Selected AbstractsThe Effects of the Absence of Emergency Medicine Residents in an Academic Emergency DepartmentACADEMIC EMERGENCY MEDICINE, Issue 11 2002Daniel French MD Objective: What are the quality effects of an emergency medicine (EM) residency, and the associated 24/7 supervision of residents by faculty, in an academic emergency department (ED)? The authors evaluated activity and quality indicators when there were no EM residents present. The hypothesis of the study was that there was no difference between the patient care provided by faculty supervising EM residents and that with an alternative model without EM residents (AbsenceEMResident). Methods: To support the weekly residency educational program (Thursday), EM residents are not scheduled clinically for a 24-hour period (ConfDay). Emergency medicine resident coverage (mean 62.7 hours) was replaced with incremental faculty and mid-level providers (mean 41.0 hours). This study was limited to adult patients (22,527 visits of 39,190 ED total) for six months (January,June 2001) and compared indicators for ConfDay (n = 23) with all other days (NotConfDay, n = 158). Results: Comparing ConfDay (2,842 visits) with NotConfDay (19,685 visits), there was no difference in mean daily visits, inpatient admissions, intensive care unit admissions, or emergency medical services arrivals. ConfDay decision-to-admit time (333 vs. 313 min, p = 0.03) and length of stay for admissions (490 vs. 445 min, p = 0.000) were longer, with no difference for treat/release patients. There was no difference in the numbers of laboratory or radiology tests, consultations, unscheduled return visits, or patient satisfaction. Conclusion: During the study period, there was no measurable difference for most of the quality indicators studied. The AbsenceEMResident model is less efficient in admitting patients. Faculty supervision results in the same number of laboratory and radiology tests and consultations. Other specialties may consider this model if off-hours care becomes a concern. [source] Evaluation of Noble Gas Recharge Temperatures in a Shallow Unconfined AquiferGROUND WATER, Issue 5 2009Bradley D. Cey Water table temperatures inferred from dissolved noble gas concentrations (noble gas temperatures, NGT) are useful as a quantitative proxy for air temperature change since the last glacial maximum. Despite their importance in paleoclimate research, few studies have investigated the relationship between NGT and actual recharge temperatures in field settings. This study presents dissolved noble gas data from a shallow unconfined aquifer heavily impacted by agriculture. Considering samples unaffected by degassing, NGT calculated from common physically based interpretive gas dissolution models that correct measured noble gas concentrations for "excess air" agreed with measured water table temperatures (WTT). The ability to fit data to multiple interpretive models indicates that model goodness-of-fit does not necessarily mean that the model reflects actual gas dissolution processes. Although NGT are useful in that they reflect WTT, caution is recommended when using these interpretive models. There was no measurable difference in excess air characteristics (amount and degree of fractionation) between two recharge regimes studied (higher flux recharge primarily during spring and summer vs. continuous, low flux recharge). Approximately 20% of samples had dissolved gas concentrations below equilibrium concentration with respect to atmospheric pressure, indicating degassing. Geochemical and dissolved gas data indicate that saturated zone denitrification caused degassing by gas stripping. Modeling indicates that minor degassing (<10% ,Ne) may cause underestimation of ground water recharge temperature by up to 2°C. Such errors are problematic because degassing may not be apparent and degassed samples may be fit by a model with a high degree of certainty. [source] Smoking cessation advice provided in 53 Norwegian pharmaciesINTERNATIONAL JOURNAL OF PHARMACY PRACTICE, Issue 4 2004Dr. Anne G. Granas Director of research Objective To investigate the quality of the smoking cessation advice on over-the-counter (OTC) nicotine replacement treatment (NRT) provided by community and hospital pharmacies in Norway and to assess any change in customer services and pharmaceutical smoking cessation advice after a change in legislation deregulating NRT from pharmacy-only to general sale. Method A mystery shopper made 106 visits to 53 pharmacies in Oslo, Norway. The first visit was when NRT was a pharmacy-only medicine and the second when it had become available from any outlet as a general sales product. The pharmacies were scored on 12 observation parameters regarding customer service and pharmaceutical advice (score 0,4, total possible score was 48). Key findings There was considerable inter- and intra-pharmacy variation in scores. The pharmacies' total score ranged from 3 to 45. On only 14.2% of the visits was the service categorised as ,Good'. There was no statistically significant difference in total score between the two test periods (P = 0.56), and hence no measurable difference in customer services and pharmaceutical advice as a result of increased competition after the switch to general sale. A scatter plot showed no relationship between the individual pharmacies' total score at the two visits. Conclusions Pharmacies' scores for the quality of their smoking cessation advice were generally low. There was no measurable change in pharmacies' customer service and pharmaceutical advice following the deregulation of NRT. There was little consistency in the total score between the first and the second visit to the same pharmacy. Improvement is needed in the smoking cessation advice provided by Norwegian pharmacies. [source] Dispersion polymerization of methyl methacrylate: Effect of stabilizer concentrationJOURNAL OF APPLIED POLYMER SCIENCE, Issue 4 2008Sheng Jiang Abstract In dispersion polymerizations of methyl methacrylate (MMA), little difference was found in the polymerization kinetics with varying stabilizer (PVP K-30) concentration, indicating that in contrast to emulsion polymerization, the kinetics of these dispersion polymerizations are independent of the number of particles. Comparing MMA precipitation and dispersion polymerizations revealed that nuclei formation in the latter results from precipitation polymerization occurring in the continuous phase, which can contribute significantly to the kinetics, and especially to the molecular weight distributions. No change was found in the molecular weight distribution of the PVP during a dispersion polymerization, nor was there any measurable difference between using fresh and recycled stabilizer. © 2007 Wiley Periodicals, Inc. Sozmen A histological, histochemical and morphometric study was performed on submandibular salivary glands from 13 dogs which had presented with a submandibular mass or swelling that proved to be a portion of non-inflammatory and non-neoplastic submandibular salivary gland. There were no consistent changes in lectin-binding histochemistry or immunohistochemical expression of various cell markers, and, in most cases, there was no measurable difference in acinar size in the affected gland. The possible explanation for the clinical salivary gland enlargement is therefore unclear. [source] Cross-species hybridization of a Borrelia burgdorferi DNA array reveals infection- and culture-associated genes of the unsequenced genome of the relapsing fever agent Borrelia hermsiiMOLECULAR MICROBIOLOGY, Issue 3 2004Jianmin Zhong Summary The known genome sequence of Borrelia burgdorferi, an agent of Lyme borreliosis, was used to study the genetic content and gene expression in B. hermsii, another spirochete pathogen and a cause of relapsing fever. Cross-species hybridization of a DNA array representing 1628 open reading frames (ORF) of B. burgdorferi with genomic DNA of B. hermsii indicated that the latter organism has at least 81% of the chromosomal genes and 43% of the plasmid genes of B. burgdorferi. We then carried out quantitative hybridization of the arrays with multiple replicates of cDNA produced from B. hermsii cells growing in the blood of infected mice or in culture medium that was adjusted to the same pH, temperature and a spirochete density as infected blood. Of 642 B. burgdorferi ORFs hybridized by all replicates under both conditions, 12 (1.9%) demonstrated differential expression by a regularized t -test and stringent criteria. BBP07 and BBG30, two plasmid-borne ORFs with the greatest measurable difference in expression between in vivo and in vitro conditions, putatively encode proteins of unknown function. Orthologues of BBP07 in B. hermsii were identified, and increased expression in infected mice was demonstrated by quantitative reverse-transcriptase polymerase chain reaction. [source] Microbiology and geochemistry of Little Hot Creek, a hot spring environment in the Long Valley CalderaGEOBIOLOGY, Issue 2 2010T. J. VICK A culture-independent community census was combined with chemical and thermodynamic analyses of three springs located within the Long Valley Caldera, Little Hot Creek (LHC) 1, 3, and 4. All three springs were approximately 80 °C, circumneutral, apparently anaerobic and had similar water chemistries. 16S rRNA gene libraries constructed from DNA isolated from spring sediment revealed moderately diverse but highly novel microbial communities. Over half of the phylotypes could not be grouped into known taxonomic classes. Bacterial libraries from LHC1 and LHC3 were predominantly species within the phyla Aquificae and Thermodesulfobacteria, while those from LHC4 were dominated by candidate phyla, including OP1 and OP9. Archaeal libraries from LHC3 contained large numbers of Archaeoglobales and Desulfurococcales, while LHC1 and LHC4 were dominated by Crenarchaeota unaffiliated with known orders. The heterogeneity in microbial populations could not easily be attributed to measurable differences in water chemistry, but may be determined by availability of trace amounts of oxygen to the spring sediments. Thermodynamic modeling predicted the most favorable reactions to be sulfur and nitrate respirations, yielding 40,70 kJ mol,1 e, transferred; however, levels of oxygen at or below our detection limit could result in aerobic respirations yielding up to 100 kJ mol,1 e, transferred. Important electron donors are predicted to be H2, H2S, S0, Fe2+ and CH4, all of which yield similar energies when coupled to a given electron acceptor. The results indicate that springs associated with the Long Valley Caldera contain microbial populations that show some similarities both to springs in Yellowstone and springs in the Great Basin. [source] Relationship Between RDX Properties and SensitivityPROPELLANTS, EXPLOSIVES, PYROTECHNICS, Issue 1 2008Abstract An interlaboratory comparison of seven lots of commercially available RDX was conducted to determine what properties of the nitramine particles can be used to assess whether the RDX has relatively high or relatively low sensitivity. The materials chosen for the study were selected to give a range of HMX content, manufacturing process and reported shock sensitivity. The results of two different shock sensitivity tests conducted on a PBX made with the RDX lots in the study showed that there are measurable differences in the shock sensitivity of the PBXs, but the impact sensitivity for all of the lots is essentially the same. Impact sensitivity is not a good predictor of shock sensitivity for these types of RDX. Although most RDX that exhibits RS has low HMX content, that characteristic alone is not sufficient to guarantee low sensitivity. Morgan Abstract Previous work has demonstrated both that there are substantial individual differences in the rate of physiological development, and that infants with risk factors for Sudden Infant Death Syndrome (SIDS) develop more slowly, suggesting that their increased vulnerability may be due to delayed neuronal development associated with compromised development in fetal or early neonatal life. This project aims to test the hypothesis that individual differences in the rate of physiological development of infants correlate with measurable differences in the rate of brain development as assessed by magnetic resonance imaging (MRI). Sixty infants were recruited to this study in three different groups that are known to have differing rates of physiological development. MRI was performed successfully in 49 cases at 6 weeks of age without sedation. Forty-one of these cases had full follow-up (15 normal; 19 IUGR; 11 ,high risk'). Postnatal physiological development was assessed by measuring age-related deep body temperature patterns during sleep. Neuronal development was assessed by subjective analysis of MRI images and objective measurements relating to myelination using T1 and diffusion weighted (23 cases) MRI images. As expected the normal group acquired the adult temperature pattern earlier, but this was not statistically significant. All MRI scan appearances were within normal limits. Ranking cases subjectively in order of maturity revealed no significant pattern. The normal group had a significantly higher myelination score than the IUGR and ,high risk' groups (P = 0.001). This trend was also shown by the diffusion weighted myelination score but did not reach statistical significance. No significant differences were seen in both the subjective and objective MRI measurements and development of nocturnal temperature patterns. The results suggest there may be differences in neurodevelopment between the different groups at 6 weeks of age but these are not linked to late development of temperature patterns. It is therefore unlikely that this related to a global delay in maturation. [source] |
