Comparison of different near-infrared spectroscopic cerebral oxygenation indices with central venous and jugular venous oxygenation saturation in children

Summary Background:, We compared two different near-infrared spectrophotometers: cerebral tissue oxygenation index (TOI) measured by NIRO 200 and regional cerebral oxygenation index (rSO2) measured by INVOS 5100 with venous oxygen saturation in the jugular bulb (SjO2) and central SvO2 from the superior caval vein (SVC) during elective cardiac catheterization in children. Methods:, A prospective observational clinical study in 31 children with congenital heart defects in a catheterization laboratory was undertaken. TOI was compared with SjO2 in the left jugular bulb and with SvO2. rSO2 was compared with SjO2 from the right jugular bulb and SvO2. Linear regression analysis and Pearson's correlation coefficient were calculated and Bland,Altman analyses were performed. Results:, Cerebral TOI and SjO2 were significantly correlated (r = 0.56, P < 0.0001), as well as TOI and SvO2 with r = 0.74 (P < 0.0001). Bland,Altman plots showed a mean bias of ,4.3% with limits of agreement of 15.7% and ,24.3% for TOI and SjO2 and a mean bias of ,4.9% with limits of agreement of 10.3% and ,20.1% for TOI and SvO2. Cerebral rSO2 and SjO2 showed a significant correlation (r = 0.83, P < 0.0001) and rSO2 and SvO2 showed excellent correlation with r = 0.93 (P < 0.0001). Bland,Altman plots showed a mean bias of ,5.2% with limits of agreement of between 8.4% and ,18.8% for rSO2 and SjO2 and a mean bias of 5.6% with limits of agreement of 13.4% and ,2.2% for rSO2 and SvO2. Conclusions:, Both near-infrared spectroscopy devices demonstrate a significant correlation with SjO2 and SvO2 values; nevertheless both devices demonstrate a substantial bias of the measurements to both SjO2 and SvO2. Obstructive Sleep Apnea Syndrome: A Comparison Between Far-East Asian and White Men

Abstract Objectives To investigate the possible differences between Far-East Asian men and white men in obstructive sleep apnea syndrome (OSAS). Study Design Prospective nonrandomized controlled study. Methods This study compared consecutive Far-East Asian men with OSAS (n = 50) with two selected groups of White men with OSAS (n = 50 in each group). One group of white men was controlled for age, respiratory disturbance index (RDI), and minimum oxygenation saturation (LSAT). Another group was controlled for age and body mass index (BMI). Cephalometric analysis was performed on all subjects. Results The majority of the Far-East Asian men were found to be nonobese (mean BMI, 26.7 ± 3.8) but had severe OSAS (mean RDI, 55.1 ± 35.1). When controlled for age, RDI, and LSAT, the white men were substantially more obese (mean BMI, 29.7 ± 5.8, P = .0055). When controlled for age and BMI, the white men had less severe illness (RDI, 34.1 ± 17.9, P = .0001). Although the posterior airway space and the distance from the mandibular plane to hyoid bone were less abnormal in the Far-East Asian men, the cranial base dimensions were significantly decreased. Conclusions The majority of the Far-East Asian men in this study were found to be nonobese, despite the presence of severe OSAS. When compared with white men, Far-East Asian men were less obese but had greater severity of OSAS. There may be differences in obesity and craniofacial anatomy as risk factors in these two groups. 2322: Reproducibility of retinal vessel hemoglobin oxygenation saturation in healthy subjects

Purpose Only recently systems for the measurement of hemoglobin oxygenation saturation in retinal vessels became commercially available. In the present study a fundus camera equipped with a dual wavelength transmission filter and a CCD camera was used (Imedos, Jena, Germany). Two monochromatic fundus images at 548 and 610 nm were recorded simultaneously and the oxygen saturation (SaO2) was calculated based on the optical density ratio. Methods A study on test/retest, short-term and day-to-day reproducibility of the technique was done in 14 healthy subjects. To assess test/retest variability fundus images were taken at each measurement cycle. To assess short-term reproducibility 2 such cycles of 3 fundus images were done within 15 minutes. Finally, this cycle of measurements was done on the next day to assess day-to-day reproducibility. Data were evaluated separately for retinal branch arteries and branch veins. The reproducibility was calculated using the standard deviation (SD) of the measurements. Results In retinal branch veins the SaO2 was 49±9% (range 32-69%, 38 vessels) and in retinal branch arteries 91±8% (range 77-112%, 40 vessels). The test/retest SD was 5.4±3.7% in retinal veins and 3.0±2.3% in retinal arteries. The short term SD was 4.7±3.7% in retinal veins and 2.9±2.1% in retinal arteries. The day-to-day SD was 6.2±4.1% in retinal veins and 4.0±2.8% in retinal arteries. Conclusion Our data indicate that the test/retest, short-term and day-to-day reproducibility of the technique is acceptable. Hence, the system may be adequate to follow oxygen saturation during disease processes. The data obtained in retinal branch arteries indicates, however, that the validity of measurements requires further studies. [source]
