    Contact sensitization from Compositae-containing herbal remedies and cosmetics

    The Compositae (Asteraceae) family of plants is currently an important cause of allergic plant contact dermatitis in Europe. The family comprises some of the oldest and most valued medicinal plants, and the increasing popularity of herbal medicine and cosmetics may theoretically result in a growing number of Compositae sensitizations from these sources. According to the literature at least 15 species, including among others arnica (Arnica montana), German and Roman chamomile (Chamomilla recutita and Chamaemelum nobile), marigold (Calendula officinalis), Echinacea and elecampane (Inula helenium), have been suspected of sensitization or elicitation of Compositae dermatitis. Epidemiological data are available for 2 species only, arnica and German chamomile, the rest of the evidence being anecdotal. Based on this, sensitization seems to occur relatively frequently with a few species such as arnica and elecampane, and occurs rarely with the majority, especially the widely used German chamomile. Sesquiterpene lactones are the most important allergens, but there are a few cases of sensitization from a coumarin, a sesquiterpene alcohol and a thiophene. The risk of elicitation of dermatitis by using Compositae-containing products in Compositae-sensitive individuals is by-and-large unknown. [source]


    Orofacial trauma and rugby in France: epidemiological survey

    Abstract ,,,A stratified epidemiological survey was carried out among the best French rugby players to assess the prevalence of trauma to the lower or middle part of the face (TLMPF) and the frequency at which the mouthguards (MGs) were worn. The 1140 randomized players (elite 1, elite 2 and national 1 clubs) filled in the same form anonymously. The variables were first subjected to univariate analysis (Chi-square, anova). Secondly, they were included in a multivariate model (logistic regression). Some 29.57% of players had already been affected by a TLMPF. The risk increased for the oldest forward players. It also increased with the number of yearly competitions and number of hours of weekly training. Some 64.3% of players used a MG. The frequency at which it was worn increased with the number of yearly competitions, for those who had experinced a previous trauma and for the pack players who had been playing for a long time.     ABSTRACT To date, bioethics and health policy scholarship has given little consideration to questions of aging and intergenerational justice in the developing world. Demographic changes are precipitating rapid population aging in developing nations, however, and ethical issues regarding older people's claim to scarce healthcare resources must be addressed. This paper posits that the traditional arguments about generational justice and age-based rationing of healthcare resources, which were developed primarily in more industrialized nations, fail to adequately address the unique challenges facing older persons in developing nations. Existing philosophical approaches to age-based resource allocation underemphasize the importance of older persons for developing countries and fail to adequately consider the rights and interests of older persons in these settings. Ultimately, the paper concludes that the most appropriate framework for thinking about generational justice in developing nations is a rights-based approach that allows for the interests of all age groups, including the oldest, to be considered in the determination of health resource allocation.     The Bangladesh Health SWAp: Experience of a New Aid Instrument in Practice

    Sector-wide approaches are being widely adopted as a new aid modality, incorporating government ownership, partnership and a move from project to programme support. The literature to date on their performance in practice is, at best, mixed. This article reviews these issues in the light of the experience of arguably the world's oldest and largest SWAp, the Bangladesh health sector programme. A positive picture emerges of an evolutionary institutional adaptation towards a programme approach, with positive systemic effects on government processes and a reduction in transaction costs in dealing with donors. There are, however, negative aspects, notably, donor dominance in ,dialogue', though with limited influence on the government's actual strategy. [source]


    Enameloid microstructure in the oldest known chondrichthyan teeth

    Abstract Previous studies on tooth enameloid microstructure in several chondrichthyan taxa spanning the phylogeny of the group provided support for the homology of chondrichthyan tooth enameloid. This hypothesis requires that a single crystallite enameloid (SCE) monolayer must be present in the teeth of the most primitive chondrichthyan. However, the dental microstructure of the earliest sharks has yet to be investigated. We have studied the tooth enameloid microstructure of the two oldest tooth-bearing shark species currently known, Leonodus carlsi Mader (1986) and Celtiberina maderi Wang (1993), from the lowermost Lochkovian (Lower Devonian) of Spain. Our study demonstrates the presence of a SCE monolayer in the teeth of both species. These results show that a superficial cap of SCE is present in the oldest shark teeth known, which suggest its presence in the most basal chondrichthyans. [source]


    Late Devonian tetrapod remains from Red Hill, Pennsylvania, USA: how much diversity?

    Abstract The remains of Late Devonian tetrapods from the Red Hill locality in Pennsylvania help to elucidate the early stages of tetrapod evolution. Red Hill is a particularly informative site that preserves a diverse fauna and flora within a depositional setting suggesting penecontemporaneous deposition of locally derived material. Here, for the first time, we report on the full suite of early tetrapod remains from Red Hill and consider the implications for tetrapod diversity within the Red Hill ecosystem. Previously described material is reviewed and considered in relation to newly reported specimens. New material described includes isolated skull elements (two jugals, a postorbital, a lacrimal and a coronoid) and postcranial elements (a femur and a gastral scale). The characteristics of many of the Red Hill tetrapod specimens conform to the morphological expectations of Late Devonian forms. Several elements, however, illustrate more derived characteristics and strongly suggest the presence of the oldest known whatcheeriid-like tetrapod. This study demonstrates the difficulty in making taxonomic associations with isolated remains, even when found in close proximity to one another. Exploration of the characteristics of each element, however, demonstrates the presence of at least three early tetrapod taxa at the Red Hill site. [source]


    Dating young geomorphic surfaces using age of colonizing Douglas fir in southwestern Washington and northwestern Oregon, USA,

    Abstract Dating of dynamic, young (<500 years) geomorphic landforms, particularly volcanofluvial features, requires higher precision than is possible with radiocarbon dating. Minimum ages of recently created landforms have long been obtained from tree-ring ages of the oldest trees growing on new surfaces. But to estimate the year of landform creation requires that two time corrections be added to tree ages obtained from increment cores: (1) the time interval between stabilization of the new landform surface and germination of the sampled trees (germination lag time or GLT); and (2) the interval between seedling germination and growth to sampling height, if the trees are not cored at ground level. The sum of these two time intervals is the colonization time gap (CTG). Such time corrections have been needed for more precise dating of terraces and floodplains in lowland river valleys in the Cascade Range, where significant eruption-induced lateral shifting and vertical aggradation of channels can occur over years to decades, and where timing of such geomorphic changes can be critical to emergency planning. Earliest colonizing Douglas fir (Pseudotsuga menziesii) were sampled for tree-ring dating at eight sites on lowland (<750 m a.s.l.), recently formed surfaces of known age near three Cascade volcanoes , Mount Rainier, Mount St. Helens and Mount Hood , in southwestern Washington and northwestern Oregon. Increment cores or stem sections were taken at breast height and, where possible, at ground level from the largest, oldest-looking trees at each study site. At least ten trees were sampled at each site unless the total of early colonizers was less. Results indicate that a correction of four years should be used for GLT and 10 years for CTG if the single largest (and presumed oldest) Douglas fir growing on a surface of unknown age is sampled. This approach would have a potential error of up to 20 years. Error can be reduced by sampling the five largest Douglas fir instead of the single largest. A GLT correction of 5 years should be added to the mean ring-count age of the five largest trees growing on the surface being dated, if the trees are cored at ground level. This correction would have an approximate error of ±5 years. If the trees are cored at about 1·4 m above the ground surface (breast height), a CTG correction of 11 years should be added to the mean age of the five sampled trees (with an error of about ±7 years).     Separating the influence of resource ,availability' from resource ,imbalance' on productivity,diversity relationships

    Abstract One of the oldest and richest questions in biology is that of how species diversity is related to the availability of resources that limit the productivity of ecosystems. Researchers from a variety of disciplines have pursued this question from at least three different theoretical perspectives. Species energy theory has argued that the summed quantities of all resources influence species richness by controlling population sizes and the probability of stochastic extinction. Resource ratio theory has argued that the imbalance in the supply of two or more resources, relative to the stoichiometric needs of the competitors, can dictate the strength of competition and, in turn, the diversity of coexisting species. In contrast to these, the field of Biodiversity and Ecosystem Functioning has argued that species diversity acts as an independent variable that controls how efficiently limited resources are utilized and converted into new tissue. Here we propose that all three of these fields give necessary, but not sufficient, conditions to explain productivity,diversity relationships (PDR) in nature. However, when taken collectively, these three paradigms suggest that PDR can be explained by interactions among four distinct, non-interchangeable variables: (i) the overall quantity of limiting resources, (ii) the stoichiometric ratios of different limiting resources, (iii) the summed biomass produced by a group of potential competitors and (iv) the richness of co-occurring species in a local competitive community. We detail a new multivariate hypothesis that outlines one way in which these four variables are directly and indirectly related to one another. We show how the predictions of this model can be fit to patterns of covariation relating the richness and biomass of lake phytoplankton to three biologically essential resources (N, P and light) in a large number of Norwegian lakes.     Temporal dynamics within a contemporary latitudinal diversity gradient

    Abstract Poleward declines in species diversity [latitudinal diversity gradients (LDG)] remain among the oldest and most widespread of macroecological patterns. However, their contemporary dynamics remain largely unexplored even though changing ecological conditions, including global change, may modify LDG and their respective ecosystems. Here, we examine temporal variation within a temperate Northwest Atlantic LDG using 31 years of annual fisheries-independent surveys and explore its dynamics in relation to a dominant climate signal [the wintertime North Atlantic Oscillation (NAO)] that varies interannually and alters the latitudinal gradient of Northwest Atlantic continental shelf bottom water temperatures. We found that the slopes of the annual LDG vary dramatically due to changes in geographic distributions of 100+ species, variations that are concealed within the cumulative, static LDG. These changes are strongly associated with changes in NAO sign and strength. This is the first illustration of temporal dynamics in a contemporary LDG and the first demonstration of the speed at which local environmental variations can alter an LDG. Our findings underscore the need to investigate factors that modify LDG separately from those that contribute to their origins. [source]


    Human impacts on the species,area relationship in reef fish assemblages

    Abstract The relationship between species richness and area is one of the oldest, most recognized patterns in ecology. Here we provide empirical evidence for strong impacts of fisheries exploitation on the slope of the species,area relationship (SAR). Using comparative field surveys of fish on protected and exploited reefs in three oceans and the Mediterranean Sea, we show that exploitation consistently depresses the slope of the SAR for both power-law and exponential models. The magnitude of change appears to be proportional to fishing intensity. Results are independent of taxonomic resolution and robust across coral and rocky reefs, sampling protocols and statistical methods. Changes in species richness, relative abundance and patch occupancy all appear to contribute to this pattern. We conclude that exploitation pressure impacts the fundamental scaling of biodiversity as well as the species richness and spatial distribution patterns of reef fish. We propose that species,area curves can be sensitive indicators of community-level changes in biodiversity, and may be useful in quantifying the human imprint on reef biodiversity, and potentially elsewhere.     Interstrand crosslink inducing agents in pretransplant conditioning therapy for hematologic malignancies

    Abstract Despite successful molecularly targeted, highly specific, therapies for hematologic malignancies, the DNA interstrand crosslinking agents, which are among the oldest and least specific cytotoxic drugs, still have an important role. This is particularly true in stem cell transplantation, where virtually every patient receives conditioning therapy with a DNA-alkylating agent-based program. However, due to concern about serious additive toxicities with combinations of different alkylating drugs, the last several years have seen nucleoside analogs, whose cytotoxic action follows vastly different molecular pathways, introduced in combination with alkylating agents. The mechanistic differences paired with different metabolic pathways for the respective drugs have clinically translated into increased safety without appreciable loss of antileukemic activity. In this report, we review pre-clinical evidence for synergistic antileukemic activity when nucleoside analog(s) and DNA-alkylating agent(s) are combined in the most appropriate manner(s), without a measurable decrease in clinical efficacy compared with the more established alkylating agent combinations. Data from our own laboratory using combinations of fludarabine, clofarabine, and busulfan as prototype representatives for these respective classes of cytotoxic agents are combined with information from other investigators to explain how the observed molecular events will result in greatly enhanced synergistic cytotoxicity. We further present possible mechanistic pathways for such desirable cytotoxic synergism. Finally, we propose how this information-backed hypothesis can be incorporated in the design of the next generation conditioning therapy programs in stem cell transplantation to optimize antileukemic efficacy while still safeguarding patient safety.     Psychoses of epilepsy in Babylon: The oldest account of the disorder

    Summary We have previously published translations of Babylonian texts on epilepsy and stroke, which we believe to be the oldest detailed accounts of these neurological disorders from the second millennium BC. We now present a short Babylonian text, which clearly describes what are today known as interictal or schizophrenia-like psychoses of epilepsy. The text includes many of the classical symptoms of the syndrome, for example, paranoid delusions, hallucinations and mood disorders, as well as religiosity and hyposexuality, which have only been crystallized in the twentieth century. The Babylonians were remarkably good observers of human disease and behavior but had little or no understanding of pathology or brain function. Although they recognized many natural causes of disease, epilepsy and behavior disorders were attributed to supernatural, usually evil forces, the forerunner of the Greek concept of the Sacred Disease.     Why are Male Columbian Ground Squirrels Territorial?

    Male territorial defence is a component of many vertebrate mating systems and is often regarded as a tactic for acquiring mates. Traditionally considered within the context of overt site-specific defence, territoriality actually may have several components which encompass a variety of behavioural tactics (e.g. post-copulatory mate-guarding, defence of resources that females need, defence of area around females) that underlie a mating system. The purpose of our study was to evaluate such influences on the territorial behaviour of male Columbian ground squirrels in southwestern Alberta, Canada. Males were dominant and territorial if they defended a minimum convex polygon activity range by chasing other males more within the activity range than they were chased. Subordinate males had no territory and were chased throughout their ranges, but they competed for mates by increasing chases in their activity range when nearby females were oestrous. Dominant males exhibited conditional breeding tactics, tending to chase other dominant males from their territory when nearby females were oestrous, but travelling outside their activity ranges to chase subordinate males when females were not oestrous. Although females mated first with a dominant male on whose territory they resided (and in order from oldest to youngest if several territories overlapped), mating pairs were not exclusive, as females usually mated with additional males. Males also guarded females after copulation and defended females directly just before oestrus, rather than defending territory per se during those times. Thus, males possess a repertoire of behaviours that complement site-specific territoriality, and territory ownership serves to facilitate a first mating with females that live on the territory.     Haemoglobin and anaemia in a gender perspective: The Tromsø Study,

    Abstract:,Objectives:,To examine the gender-specific distribution of haemoglobin (Hb) and the World Health Organization (WHO) criteria for anaemia compared with the 2.5 percentile for Hb. Methods:,A population-based study from Tromsø, Northern Norway. All inhabitants above 24 yr were invited. In total, 26 530 (75%) had their Hb analysed. Results:,The 2.5,97.5 percentile of Hb was 129,166 and 114,152 g/L for all men and women, respectively. In men, mean Hb decreased from 148 to 137 g/L between 55,64 and 85+ yr. In women, mean Hb increased from 132 to 137 g/L between 35,44 and 65,74 yr and then decreased to 131 g/L among the oldest. Using the WHO criteria for anaemia (Hb: <130 and <120 g/L, men and women respectively), the prevalence of anaemia in men increased with age from 0.6% aged 25,34 to 29.6% aged 85+. For women, the prevalence of anaemia varied from 9.1%, 2.2% and 16.5% in the age groups of 35,44, 55,64 and 85+ yr, respectively. The WHO criteria gave a two to three times higher prevalence of anaemia compared with the 2.5 percentile of Hb in women, but the difference was small in men. Poor self-rated health was not associated with low values of Hb in women. In men, there was an association in some age groups. Conclusion:,The WHO criteria for anaemia and the 2.5 percentile for Hb corresponded well for men, but not for women. The WHO criteria of anaemia may result in medicalization of healthy women.     The geoarchaeological and paleoenvironmental context of Paleoindian sites in western Middle Park, Colorado, USA

    Geoarchaeological investigations in western Middle Park provide important information for understanding the soil-stratigraphic context of Paleoindian components, as well as the latest Quaternary environmental change and landscape evolution in a Southern Rocky Mountain intermontane basin. Paleoindian components are associated with the oldest two of four latest Quaternary stratigraphic units (1,4) recognized in co-alluvial mantles (combined slopewash and colluvium) in uplands and in alluvial valley fills. Limited data suggest accumulation of unit 1 as early as ,12,500 14C yr B.P. in alluvial valleys and by at least ,11,000 14C yr B.P. in uplands was followed by brief stability and soil formation. A relatively widespread disconformity marks earliest Holocene erosion and substantial removal of latest Pleistocene deposits in upland and alluvial settings followed by unit 2 deposition ,10,000,9000 14C yr B.P., perhaps signaling the abrupt onset of an intensified summer monsoon. In situ Paleoindian components in uplands are found in a moderately developed buried soil (the Kremmling soil) formed in units 1 and 2 in thin (,1m) hillslope co-alluvial mantles. The Kremmling soil reflects geomorphic stability in upland and alluvial settings ,9000,4500 14C yr BP, and represents a buried landscape with the potential to contain additional Paleoindian components, although elsewhere in western Middle Park Early Archaic components are documented in morphologically similar soils. Kremmling soil morphology, the relative abundance of charcoal in unit 2 relative to younger units, and charcoal morphology indicate the expansion of forest cover, including Pinus, and grass cover during the early and middle Holocene, suggesting conditions moister than present. © 2010 Wiley Periodicals, Inc.     Transition from arc- to post-collision extensional setting revealed by K,Ar dating and petrology: an example from the granitoids of the Eastern Pontide Igneous Terrane, Arakl,-Trabzon, NE Turkey

    Abstract The Eastern Pontide Igneous Terrane (EPIT) includes several Cretaceous to Neogene intrusive rocks, ranging in composition from low-K tholeiitic gabbros through calc-alkaline and high-K calc-alkaline metaluminous granitoids or peraluminous leucogranites to alkaline syenites. Such high diversity in age and composition is also accompanied by a broad spectrum in terms of geodynamics,i.e. from arc through syn-collisional thickening to post-collisional extensional regimes. Shallow-seated porphyritic acidic to intermediate rocks are from oldest to youngest, on the basis of field relations, the Gündo,du altered microgranite, the Bo,al, K-feldspar-megacrystic monzogranite and the Uzuntarla porphyritic granodiorite. These rocks, exposed in the southern part of the Arakl, region, east of Trabzon, Turkey, were studied in terms of their mineralogy and petrography, whole-rock geochemistry and hornblende K,Ar dating. The mineralogical and geochemical data reveal an apparent diversity in incompatible-element enrichment and depletion, for the Bo,al, unit and Uzuntarla unit, respectively. The Bo,al, and Uzuntarla units yield hornblende K,Ar ages ranging from 75.7,±,1.55 to 61.4,±,1.47,Ma and from 42.4,±,0.87 to 41.2,±,0.89,Ma, respectively. The diversity in both mineralogy,geochemistry and hornblende K,Ar ages suggests that the Bo,al, and Uzuntarla units are parts of the Cretaceous arc and Eocene post-collision extensional-related igneous activity, respectively, in the EPIT of northern Turkey.     Can the tropical conservatism hypothesis explain temperate species richness patterns?

    An inverse latitudinal biodiversity gradient in the New World snake tribe Lampropeltini
    ABSTRACT Aim, A latitudinal gradient in species richness, defined as a decrease in biodiversity away from the equator, is one of the oldest known patterns in ecology and evolutionary biology. However, there are also many known cases of increasing poleward diversity, forming inverse latitudinal biodiversity gradients. As only three processes (speciation, extinction and dispersal) can directly affect species richness in areas, similar factors may be responsible for both classical (high tropical diversity) and inverse (high temperate diversity) gradients. Thus, a modified explanation for differential species richness which accounts for both patterns would be preferable to one which only explains high tropical biodiversity. Location, The New World. Methods, We test several proposed ecological, temporal, evolutionary and spatial explanations for latitudinal diversity gradients in the New World snake tribe Lampropeltini, which exhibits its highest biodiversity in temperate regions. Results, We find that an extratropical peak in species richness is not explained by latitudinal variation in diversification rate, the mid-domain effect, or Rapoport's rule. Rather, earlier colonization and longer duration in the temperate zones allowing more time for speciation to increase biodiversity, phylogenetic niche conservatism limiting tropical dispersal and the expansion of the temperate zones in the Tertiary better explain inverse diversity gradients in this group. Main conclusions, Our conclusions are the inverse of the predictions made by the tropical conservatism hypothesis to explain higher biodiversity near the equator. Therefore, we suggest that the processes invoked are not intrinsic to the tropics but are dependent on historical biogeography to determine the distribution of species richness, which we refer to as the ,biogeographical conservatism hypothesis'.     Understanding Sierra Leone in Colonial West Africa: A Synoptic Socio-Political History

    Sierra Leone played a pivotal role in the success of the British colonial project in West Africa in the 19th century. In 1866, it served as the administrative headquarters for the colonies of Lagos, the Gold Coast now Ghana and Bathurst Gambia. Professionals, clergymen, and intellectuals from Sierra Leone, particularly Creoles, served as civil servants, engineers, and medical doctors in Nigeria, Ghana, and the Gambia in the 18th and 19th centuries. Sierra Leone also educated one of the first crops of Western-educated elites in West Africa. In short, despite experiencing a devastating civil war, evidently Sierra Leone is gradually regaining its quintessential role as one of the oldest, peaceful, and democratic states in West Africa.     Slavery and the Founding

    One of the oldest but hottest debates over the U.S. Constitution has concerned its relationship to slavery, a dispute that goes right to the heart of the meaning of the founding of the republic. Some observers have portrayed slavery as a , even the , vital problem of the Founding, and denounced the Constitution as a proslavery document. Other scholars contend that this picture unduly elevates the importance of slavery in the contest over the Constitution and distorts the intent of the Founders. This essay reviews recent entries in this debate to frame its argument that slavery figured largely as a weapon that Federalists and Antifederalists wielded to press their more primary points. It examines the debates over , and using , the various slavery clauses in the document to conclude that slavery was a versatile tool in rather than a core concern of the Constitutional Convention and ratification debates.     The barnacle and the building: a modern morality tale

    Abstract A rare and almost complete barnacle fossil, previously described on the basis of two isolated shell fragments, was recently exposed in a limestone block on the outer wall of Melbourne's Old Magistrates' Courts in Victoria, Australia. These courts comprise one of the oldest and grandest buildings in Melbourne and because of this they have a heritage listing. As heritage-listed buildings are protected from alteration by law, and as removal of the fossil would be deemed "alteration", official permission had to be obtained to extract the specimen. This paper discusses the processes involved with extraction of a unique specimen from a protected building and provides an overview of the palaeontological significance of the fossil. Consideration is given to the likely fate of a fossil of this nature, situated a little below eye level on a busy city street, if it was left in situ; finally, the implications of designating a holotype from material removed from a building are assessed.     A possible case of spondyloarthropathy in a prehistoric Japanese skeleton

    Abstract Palaeopathology helps to define the migration of past diseases. Genetic and environmental factors play a role in the development of spondyloarthropathy (SpA). We report skeletal remains with SpA from the Jomon period in Japan. The skeleton is of a female who died at a young adult age. The skeleton had characteristic features seen in SpA as follows: (1) polyarticular arthritis; (2) erosions accompanying some bone formation; (3) enthesial ossification; and (4) periostitis in lower long bones. The findings suggest that SpA was present in prehistoric Japan before contact with European civilisation, and the present example of SpA is the oldest in Asia and the Old World.     Evaluation of the nano-oligosaccharide factor lipido-colloid matrix in the local management of venous leg ulcers: results of a randomised, controlled trial

    Abstract The nano-oligosaccharide factor (NOSF) is a new compound aiming to promote wound closure mainly through inhibition of matrix metalloproteinase (MMP) activity. This factor is incorporated within a lipido-colloid matrix (Techonology Lipido-Colloid-NOSF matrix) and locally released in the wound. The objective of this study was to document the performance (non inferiority or superiority) of the NOSF relative to the Promogran® matrix (oxidised regenerated cellulose, ORC) effect in the local management of venous leg ulcers (VLUs). This was a 12-week, open, two-arm, multicentre, randomised study. Patients were selected if the area of their VLU [ankle brachial pressure index ,0·80] ranged from 5 to 25 cm2 with a duration ,3 months. Ulcers had to be free from necrotic tissue. In addition to receiving compression bandage therapy, patients were randomly allocated to either NOSF matrix or ORC treatment for 12 weeks. The VLUs were assessed on a weekly basis and wound tracings were recorded. Percentage wound relative reduction (%RR) was the primary efficacy criterion. Secondary objectives were wound absolute reduction (AR), healing rate (HR) and % of wounds with ,40% reduction compared with baseline. A total of 117 patients were included (57 NOSF matrix and 60 ORC). Mean population age was 71·3 ± 13·5 years, body mass index was ,30 kg/m2 in 39·3% and 15·4% were diabetics. Fifty-six per cent of the VLUs were present for >6 months, 61% were recurrent and 68% were stagnating despite appropriate care. Mean wound area at baseline was 11·2 ± 7·4 cm2. At the last evaluation, mean difference between the groups for %RR was 33·6 ± 15·0% in favour of NOSF matrix with a unilateral 95% confidence interval (CI) lower limit of 8·6% not including the null value. Therefore, a superiority of NOSF matrix effect compared with ORC was concluded (P = 0·0059 for superiority test). The median of the wound area reduction was 61·1% and 7·7% in the NOSF matrix and control groups, respectively (per-protocol analysis), or 54·4% versus 12·9% in intent-to-treat analysis (p = 0·0286). Median AR was 4·2 cm2 in the NOSF group and 1·0 cm2 with ORC (P = 0·01). Median HR was ,0·056 and ,0·015 cm2/day in NOSF and ORC groups, respectively (P = 0·029). By logistic regression, the NOSF versus control odds ratio to reach 40% area reduction was 2·4 (95% CI: 1·1,5·3; P = 0·026). In the oldest and largest VLUs, a strong promotion of healing effect was particularly observed in the NOSF matrix group compared with the control group. NOSF matrix is a very promising option for the local management of chronic wounds, especially for VLUs with poor healing prognosis. [source]


    Prevention of Hip Fractures in Long-Term Care: Relevance of Community-Derived Data

    Osteoporosis and falling are two major contributing factors to fractures in older persons; the relevant contribution of these may vary according to age, setting, and frailty. The purpose of this review was to examine the existing evidence on osteoporosis treatments to determine whether participants in clinical trials include or resemble the older and frailer adult population living in long-term care (LTC). The trials (N=50) used to support major Canadian guidelines for osteoporosis treatment were reviewed because these are used to recommend treatment for all older adults, and several more-recent studies were added. Trials conducted specifically with participants living in LTC were also reviewed (N=6). The majority of studies (96.0%) on osteoporosis treatments were conducted with community-dwelling participants, with many excluding participants resembling the LTC population. Mean ages ranged from 52 to 84, although for the majority of studies, the mean age was younger than 70. Similarly, 80.0% of studies conducted in LTC included only residents who were ambulatory, mobile, able to transfer independently, or not permanently bedridden. Mean ages in these studies ranged from 83 to 85. These findings suggest that frail older adults, particularly the oldest and frailest adults in LTC, are neglected in clinical trials of osteoporosis fracture prevention. There is little evidence to support the application of community-based guidelines to the LTC population, and studies directly involving this population are needed. The role of age, frailty, and the mechanics of falls in hip fracture are discussed. [source]


    Do We Need to Create Geriatric Hospitals?

    During a single illness episode, the sickest, frailest older patients are often treated in multiple distinct sites, including the emergency room, the intensive care unit, a general medical floor, and a skilled nursing facility. Such frequent transfers involve changes in physician, changes in nursing care, the rewriting of orders, and physical dislocation, all of which can adversely affect outcomes. This system, although efficient, increases the chance of medical errors, promotes delirium, and undermines the doctor-patient relationship. Partial solutions include a team approach to care, an electronic medical record, and substitution of home for hospital care. A more comprehensive solution is to create a geriatric hospital for treatment of the most common medical and surgical problems and for provision of rehabilitative or skilled nursing care. Designing new institutions for geriatric care will require new legislation and a new set of regulations but should be considered for the oldest and frailest patients. [source]


    Molecular clocks keep dispersal hypotheses afloat: evidence for trans-Atlantic rafting by rodents

    Abstract Aim, In order to resolve disputed biogeographical histories of biota with Gondwanan continental distributions, and to assess the null hypothesis of vicariance, it is imperative that a robust geological time-frame be established. As an example, the sudden and coincident appearance of hystricognath rodents (Rodentia: Hystricognathi) on both the African and South American continents has been an irreconcilable controversy for evolutionary biologists, presenting enigmas for both Gondwanan vicariance and Late Eocene dispersal hypotheses. In an attempt to resolve this discordance, we aim to provide a more robust phylogenetic hypothesis and improve divergence-date estimates, which are essential to assessing the null hypothesis of vicariance biogeography. Location, The primary centres of distribution are in Africa and South America. Methods, We implemented parsimony, maximum-likelihood and Bayesian methods to generate a phylogeny of 37 hystricognath taxa, the most comprehensive taxonomic sampling of this group to date, on the basis of two nuclear gene regions. To increase phylogenetic resolution at the basal nodes, these data were combined with previously published data for six additional nuclear gene regions. Divergence dates were estimated using two relaxed-molecular-clock methods, Bayesian multidivtime and nonparametric rate smoothing. Results, Our data do not support reciprocal monophyly of African and South American lineages. Indeed, Old World porcupines (i.e. Hystricomorpha) appear to be more closely related to New World lineages (i.e. Caviomorpha) than to other Old World families (i.e. Bathyergidae, Petromuridae and Thryonomyidae). The divergence between the monophyletic assemblage of South American lineages and its Old World ancestor was estimated to have occurred c. 50 Ma. Main conclusions, Our phylogenetic hypothesis and divergence-date estimates are strongly at odds with Gondwanan-vicariance isolating mechanisms. In contrast, our data suggest that transoceanic dispersal has played a significant role in governing the contemporary distribution of hystricognath rodents. Molecular-clock analyses imply a trans-Tethys dispersal event, broadly confined to the Late Cretaceous, and trans-Atlantic dispersal within the Early Eocene. Our analyses also imply that the use of the oldest known South American rodent fossil as a calibration point has biased molecular-clock inferences. [source]


    Imputation of 10-year osteoporotic fracture rates from hip fractures: A clinical validation study

    Abstract The World Health Organization (WHO) fracture risk assessment system (FRAX) allows for calibration from country-specific fracture data. The objective of this study was to evaluate the method for imputation of osteoporotic fracture rates from hip fractures alone. A total of 38,784 women aged 47.5 years or older at the time of baseline femoral neck bone mineral density (BMD) measurement were identified in a database containing all clinical dual energy X-ray absorptiometry (DXA) results for the Province of Manitoba, Canada. Health service records were assessed for the presence of nontrauma osteoporotic fracture codes after BMD testing (431 hip, 787 forearm, 336 clinical vertebral, and 431 humerus fractures). Ten-year hip and osteoporotic fracture rates were estimated by the Kaplan-Meier method. The population was stratified by age (50 to 90 years, 5-year width strata) and again by femoral neck T -scores (,4.0 to 0.0, 0.5 SD width strata). Within each stratum, the ratio of hip to osteoporotic fractures was calculated and compared with the predicted ratio from FRAX. Increasing age was associated with greater predicted hip-to-osteoporotic ratios (youngest 0.07 versua oldest 0.41) and observed ratios (youngest 0.10 versus oldest 0.48). Lower T -scores were associated with greater predicted (highest 0.04 versus lowest 0.71) and observed ratios (highest 0.06 versus lowest 0.44). There was a strong positive correlation between predicted and observed ratios (Spearman r,=,0.90,0.97, p,<,.001). For 14 of the 18 strata, the predicted ratio was within the observed 95% confidence interval (CI). Since collection of population-based hip fracture data is considerably easier than collection of non,hip fracture data, this study supports the current emphasis on using hip fractures as the preferred site for FRAX model calibration. © 2010 American Society for Bone and Mineral Research [source]


    Reproductive evaluation of elephants culled in Kruger National Park, South Africa between 1975 and 1995

    Abstract To reduce elephant densities and preserve biological diversity, 14,629 elephants were culled from Kruger National Park, South Africa (1967,1999). Data were catalogued between 1975 and 1996 on 2737 male and female elephants, including pregnancy and lactational status for 1620 females (,5 years of age) and, uterine and/or ovarian characteristics for 1279. This study used these data to investigate the effects of age and precipitation on reproduction. The youngest age of conception was 8 years (n = 6) and by 12 years of age all females were sexually mature. From the age of 14 years, the percentage of reproductively active females (pregnant and/or lactating) was >90%; however, this percentage declined when females reached 50 years of age. Overall, one-tenth of females were nonreproductive (not pregnant or lactating) at any given time, mostly in the youngest (<15 years) and oldest (>50 years) age classes. Eighteen (3.3%) of the nonpregnant females had reproductive tract pathologies, including endometrial, uterine or ovarian cysts. There was a seasonal distribution of mating activity that correlated with the rainy season. As has been demonstrated in other populations of free-ranging African elephants, most of the females in Kruger National Park were reproductively active; however, age and climate affected reproductive activity. Résumé Pour réduire la densité des éléphants et préserver la diversité biologique, 14.629 éléphants ont été abattus dans le Parc National Kruger, en Afrique du Sud, entre 1967 et 1999. Des données ont été recueillies entre 1975 et 1999, sur 2 737 éléphants mâles et femelles, y compris le statut de gestation ou de lactation de 1 620 femelles (,5 ans) et les caractéristiques utérines et/ou ovariennes de 1 279 d'entre elles. Cette étude a utilisé ces données pour rechercher les effets de l'âge et des précipitations sur la reproduction. L'âge de conception le plus précoce était de huit ans (n = 6), et à l'âge de 12 ans, toutes les femelles étaient sexuellement mâtures. À partir de l'âge de 14 ans, le pourcentage de femelles en reproduction active (pleines ou allaitantes) était >90%; cependant, ce pourcentage diminuait lorsque les femelles atteignaient l'âge de 50 ans. Globalement, à tout moment, un dixième des femelles n'avaient pas de reproduction active (ni pleines, ni allaitantes) surtout parmi les plus jeunes (<15 ans) et les plus vieilles (>50 ans).     Influence of adjuvant chemotherapy with cyclophosphamide, methotrexate and 5-fluorouracil on plasma melatonin and chosen hormones in breast cancer premenopausal patients

    Objective:,To investigate the effect of chemotherapy on levels of melatonin in patients with breast cancer. Background:,In light of reports on the possible oncostatic role of melatonin in breast cancer patients, it is essential to know the influence of adjuvant chemotherapy with cyclophosphamide, methotrexate and 5-fluorouracil (CMF) on plasma melatonin concentration as well as on its contributing factors, e.g. current hormonal state of the organism. The combination therapy is one of the oldest, safest and most commonly prescribed adjuvant treatments. Method:,Twenty-four breast cancer patients on CMF chemotherapy were studied along with a control group of 16 healthy pre-menopausal women. Results:,Plasma melatonin concentration (determined by RIA method) in breast cancer patients prior to treatment did not differ significantly from that of healthy women, but it was significantly increased after the initial cycle of CMF, and significantly increased as compared to a group of healthy women. We did not notice any significant interactions between plasma melatonin and growth hormone, prolactin, estradiol, progesterone, cortisol and met-enkephalin concentrations in all studied groups. Conclusion:,The possible oncostatic action of melatonin warrants further investigation to elucidate whether the induced increase of blood melatonin concentration is essential to successful CMF chemotherapy.     Barley yellow dwarf viruses (BYDVs) preserved in herbarium specimens illuminate historical disease ecology of invasive and native grasses

    Summary 1In plant invasion ecology, viruses and other pathogens are often considered in terms of the enemy release hypothesis, which predicts that plants become invasive in new ranges if they escape pathogens from their home range. However, pathogens may sometimes facilitate host spread rather than hinder it. 2Previously, we hypothesized that apparent competition mediated by barley and cereal yellow dwarf viruses (Luteoviridae: BYDVs, CYDVs) may have facilitated historical grassland invasion in California, USA, where Eurasian grasses displaced native grasses in the 18th and 19th centuries (the disease facilitation hypotheses). However, this could have happened only if the viruses were present during the invasion, which is unknown. 3To investigate the historical ecology of BYDVs in California grasses, we analysed preserved virus infections in herbarium specimens and used the historical virus sequences to determine rough time estimates of relevant phylogenetic events. 4The historical viral RNA sequences we identified in invasive and native grasses date from 1917 and are among the oldest recovered from plants thus far and the oldest from North America. 5Herbarium evidence and phylogenetic analysis suggest that BYDVs were likely to have been present in wild grasses during the California grassland invasion and to have shared some functional characteristics with present-day isolates, supporting the disease facilitation hypothesis. 6We found evidence of virus spread from California to Australia (or, less likely, from Australia to California) in the late 19th century, when much horticultural exchange occurred, as well as potential correspondence in the timing of virus diversification events and the beginning of extensive human exchange between the Old and New Worlds. 7Synthesis. Increasing evidence indicates that viruses are important in the ecology of grasslands and may, in some cases, mediate apparent competition among species. Historical data provide essential insight into plant virus ecology and suggest the need to examine human influence on plant virus diversification and spread within natural ecosystems. [source]


    About the oldest domesticates among fishes

    Domestication of mammals such as cattle, dogs, pigs and horses preceded that of fishes by at least 10 000 years. The first domesticated fish was the common carp Cyprinus carpio. Initially it was held as an exploited captive and did not undergo major changes in body shape or colour variations. About 2000 years ago, wild common carp were most abundant in the inland delta of the Danube River. These fish were torpedo shaped, golden-yellow in colour and had two pairs of barbels and a mesh-like scale pattern. Large schools of them thrived and reproduced on the flood plains of the Danube. The Romans kept fishes in specially built ponds at that time. The common carp was an ideal candidate and its rearing became more popular in medieval times. Common carp culture gradually became the most profitable branch of agriculture in central Europe and many special ponds were built. Soon common carp were being produced in pond systems including spawning and growing ponds. Unintentional artificial selection had taken place between the 12th and mid-14th century, and deep bodied and variously scaled or scaleless domesticated forms appeared in nearly every pond system. Some colour aberrations appeared in the 1950s in Japan, which, as koi, became the most expensive of fish. Common carp were not originally domesticated in China but wild ,chi'Carassius auratus occasionally appeared as a xanthic form that, as the goldfish, has been known since 960 A.D. By the 1200s the fish were used as ornamental animals in the garden pools of rich landowners. Circa 1276 to 1546, the Chinese began keeping golden chi in aquarium-like vessels and soon rich and poor alike became breeders of the fancy domesticated goldfish. The variously shaped monstrosities and colour aberrants were freaks but they became very fashionable at that time and still are. Domesticated goldfish monstrosities were first exported from China to Japan and much later to Europe and around the world. More recently other species have been domesticated by aquarists, such as the guppy Poecilia reticulata or the neon tetra Paracheirodon innesi. Other fishes kept as ornamentals, like swordtails Xiphophorus hellerii and platies Xiphophorus maculatus, the discus and angelfishes (Cichlidae), as well as those cultured for food like the rainbow trout Oncorhynchus mykiss, channel catfish Ictalurus punctatus or sturgeons (Acipenseridae) are merely exploited captives.
