Selected AbstractsEvaluation of risk of falls in patients at a memory impairment outpatient clinicGERIATRICS & GERONTOLOGY INTERNATIONAL, Issue 3 2009Reiko Kikuchi Aim: We investigated the usefulness of the fall-predicting score, a simple screening test to identify patients at high risk of falls in outpatients with cognitive impairment. Methods: This was a 1-year prospective study. Seventy-nine patients (28 men and 51 women, 78.1 ± 5.9 years old) in the Memory Impairment Outpatient Clinic of Kyorin University Hospital. History of falls in the past year, record of falls in the follow-up period (1 year), fall-predicting score, time of standing on one foot, timed Up & Go test, tandem gait, functional reach, grip strength, maximum circumference of the legs and blood laboratory tests were measured. Results: Of the 79 subjects, 38 (48.1%) had experienced falls in the past year, and 29 (36.7%) experienced falls during the follow-up period. Comparing the two groups with and without a history of falls during the follow-up period, a significant difference was observed in fall-predicting score, timed Up & Go test, tandem gait and functional reach. Logistic regression analysis revealed that fall-predicting score was the only significant determinant for predicting future falls. Furthermore, fall-predicting score correlated with timed Up & Go, duration of standing on one foot, functional reach, grip strength and tandem gait. When the ,2 -test was performed to investigate the correlation between individual items of the fall-predicting questions and falls during the follow-up period, "Do you use a stick when you walk?" and "Are there any obstacles in your house?" showed a significant difference (P < 0.05). Conclusion: Fall-predicting score is useful as a screening test to predict future falls in patients with cognitive decline. Hewett Abstract The purpose of this study was to identify gender differences in hip motion and kinetics during a single leg bidirectional deceleration maneuver. The rationale for the development of this maneuver was to test dynamic hip control during the deceleration of three different types of single-leg landings. The hypothesis was that female athletes would display increased hip adduction angles and moments during the maneuver compared to male athletes. Thirty-six collegiate soccer players (19 female, 17 male) volunteered to participate. Subjects were instructed to start the maneuver balancing on one foot, to hop through an agility-speed ladder on the same leg "up two boxes, back one, and then up one and hold it." Hip kinematics and kinetics during all three landings were examined. Females demonstrated significantly greater hip adduction angles at initial contact during all three landings and greater maximal hip adduction during landings 1 and 2 compared to male athletes. Females also exhibited significantly increased external hip adduction moments during landing 1, however, no differences were found between genders during landings 2 and 3. © 2006 Orthopaedic Research Society. Bellone The clinical separation of CMT2 from HSAN I may be difficult in some kindreds in which the sensory and motor symptoms and deficits are approximately alike. The genetic studies of CMT2 families are also controversial: one form of CMT2 was shown to map on chromosome 3q13-q22 and named CMT2B; the HSAN I locus was mapped to 9q22.1. We describe a family with an autosomal dominant inheritance in which at least three members, belonging to three generations, developed a progressive neuropathy that combined limb weakness, wasting, and severe distal sensory loss leading to prominent mutilating changes. The onset was in late childhood, with progressive weakness in the lower limbs and later in the hands, resulting in a severe paralysis in the feet in one patient. Sensory disturbances were pronounced in 2 patients, and led to poorly healing ulcerations with osteomyelitis and amputations in one foot and mutilating lesions of both hands. Electrophysiological investigation revealed an axonopaty with consistent motor damage. Sural nerve biopsy showed a reduction in the density of both myelinated and unmyelinated fibers, with regenerating clusters. Linkage analysis using 5 microsatellite markers within to the critical 9q22 region was performed. Lod scores of this family calculated by LINKAGE package excluded association to this locus. We also performed linkage studies with chr. 3q13-q22 markers associated to the CMT2b locus. Lod scores excluded this locus as well as responsiblity of the familial phenotype. The severity of motor involvement would suggest classifying the disorder of this family as a form of HSMN II rather than HSAN, indicating that a new locus is involved in the pathogenesis of this disorder. [source] Migration and the Tiebout-Tullock Hypothesis RevisitedAMERICAN JOURNAL OF ECONOMICS AND SOCIOLOGY, Issue 2 2009Richard J. Cebula This study investigates, using state-level data for the period 2000,2005, the Tiebout hypothesis (as extended by Tullock) of "voting with one's feet." This analysis differs from previous related studies not only in its adoption of more current migration and other data but also in other ways. First, unlike most earlier related studies, it includes a separate measure of the overall cost of living; second, it examines per pupil (rather than per capita) outlays on public primary and secondary education; and third, in addition to property taxes, it also focuses on per capita state income tax burdens. Inclusion of the last of these variables in the analysis is based on studies that have found the existence of a state income tax to have influenced migration patterns and other studies that have found higher state income tax levels to have resulted in reduced per capita income growth over time. Moreover, including both property tax burdens and income tax burdens broadens the scope of the hypothesis. Strong empirical support for the Tiebout-Tullock hypothesis (as interpreted here) is obtained for the study period. [source] |
