Physical Activity Pattern of Prepubescent Filipino School Children During School Days

ABSTRACT Background:, Little is known about pre-pubescent Filipino children's involvement in moderate-to-vigorous physical activity (MVPA). There are international guidelines regarding required levels of MVPA for healthy children. Methods:, This study describes participation of 11- to 12-year-olds in randomly selected public and private schools in San Juan, Metromanila, in MVPA and sports during a school day. The Filipino-modified Physical Activity Questionnaire for Older Children (F_PAQ_C) was administered in English and Filipino. Additional data was collected on sex, age, type of school, and amount of time spent using television and computers. Children's self-assessment of physical activities (1 question in the F_PAQ_C) was correlated with their cumulative F_PAQ_C score. Results:, Three hundred eighty subjects (167 boys, 213 girls) participated. Participation in MVPA varied between sex and age groups, from 56.1% to 65.0%. Fewer than 10% of participants were very active. The children were more active during physical education classes than at recess or lunch, after class, or in the evening. Walking for exercise, jumping, jogging and running, free play, and dance were most common. Boys, younger children, and private school students most commonly engaged in MVPA. Self-assessed physical activity had modest correlation (r2= 0.21) with cumulative F_PAQ_C score, after adjusting for sex, age, and school type. Conclusion:, Most children were not physically active during the school day, except in physical education classes. To reduce the gap between recommended and current activity levels, more opportunities should be provided for preteen Filipino children to engage in MVPA during and after school. [source]


The development of change blindness: children's attentional priorities whilst viewing naturalistic scenes

Change blindness describes the surprising difficulty of detecting large changes in visual scenes when changes occur during a visual disruption. In order to study the developmental course of this phenomenon, a modified version of the flicker paradigm, based on Rensink, O'Regan & Clark (1997), was given to three groups of children aged 6,12 years and to a group of adults. This paradigm tested the ability to detect single colour, presence/absence and location changes of both high and low semantic importance in a complex scene. Semantically important changes were detected more quickly and accurately than less semantically important changes, by all age groups, indicating that children had the same attentional priorities as adults. Older children achieved more efficient and accurate detection of changes than younger children and reached almost adult level at 10,12 years old. These improvements parallel age-related developments in attention and visual perception. [source]


Origins of a stereotype: categorization of facial attractiveness by 6-month-old infants

Like adults, young infants prefer attractive to unattractive faces (e.g. Langlois, Roggman, Casey, Ritter, Rieser-Danner & Jenkins, 1987; Slater, von der Schulenburg, Brown, Badenoch, Butterworth, Parsons & Samuels, 1998). Older children and adults stereotype based on facial attractiveness (Eagly, Ashmore, Makhijani & Longo, 1991; Langlois, Kalakanis, Rubenstein, Larson, Hallam & Smooth, 2000). How do preferences for attractive faces develop into stereotypes? Several theories of stereotyping posit that categorization of groups is necessary before positive and negative traits can become linked to the groups (e.g. Tajfel, Billig, Bundy & Flament, 1971; Zebrowitz-McArthur, 1982). We investigated whether or not 6-month-old infants can categorize faces as attractive or unattractive. In Experiment 1, we familiarized infants to unattractive female faces; in Experiment 2, we familiarized infants to attractive female faces and tested both groups of infants on novel faces from the familiar or novel attractiveness category. Results showed that 6-month-olds categorized attractive and unattractive female faces into two different groups of faces. Experiments 3 and 4 confirmed that infants could discriminate among the faces used in Experiments 1 and 2, and therefore categorized the faces based on their similarities in attractiveness rather than because they could not differentiate among the faces. These findings suggest that categorization of facial attractiveness may underlie the development of the ,beauty is good' stereotype. [source]


Children's, adolescents', and adults' reference to goals to explain interpersonal actions

Abstract This study investigated children's, adolescents', and adults' references to an actor's goals when explaining interpersonal actions. Participants were presented with eight brief stories containing a variety of social events and were asked to explain why the actor in each story performed the central action. Children, adolescents, and adults mentioned goals for most of the stories. Adults and adolescents mentioned psychological goals much more often than did children. Older children, adolescents, and adults mentioned complex psychological goals more often than did younger children. Younger children often mentioned instrumental or social goals. When making goal attributions, children, adolescents, and adults considered both the nature of the action and the social context in which it occurred. Participants mentioned psychological goals more often when explaining an antisocial action and social goals when explaining a prosocial action. Participants were also more likely to mention psychological goals when interpersonal events immediately preceding the central action were described. Thus, goals were offered as explanations for interpersonal actions by all age groups, but the type of goals mentioned varied by the age of the participant, the social context, and the valence of the action. Familial Factors Associated With the Use of Multiple Child-Care Arrangements

This study examined the use of multiple, concurrent, nonparental child-care arrangements among children under 5 with employed mothers in the NICHD Study of Early Child Care and Youth Development (N= 759). Older children, those primarily cared for in informal child care, those living in cohabitating or single-parent households, and those whose mothers were employed for 40 or fewer hours per week were likely to be in multiple arrangements. Higher quality primary child-care and lower maternal satisfaction with primary care predicted the subsequent use of multiple arrangements. Little support for income differences in selection into multiple arrangements was found. Findings highlight the importance of child-care characteristics and structure in child-care choice. Policy implications are discussed. [source]


Locomotor skills and balance strategies in children with internal rotations of the lower limbs

Abstract The purpose of this study was to investigate the functional effects of a structural deformation, internal rotations (IR) of the lower limbs, on upper body balance strategies used during locomotion in 5,6 year-old and 7,10 year-old children. Balance control was examined in terms of rotation around the longitudinal axis in horizontal plane (yaw) and around the sagittal axis in a frontal plane (roll). Kinematics of foot, pelvis, shoulder, and head rotations were measured with an automatic optical TV image processor and used to calculate angular dispersions and segmental stabilizations. Older children with IR showed a lower gait velocity, particularly in difficult balance conditions than typically developing (TD) children. In younger children, the effect of the local biomechanical deficit remained limited to the lower limbs and did not affect upper body coordination. By contrast, in older children with IR, the development of head stabilization in space was affected. This was demonstrated by an "en bloc" instead of an articulated mode of head-trunk unit systematically adopted by the control group. As pelvic stabilization remains the main reference frame to organize balance control in older children with IR, we conclude that the structural deformity of the legs affect and possibly delay the acquisition of the head stabilization in space strategy. © 2007 Orthopaedic Research Society. Differences in Children's Recess Physical Activity: Recess Activity of the Week Intervention

BACKGROUND: The increased prevalence in recent childhood obesity rates raises concern about youth health and the role that lack of physical activity plays in this trend. A focus on how children today choose to spend their discretionary time is one approach that may yield ideas for how to reduce childhood obesity. The purpose of the present study was to examine whether 3 separate recess activities of the week (RAWs) would make a difference in children's discretionary time physical activity levels. METHODS: Children (N = 65: 30 boys, 35 girls; 32 first and second graders; 33 third and fourth graders; 45 healthy body mass index [BMI], 20 overweight BMI) at 1 Midwest elementary school wore pedometers for each 15-minute morning recess period for 4 weeks. Following 1 no RAW (#1), a new RAW was introduced each subsequent week: #2, circuit course; #3, obstacle course; and #4, Frisbees. RESULTS: Repeated measures factorial analysis of variance results revealed that children were significantly more active during the no RAW and circuit course week than the Frisbee week. Males were significantly more physically active than females during the obstacle course week. Older children were significantly more active during the Frisbee week than younger children. Healthy BMI children were significantly more physically active during the circuit course week than children in the overweight/obese BMI category. CONCLUSIONS: Results imply that it is important for schools to consider demographic factors in the creation of recess opportunities to increase physical activity. [source]


Time Spent on Treatment of Atopic Dermatitis: A New Method of Measuring Pediatric Morbidity?

The TSOT (min/day) was studied in a group of 42 children with atopic dermatitis (AD) (16 girls and 26 boys; mean age 7.07 years). The TSOT included time spent on all types of topical treatment, on extra cleaning, and on visits to doctors. Objective Scoring Atopic Dermatitis (SCORAD) assessment was performed at each visit. A significant correlation was found between TSOT and SCORAD scores for all visits (p < 0.0001). There was no correlation between TSOT and age or sex or between TSOT/SCORAD and age (p < 0.08). For the 65 visits (by 42 children), TSOT/SCORAD ranged from 0.08 min/point to 28.67 min/point. Older children (10,15 years of age) had a lower TSOT/SCORAD ratio compared to younger children (1,5 years of age). Our data suggest that TSOT in itself may be a useful measure of morbidity among pediatric AD patients. It is speculated that patients with a very high TSOT/SCORAD rate or a very low rate have coping problems and would therefore be suitable candidates for intensified efforts in programs such as "eczema schools." Parental communication and children's behaviour following diagnosis of childhood leukaemia

Many parents find decisions about what to tell their child with cancer difficult. Open communication is generally considered the best policy and most health care professionals encourage parents to talk openly and honestly about the illness. However, parents differ in their views about what to tell the child. In this study 55 parents of children (36 boys and 19 girls, mean age = 7.33 years) newly diagnosed with acute lymphoblastic leukaemia (ALL) were interviewed about (i) the child's reactions and behaviour following diagnosis, (ii) their views about what to tell their child and (iii) factors influencing parents' communication with the child. Interviews were analysed using thematic analysis. Most children showed behavioural and mood difficulties after diagnosis. Older children were given more information. In addition, parents' perceptions of childhood cancer affect the way they communicate with their child. Gaining insight into long-term effects of inviting speculation: Does recantation help?,

The effects of speculation on delayed recall and on source monitoring (SM) as a recantation device were examined in first and fourth graders. Two weeks after a clown show, the speculation group recalled all actions and speculated on half. The control group recalled all actions without speculating. One month and 6 months later, all children recalled all actions. Half the children were given the opportunity to recant at 1 month and all children at 6 months. Even after 6 months, speculation negatively impacted recall. Only first-graders' script-consistent false responses increased with time. In the speculation group, 19% of previously given script-consistent speculations were recanted after 1 month and 20% after 6 months. Older children were better at recanting than younger children. Although recantation via SM immediately after 6-month recall had a positive effect, SM at 1 month had no prospective effect on 6-month recall. Copyright © 2009 John Wiley & Sons, Ltd. [source]


The Effects of repeated interviewing on children's forensic statements of sexual abuse

Multiple interviews with children alleging sexual abuse are not uncommon. Researchers expressed concern that repeated investigations may create and preserve inaccurate details. However, studies indicated that repeated open-ended interviews are not necessarily harmful and may have advantages. Forensic interviews were conducted with 40 children, alleged victims of sexual abuse, according to the NICHD investigative protocol. The children were re-interviewed after a short break. The information obtained in the second interview was almost 25% new. The first interview yielded a larger number of details, both central and peripheral, but the proportion of central details was larger in the second interview. The proportion of details repeated in both interviews was surprisingly low, and most of the original information was not included. Older children repeated more information than younger ones. Dyadic Analyses of Friendship in a Sample of Preschool-Age Children Attending Head Start: Correspondence between Measures and Implications for Social Competence

Friendships among a large sample of preschool-age children (N= 471) attending Head Start were assessed. Based on sociometric data, friendship dyads were identified as reciprocated (mutual choice) or nonreciprocated (unilateral choice). Dyads were further classified with respect to gender composition as either same- or mixed-gender dyads. Older children were more likely to participate in a reciprocated friendship than were younger children and reciprocated dyads were more likely to be same-gender than were nonreciprocated dyads. Analyses of interaction between dyad partners revealed that reciprocated friends interacted more frequently across all categories of interaction coded and looked at each other more frequently than did members of nonreciprocated dyads. For the positive interaction subscore, the friendship status effect was modified by a significant interaction with gender composition such that significant effects of friendship status were obtained only for same-gender dyads. Additional analyses indicated that the average social competence level was greater for reciprocated dyads than for nonreciprocated dyads. The findings suggest that reciprocated friendships are meaningful for preschool-age children and may serve as special socialization contexts in which the repertoire of behavior can be exercised and perhaps improved. They also highlight the salience of same-gender friendships in the preschool classroom. [source]


Time use in Russian Baby Homes

Abstract Background We investigated time use of Russian children residing in Baby Homes to document their opportunities and experiences during institutional care. Methods Time use of 138 non-special needs, randomly selected children (65 female vs. 73 male, age 1 month to 4 years) in Baby Homes in Murmansk, Russia, was analysed. Baby Home specialists were trained in time-use spot observation methodology. Each child was observed every 10 min for 5 h (31 observations/child; >4000 data points). At each observation, we coded: who the child was with, adult role, what the child was doing, location and vocalizations. Results Children spent 50% of their time alone, 27% with a caregiver, 15% with another adult, and 7% with another child. Infants spent significantly more time alone (65%) than toddlers (43%) or pre-schoolers (46%) [F(2,133) = 13.05, P < 0.0001]. Caregivers supervised children (58%), led group activities (16%), cared for individuals (14%), or were absent from the room (12%). Children spent the most time (32%) in activities of daily living; remaining time was spent in meaningful play (27%), non-purposeful activity (16%) or sleep (18%). The percentage of time spent in meaningful play significantly increased across age groups [infants 10%, toddlers 25%, pre-schoolers 36%, F(2,133) = 26.9, P < 0.001]. Infants (23%) and toddlers (20%) spent significantly more time than the older group (10%) in non-purposeful activity [F(2,133) = 26.9, P < 0.001]. In 12% of observations, an adult was speaking to the child, in 10% to the group, and <1% to another adult. Child vocalizations varied by age: infants 42% of observations, toddlers 56%, and pre-schoolers 59%. Older children directed more vocalizations to adults than younger children [F(2,133) = 24.47, P < 0.001]. Conclusion Time use of children residing in Baby Homes is limited by routinized schedules and care, at the expense of child-directed or interactive play with adults. Despite close proximity and living in group care, children's vocalizations and interactions with others are limited. [source]


Pulmonary Function and Ventilatory Limitation to Exercise in Congenital Heart Disease

ABSTRACT Pulmonary function in older children and adolescents following surgical repair of congenital heart disease is often abnormal for various reasons. Many of these patients report symptoms of exercise intolerance although the reason(s) for this symptom can be complicated and sometimes interrelated. Is it simply deconditioning due to inactive lifestyle, chronotropic or inotropic insufficiency? or could there indeed be ventilatory limitation to exercise? These are the questions facing the clinician with the increasing frequency of patients undergoing repair early in life and growing into adulthood. Understanding pulmonary functional outcomes and means of determining ventilatory limitation to exercise is essential to thoroughly address the problem. This article reviews pulmonary function in patients with congenital heart disease and then describes a newer technique that should be applied to determine ventilatory limitation to exercise. A CROSS-CULTURAL EXAMINATION OF THE LINK BETWEEN CORPORAL PUNISHMENT AND ADOLESCENT ANTISOCIAL BEHAVIOR,

Several studies with older children have reported a positive relationship between parental use of corporal punishment and child conduct problems. This has lead some social scientists to conclude that physical discipline fosters antisocial behavior. In an attempt to avoid the methodological difficulties that have plagued past research on this issue, the present study used a proportional measure of corporal punishment, controlled for earlier behavior problems and other dimensions of parenting, and tested for interaction and curvilinear effects. The analyses were performed using a sample of Iowa families that displayed moderate use of corporal punishment and a Taiwanese sample that demonstrated more frequent and severe use of physical discipline, especially by fathers. For both samples, level of parental warmth/control (i.e., support, monitoring, and inductive reasoning) was the strongest predictor of adolescent conduct problems. There was little evidence of a relationship between corporal punishment and conduct problems for the Iowa sample. For the Taiwanese families, corporal punishment was unrelated to conduct problems when mothers were high on warmth/control, but positively associated with conduct problems when they were low on warmtwcontrol, An interaction between corporal punishment and warmth/Wcontro1 was found for Taiwanese fathers as well. For these fathers, there was also evidence of a curvilinear relationship, with the association between corporal punishment and conduct problems becoming much stronger at extreme levels of corporal punishment. Overall, the results are consistent with the hypothesis that it is when parents engage in severe forms of corporal punishment, or administer physical discipline in the absence of parental warmth and involvement, that children feel angry and unjustly treated, defy parental authority, and engage in antisocial behavior. Developmental profiles for multiple object tracking and spatial memory: typically developing preschoolers and people with Williams syndrome

The ability to track moving objects, a crucial skill for mature performance on everyday spatial tasks, has been hypothesized to require a specialized mechanism that may be available in infancy (i.e. indexes). Consistent with the idea of specialization, our previous work showed that object tracking was more impaired than a matched spatial memory task in individuals with Williams syndrome (WS), a genetic disorder characterized by severe visuo-spatial impairment. We now ask whether this unusual pattern of performance is a reflection of general immaturity or of true abnormality, possibly reflecting the atypical brain development in WS. To examine these two possibilities, we tested typically developing 3- and 4-year-olds and people with WS on multiple object tracking (MOT) and memory for static spatial location. The maximum number of objects that could be correctly tracked or remembered (estimated from the k -statistic) showed similar developmental profiles in typically developing 3- and 4-year-old children, but the WS profile differed from either age group. People with WS could track more objects than 3-year-olds, and the same number as 4-year-olds, but they could remember the locations of more static objects than both 3- and 4-year-olds. Combining these data with those from our previous studies, we found that typically developing children show increases in the number of objects they can track or remember between the ages of 3 and 6, and these increases grow in parallel across the two tasks. In contrast, object tracking in older children and adults with WS remains at the level of 4-year-olds, whereas the ability to remember multiple locations of static objects develops further. As a whole, the evidence suggests that MOT and memory for static location develop in tandem typically, but not in WS. Atypical development of the parietal lobe in people with WS could play a causal role in the abnormal, uneven pattern of performance in WS. This interpretation is consistent with the idea that multiple object tracking engages different mechanisms from those involved in memory for static object location, and that the former can be particularly disrupted by atypical development. Seven-year-olds allocate attention like adults unless working memory is overloaded

Previous studies have indicated that visual working memory performance increases with age in childhood, but it is not clear why. One main hypothesis has been that younger children are less efficient in their attention; specifically, they are less able to exclude irrelevant items from working memory to make room for relevant items. We examined this hypothesis by measuring visual working memory capacity under a continuum of five attention conditions. A recognition advantage was found for items to be attended as opposed to ignored. The size of this attention-related effect was adult-like in young children with small arrays, suggesting that their attention processes are efficient even though their working memory capacity is smaller than that of older children and adults. With a larger working memory load, this efficiency in young children was compromised. The efficiency of attention cannot be the sole explanation for the capacity difference. [source]


Small and large number processing in infants and toddlers with Williams syndrome

Previous studies have suggested that typically developing 6-month-old infants are able to discriminate between small and large numerosities. However, discrimination between small numerosities in young infants is only possible when variables continuous with number (e.g. area or circumference) are confounded. In contrast, large number discrimination is successful even when variables continuous with number are systematically controlled for. These findings suggest the existence of different systems underlying small and large number processing in infancy. How do these develop in atypical syndromes? Williams syndrome (WS) is a rare neurocognitive developmental disorder in which numerical cognition has been found to be impaired in older children and adults. Do impairments of number processing have their origins in infancy? Here this question is investigated by testing the small and large number discrimination abilities of infants and toddlers with WS. While infants with WS were able to discriminate between 2 and 3 elements when total area was confounded with numerosity, the same infants did not discriminate between 8 and 16 elements, when number was not confounded with continuous variables. These findings suggest that a system for tracking the features of small numbers of object (object-file representation) may be functional in WS, while large number discrimination is impaired from an early age onwards. Finally, we argue that individual differences in large number processing in infancy are more likely than small number processing to be predictive of later development of numerical cognition. Eye remember you two: gaze direction modulates face recognition in a developmental study

The effects of gaze direction on memory for faces were studied in children from three different age groups (6,7, 8,9, and 10,11 years old) using a computerized version of a task devised by Hood, Macrae, Cole-Davies and Dias (2003). Participants were presented with a sequence of faces in an encoding phase, and were then required to judge which faces they had previously encountered in a surprise two-alternative forced-choice recognition test. In one condition, stimulus eye gaze was either direct or deviated at the viewing phase, and eyes were closed at the test phase. In another condition, stimulus eyes were closed at the viewing phase, with either direct or deviated gaze at the test phase. Modulation of gaze direction affected hit rates, with participants demonstrating greater accuracy for direct gaze targets compared to deviated gaze targets in both conditions. Reaction times (RT) to correctly recognized stimuli were faster for direct gaze stimuli at the viewing phase, but not at the test phase. The age group of participants differentially affected these measures: there was a greater hit rate advantage for direct gaze stimuli in older children, although RTs were less affected by age. These findings suggest that while the facilitation of face recognition by gaze direction is robust across encoding and recognition stages, the efficiency of the process is affected by the stage at which gaze is modulated. A visit from the Candy Witch: factors influencing young children's belief in a novel fantastical being

Factors hypothesized to affect beliefs in fantastical beings were examined by introducing children to a novel fantastical entity, the Candy Witch. Results revealed that among older preschoolers, children who were visited by the Candy Witch exhibited stronger beliefs in the Candy Witch than did those who were not. Among children who were visited, older children had stronger beliefs than did younger children. Among children who were not visited, those with a high Fantasy Orientation believed more strongly than did those with a low Fantasy Orientation. Belief remained high one year later. At both time points, the number of other fantastical beings in which a child believed was significantly related to belief in the Candy Witch. [source]


Applying the choice/no-choice methodology: the case of children's strategy use in spelling

The present study illustrates the usefulness of the choice/no-choice method to investigate age-related changes in children's strategies. This method enables independent assessments of strategy use and execution. It is applied in children's spelling strategies. Third- and fifth-grade children were asked to write down words using a dictionary or not (choice condition). Then, they were successively required to spell words using each strategy (no-choice condition). Performance showed that (a) strategy choices and accuracy differed in younger and older children, (b) strategy effectiveness was the strongest predictor of children's strategy use, and (c) having the choice resulted in higher level of spelling accuracy (especially in older children) than having no choice. We discuss the implications of these results on how the choice/no-choice method can be useful for understanding and investigating children's strategy choices in spelling and other cognitive domains. Fluency remediation in dyslexic children: does age make a difference?

Abstract This study tested the hypothesis whether older dyslexic children may obtain fewer gains on fluency and accuracy with respect to their younger peers after specific remediation. Changes in accuracy and fluency of a group of children with a diagnosis of dyslexia attending third and fourth grades were compared with those obtained by a group of children attending the sixth, seventh or eighth grade in two different treatments, one based on the Balance model (Bakker) and the second based on the automatization of syllable recognition (sublexical). Among all comparisons between the gains in accuracy and fluency obtained by the two groups, only the younger group in the sublexical treatment obtained a statistically significant gain with respect to their older peers' accuracy in reading words. These outcomes suggest that, at least for the chronological ages and types of treatments considered in this study, older children with dyslexia may obtain comparable gains to their younger peers, suggesting that ,it is never too late' to remediate reading fluency and accuracy. Genetic influences on reading difficulties in boys and girls: the Colorado twin study

Abstract To test the hypothesis that the genetic etiology for reading disability may differ in males and females, data from identical and fraternal twin pairs were analysed using both concordance and multiple regression methods. The sample included 264 identical (129 male, 135 female) and 214 same-sex fraternal (121 male, 93 female) twin pairs in which at least one member of each pair had reading difficulties. The difference between the identical and fraternal twin pair concordance rates was slightly larger for females than for males, suggesting a possible sex difference in etiology; however, a loglinear analysis of the three-way interaction of sex, zygosity, and concordance was not significant (p,0.17). The estimate of group heritability (hg2), a standardized measure of the extent to which reading difficulties are due to genetic influences, was somewhat greater for females than males (0.65 vs 0.54), but this difference was also not significant (p,0.35). Gender differences in hg2 were larger for younger children (less than 11.5 years of age) than for older children. However, the three-way interaction of sex, zygosity, and age was not significant when age was treated either categorically (p,0.86) or continuously (p,0.71). Parents and Practitioners Are Poor Judges of Young Children's Pain Severity

Objective: Visual analog pain scales are reliable measures in older children and adults; however, pain studies that include young children often rely on parental or practitioner assessments for measuring pain severity. The authors correlated patient, parental, and practitioner pain assessments for young children with acute pain. Methods: This was a prospective, descriptive study of a convenience sample of 63 emergency department patients aged 4-7 years, with acute pain resulting from acute illness or painful invasive procedures. A trained research assistant administered a structured pain survey containing demographic and historical features to all parents/guardians. Children assessed their pain severity using a validated ordinal scale that uses five different faces with varying degrees of frowning (severe pain) or smiling (no pain). Each face was converted to a numeric value from 0 (no pain) to 4 (severe pain). Parents and practitioners independently assessed their child's pain using a validated 100-mm visual analog scale (VAS) marked "most pain" at the high end. Pairwise correlations between child, parent, and practitioner pain assessments were performed using Spearman's or Pearson's test as appropriate. The association between categorical data was assessed using ,2 tests. Results: Sixty-three children ranging in age from 4 to 7 were included. Mean age (±SD) was 5.7 (±1.1); 42% were female. Fifty-seven successfully completed the face scale. The distribution of the children's scores was 0-17%, 1-9%, 2-30%, 3-14%, and 4-30%. Mean parental and practitioner scores (±SD) on the VAS were 61 (±26) mm and 37 (±26) mm, respectively (maximal = 100 mm). Correlation between child and parent scores was 0.47 (p < 0.001). Correlation between child and practitioner scores was 0.08 (p = 0.54). Correlation between parent and practitioner scores was 0.04 (p = 0.001). Conclusions: There is poor agreement between pain ratings by children, parents, and practitioners. It is unclear which assessment best approximates the true degree of pain the child is experiencing. The Relationship Between Classroom Motivation and Academic Achievement in Elementary-School-Aged Children

The relationship between motivation and academic success has been better established with older children and adults than with younger children. As part of a larger project, the purpose of this study was to examine the relation-ship between classroom motivation and academic achievement in young elementary-school-aged children. The participants were 122 first-grade and 129 third-grade children from a mid-sized city in the southern United States. The findings from the current study were consistent with previous research in that higher levels of mastery motivation and judgment motivation were found to be related to higher math and reading grades in third graders. However, higher levels of mastery motivation, not judgment motivation, were related to higher math and reading grades in first graders. [source]


CHILDREN'S AND PARENTS' PERCEPTIONS ON CHILDREN'S PARTICIPATION IN DECISION MAKING AFTER PARENTAL SEPARATION AND DIVORCE*

This article outlines the views of children and parents involved in family law disputes, about the need for and appropriateness of children's participation in decisions regarding residence and contact arrangements. Ninety parents and 47 children (ranging in age from 6 to 18 years) who had been through parental separation, were interviewed. Both parents and children had a range of views about the general appropriateness and fairness of children being involved, but the great majority, particularly of parents, thought that children should have a say in these matters. Core findings of the study include the considerable influence that older children had over the arrangements either in the aftermath of the separation or in making further changes over time, and the higher stated need of children who had experienced violence, abuse, or high levels of conflict to be heard than those in less problematic and noncontested matters. Parents involved in contested proceedings supported the participation of children at a younger age than those who were not. There was a reasonable degree of agreement between parents and children about the need for children to be acknowledged and the value of their views being heard in the decision-making process. Parents, however, expressed concern about the pressure and manipulation that children can face and exert in this process, whereas children were generally more concerned about the fairness of the outcomes, and maintaining their relationships with their parents and siblings. High incidence of t(7;12)(q36;p13) in infant AML but not in infant ALL, with a dismal outcome and ectopic expression of HLXB9

The t(7;12)(q36;p13) is a recurrent translocation involving the ETV6/TEL gene (12p13) and a heterogeneous breakpoint at 7q36. A fusion transcript between HLXB9 and ETV6 in AML with t(7;12) is occasionally found. To study the incidence of t(7;12) in infant and childhood acute leukemia, we screened 320 cases <36 months using FISH. Additionally, 28 pediatric cases >36 months with cytogenetic breakpoints at 12p and 7q were investigated. We studied the presence of an HXLB9-ETV6 fusion transcript and quantified the expression of various genes located in the 7q36 breakpoint region. In total, six AML patients carried the t(7;12) of which five were infants and one child of 18 months. Only one out of 99 infant ALL patients harbored the t(7;12). No t(7;12) was found in older children with AML or ALL. AML patients carrying a t(7;12) had a poor outcome with a 3-year EFS of 0%. A fusion of HLXB9 to ETV6 was found in four AML cases with t(7;12). The 7q36 genes NOM1, LMBR1, RNF32, and SHH were equally expressed among t(7;12)-positive AML versus t(7;12)-negative AML, t(7;12)-negative ALL, or normal bone marrow. However, the HLXB9 expression was highly increased in t(7;12)-positive cases, including those with an HLXB9-ETV6 fusion. We conclude that the t(7;12) is almost exclusively present in infant AML and covers 30% of infant AML, while it is extremely rare in infant ALL and older children. AML1/RUNX1 mutations are infrequent, but related to AML-M0, acquired trisomy 21, and leukemic transformation in pediatric hematologic malignancies

AML1/RUNX1, located on chromosome band 21q22, is one of the most important hematopoietic transcription factors. AML1 is frequently affected in leukemia and myelodysplastic syndrome with 21q22 translocations. Recently, AML1 mutations were found in adult hematologic malignancies, especially acute myeloid leukemia (AML),M0 or leukemia with acquired trisomy 21, and familial platelet disorder with a predisposition toward AML. Through the use of polymerase chain reaction,single-strand conformation polymorphism analysis, we examined the AML1 gene for mutations in 241 patients with pediatric hematologic malignancies, and we detected AML1 mutations in seven patients (2.9%). Deletion was found in one patient, and point mutations in four patients, including three missense mutations, two silent mutations, and one mutation within an intron resulting in an abnormal splice acceptor site. All of the mutations except for one were heterozygous. Mutations within the runt domain were found in six of seven patients. Six of seven patients with AML1 mutations were diagnosed with AML, and one had acute lymphoblastic leukemia. In three of these seven patients, AML evolved from other hematologic disorders. AML1 mutations were found in two of four AML-M0 and two of three patients with acquired trisomy 21. Patients with AML1 mutations tended to be older children. Three of four patients with AML1 mutations who received stem cell transplantation (SCT) are alive, whereas the remaining three patients with mutations without SCT died. These results suggest that AML1 mutations in pediatric hematologic malignancies are infrequent, but are possibly related to AML-M0, acquired trisomy 21, and leukemic transformation. PENTA 2009 guidelines for the use of antiretroviral therapy in paediatric HIV-1 infection

PENTA Guidelines aim to provide practical recommendations for treating children with HIV infection in Europe. Changes to guidance since 2004 have been informed by new evidence and by expectations of better outcomes following the ongoing success of antiretroviral therapy (ART). Participation in PENTA trials of simplifying treatment is encouraged. The main changes are in the following sections: ,When to start ART': Treatment is recommended for all infants, and at higher CD4 cell counts and percentages in older children, in line with changes to adult guidelines. The number of age bands has been reduced to simplify and harmonize with other paediatric guidelines. Greater emphasis is placed on CD4 cell count in children over 5 years, and guidance is provided where CD4% and CD4 criteria differ. ,What to start with': A three-drug regimen of two nucleoside reverse transcriptase inhibitors (NRTIs) with either a nonnucleoside reverse transcriptase inhibitor (NNRTI) or a boosted protease inhibitor (PI) remains the first choice combination. Lamivudine and abacavir are the NRTI backbone of choice for most children, based on long-term follow-up in the PENTA 5 trial. Stavudine is no longer recommended. Whether to start with an NNRTI or PI remains unclear, but PENPACT 1 trial results in 2009 may help to inform this. All PIs should be ritonavir boosted. Recommendations on use of resistance testing, therapeutic drug monitoring and HLA testing draw from data in adults and from European paediatric cohort studies. Recently updated US and WHO paediatric guidelines provide more detailed review of the evidence base. Differences between guidelines are highlighted and explained. [source]


Children's understanding of advertising: an investigation using verbal and pictorially cued methods

Abstract Conflicting results on children's understanding of advertising may stem from differences in research methods. Most studies are conducted using interviewing techniques, employing only verbal questioning. In the present study, 136 children of two age groups (7 and 10 years) were first asked what advertising was for and, after responding, shown depictions of possible reasons. The results indicate that although older children are more likely than younger ones to understand that advertising seeks to promote selling, pictorial cues allow a much larger proportion of all children to indicate their understanding than verbal questioning does on its own, with younger children especially showing improvement. Thus, seven-year-olds seem to have an implicit understanding of the persuasive intent of advertising that they are unable to articulate in response to investigators' questions.
