Selected AbstractsSafety and efficacy of intravitreal triamcinolone for cystoid macular oedema in uveitisCLINICAL & EXPERIMENTAL OPHTHALMOLOGY, Issue 1 2001Stephanie Young FRACO ABSTRACT Purpose: To report the safety and efficacy of intravitreal triamcinolone in the treatment of inflammatory cystoid macular oedema (CMO) in six patients who were resistant to other forms of therapy. Methods: An open-label unmasked prospective non- randomized pilot study of six patients with idiopathic uveitis and visually significant macular oedema, resistant to periocular and/or systemic corticosteroid treatment, was carried out. Baseline examination and investigations were performed, including fundus fluorescein angiography, and the patients were given a single intravitreal injection of triamcinolone (4 mg/0.1 mL). The primary outcome measure was angiographic resolution of CMO. Patients were reviewed at intervals of 2,4 weeks for 12 months. Results: A single intravitreal injection of triamcinolone induced clinical and angiographic resolution of inflammatory macular oedema in all patients for varying periods of time up to 6 months. Five patients experienced increased intraocular pressure to 30 mmHg or greater which required treatment. Two patients developed posterior subcapsular cataract. Conclusion: One injection of intravitreal triamcinolone was an effective short-term treatment for resistant CMO in uveitis. As with steroids given by other routes, raised intraocular pressure and cataract may occur. As it was so effective in these eyes with resistant CMO, a larger study is warranted to evaluate this form of therapy. [source] Long-term follow-up of achalasic patients treated with botulinum toxinDISEASES OF THE ESOPHAGUS, Issue 2 2000D'Onofrio Botulinum toxin A (BoTx), a potent inhibitor of acetylcholine release from nerve endings both within the myenteric plexus and at the nerve,muscle junction, has been shown to decrease the lower esophageal sphincter (LES) pressure in patients with achalasia. Because of this property, the esophageal injection of BoTx has been suggested as an alternative treatment in achalasia. The objective of this study was to determine the long-term efficacy and safety of intrasphincteric injection of BoTx in a group of achalasic patients. Nineteen patients (mean age 56.1 ± 19.2 years) were enrolled in the study. All of them were injected endoscopically with 100 U of BoTx by sclerotherapy needle at different sites of the LES. Symptom score (dysphagia, regurgitation and chest pain, each on a 0,3 scale), esophageal manometer and esophageal radionuclide emptying were assessed before the treatment and at 4 weeks, 3 months and 1 year after BoTx injection. In case of failure or relapse (symptom score >2), the treatment was repeated. All but five patients (74%) were in clinical remission at 1 month. Mean symptom score after 1 month of BoTx decreased from 7.1 ± 0.9 to 2.2 ± 2.5 (p < 0.05). LES pressure decreased from 38.4 ± 13.7 to 27.4 ± 13.5 mmHg (p < 0.05) and 10-min radionuclide retention decreased from 70.9 ± 20.7% to 33.8 ± 27.0% (p < 0.05). Side-effects (transient chest pain) were mild and infrequent. At 12 months, the clinical score was 0.9 ± 0.5 (p < 0.05 vs. basal); mean LES pressure was 22.0 ± 7.1 (p < 0.05 vs. basal) and 10-min radionuclide retention was 15.8 ± 6.0% (p < 0.05 vs. basal). The efficacy of the first injection of BoTx lasted for a mean period of 9 months (range 2,14 months). At the time of writing (follow-up period mean 17.6 months, range 2,31), 14 patients (10 with one injection) were still in remission (74%). Our results showed that one or two intrasphincteric injections of BoTx resulted in clinical and objective improvement in about 74% of achalasic patients and are not associated with serious adverse effects; the efficacy of BoTx treatment was long lasting; this procedure could be considered an attractive treatment, especially in elderly patients who are poor candidates for more invasive procedures. [source] Postnatal neurogenesis in the dentate gyrus of the guinea pigHIPPOCAMPUS, Issue 3 2005Sandra Guidi Abstract In all species examined, the dentate gyrus develops over an extended period that begins during gestation and continues up to adulthood. The aim of this study was to investigate the pattern of postnatal cell production in the dentate gyrus of the guinea pig, a rodent whose brain development has features more closely resembling the human condition than the most commonly used rodents (rat and mouse). Animals of different postnatal (P) ages received one or multiple injections of bromodeoxyuridine (BrdU), and the number of labeled cells in the dentate gyrus was counted after time intervals of 24 h or longer. The total granule cell number and the volume of the granule cell layer were evaluated in Nissl-stained brain sections from P1 and P30 animals. P1,P5 animals were treated with MK-801 to analyze the effect of NMDA receptor blockade on cell proliferation. Cell production occurred at a high rate (9,000,13,000 labeled cells 24 h after one injection) from P1 to P20, with a peak at 3,6 days of age, and then slowly declined from P20 to P30. The production of new cells continued in adult animals, although at a much-reduced rate (400 cells 24 h after one injection). About 20% of the labeled cells survived after a 17-day period and most (60%) of these cells had a neuronal phenotype. The total number of granule cells increased over the first postnatal month; in 30-day-old animals, it was 20% greater than in 1-day-old animals. Administration of MK-801 to P1,P5 animals caused an increase in cell proliferation restricted to the dorsal dentate gyrus. The present data show that, although the guinea pig dentate gyrus develops largely before birth, the production of new neurons continues at a high rate during the first postnatal month, leading to a considerable increase in cell number. This developmental pattern, resembling the human and nonhuman primate condition, may make the guinea pig a useful rodent model in developmental studies on dentate gyrus neurogenesis. © 2004 Wiley-Liss, Inc. [source] Exact mass measurement on an electrospray ionization time-of-flight mass spectrometer: error distribution and selective averagingJOURNAL OF MASS SPECTROMETRY (INCORP BIOLOGICAL MASS SPECTROMETRY), Issue 10 2003Jiejun Wu Abstract An automated, accurate and reliable way of acquiring and processing flow injection data for exact mass measurement using a bench-top electrospray ionization time-of-flight (ESI-TOF) mass spectrometer is described. Using Visual Basic programs, individual scans were selected objectively with restrictions on ion counts per second for both the compound of interest and the mass reference peaks. The selected ,good scans' were then subjected to two different data-processing schemes (,combine-then-center' and ,center-then-average'), and the results were compared at various ion count limit settings. It was found that, in general, the average of mass values from individual scans is more accurate than the centroid mass value of the combined (same) scans. In order to acquire a large number of good scans in one injection (to increase the sampling size for statistically valid averaging), an on-line dilution chamber was added to slow down the typically rapid mass chromatographic peak decay in flow-injection analysis. This simple addition worked well in automation without the need for manual sample dilution. In addition, by dissolving the reference compound directly into the mobile phase, manual syringe filling can be eliminated. Twenty-seven samples were analyzed with the new acquisition and process routines in positive electrospray ionization mode. For the best method found, the percentage of samples with RMS error less than 5 ppm was 100% with repetitive injection data (6 injections per sample), and 95% with single injection data. Afterwards, 31 other test samples were run (with MW ranging from 310 to 3493 Da, 21 samples in ESI+ and 10 in ESI, mode) and processed with similar parameters and 100% of them were mass-calculated to RMS error less than 5 ppm also. [source] Effects of long-term vardenafil treatment on the development of fibrotic plaques in a rat model of Peyronie's diseaseBJU INTERNATIONAL, Issue 3 2006MONICA G. FERRINI OBJECTIVES To determine whether the phosphodiesterase-5 (PDE5) inhibitor, vardenafil, given orally and in different regimens, has a similar effect to that of the PDE5 inhibitor sildenafil, which prevented the development of a Peyronie's disease (PD)-like plaque formation induced by injecting transforming growth factor ,1 (TGF-,1) into the tunica albuginea of the rat. MATERIALS AND METHODS Vardenafil was given to male rats (eight per group) either in the drinking water or as an oral instillation once daily, at ,,1 and ,,3 mg/kg/day for 45 days after one injection with TGF-,1 into the tunica albuginea, as an ,early preventive' treatment for TGF-,1-induced formation of a PD-like plaque. Other groups received the two doses of vardenafil only in the drinking water, starting with a well-formed plaque, for 42 days (,late, therapeutic' administration). Sections of penile tissue were stained histochemically or immunohistochemically, followed by quantitative image analysis for collagen/smooth muscle and collagen III/I ratios, myofibroblast content (,-smooth muscle actin), TGF-,1 expression, and apoptotic index. RESULTS Preventative treatment with vardenafil at the higher dose (both continuous and once-daily treatments) reduced the collagen/smooth muscle and collagen III/I ratios, and the numbers of myofibroblasts and TGF-,1-positive cells, and selectively increased the apoptotic index in the PD-like plaque. The lower dose was less effective, When vardenafil was given continuously in the drinking water for 41 days after the PD-like plaque was formed, there was only a partial reduction of the plaque. CONCLUSIONS Long-term oral treatment with vardenafil slows and reverses the early stages of an experimental PD-like plaque in the rat, and might ameliorate a more advanced plaque. [source] |
