The impact of personal characteristics on engagement in nursing home residents with dementia

Abstract Objective To examine the impact of personal attributes on engagement in persons with dementia. Methods Participants were 193 residents of seven Maryland nursing homes. All participants had a diagnosis of dementia. Cognitive functioning was assessed via the Mini-Mental State Examination, and engagement was assessed via the Observational Measure of Engagement. Data pertaining to activities of daily living were obtained from the Minimum Data Set. Results Women had longer mean engagement duration than men, and significant results were not seen with the other demographic variables. Significant, positive correlations were found between higher cognitive functioning and longer engagement duration, more attention, a more positive attitude, and a higher refusal rate. There was a positive and significant correlation between the comorbidity index and engagement duration, and between the number of medications and attention. All functional status variables yielded significance in a positive direction. Participants with poor hearing had a higher refusal rate. Cognitive status was the most consistent and potent predictor of engagement in this population. Conclusion Despite a higher refusal rate among those with higher cognitive levels, their overall engagement with stimuli is higher. Caregivers should anticipate higher refusal rates in those with poor hearing, and therefore compensatory methods should be used in presenting stimuli in this population. The potent role of cognitive and functional status on engagement of persons with dementia underscores the importance of tailoring activities to nursing home residents' needs, interests, and limitations. The Relation between Observational Measures of Social Problem Solving and Familial Antisocial Behavior: Genetic and Environmental Influences

Deficits in social problem-solving skills are often associated with antisocial behavior, particularly in children's extrafamilial relationships. The current study was designed to examine this association in several new ways: the association was examined at two times in an adolescent sample within the context of the family; genetic models were used to estimate genetic and environmental effects on observational measures of problem solving and antisocial behavior and on the association between the two. The analyses were conducted as part of the Nonshared Environment in Adolescent Development project, consisting of 720 families at Time 1 (mean adolescent age: 14.5 years) and 440 families at Time 2 (mean adolescent age: 16.1 years). Genetic influence was found for antisocial behavior, but not for problem solving. The findings of shared environmental influences on these measures and their association are unusual in the behavioral genetic literature and are important in that respect. [source]


The Leader Observation Tool: a process skills treatment fidelity measure for the Incredible Years parenting programme

Abstract Background Despite recognition of the need to deliver evidence-based programmes in the field of mental health, there is little emphasis on implementing such programmes with fidelity. Attempts by programme developers to ensure adherence to their programmes include the development of training, manuals and content scales, but these alone may be insufficient to ensure fidelity in replication. Observational measures lend themselves as a potentially useful assessment of intervention outcomes, providing accurate and objective accounts of the intervention process. Aim To develop a reliable and valid observational treatment fidelity tool of process skills required to deliver the Incredible Years (IY) BASIC parenting programme effectively. Methods An objective observational fidelity measure was developed to assess adherence to the IY BASIC parenting programme protocol. Observations were conducted on 12 IY BASIC parenting programme groups, attended by parents of pre-school children displaying signs of early onset conduct disorder. Results The Leader Observation Tool (LOT) achieved high internal reliability and good code,recode and inter-rater reliability. Evidence of concurrent validity was also obtained. Conclusions Having demonstrated that the LOT is a reliable and valid measure of implementation fidelity, further research is necessary to examine the relationship between LOT scores and intervention outcome. Working together: neonatal nurse practitioners in practice

The aim of this study was to examine the relatively new role of neonatal nurse practitioners (NNPs) in the United Kingdom, comparing practice in different types of neonatal units and work undertaken by junior medical staff (JMS). Diary checklists sent to the total population of qualified NNPs in neonatal units (NNUs) and JMS in six regional centres with qualified NNPs were returned from 68 out of 109 qualified NNPs (62%), working in 50 different NNUs and from 25 out of 48 JMS (52%). Direct observations (totalling 263.5 h) were made by an experienced neonatal nurse researcher on 30 different NNPs. Frequencies of activities and specific procedures were compared between groups. Observational measures included type and duration of activity and interactions with other members of staff. NNPs were found to be undertaking a range of activities: in the NNU, which usually involved blood sampling, siting of intravenous cannulae, presenting at ward rounds and teaching. Outside the unit, NNPs attended the delivery suite and the postnatal ward. Significant differences were found in the nature and organization of their work in different types of NNUs. A comparison between NNPs and JMS showed similar activities, with greater direct involvement by NNPs in the NNU and in teaching. The diary data were supported by observations and together these are evidence of current NNP practice. Conclusion: To a large extent there is an overlap in the work of JMS and NNPs in neonatal units, but although the clinical work and areas of activity are similar, there are differences in emphasis and in work organization. An observational measure of children's behavioural style: Evidence supporting a multi-method approach to studying temperament

Abstract This study demonstrates the potential utility of the Behavioural Style Observational System (BSOS) as a new observational measure of children's behavioural style. The BSOS is an objective, short and easy to use measure that can be readily adapted to a variety of home and laboratory situations. In the present study, 160 mother,child dyads from the Concordia Longitudinal Risk Project (CLRP) were observed during an 11-min behavioural sample. Videotaped interactions were coded using the BSOS for children's mood, activity level, vocal reactivity, approach to toys, mood consistency and adaptability. Comparisons between the BSOS observational ratings and mothers' ratings of the child on the EAS Temperament Survey (EAS) provided support for modest congruence between these two measurement systems, and revealed a differential predictive pattern of children's functioning. Specifically, the observation-based BSOS predicted children's cognitive performance and adaptive behaviour during testing, whereas the mother-rated EAS predicted maternal ratings of children's internalizing and externalizing behaviour problems. Both measures were found to independently predict mothers' ratings of parenting stress. Overall, the findings imply that neither observational measures nor maternal ratings alone are sufficient to understand children's behavioural style, and that comprehensive evaluations of children's temperament should optimally include both types of measures. Teacher's PETS: a new observational measure of experiential training interactions

Government policy now stresses the importance of staff training in fostering evidence-based practice, but what is done in the name of training is rarely reported and there are few instruments with which to analyse training. Therefore, the purpose of the present study was to develop a new measure of training processes and mini-outcomes, and to provide a case study illustration. A single subject (N = 1) withdrawal design was used to assess the empirical validity of the measure. The measure (Teacher's PETS) was derived from the applied psychology literature in order to operationalize Kolb's (1984) integrative theory of experiential learning. Reliability and validity assessments were conducted. During the intervention phase of the case study baseline data from PETS were fed back to the trainer. The effectiveness of this intervention was assessed structurally and functionally, in relation to the learners, i.e. mental health staff (N = 31) receiving training in evidence-based practice (psychosocial interventions in severe mental illness). PETS was found to have very good inter-rater reliability (K = 0.84) and promising content, empirical and concurrent validity. The case study illustrated that, at baseline, the training did not correspond to a ,training workshop'. However, the intervention of feedback and modelling resulted in more appropriate training processes and outcomes, which were maintained at a 1-month follow-up assessment. It is concluded that PETS shows promise as an instrument and has the advantage of affording detailed, transactional information to improve the efficiency of training. Personality and marital satisfaction: a behavioural genetic analysis

Previous research has found that genetic and nonshared environmental factors influence marital quality (Spotts et al., 2004). The current study explored personality as a source for these genetic and environmental individual differences. A sample of 752 Swedish twin women and their spouses were used. Genetic and environmental influences were found for self-report measures of marital quality, but only environmental factors contributed to the variance of observational measures of marital quality. Wives' personality characteristics accounted for genetic and nonshared environmental variance in the wives' own marital satisfaction, their husbands' marital satisfaction, and the agreement between the spouses on the quality of their marriage. Genetic influences on the correlation between wives' genetically influenced personality characteristics and their husbands' marital satisfaction indicate a gene,environment correlation. Contrary to expectations, husbands' personality did not explain large portions of wives' marital satisfaction beyond that explained by wives' personality. This study emphasizes the importance of spousal personality to the well-being of marriages, and results are discussed within the context of three different theories regarding associations between personality and marital quality. Marital Research in the 20th Century and a Research Agenda for the 21st Century

In this article we review the advances made in the 20th century in studying marriages. Progress moved from a self-report, personality-based approach to the study of interaction in the 1950s, following the advent of general systems theory. This shift led, beginning in the 1970s, to the rapid development of marital research using a multimethod approach. The development of more sophisticated observational measures in the 1970s followed theorizing about family process that was begun in the decade of the 1950s. New techniques for observation, particularly the study of affect and the merging of synchronized data streams using observational and self-report perceptual data, and the use of sequential and time-series analyses produced new understandings of process and power. Research in the decades of the 1980s and 1990s witnessed the realization of many secular changes in the American family, including the changing role of women, social science's discovery of violence and incest in the family, the beginning of the study of cultural variation in marriages, the expansion of the measurement of marital outcomes to include longevity, health, and physiology (including the immune system), and the study of co-morbidities that accompany marital distress. A research agenda for the 21st century is then described. [source]


Do Premarital Education Programs Really Work?

Previous studies (J. S. Carroll & W. J. Doherty, 2003) have asserted that premarital education programs have a positive effect on program participants. Using meta-analytic methods of current best practices to look across the entire body of published and unpublished evaluation research on premarital education, we found a more complex pattern of results. We coded 47 studies and found that premarital education programs do not improve relationship quality/satisfaction when unpublished studies are included in the analysis, although studies that follow couples past the honeymoon stage to detect prevention effects are rare. In contrast, premarital education programs appear to be effective at improving couple communication, with studies that employed observational measures rather than self-report measures producing large effects. Still, given the mixed, modest results, there is ample room and a real need to improve the practice of premarital education. [source]


The behaviour style observation system for young children predicts teacher-reported externalizing behaviour in middle childhood

Abstract The Behaviour Style Observation System for Young Children (BSOS) was used to predict preschool-aged children's externalizing and internalizing behaviour problems in middle childhood, 3,5 years after the initial assessment. This observational measurement tool was designed to sample and assess young children's disruptive, non-compliant, and unresponsive behaviour, during a brief (11,min) observation in the child's home. In the current study, the BSOS was used to predict parent and teacher ratings of child behaviour problems after school entry in a longitudinal sample (N=81) of at-risk children at time 2. The BSOS predicted teacher-reported externalizing problems at time 2. In contrast, parent reports of behaviour problems, although correlated with repeated parent reports at time 2, were not significantly predictive of teacher-reported behaviour problems at school age. The BSOS was not associated with either parent or teacher reports of internalizing problems. These findings emphasize the importance and utility of using observational measures when examining the continuity of behaviour problems in young children over time. Biological and caregiver correlates of behavioral inhibition

Abstract The aim of this study was to explore the relationships among measures of behavioral inhibition, physiological reactivity, and caregiver attitudes. Measures comprised of heart rate; survey assessments of caregiver empathy with the child, appropriateness of caregiver developmental expectations of the child, and use of positive discipline techniques; and observational measures of behavioral inhibition were gathered in a laboratory setting on 72, 5,6-year old children. It was found that heart rate and behavioral inhibition were positively correlated (0.595), whereas behavioral inhibition was negatively correlated with caregiver attitudes of empathy (,0.356), appropriate expectations (,0.366), and use of positive discipline techniques (,0.275). However, there was no correlation between heart rate and caregiver attitudes. These data suggest that physiological reactivity to novel stimuli and caregiver attitudes of empathy, appropriate expectations, and use of positive discipline techniques conjointly influence behavioral inhibition in early childhood. Further, these three caregiver variables accounted for significant variance in children who had high reactive physiological profiles associated with inhibited behavior patterns, but did not display such patterns. Fits and starts: A mother,infant case-study involving intergenerational violent trauma and pseudoseizures across three generations

This case-study presents in detail the clinical assessment of a 29-year-old mother and her daughter who first presented to infant mental health specialists at age 16 months, with a hospital record suggesting the presence of a dyadic disturbance since age eight months. Data from psychiatric and neurological assessments, as well as observational measures of child and mother, are reviewed with attention to issues of disturbed attachment, intergenerational trauma, and cultural factors for this innercity Latino dyad. Severe maternal affect dysregulation in the wake of chronic, early-onset violent-trauma exposure manifested as psychogenic seizures, referred to in the mother's native Spanish as "ataques de nervios," the latter, an idiom of distress, commonly associated with childhood trauma and dissociation. We explore the mechanisms by which the mothers' reexperiencing of violent traumatic experience, together with physiologic hyperarousal and associated negative affects, are communicated to the very young child and the clinician-observer via action and language from moment to moment during the assessment process. High risk studies and developmental antecedents of anxiety disorders,

Abstract The past two decades have witnessed significant growth in our understanding of the developmental antecedents of anxiety disorders. In this article, we review studies of offspring at risk for anxiety disorders, longitudinal studies of the course of anxiety disorders in clinical, epidemiologic, and at-risk samples, studies of hypothesized temperamental risk factors for anxiety, and give a brief overview of the literature on environmental risk factors. Clear developmental antecedents to anxiety disorders identified include (1) childhood anxiety disorders [in particular, separation anxiety and overanxious disorder/general anxiety disorder (GAD)], (2) behavioral inhibition which predicts later social phobia, (3) anxiety sensitivity which predicts later panic disorder, and (4) negative affectivity, which predicts a spectrum of psychopathology including anxiety disorders. Further prospective studies are needed to examine the roles of environmental factors such as parenting practices, peer influences, stressful life events, and perinatal stressors. Future studies could benefit from (1) beginning earlier in development and following individuals into adulthood, (2) assessing the overlap between multiple temperamental constructs, (3) greater use of observational measures of temperament and of parent,child and peer interactions, (4) greater attention to parental psychopathology which may confound associations noted, (5) exploration of other features of anxiety disorders (neurofunctional correlates, cognitive features, other aspects of emotional regulation) as potential precursors, and (6) intervention studies exploring whether modifying developmental antecedents can alter the course of anxiety disorders. © 2008 Wiley-Liss, Inc. Links Between Systems of Inhibition From Infancy to Preschool Years

Links between early and late emerging inhibitory characteristics of young children are poorly understood. Based on existing evidence, this study proposed an indirect link between reactive inhibition to novelty (fearfulness) and later emerging effortful inhibition of prepotent response tendencies. A model was tested that posits that fearful children have opportunistic advantages in developing a nonimpulsive approach style in contexts that are not imbued with salient novelty cues. That nonimpulsive approach style, in turn, facilitates future effortful inhibition. Longitudinal panel data, all based on observational measures, at 9, 14, 22, 33, and 45 months were used to support the proposed indirect link. The implications for understanding regulatory influences of fearful behavior and developmental complexity in child characteristics are discussed. [source]
