    Teaching Culturally Appropriate Care: A Review of Educational Models and Methods

    Abstract The disparities in health care and health outcomes between the majority population and cultural and racial minorities in the United States are a problem that likely is influenced by the lack of culturally competent care. Emergency medicine and other primary-care specialties remain on the front lines of this struggle because of the nature of their open-door practice. To provide culturally appropriate care, health care providers must recognize the factors impeding cultural awareness, seek to understand the biases and traditions in medical education potentially fueling this phenomenon, and create a health care community that is open to individuals' otherness, thus leading to better communication of ideas and information between patients and their health care providers. This article highlights the rationale for and current problems in teaching cultural competency and examines several different models implemented to teach and promote cultural competency along the continuum of emergency medicine learners. However, the literature addressing the true efficacy of such programs in leading to long-lasting change and improvement in minority patients' clinical outcomes remains insufficient. [source]

    Is the Open Method of Coordination Appropriate for Organising Activities at European Level in Sensitive Policy Areas?

    This article addresses the question of the relevance of the most recent soft policy instrument of the EU, the open method of coordination (OMC), for organising actions at European level in politically sensitive areas. In addition to describing its origins and operational principles, we will compare its application to the areas of employment and social inclusion. Two hypotheses make up the structure of the text. The first is that the discourses produced in the framework of OMC in the areas of employment and social inclusion are broad enough to cater to the different welfare models, but that the changes to be made by the Member States to be in line with the European discourses differ considerably, depending on their welfare state family and their initial situation. The second is that the form of OMC is variable, depending on policy area. Our conclusions confirm both of these hypotheses. [source]


    Systematic review on embracing cultural diversity for developing and sustaining a healthy work environment in healthcare

    Abstract Objectives, The objective of this review was to evaluate evidence on the structures and processes that support development of effective culturally competent practices and a healthy work environment. Culturally competent practices are a congruent set of workforce behaviours, management practices and institutional policies within a practice setting resulting in an organisational environment that is inclusive of cultural and other forms of diversity. Inclusion criteria, This review included quantitative and qualitative evidence, with a particular emphasis on identifying systematic reviews and randomised controlled trials. For quantitative evidence, other controlled, and descriptive designs were also included. For qualitative evidence, all methodologies were considered. Participants were staff, patients, and systems or policies that were involved or affected by concepts of cultural competence in the nursing workforce in a healthcare environment. Types of interventions included any strategy that had a cultural competence component, which influenced the work environment, and/or patient and nursing staff in the environment. The types of outcomes of interest to this review included nursing staff outcomes, patient outcomes, organisational outcomes and systems level outcomes. Search strategy, The search sought both published and unpublished literature written in the English language. A comprehensive three-step search strategy was used, first to identify appropriate key words, second to combine all optimal key words into a comprehensive search strategy for each database and finally to review the reference lists of all included reviews and research reports. The databases searched were CINAHL, Medline, Current Contents, the Database of Abstracts of Reviews of Effectiveness, The Cochrane Library, PsycINFO, Embase, Sociological Abstracts, Econ lit, ABI/Inform, ERIC and PubMed. The search for unpublished literature used Dissertation Abstracts International. Methodological quality, Methodological quality was independently established by two reviewers, using standardised techniques from the Joanna Briggs Institute (JBI) System for the Unified Management, Assessment and Review of Information (SUMARI) package. Discussion with a third reviewer was initiated where a low level of agreement was identified for a particular paper. Following inclusion, data extraction was conducted using standardised data extraction tools from the JBI SUMARI suite for quantitative and qualitative research. Data synthesis was performed using the JBI Qualitative Assessment and Review Instrument and JBI Narrative, Opinion and Text Assessment and Review Instrument software to aggregate findings by identifying commonalities across texts. Quantitative data were presented in narrative summary, as statistical pooling was not appropriate with the included studies. Results, Of the 659 identified papers, 45 were selected for full paper retrieval, and 19 were considered to meet the inclusion criteria for this review. The results identified a number of processes that would contribute to the development of a culturally competent workforce. Appropriate and competent linguistic services, and intercultural staff training and education, were identified as key findings in this review. Conclusions, The review recommends that health provider agencies establish links with organisations that can address needs of culturally diverse groups of patients, include cultural competence in decision support systems and staff education as well as embed them in patient brochures and educational materials. The review also concluded that staff in-service programs consider the skills needed to foster a culturally competent workforce, and recruitment strategies that also explicitly address this need.     Evaluating Commodity Market Efficiency: Are Cointegration Tests Appropriate?

    This paper investigates the claim that the finding of cointegration between commodity spot and lagged futures rates reflects the existence of commodity arbitrage and not, as is generally accepted, long-run market efficiency. The methodology of Kellard et al. (1999) is employed to match spot and lagged futures rates correctly for the UK wheat futures contract traded at LIFFE. Bi-variate analysis shows that spot and lagged futures rates are cointegrated with the vector (1, -1), a necessary condition for market efficiency. However, at variance with asymptotic theory, in a tri-variate VECM estimation, the spot rate, lagged futures rate and lagged domestic interest rate are shown to be cointegrated with the vector (1, ,1, 1). The "cointegration" paradox is explained by investigating the relative magnitudes of the forecast error and the domestic interest rate. The small sample results demonstrate that it is impossible to distinguish between the influence of commodity arbitrage and the existence of market efficiency using cointegration-based tests. In summary, this work implies that such tests are not wholly appropriate for evaluating commodity market efficiency. [source]


    Unit cost of Mohs and Dermasurgery Unit

    Abstract Background, Appropriate pricing for medical services of not-for-profit hospital is necessary. The prices should be fair to the public and should be high enough to cover the operative costs of the organization. Objective, The purpose of this study was to determine the cost and unit cost of medical services performed at the Mohs and Dermasurgery Unit (MDU), Department of Dermatology, The University of Texas , MD Anderson Cancer Center, Houston, TX from the healthcare provider's perspective. Methods, MDU costs were retrieved from the Financial Department for fiscal year 2006. The patients' statistics were acquired from medical records for the same period. Unit cost calculation was based on the official method of hospital accounting. Results, The overall unit cost for each patient visit was $673.99 United States dollar (USD). The detailed unit cost of nurse visit, new patient visit, follow-up visit, consultation, Mohs and non-Mohs procedure were, respectively, $368.27, $580.09, $477.82, $585.52, $1,086.12 and $858.23 USD. With respect to a Mohs visit, the unit cost per lesion and unit cost per stage were $867.89 and $242.30 USD respectively. Conclusions, Results from this retrospective study provide information that may be used for pricing strategy and resource allocation by the administrative board of MDU. [source]


    The Politics of Recognition in Culturally Appropriate Care

    Over the last 20 years, the concept of culturally appropriate health care has been gradually gaining popularity in medicine and public health. In calling for health care that is culturally appropriate, minority groups seek political recognition of often racialized constructions of cultural difference as they intervene in health care planning and organization. Based on interview narratives from people involved in community organizing to establish a federally funded community health center in a mid-size New England city, I chart the emergence of a language of "culturally appropriate health care" in language used to justify the need for a health center. An identity model of recognition underlies the call for ethnic resemblance between patient and provider seen in many culturally appropriate care programs. I contrast this model of health care with earlier calls for community access and control by activists in the 1970s and explore the practical and theoretical implications of each approach. [source]


    Oncological and Aesthetic Considerations of Conservational Surgery for Multifocal/Multicentric Breast Cancer

    Abstract:, Conventional indications for mastectomy (MX) reflect circumstances where breast conserving therapy (BCT) could compromise oncological or cosmetic outcome. MX continues to be recommended for the majority of women with multiple lesions within the same breast. In this article, we review the oncological safety and aesthetic considerations of BCT in the context of multifocal (MF) or multicentric (MC) breast cancer. Literature review facilitated by Medline and PubMed databases. Published studies have reported divergent results regarding the oncological adequacy of BCT in the management of MF or MC disease. Earlier studies demonstrated high rates of local recurrence (LR) for BCT. More recent series have found BCT to be comparable to MX in terms of LR, distant failure, disease free and overall survival. Few studies have adequately evaluated cosmetic outcomes following BCT for MF or MC breast cancer. Contemporary oncoplastic techniques have extended the clinical utility of BCT and are of particular relevance to breast conservation in the context of MF or MC lesions. Appropriate case selection, preoperative oncological and aesthetic planning, satisfactory clearance of the surgical margins and adjuvant radiotherapy are of paramount importance. In the absence of level-1 guidance concerning the management of women with MF or MC disease, each case requires discussion with regard to tumor and patient related factors in the context of the multidisciplinary team. In selected patients with MF or MC disease, BCT is oncologically safe and cosmetically acceptable. Uniformity of practice and the establishment of a standard of care will require an evidence-base from prospective studies. [source]


    Implantable Cardioverter-Defibrillator Shock: Appropriate or Inappropriate?

    A 76-year-old female with a single chamber implantable cardioverter-defibrillator implanted for secondary prevention was referred due to multiple discharges. The device was programmed for ventricular tachycardia (VT) detection at 400 ms, fast VT detection at 280 ms, and ventricular fibrillation detection at 320 ms. Antitachycardia pacing (ATP) during charge was enabled. Interrogation revealed a VT episode with a mean cycle length of 270 ms, which was successfully terminated with ATP during charge. Seconds later, the device delivered a shock. This case illustrates the importance of understanding programming algorithms as part of troubleshooting when facing a scenario of device discharge.     Abductive Diagnosis Using Time-Objects: Criteria for the Evaluation of Solutions

    Diagnostic problem solving aims to account for, or explain, a malfunction of a system (human or other). Any plausible potential diagnostic solution must satisfy some minimum criteria relevant to the application. Often there will be several plausible solutions, and further criteria will be required to select the "best" explanation. Expert diagnosticians may employ different, complex criteria at different stages of their reasoning. These criteria may be combinations of some more primitive criteria, which therefore should be represented separately and explicitly to permit their flexible and transparent combined usage. In diagnostic reasoning there is a tight coupling between the formation of potential solutions and their evaluation. This is the essence of abductive reasoning. This article presents an abductive framework for diagnostic problem solving. Time-objects, an association of a property and an existence, are used as the representation formalism and a number of primitive, general evaluation criteria into which time has been integrated are defined. Each criterion provides an intuitive yardstick for evaluating the space of potential solutions. The criteria can be combined as appropriate for particular applications to define plausible and best explanations. The central principle is that when time is diagnostically significant, it should be modeled explicitly to enable a more accurate formulation and evaluation of diagnostic solutions. The integration of time and primitive evaluation criteria is illustrated through the Skeletal Dysplasias Diagnostician (SDD) system, a diagnostic expert system for a real-life medical domain. SDD's notions of plausible and best explanation are reviewed so as to show the difficulties in formalizing such notions. Although we illustrate our work by medical problems, it has been motivated by consideration of problems in a number of other domains (fermentation monitoring, air and ground traffic control, power distribution) and is intended to be of wide applicability.     Growth hormone in short children: beyond medicine?

    The indications for growth hormone (GH) treatment in non-GH-deficient short children are in debate, with some arguing that this treatment does not belong solely in the medical domain. We describe three different approaches to the issue, and argue that neither a disease-oriented nor client-oriented approach is sufficient. Both lead to withdrawal of medical interventions or to an undesirable application. Conclusion: An approach focusing on suffering as an indication for treatment of short stature is the most appropriate. The challenge is to develop proper tools by which to evaluate suffering and the efficacy of GH treatment in these children in order to relieve or prevent suffering. [source]


    Superresolution Reflectance Fields: Synthesizing images for intermediate light directions

    Reflectance Fields; Image-Based Lighting; Upsampling Abstract Captured reflectance fields tend to provide a relatively coarse sampling of the incident light directions. As a result, sharp illumination features, such as highlights or shadow boundaries, are poorly reconstructed during relighting; highlights are disconnected, and shadows show banding artefacts. In this paper, we propose a novel interpolation technique for 4D reflectance fields that reconstructs plausible images even for non-observed light directions. Given a sparsely sampled reflectance field, we can effectively synthesize images as they would have been obtained from denser sampling. The processing pipeline consists of three steps: (1) segmentation of regions where intermediate lighting cannot be obtained by blending, (2) appropriate flow algorithms for highlights and shadows, plus (3) a final reconstruction technique that uses image-based priors to faithfully correct errors that might be introduced by the segmentation or flow step. The algorithm reliably reproduces scenes that contain specular highlights, interreflections, shadows or caustics. [source]


    Mobile Construction Supply Chain Management Using PDA and Bar Codes

    However, extending the construction project control system to job sites is not considered efficient because using notebooks in a harsh environment like a construction site is not particularly a conventional practice. Meanwhile, paper-based documents of the site processes are ineffective and cannot get the quick response from the office and project control center. Integrating promising information technologies such as personal digital assistants (PDA), bar code scanning, and data entry mechanisms, can be extremely useful in improving the effectiveness and convenience of information flow in construction supply chain control systems. Bar code scanning is appropriate for several construction applications, providing cost savings through increased speed and accuracy of data entry. This article demonstrates the effectiveness of a bar-code-enabled PDA application, called the mobile construction supply chain management (M-ConSCM) System, that responds efficiently and enhances the information flow between offices and sites in a construction supply chain environment. The advantage of the M-ConSCM system lies not only in improving the efficiency of work for on-site engineers, but also providing the Kanban-like visual control system for project participants to control the whole project.     Evaluation of Uncertainties Associated with Geocoding Techniques

    Geocoded data play a major role in numerous engineering applications such as transportation and environmental studies where geospatial information systems (GIS) are used for spatial modeling and analysis as they contain spatial information (e.g., latitude and longitude) about objects. The information that a GIS produces is impacted by the quality of the geocoded data (e.g., coordinates) stored in its database. To make appropriate and reasonable decisions using geocoded data, it is important to understand the sources of uncertainty in geocoding. There are two major sources of uncertainty in geocoding, one related to the database that is used as a reference data set to geocode objects and one related to the interpolation technique used. Factors such as completeness, correctness, consistency, currency, and accuracy of the data in the reference database contribute to the uncertainty of the former whereas the specific logic and assumptions used in an interpolation technique contribute to the latter. The primary purpose of this article is to understand uncertainties associated with interpolation techniques used for geocoding. In doing so, three geocoding algorithms were used and tested and the results were compared with the data collected by the Global Positioning System (GPS). The result of the overall comparison indicated no significant differences between the three algorithms. [source]


    Cross-correlated and conventional dipolar carbon-13 relaxation in methylene groups in small, symmetric molecules

    Abstract A theory for dipolar cross-correlated relaxation processes in AMX or AX2 spin system, with special reference to 13C-methylene groups, is reviewed briefly. Simple experiments and protocols for measuring the transfer rates between the carbon-13 Zeeman order and the three-spin order, and for their analogues in the transverse plane, are discussed using a concentrated solution of the disaccharide trehalose as a model system. Experimental data sets consisting of conventional carbon-13 relaxation parameters (T1, T2, and NOE), along with the cross-correlated relaxation rates, are also presented for some small, rigid, polycyclic molecules. These data are interpreted using spectral density functions appropriate to spherical or symmetric tops reorienting according to small-step rotational diffusion model.     Investigation of prolonged neonatal jaundice

    Jaundice persisting beyond 14 d of age (prolonged jaundice) can be a sign of serious underlying liver disease. Protocols for investigating prolonged jaundice vary in complexity and the yield from screening has not been assessed. In order to address these issues, we carried out a prospective study of term infants referred to our neonatal unit with prolonged jaundice over an 18 mo period. Infants were examined by a paediatrician and had the following investigations: a total and conjugated serum bilirubin, liver function tests, full blood count, packed cell volume, group and Coombs' test, thyroid function tests, glucose-6-phosphate dehydrogenase levels and urine for culture. One-hundred-and-fifty-four infants were referred with prolonged jaundice out of 7139 live births during the study period. Nine infants were referred to other paediatric specialties. One infant had a conjugated hyperbilirubinaemia, giving an incidence of conjugated hyperbilirubinaemia of 0.14 per 1000 live births. Diagnoses included: giant cell hepatitis (n= 1), hepatoblastoma (n= 1), trisomy 9p (n= 1), urinary tract infections (n= 2), glucose-6-phosphate dehydrogenase deficiency (n= 3) and failure to regain birthweight (n= 1). Conclusions: In conclusion, a large number of infants referred to hospital for prolonged jaundice screening had detectable problems. The number of investigations may safely be reduced to: a total and conjugated bilirubin, packed cell volume, glucose-6-phosphate dehydrogenase level (where appropriate), a urine for culture and inspection of a recent stool sample for bile pigmentation. Clinical examination by a paediatrician has a vital role in the screening process. [source]

    Abstract A unified scale is recommended for reporting the NMR chemical shifts of all nuclei relative to the 1H resonance of tetramethylsilane. The unified scale is designed to provide a precise ratio, ,, of the resonance frequency of a given nuclide to that of the primary reference, the 1H resonance of tetramethylsilane (TMS) in dilute solution (volume fraction, , < 1%) in chloroform. Referencing procedures are discussed, including matters of practical application of the unified scale. Special attention is paid to recommended reference samples and values of , for secondary references on the unified scale are listed, many of which are the results of new measurements. Some earlier recommendations relating to the reporting of chemical shifts are endorsed. The chemical shift, ,, is redefined to avoid previous ambiguities but to leave practical usage unchanged. Relations between the unified scale and recently published recommendations for referencing in aqueous solutions (for specific use in biochemical work) are discussed, as well as the special effects of working in the solid state with magic-angle spinning. In all, nine new recommendations relating to chemical shifts are made. Standardized nuclear spin data are also presented in tabular form for the stable (and some unstable) isotopes of all elements with non-zero quantum numbers. It begins with the premise that to say in absolute terms that mediation is or is not an effective way of dealing with power imbalance is to ignore the complexity of both the concept of power and the range of processes that can be deemed mediation. This article examines sources of power in an effort to develop a clear, yet thorough, understanding of negotiating power.     The Need to Rationalize and Prioritize Threatening Processes Used to Determine Threat Status in the IUCN Red List

Although the system used to determine risk of extinction is rigorously and objectively applied, the list of threatening processes affecting a species is far more subjectively determined and has not had adequate review. I reviewed the threats listed in the IUCN Red List for randomly selected groups within the three most threatened orders of mammals: Artiodactyla, Carnivora, and Primates. These groups are taxonomically related and often ecologically similar, so I expected they would suffer relatively similar threats. Hominoid primates and all other terrestrial fauna faced similar threats, except for bovine artiodactyls and large, predatory carnivores, which faced significantly different threats. Although the status of bovines and hominoids and the number of threats affecting them were correlated, this was not the case for large carnivores. Most notable, however, was the great variation in the threats affecting individual members of each group. For example, the endangered European bison (Bison bonasus) has no threatening processes listed for it, and the lion (Panthera leo) is the only large predator listed as threatened with extinction by civil war. Some threatening processes appear spurious for the conservation of the species, whereas other seemingly important factors are not recorded as threats. The subjective nature of listing threatening processes, via expert opinion, results in substantial biases that may be allayed by independent peer review, use of technical manuals, consensus among multiple assessors, incorporation of probability modeling via decision-tree analysis, and adequate coordination among evaluators. The primary focus should be on species-level threats rather than population-level threats because the IUCN Red List is a global assessment and smaller-scale threats are more appropriate for national status assessments. Until conservationists agree on the threats affecting species and their relative importance, conservation action and success will be hampered by scattering scarce resources too widely and often by implementing conflicting strategies.     Beyond Species Richness: Community Similarity as a Measure of Cross-Taxon Congruence for Coarse-Filter Conservation

    Species richness is only one measure of species diversity, however, and recent studies suggest that investigations of cross-taxon congruence should consider a broader range of assessment techniques. The cross-taxon congruence of community similarity between sites among taxa has rarely been examined and may be the most relevant measure of species diversity in the context of coarse-filter conservation strategies. We examined cross-taxon congruence patterns of species richness and community similarity (Bray-Curtis similarity) among birds, butterflies, and vascular plants in montane meadow habitats in the Greater Yellowstone Ecosystem. Although patterns of species richness (Spearman rank correlation) varied between taxa, we consistently found a positive correlation in community similarity (Mantel test) between all pair-wise comparisons of the three taxa (e.g., sites with similar bird communities also had similar butterfly communities). We suggest that the success of a surrogate taxon depends on the technique used to assess surrogacy and the specific approach to conservation planning. In the context of coarse-filter conservation, measures of community similarity may be more appropriate than measures of species richness. Furthermore, the cross-taxon congruency of community similarity in our study suggests that coarse-filter conservation may be tenable in montane meadow communities.     Nonindigenous Species: Ecological Explanation, Environmental Ethics, and Public Policy

    Misunderstandings and tension exist regarding the science, values, environmental ethics, and public policy relevant to invasive species, which are the subset of nonindigenous species that cause economic or environmental damage. Although there is a natural background rate at which species invasions occur, it is much lower than the current human-induced rates at which species are being moved around the globe. Contrary to some recently voiced opinions , the fact that some species invasions occur without human assistance does not confer acceptability on all species invasions. Also, despite claims to the contrary, the reductions of native biodiversity caused by nonindigenous species are large and well documented. Even if that were not true, an emphasis on species numbers alone as a metric for the impact of nonindigenous species does not adequately incorporate the high value many humans place on the uniqueness of regional biota. Because regional biota are being homogenized by species invasions, it has become an appropriate and official public policy goal in the United States to reduce the harm done by invasive species. The goal is not, however, a reduction of numbers of nonindigenous species per se, as recently claimed by some authors, but a reduction in the damage caused by invasive species, including many sorts of environmental and economic damage. A major challenge remaining for ecology, environmental ethics, and public policy is therefore the development of widely applicable risk-assessment protocols that are acceptable to diverse constituencies. Despite apparent disagreements among scholars, little real disagreement exists about the occurrence, effects, or public-policy implications of nonindigenous species.     Studying Biodiversity on Private Lands

    Private lands harbor a great amount of biodiversity, including at least some habitat for 95% of the federally listed species in the United States. It is important to conduct conservation biology research on private lands, but our review of the literature indicates that few conservation-oriented field studies are conducted on private property. Based on our success in obtaining permission to conduct research on 43 land parcels in Sonoma County, California, we developed methods to enhance a conservation biologist's chance of obtaining permission to work on private lands. We provide guidelines for researchers to conduct studies successfully on private land with the goal of improving access, data collection, and relationships with private landowners. We also discuss constraints researchers face, such as designing studies appropriate for working on privately owned parcels. In light of the importance of these lands to biodiversity conservation, greater effort should be made to conduct research on private lands.     A Transdisciplinary Approach to Conservation Land Acquisition

    I integrated principles of conservation biology, ecological economics, and political science to outline an approach to conservation land acquisition in the United States. American political economy, especially, has profound implications for conservation land acquisition, yet these implications have been largely neglected by public land conservation agencies. I derived three general recommendations for the conservation of biodiversity via land acquisition: (1) relatively inexpensive lands in relatively intact ecosystems should be prioritized for acquisition, (2) fee-title acquisition should be favored over easement acquisition, and (3) low-lying coastal properties should receive lower priority. These recommendations contribute to an ecologically macroeconomic approach to conservation land acquisition, and each will become more appropriate as the sizes of the U.S. and global economies increase. Ultimately, however, the conservation of biodiversity will require a new political economy predicated on sustainability rather than growth. Resumen: La adquisición de tierras es uno de los aspectos claves para la conservación de la biodiversidad y es una empresa transdisciplinaria que requiere de la consideración de fenómenos sociales y naturales.     Evaluation of 2 self-administered questionnaires to ascertain dermatitis among metal workers and its relation with exposure to metalworking fluids

    We performed an exploratory study to evaluate 2 self-administered questionnaires assessing hand dermatitis and investigate a possible exposure,response relationship between dermal exposure to semi-synthetic metalworking fluids (SMWF) and dermatitis. In a cross-sectional survey on dermatitis, a symptom-based questionnaire and a picture-based skin screening list were applied in 80 SMWF-exposed workers and 67 referents. To evaluate the accuracy of the questionnaires, 47 subjects were examined by a dermatologist. Dermal exposure levels to SMWF were assessed on hands, forearms, and face with a observational method that was validated using a fluorescent tracer method. The symptom-based questionnaire had a relatively high sensitivity (0.86) but moderate specificity (0.64), and the skin screening list had a low sensitivity (0.36) and a relatively high specificity (0.84). The skin screening list seemed to represent the more severe cases of dermatitis and showed a significant relation with exposure for dermatitis on hands, forearms, or face. In epidemiological surveys where workers are not seen by a dermatologist, the skin screening list seems to be more appropriate to detect cases of dermatitis, as its higher specificity results in less false positives. Alternatively, it would be preferable applying the symptom-based questionnaire; workers with symptoms should be seen by a dermatologist to identify false positives. [source]


    The relation between the localization of foot dermatitis and the causative allergens in shoes: a 13-year retrospective study

    The purpose of this retrospective study was to test whether the initial pattern of clinical presentation of shoe dermatitis could indicate the causative allergen(s) and to estimate the odds on foot dermatitis in patients with a positive patch test versus those with a negative patch-test result. Between 1990 and 2002, 8543 patients were patch tested with the standard series (and additional allergens, if appropriate). Of them, 1168 (14%) had been referred because of foot dermatitis and 474 of these patients (5.5% of the total group) presented a positive reaction to one or more substances related to shoes. We found that 6 standard allergens in the male group and 8 standard allergens in the female group were statistically significant for the shoe dermatitis group. The data showed a relationship between the distribution pattern of the foot lesions and most of the allergens. These results have clinical applications since the gender of the patients and the localization of the foot eruptions can, indeed, indicate what allergen is involved. [source]

State-level panel data analysis provides evidence that, ceteris paribus, increasing the federal matching formula has a negative impact on prenatal care. This aggregate result masks significant differences between high- and low-spending states and appears to be driven by the high-spending states thus implying that a 2-track approach to Medicaid funding may be more appropriate than the current system.     Abstract The lymphatic system is a complex network of lymph vessels, lymphatic organs and lymph nodes. Traditionally, imaging of the lymphatic system has been based on conventional imaging methods like computed tomography (CT) and magnetic resonance imaging (MRI), whereby enlargement of lymph nodes is considered the primary diagnostic criterion for disease. This is particularly true in oncology, where nodal enlargement can be indicative of nodal metastases or lymphoma. CT and MRI on their own are, however, anatomical imaging methods. Newer imaging methods such as positron emission tomography (PET), dynamic contrast-enhanced MRI (DCE-MRI) and color Doppler ultrasound (CDUS) provide a functional assessment of node status. None of these techniques is capable of detecting flow within the lymphatics and, thus, several intra-lymphatic imaging methods have been developed. Direct lymphangiography is an all-but-extinct method of visualizing the lymphatic drainage from an extremity using oil-based iodine contrast agents. More recently, interstitially injected intra-lymphatic imaging, such as lymphoscintigraphy, has been used for lymphedema assessment and sentinel node detection. Nevertheless, radionuclide-based imaging has the disadvantage of poor resolution. This has lead to the development of novel systemic and interstitial imaging techniques which are minimally invasive and have the potential to provide both structural and functional information; this is a particular advantage for cancer imaging, where anatomical depiction alone often provides insufficient information. At present the respective role each modality plays remains to be determined. Indeed, multi-modal imaging may be more appropriate for certain lymphatic disorders. The field of lymphatic imaging is ever evolving, and technological advances, combined with the development of new contrast agents, continue to improve diagnostic accuracy.     Sheath Physics and Boundary Conditions for Edge Plasmas

    Abstract The boundary conditions of mass, momentum, energy, and charge appropriate for fluid formulations of edge plasmas are surveyed. We re-visit the classic problem of 1-dimensional flow, and note that the "Bohm sheath criterion" is requirement of connectivity of the interior plasma with the external world, not the result of termination of the plasma by a wall. We show that the nature of the interior plasma solution is intrinsically different for ion sources that inject above and below the electron sound speed. We survey the appropriate conditions to apply, and resultant fluxes, for a magnetic field obliquely incident on a wall, including the presence of drifts and radial transport. We discuss the consequences of toroidal asymmetries in wall properties, as well as experimental tests of such effects.     What really happens with the electron gas in the famous Franck-Hertz experiment?

    Abstract The interpretation of the anode current characteristics obtained in the famous Franck-Hertz experiment of 1914 led to the verification of Bohr's predictions of quantised atomic states. This fundamental experiment has been often repeated, and nowadays is generally part of the curriculum in modern physics education. However, the interpretation of the experiment is typically based upon significant simplifying assumptions, some quite unrealistic. This is the case especially in relation to the kinetics of the electron gas, which is in reality quite complex, due mainly to non-uniformities in the electric field, caused by a combination of accelerating and retarding components. This non-uniformity leads to a potential energy valley in which the electrons are trapped. The present state of understanding of such effects, and their influence upon the anode characteristics, is quite unsatisfactory. In this article a rigorous study of a cylindrical Franck-Hertz experiment is presented, using mercury vapour, the aim being to reveal and explain what really happens with the electrons under realistic experimental conditions. In particular, the anode current characteristics are investigated over a range of mercury vapour pressures appropriate to the experiment to clearly elaborate the effects of elastic collisions (ignored in typical discussions) on the power budget, and the trapping of electrons in the potential energy valley.
