    Regiospecific Synthesis of 4-Deoxy- D - threo -hex-3-enopyranosides by Simultaneous Activation,Elimination of the Talopyranoside Axial 4-OH with the NaH/Im2SO2 System: Manifestation of the Stereoelectronic Effect

    Abstract A new and high-yielding method for the regioselective preparation of 4-deoxy- and 2,4-dideoxy-2-acetamido-,- D - threo -hex-3-enopyranosides has been developed. The process involves a simultaneous activation,elimination of the OH-4 group of ,- D -talopyranosides and 2-acetamido-2-deoxy-,- D -talopyranosides, mediated by the NaH/N,N,-sulfuryldiimidazole system at ,30 °C. The same reaction applied on the analogous ,- D -galactopyranosides takes place without any regioselectivity, affording mixtures of hex-3- and hex-4-enopyranosides. In the case of the methyl 2,3,6-tri- O -benzyl-,- D -talo- and ,- D -galactopyranosides, the corresponding 4- O -imidazylates can be isolated by quenching the reactions at ,30 °C. Upon warming these crude products to room temperature, the ,- talo -4- O -imidazylate gives the corresponding hex-3-eno derivative in very high yield, but its ,- galacto analogue gives the hex-4-enopyranoside enol ether in poor yield. The different regiochemical outcome between the talo and the galacto series has been attributed to the stereoelectronic effect exerted, exclusively in talo -configured compounds, by the axially disposed C-2 electronegative substituent, which selectively accelerates the breaking of the antiperiplanar C(3),H bond.     Cultural Variability in the Manifestation of Expressed Emotion

    We examined the distribution of expressed emotion (EE) and its indices in a sample of 224 family caregivers of individuals with schizophrenia pooled from 5 studies, 3 reflecting a contemporary sample of Mexican Americans (MA 2000, N=126), 1 of an earlier study of Mexican Americans (MA 1980, N=44), and the other of an earlier study of Anglo Americans (AA, N=54). Chi-square and path analyses revealed no significant differences between the 2 MA samples in rates of high EE, critical comments, hostility, and emotional over-involvement (EOI). Only caregiver warmth differed for the 2 MA samples; MA 1980 had higher warmth than MA 2000. Significant differences were consistently found between the combined MA samples and the AA sample; AAs had higher rates of high EE, more critical comments, less warmth, less EOI, and a high EE profile comprised more of criticism/hostility. We also examined the relationship of proxy measures of acculturation among the MA 2000 sample. The findings support and extend Jenkins' earlier observations regarding the cultural variability of EE for Mexican Americans. Implications are discussed regarding the cross-cultural measurement of EE and the focus of family interventions. RESUMEN Examinamos la distribución de emoción expresada y sus índices en una muestra de 224 cuidadores parientes de personas con esquizofrenia tomadas de 5 estudios, tres que reflejaban una muestra contemporánea de personas méxico-estadounidenses (ME 2000, N=126), una de un estudio anterior de méxico-estadounidenses (ME 1980, N=44, Karno et al., 1987) y la otra de un estudio anterior de angloamericanos (AA, N=54, Vaughn et al., 1984). La distribución ji-cuadrado y los análisis de pautas no revelaron diferencias significativas entre las dos muestras de méxico-estadounidenses en cuanto a los índices de alta emoción expresada, comentarios críticos, hostilidad y sobreimplicación emocional. Solo la calidez de los cuidadores fue distinta en las dos muestras de méxico-estadounidenses; el grupo ME 1980 demostró mayor calidez que el grupo ME 2000. Se encontraron sistemáticamente diferencias considerables entre las dos muestras de méxico-estadounidenses y la muestra de angloamericanos; los angloamericanos demostraron índices más altos de alta emoción expresada, más comentarios críticos, menos calidez, menos sobreimplicación emocional y un perfil de alta emoción expresada compuesto mayormente por crítica y hostilidad. También examinamos la relación de los cálculos aproximados de aculturación entre la muestra ME 2000. Los resultados respaldan y amplían las observaciones anteriores de Jenkins (1991) con respecto a la variabilidad cultural de emoción expresada en los méxico-estadounidenses.     Manifestation of a Chiral Smectic C Phase in Diphenylbutadiene-Cored Bolaamphiphilic Sugars,

    Abstract A series of symmetrical bolaamphiphiles possessing a diphenylbutadiene core and glucopyranoside head groups linked together by oligomethylene spacers, were synthesized and their thermotropic liquid crystalline properties investigated by polarized light optical microscopy, differential scanning calorimetry, X-ray diffraction and electro-optic switching. In spite of the presence of chiral centers, amphiphilic sugars in general do not exhibit macroscopic chirality and this phenomenon is attributed to strong hydrogen bonding between sugar head groups resulting in microphase-segregated layer like arrangements. In the present study all the molecules investigated exhibited the smectic C* phase, i.e., tilted lamellar phase with macroscopic chiral ordering of the molecules. The stability of this phase increased with increase in the length of the oligomethylene spacers. Whereas for derivatives with spacers containing ,4 methylene groups, the smectic C* phase was observed only in the cooling phase, for those containing spacers with ,5 methylene groups this phase was observed both in the heating and cooling cycles. The absorption and fluorescence spectra of these materials suggest that the unusual observation of macroscopic chirality in these bolaamphiphiles containing free hydroxyl groups could be attributed to self-aggregating behavior of the diphenylbutadiene core.     Arnold-Chiari type I malformations usually manifest clinically with short-lasting headaches typically involving the occipital-nuchal region and precipitated by the Valsalva maneuver, coughing, sudden postural change, or physical exertion. We describe the case of an adult patient who presented with symptomatic trigeminal neuralgia caused by an Arnold-Chiari type I malformation.     Thrombogenic Vasculopathy and Interstitial to Diffuse Dermal Neutrophilic Inflammation as a Histologic Manifestation of Tick Bite Reaction

    Ticks are ectoparasites that cause dermatologic disease both directly through physical trauma to the skin, salivary secretions, or remnant body parts, and indirectly through transmission of disease. Lyme disease, Rocky Mountain spotted fever, tularemia, and babesiosis are known tick-transmitted diseases. The histopathology related to a primary tick bite, similar to other arthropod bites, classically consists of a perivascular infiltrate composed of lymphocytes, neutrophils, histiocytes, plasma cells, and eosinophils in varying amounts. We describe five patients with a novel histologic reaction to embedded tick parts., Each case demonstrates a thrombotic vasculopathy consisting of intraluminal eosinophilic deposits that stain strongly with Periodic acid Schiff stain. The adjacent tissue shows dermal necrosis with surrounding interstitial to diffuse dermal neutrophilic inflammation. We postulate that diffuse dermal nutrophilic infiltrates in association with thrombogenic vasculopathy is an unusual histologic picture of tick bite reactions that may be attributable to remnant tick parts.     Pathological Laughing As a Manifestation in a Clinically Isolated Brainstem Syndrome: A Case Report

    ABSTRACT The prevalence of pathological laughing and crying in multiple sclerosis (MS) is 10%. It has been speculated that the anatomical lesion responsible for the pathological laughing is located in the pontine base, prefrontal cortex, and cerebellum. We report an 18-year-old male patient presenting with pathological laughing and hypomania. In his neurological examination, he had a euphoric effect with ataxic walking and dysarthria speech. He had a bilateral conjugated gaze limitation, with a prominent bilateral horizontal nystagmus on left gaze, dysmetria, dysdiadokokinesia, and remarkable dysfunction in a heel-to-shin test on the left. The IgG index in cerebrospinal fluid was normal with an oligoclonal band was present. In cranial MRI, there was a lesion on central pons which was hypointense in T1 images with contrast enhancement and hyperintense in T2 and flair images. Also another lesion in right brachium pontis which did not contrast enhancement but was hyperintense on T2 and flair images was present. There was an elevation of myoinositol/creatine ratio and choline and a reduction of NAA in proton MR spectroscopy. MR spectroscopic evaluation of the patient demonstrated the demyelination process. There has been no report of patients in whom pathological laughter was the presenting symptom of clinically isolated brainstem syndrome. [source]


    Pure Sensory Stroke as an Isolated Manifestation of the Lateral Medullary Infarction

    ABSTRACT Pure sensory stroke is a common manifestation of a thalamic stroke and may occur less frequently in the infarction of the brainstem, internal capsule, and parietal cortex. The authors report a 47-year-old man who presented with loss of pain and temperature sensation in the left face, arm, trunk, and leg and was found to have a right lateral medullary infarction on diffusion-weighted magnetic resonance imaging. To the authors' knowledge, this is the first case describing pure sensory stroke as a single, isolated manifestation of the lateral medullary infarction that was detected by diffusion-weighted magnetic resonance imaging. This case suggests that with the availability of diffusion-weighted magnetic resonance imaging, pure sensory stroke may carry a broader spectrum of the anatomical localizations than previously determined and can localize to the lateral medulla. Therefore, a possibility of medullary infarction should be considered when a patient presents with pure sensory stroke, especially when diffusion-weighted magnetic resonance imaging is not immediately available to provide radiographic correlation.     Manifestation of emerging specialties in journal literature: A growth model of papers, references, exemplars, bibliographic coupling, cocitation, and clustering coefficient distribution

    A model is presented of the manifestation of the birth and development of a scientific specialty in a collection of journal papers. The proposed model, Cumulative Advantage by Paper with Exemplars (CAPE) is an adaptation of Price's cumulative advantage model (D. Price, 1976). Two modifications are made: (a) references are cited in groups by paper, and (b) the model accounts for the generation of highly cited exemplar references immediately after the birth of the specialty. This simple growth process mimics many characteristic features of real collections of papers, including the structure of the paper-to-reference matrix, the reference-per-paper distribution, the paper-per-reference distribution, the bibliographic coupling distribution, the cocitation distribution, the bibliographic coupling clustering coefficient distribution, and the temporal distribution of exemplar references. The model yields a great deal of insight into the process that produces the connectedness and clustering of a collection of articles and references. Two examples are presented and successfully modeled: a collection of 131 articles on MEMS RF (microelectromechnical systems radio frequency) switches, and a collection of 901 articles on the subject of complex networks.     Epilepsy in Border Collies: Clinical Manifestation, Outcome, and Mode of Inheritance

    Background: There is a lack of data on idiopathic epilepsy (IE) in Border Collies (BCs) in the veterinary literature. Hypothesis: Genetic epilepsy occurs in BCs and is frequently characterized by a severe clinical course and poor response to medical treatment. Animals: Forty-nine BCs diagnosed with IE. Methods: Medical records, seizure data, treatment data, and pedigree information of affected dogs were collected. Cases were classified phenotypically as affected or not affected; mild, moderate, or severe clinical course; active epilepsy (AE) or remission; and drug resistant or not drug resistant. Results: Clinical manifestations were classified as having a moderate (33%) or severe clinical course (49%), characterized by a high prevalence of cluster seizures and status epilepticus. Survival time was significantly decreased in dogs <2 years of age at seizure onset, and in dogs with a severe clinical course. Drug resistance was apparent in 71% of 24 dogs treated with ,2 antiepileptic drugs. The epilepsy remission rate was 18%. Median age at onset was significantly higher and initial seizure frequency was significantly lower in dogs with remission compared with dogs with AE. Pedigree analyses indicated a strong genetic founder effect in the appearance of epilepsy, resembling autosomal recessive inheritance. Conclusion and Clinical Importance: The present study confirms the occurrence of genetically mediated epilepsy with a frequent severe clinical course and drug resistance in BCs. The results provide information about the long-term prognosis of IE in BCs for veterinarians and concerned owners, and may benefit breeders as well. [source]

    Acquired Ichthyosis as a Manifestation of Acute Cutaneous Graft-Versus-Host Disease

    While some authors have suggested the association of acquired ichythosis with cutaneous graft-versus-host disease, the evidence to support this association is rare. We describe a patient who developed enteritis secondary to acute graft-versus-host disease and had concomitant ichthyosiform lesions. Several months later, he was diagnosed with cutaneous graft-versus-host disease. This patient is presented to suggest that acquired ichthyosis is an underrecognized manifestation of acute cutaneous graft-versus-host disease. Given the important prognostic implications of this diagnosis, we recommend a low threshold for performing a skin biopsy to rule out GVHD in the appropriate clinical setting. [source]


    Cytomegalovirus infection in children who underwent hematopoietic stem cell transplantation at a single center: A retrospective study of the risk factors

    Abstract:, CMV infection is one of the major causes of morbidity and mortality after HSCT. The aim of this single center retrospective study was to analyze risk factors for CMV infection in pediatric patients who underwent HSCT. We retrospectively reviewed the medical records of 117 pediatric patients who underwent allogeneic HSCT at Asan Medical Center between December 2000 and January 2007. After HSCT, CMV antigenemia was detected by identifying CMV pp65 early antigen in white blood cells. The incidence of CMV antigenemia was 24% (28/117) at a median of 38 days (range: 19,123 days) after HSCT. In multivariate analysis, CMV antigenemia occurred significantly more often in CMV seropositive recipients, patients who received grafts from alternative donors, T-cell depleted grafts, patients on ATG-containing conditioning regimens, or patients who received steroid for acute GVHD (p < 0.05). CMV antigenemia tend to develop earlier in patients who received ATG-containing conditioning regimens (p = 0.09). A second episode of CMV antigenemia was observed in three out of 28 patients (11%). The incidence of CMV disease was 5.9% (7/117) at a median of 97 days (range: 34,120 days). Manifestation of CMV disease included retinitis in two, pneumonitis in two, hepatitis in one, hepatitis with colitis in one, and gastritis in one. Six of the 12 patients (50%) with HG antigenemia (CMV pp65 antigen positivity ,40 cells) developed clinical CMV disease, a rate that was significantly higher than seen in patients with LG antigenemia (6.25%; p < 0.01). We recommend that patients with these risk factors should carefully undergo regular evaluations for CMV infection. We also suggest that earlier and more aggressive preemptive treatment and serial follow-up of CMV disease is necessary in patients with HG-antigenemia. [source]


    Manifestation of the equilibrium hole distribution in photoluminescence of n-InN

    Abstract Photoluminescence (PL) of n-InN grown by molecular beam epitaxy with Hall concentrations from 3.6 to 7.3 × 1017 cm,3 demonstrates dependences on carrier concentration, temperature, and excitation density which give evidences of a fast energy relaxation rate of photoholes and their equilibrium distribution over localized states. The structure of the PL spectra observed at 4.2 and 77 K in the energy interval from 0.50 to 0.67 eV indicates that a considerable part of holes is trapped by deep and shallow acceptors before the interband recombi- nation with degenerate electrons occurs. At room temperature, the band-to-band recombination of free holes and electrons dominates in PL. Experimental results on PL and absorption are described by model calculations under the assumptions of a band gap equal to 0.665,0.670 eV at zero temperature and zero carrier concentration and a non-parabolic conduction band with the effective mass at the G -point equal to 0.07 of the free electron mass.     Pulmonary Capillaritis as a Manifestation of Acute Humoral Allograft Rejection Following Infant Lung Transplantation

    Pulmonary capillaritis has been described in adult lung transplant recipients, but has not been previously reported in pediatric recipients. We report a case of posttransplant pulmonary capillaritis in an 8-month-old infant, and demonstrate evidence of C4d deposition and B-lymphocytes in the allograft, donor anti-HLA antibodies in the serum and a clinical and immunohistochemical response to anti-CD20 monoclonal antibody (rituximab) therapy. These findings strongly support the hypothesis that pulmonary capillaritis may represent a form of acute humoral rejection in the lung allograft that is less common than, and clinically and histologically distinct from, typical acute cellular rejection.     A Novel Point Variant in NTRK3, R645C, Suggests a Role of this Gene in the Pathogenesis of Hirschsprung Disease

    Summary Hirschsprung disease (HSCR) is a developmental disorder characterized by the absence of ganglion cells in the myenteric and submucosal plexuses due to a defect in the migration process of neural crest neuroblasts. Manifestation of the disease has been linked to the dysfunction of two principal signalling pathways involved in the enteric nervous system (ENS) formation: the RET-GDNF and the EDN3-EDNRB receptor systems. However, the NTF3/NTRK3 signalling pathway plays an essential role in the development of the ENS suggesting a potential role for those genes in the pathogenesis of HSCR. We have sought to evaluate the candidature of the NTRK3 gene, which encodes the TrkC receptor, as a susceptibility gene for Hirschsprung disease. Using dHPLC technology we have screened the NTRK3 coding region in 143 Spanish HSCR patients. A total of four previously described polymorphisms and 12 novel sequence variants were detected. Of note, the novel R645C mutation was detected in 2 affected siblings of a HSCR family also carrying a RET splicing mutation. Using bioinformatics tools we observed that the presence of an additional cysteine residue might implicate structural alterations in the mutated protein. We propose haploinsufficiency as the most probable mechanism for the NTRK3 R645C mutation. NTRK3 and RET mutations in this family only appear together in the HSCR patients, suggesting that they per se are necessary but not sufficient to produce the phenotype. In addition, it is quite probable that the contribution of other still unidentified modifier genes, may be responsible for the different phenotypes (length of aganglionosis) in the two affected members.     Dose-dependent pharmacokinetics of 1-(2-Deoxy- , - D - ribofuranosyl)-2,4-difluoro-5-iodobenzene: A potential mimic of 5-iodo-2,-deoxyuridine

    Abstract The dose-range pharmacokinetics of l-(2-deoxy- , - D -ribofuranosyl)-2,4-difluoro-5-iodobenzene (5-IDFPdR), a C -aryl nucleoside mimic of IUdR, were studied in male Sprague-Dawley rats following single intravenous (i.v.) and oral doses. After i.v. administration, the blood clearance decreased from ,32 ml/min/kg at a dose of 15 mg/kg, to ,19 ml/min/kg when dosed at 54 mg/kg, and the elimination half-life increased from 8.4 min to 21.5 min, for the respective doses. While the dose-normalized area under the concentration-time curve (AUCnorm) remained practically unchanged (0.132 kg min ml,1) upon increasing the i.v. dose from 5 to 15 mg/kg, it increased by about 44% (,0.19 kg min ml,1) when the i.v. dose was increased from 15 to 54 mg/kg. Similarly, there was a dose-dependent increase in AUCnorm with increasing oral doses: AUCnorm increased by 49% as the oral dose increased from 20 to 40 mg/kg, and further by 55% as the oral dose was increased from 40 mg/kg to 54 mg/kg. For the respective oral doses, the elimination half-life increased from 24.5 min to 176 min, while blood clearance was reduced from ,37 ml/min/kg to ,17 ml/min/kg. The urinary recoveries of unchanged 5-IDFPdR and its glucuronides (as percent of the dose) were somewhat increased at higher doses. This increase was more pronounced following the highest oral dose. The total biliary recovery of 5-IDFPdR (as percent of the dose) was, however, decreased with increasing doses. The overall kinetic profile of 5-IDFPdR based on these data is suggestive of dose-dependent pharmacokinetics. Decreased elimination of 5-IDFPdR with increasing dose, as supported by longer elimination half-lives at higher doses, is one likely mechanism contributing to the dose-dependent behaviour of this compound. Saturable non-renal metabolism might explain the reduced total body clearance of 5-IDFPdR at higher doses, despite the unchanged or increased urinary clearance. For drugs exhibiting nonlinear kinetics, the dosage regimens may need to be carefully designed to avoid potential unpredictable toxicity and/or lack of pharmacological response associated with the disproportional changes in steady state drug concentrations on changing dose. Manifestation in the rat of nonlinear kinetics at doses of 5-IDFPdR, which may be of therapeutic relevance, warrants extended dose-range evaluations of this compound in future preclinical and clinical studies, to establish safe and efficacious dosage regimens.     Voltammetric Manifestation of the Ultraslow Dynamics at the Interface between Water and an Ionic Liquid,

    Abstract The ultraslow relaxation (on the order of minutes) of the electrical double-layer structure, related to a change in the phase-boundary potential across the interface between water (W) and the ionic liquid (IL) trioctylmethylammonium bis(nonafluorobutanesufonyl)amide ([TOMA+][C4C4N,]) (Y. Yasui et al., J. Phys. Chem. B. 2009, 113, 3273), appears to be invisible in the transfer of tetrapropylammonium ions across the [TOMA+][C4C4N,]|W interface, provided that the charging current, which shows an unusual dependence on the voltage scan rate, is subtracted to obtain the faradaic current. This counterintuitive observation can be explained by the differences in the timescales of the fast and slow components of the relaxation dynamics of the electrical double layer on the IL side (ms and min). In contrast, the effect of the slow dynamics becomes surfaced in ion-transfer voltammetry when the ion is surface-active. The transfer of pentadecafluorooctanoate across the [TOMA+][C4C4N,]|W interface is irreversible, which is attributable to the self-inhibition of pentadecafluorooctanoate ions transferred to the IL phase. This process is likely to be affected by the ultraslow structural change of the IL side of the interface. [source]


    Manifestation of Coats' disease by age in Taiwan

    Abstract Background:, To compare the differences in clinical manifestation of Coats' disease between younger and older patients in the Far East. Methods:, Coats' disease cases diagnosed at one Taiwanese hospital from July 1986 to June 2004 were retrospectively reviewed (n = 30; 32 eyes). Patients were stratified into groups according to the initial diagnosis of Coats' disease at the age of <20 years (Group Y) or ,20 years (Group O). The clinical manifestations of Coats' disease in Group Y (19 eyes) and Group O (13 eyes) were compared. Results:, A higher proportion of female patients was noted in Group O (P = 0.046). Diseases were generally limited geographically, with 14 eyes (73.7%) manifesting involvement greater than 6 clock hours in Group Y and four eyes (30.8%) in Group O. The involved area including retinal telangiectasia and exudates was smaller in Group O (P = 0.016). Patients without posterior pole involvement were associated with better visual outcome (adjusted odds ratio, 6.5; 95% confidence interval, 1.1,40.1, P = 0.044). Conclusion:, Coats' disease manifestation was different between different age groups. Treatment is important to prevent disease progression. Visual prognosis is associated with posterior pole involvement. [source]


    An Unusual Manifestation of Tako-tsubo Cardiomyopathy

    Abstract Takotsubo cardiomyopathy typically presents with chest pain, ST changes, and transient left ventricular apical ballooning in the absence of epicardial coronary artery disease. This process is reversible and usually benign. An unusual manifestation is that of left ventricular outflow tract obstruction with systolic anterior motion of the mitral valve. Recognition of this finding is critical in patient management especially in the setting of cardiogenic shock, as inotropes are likely to aggravate and worsen the clinical condition.     Manifestation of depressive tendency in color perception and colors utilized in creating a self-portrait

    Abstract There are a large number of studies on color and its influence upon human emotions, but there have been only a few studies on the correlation between color and depressive mental disorders. This study used color preference, association, and creation of self-portraits to explore the relationship between color and depressive tendency. In this study, 337 freshman design students participated in the experiment, and the center for epidemiologic studies-depression scale (CES-D) was used to determine the extent of depression in the subjects. After classifying the subject as "depressive tendency" and "no depressive tendency," the semantic differential scale and color association methods were used to explore the subjects' emotional, cognitive, and perceptive responses to Munsell 14 colors. In the final portion of the study, the 233 subjects created a colored self-portrait. The main conclusions found by this study are: (1) when color is used to determine whether subjects have depressive tendency, then the three colors of Yellowish Red, Purple, and Dark Gray are important discriminant variables. (2) those who have depressive tendency (DT) or do not have depressive tendency (NoDT) have similar results in color association, except when viewing dark Gray in association with abstract concepts, wherein DT subjects chose more negative emotional words such as "hopeless," "fear," and "depression"; (3) among the 233 self-portraits, the existence of depressive tendency correlated with the colors used on the face in the self-portrait.     Different Electroclinical Manifestations of the Epilepsy Associated with Hamartomas Connecting to the Middle or Posterior Hypothalamus

    Summary:,Purpose: The epilepsy associated with hypothalamic hamartomas (HHs) has typical clinical, electrophysiologic, and behavioral manifestations refractory to drug therapy and with unfavorable evolution. It is well known that only sessile lesions produce epilepsy, but no correlation has been established between the different types of sessile hamartomas and the diverse manifestations of the epilepsy. We correlate anatomic details of the hamartoma and the clinical and neurophysiologic manifestations of the associated epilepsy. Methods: HHs of seven patients with epilepsy (ages 2, 25 years) were classified as to lateralization and connection to the anteroposterior axis of the hypothalamus by using high-resolution brain magnetic resonance imaging. We correlated the anatomic classification with the clinical and neurophysiologic manifestations of the epilepsy as evaluated in long-term (24 h) video-EEG recordings. Results: HHs ranged in size from 0.4 to 2.6 cc, with complete lateralization in six of seven patients. Ictal manifestations showed good correlation with the lobar involvement of ictal/interictal EEGs. These manifestations suggest the existence of two types of cortical involvement, one associated with the temporal lobe, produced by hamartomas connected to the posterior hypothalamus (mamillary bodies), and the other associated with the frontal lobe, seen in lesions connecting to the middle hypothalamus. Conclusions: A consistent clinical and neurophysiologic pattern of either temporal or frontal lobe cortical secondary involvement was found in the patients of our series. It depends on whether the hamartoma connects to the mamillary bodies (temporal lobe cases) or whether it connects to the medial hypothalamus (frontal lobe cases). [source]

    Manifestations of Relationship Conceptualizations in Conversation

    A question fundamental to the study of interpersonal communication in close relationships is how the characteristics people associate with their relationships are evident in their conversation. We begin by explicating 3 key aspects of relationship conceptualizations: reliance on relational knowledge, interdependence, and mutual commitment. Then we advance hypotheses linking those relationship conceptualizations with features of people's content and relational messages. We report the results of a study in which 120 dating couples engaged in videotaped conversations. Findings offered modest support for our predictions regarding reliance on relational knowledge; results were less consistent with our hypotheses involving interdependence and mutual commitment. We conclude by discussing the implications of our findings for understanding how perceptions of relationships are apparent in conversation. [source]


    Homocysteine Level and Cognitive Function in Patients with Arterial Disease: The Second Manifestations of ARTerial Disease Study

    OBJECTIVES: To assess the relationship between total plasma homocysteine (tHcy) level and cognitive function in patients with manifest arterial disease. DESIGN: Cross-sectional. SETTING: Patients with symptomatic cerebrovascular disease, cardiovascular disease, peripheral arterial disease, or abdominal aortic aneurysm included in the Second Manifestations of ARTerial disease study, a single-center, longitudinal study with an extensive screening program at baseline. PARTICIPANTS: Three hundred forty-five consecutively included patients, mean age 59. MEASUREMENTS: The patients underwent an extensive neuropsychological test. The cognitive domains assessed were memory, executive function, attention, and visuoperception and construction. Each raw score was transformed into standardized z-scores, and a sum score for global cognitive function was determined. Risk factors and vascular damage were measured in detail. RESULTS: Linear regression showed that elevated levels of tHcy were related to lower global cognitive function (,=,0.065, 95% confidence interval (CI)=,0.116 to ,0.013) and, more specifically, lower performance on memory (,=,0.078, 95% CI=,0.155 to ,0.002), attention (,=,0.079, 95% CI=,0.163 to ,0.005), and visuoperception and construction (,=,0.125, 95% CI=,0.236 to ,0.014) per standard deviation increase in tHcy (SD=6.4 mol/L), after adjustment for age, sex, educational level, extent of atherosclerosis, and location of vascular disease. Silent cerebral infarcts did not influence this relationship. CONCLUSION: A relationship was found between tHcy levels and cognitive function that was independent of extent and location of arterial disease. The results suggest that vascular mechanisms are not responsible for the relationship between tHcy and cognitive function. [source]
