acoustiques chez les éléphants de forêts africains Loxodonta africana cyclotis. Nous avons utilisé des études visuelles et des vocalisations d'une clairière forestière de République Centrafricaine pour établir que le taux d'appel à basse fréquence des éléphants est un indice intéressant du nombre d'éléphants (régression linéaire P < 0,001, radj² = 0,58). La superficie d'échantillonnage effective était de 3,22 km² par senseur acoustique, ce qui est une augmentation spectaculaire par rapport à la couverture des transects d'études par comptage des crottes. To compare 3-dimensional saline infusion sonohysterography (3DSIS) with 2-dimensional sonohysterography (2DSIS) using hysteroscopy and histologic diagnosis as the gold standard. Method. A retrospective analysis of all SIS examination performed between July 1, 2005 and April 30, 2007 in our gynecological sonographic unit. 2DSIS or 3DSIS sonographic techniques were used randomly. Management decisions and operative procedures were done by the referring provider. Patients that had diagnostic hysteroscopy and complete histologic evaluation were included. Concordance between the various assessment methods as well as sensitivity and specificity were compared between 2DSIS and 3DSIS. Student's t test and chi-square test were used for statistical analysis. p < 0.05 was considered statistically significant. Result. Of 804 patients that had SIS, 125 patients met the inclusion criteria. Patient median age was 48 (range 19,82). Also, 77 patients were premenopausal, and 48 patients were postmenopausal. Furthermore, 43 patients had 2DSIS and 82 patients had 3DSIS. 3DSIS was found to correlate with hysteroscopic findings more often than 2DSIS (p < 0.05). A trend for higher specificity of 3DSIS with the different uterine pathologies was seen, but it did not reach statistical significance. No difference in sensitivity of 3DSIS compared with 2DSIS was found for all pathologic diagnoses. Conclusion. 3DSIS correlated better with hysteroscopy than 2DSIS. Specificity for histologic diagnosis appears to be higher with 3DSIS compared with 2DSIS but larger-scale studies are needed to confirm this finding. © 2009 Wiley Periodicals, Inc. Casas DDS Abstract Purpose The purpose of this study was to evaluate the ability of 4 liquid boluses to enhance pixel brightness and the ease with which the boluses could be identified during the sonographic evaluation of oral swallowing in healthy young adults. Methods Ten healthy adult volunteers (5 men and 5 women), ranging in age from 21 to 31 years, underwent sonographic evaluation of the oral phase of swallowing while sitting in their usual feeding position. We compared the ability of the 4 following liquids to improve sonographic visualization of swallowing with that of water: a carbonated cola beverage, 5.0 ml of Thick-It in 120 ml of water, 2.5 ml of Thick-It in 120 ml of water, and 7.5 ml of confectioners' sugar in 120 ml of water. Water was used as a control. In each case, 5 ml of the liquid was introduced into the subject's oral cavity using a syringe, and the subject was instructed to swallow. Digitized still images and recorded video sequences of sonographic examinations of the swallowing were analyzed. The brightness of the bolus image on selected digitized video frames was measured digitally using Image Analyst software. Pixel brightness within selected regions of interest for each of the test liquids was statistically compared with that for water. Seven clinicians rated the visualization of each test liquid and water on paired sonographic videotape sequences. These ratings and the level of agreement between them were statistically tested. Results Only the carbonated cola beverage demonstrated statistically greater pixel brightness than that of water on digitized video frames (p = 0.01), whereas both cola (with a moderate inter-rater agreement, , = 0.50) and 5.0 ml Thick-It mixed with 120 ml of water (with a fair inter-rater agreement, , = 0.24) were significantly better visualized on sonographic video sequences. Conclusions The digital still-frame analysis confirmed the clinicians' ratings of bolus visualization on real-time sonography, but dynamic sonography is more important than still frames in assessing sonographic swallow media because the dynamic images more closely parallel what is seen in clinical practice. Future investigations of sonographic contrast agents for use in the examination of the oral phase of swallowing should use both static digital (still-frame) and dynamic (real-time) assessment methods, as well as expert reviewers. © 2002 John Wiley & Sons, Inc. Stephan Ph.D. Abstract:, The success of facial approximation is thought to depend, at least in part, upon the "accuracy" of the constructed face. However, methods of accuracy assessment are varied and this range in methods may be responsible for the disparate results reported in the literature. The aim of this study was to determine if the accuracy results of one facial approximation were comparable across two different assessment methods (resemblance ratings and simultaneous face array tests using unfamiliar assessors) and if resemblance ratings co-varied with recognition responses. True-positive recognition performance from the facial approximation was poor (21%) while resemblance scores using the same facial approximation were moderately high (3 out of 5 on a five-point scale). These results are not, therefore, consistent and indicate that either different variables are being evaluated by the methods, or the same variable is being examined but with different weight/calibration. Further resemblance ratings tests of the facial approximation to three foil faces from the face array revealed that resemblance scores were similar irrespective of which face was compared, and did not closely correspond with the degree of recognition performance. This was especially the case for isolated comparisons of single faces to the facial approximation. Collectively, these results indicate that resemblance ratings are: (i) insensitive measures of a facial approximation's accuracy; and (ii) inconsistent with results of unfamiliar simultaneous face-array recognition results. These data suggest that familiar and unfamiliar recognition tests should be given increased weight in contrast to current resemblance rating tests. Methods, A systematic literature search was completed and each paper independently reviewed. All health care organizations in Northern Ireland submitted details of their performance assessments. Each was critically appraised using a utility index. Results, Performance was not universally defined. A broad range of assessments were identified, each method had advantages and disadvantages. Although many lacked rigorous testing, areas of good practice were also noted. Conclusions, No single method is appropriate for assessing clinical performance. Rather, this study endorses proposals for a multi-method strategy to ensure that performance assessment demonstrates all attributes required for effective nursing and midwifery practice. Although bruxism is not a life-threatening disorder, it can influence the quality of human life, especially through dental problems, such as tooth wear, frequent fractures of dental restorations and pain in the oro-facial region. Therefore, various clinical methods have been devised to assess bruxism over the last 70 years. This paper reviews the assessment of bruxism, provides information on various assessment methods which are available in clinical situations and discusses their effectiveness and usefulness. Currently, there is no definitive method for assessing bruxism clinically that has reasonable diagnostic and technical validity, affects therapeutic decisions and is cost effective. One future direction is to refine questionnaire items and clinical examination because they are the easiest to apply in everyday practice. Another possible direction is to establish a method that can measure actual bruxism activity directly using a device that can be applied to patients routinely. More clinical studies should examine the clinical impact of bruxism on oral structures, treatment success and the factors influencing the decision-making process in dental treatment. Jones Abstract A transiently supersaturated drug delivery system has the potential to enhance topical drug delivery via heightened thermodynamic activity. The aim of this work was to quantify the degree of saturation (DS) for transiently supersaturated formulations using three traditional and one novel in vitro assessment methods. Metered dose aerosols (MDA) were formulated containing saturated levels of beclomethasone dipropionate monohydrate (BDP) or betamethasone 17-valerate (BMV) within a pressurised canister, and included ethanol (EtOH), hydrofluoroalkane 134a propellant and poly(vinyl pyrrolidone). Attempts to determine the DS via the measurement of drug flux through synthetic membranes did not correlate and was shown to be dependent on the EtOH concentration. The inability of these methods to accurately assess the drug DS may be due to the transient nature of the formulation and the volatile solvents dehydrating the membrane. A mathematical equation that used the evaporation rate of the formulation was derived to determine the theoretical DS at various time points after MDA actuation. Evidence based psychotherapies and pharmacotherapies are available. However, when administered to heterogeneous populations of patients effect sizes are only modest. We present the rationale and design of a double-blind randomized trial comparing acamprosate, naltrexone, and placebo. Additionally we subtype patients on the basis of biological and psychometric measures and explore their treatment response to both acamprosate and naltrexone. According to our initial hypothesis, the "relief drinker/craver" is an endophenotype associated with glutamatergic dysfunction who responds to acamprosate. The "reward drinker/craver" is mainly associated with alterations in the dopaminergic and opioidergic system and responds to naltrexone. Methods:, The study is planned for 430 patients (2:2:1 for both drugs and placebo) over 12 weeks of medication. All receive manualized counselling to improve compliance (Medical Management) which is extended to 6 months. Subtyping is primarily done using the acoustic startle reflex, functional magnetic resonance imaging, positron emission tomography (in a subset of patients), and the Inventory of Drinking Situations. Relapsers will be re-randomized into a second study where additional psychotherapy (Cognitive Behavioral Intervention) is used in a stepped care approach. Genotyping and additional analyses such as health economy are being done as well. The study follows the assessment methods, treatments, and medications used in the U.S. based COMBINE study, which will allow for a direct comparison between this U.S. study trial and a study performed in Europe. Dori Matriculation 2000 was a 5-year project aimed at moving from the nationwide traditional examination system in Israel to a school-based alternative embedded assessment. Encompassing 22 high schools from various communities in the country, the Project aimed at fostering deep understanding, higher-order thinking skills, and students' engagement in learning through alternative teaching and embedded assessment methods. This article describes research conducted during the fifth year of the Project at 2 experimental and 2 control schools. The research objective was to investigate students' learning outcomes in chemistry and biology in the Matriculation 2000 Project. The assumption was that alternative embedded assessment has some effect on students' performance. The experimental students scored significantly higher than their control group peers on low-level assignments and more so on assignments that required higher-order thinking skills. The findings indicate that given adequate support and teachers' consent and collaboration, schools can transfer from nationwide or statewide standardized testing to school-based alter-native embedded assessment. © 2003 Wiley Periodicals, Inc. VAN DEN BERG Summary Sleep duration is an important concept in epidemiological studies. It characterizes a night's sleep or a person's sleep pattern, and is associated with numerous health outcomes. In most large studies, sleep duration is assessed with questionnaires or sleep diaries. As an alternative, actigraphy may be used, as it objectively measures sleep parameters and is feasible in large studies. However, actigraphy and sleep diaries may not measure exactly the same phenomenon. Our study aims to determine disagreement between actigraphic and diary estimates of sleep duration, and to investigate possible determinants of this disagreement. This investigation was embedded in the population-based Rotterdam Study. The study population consisted of 969 community-dwelling participants aged 57,97 years. Participants wore an actigraph and kept a sleep diary for, on average, six consecutive nights. Both measures were used to determine total sleep time (TST). In 34% of the participants, the estimated TST in the sleep diaries deviated more than 1 h from actigraphically measured TST. The level of disagreement between diary and actigraphic measures decreased with subjective and actigraphic measures of sleep quality, and increased with male gender, poor cognitive function and functional disability. Actigraphically measured poor sleep was often accompanied by longer subjective estimates of TST, whereas subjectively poor sleepers tended to report shorter TST in their diaries than was measured with actigraphy. We recommend, whenever possible, to use multiple measures of sleep duration, to perform analyses with both, and to examine the consistency of the results over assessment methods. [source] |
