Selected AbstractsTowards a decision support system for health promotion in nursingJOURNAL OF ADVANCED NURSING, Issue 2 2003Kate Caelli PhD RN RM Aims. This study was designed to investigate what type of models, techniques and data are necessary to support the development of a decision support system for health promotion practice in nursing. Specifically, the research explored how interview data can be interpreted in terms of Concept Networks and Bayesian Networks, both of which provide formal methods for describing the dependencies between factors or variables in the context of decision-making in health promotion. Background. In nursing, the lack of generally accepted examples or guidelines by which to implement or evaluate health promotion practice is a challenge. Major gaps have been identified between health promotion rhetoric and practice and there is a need for health promotion to be presented in ways that make its attitudes and practices more easily understood. New tools, paradigms and techniques to encourage the practice of health promotion would appear to be beneficial. Concept Networks and Bayesian Networks are techniques that may assist the research team to understand and explicate health promotion more specifically and formally than has been the case, so that it may more readily be integrated into nursing practice. Methods. As the ultimate goal of the study was to investigate ways to use the techniques described above, it was necessary to first generate data as text. Textual descriptions of health promotion in nursing were derived from in-depth qualitative interviews with nurses nominated by their peers as expert health promoting practitioners. Findings. The nurses in this study gave only general and somewhat vague outlines of the concepts and ideas that guided their practice. These data were compared with descriptions from various sources that describe health promotion practices in nursing, then examples of a Conceptual Network and a representative Bayesian Network were derived from the data. Conclusions. The study highlighted the difficulty in describing health promotion practice, even among nurses recognized for their expertise in health promotion. Nevertheless, it indicated the data collection and analysis methods necessary to explicate the cognitive processes of health promotion and highlighted the benefits of using formal conceptualization techniques to improve health promotion practice. [source] Women's Decision Making About the Use of Hormonal and Nonhormonal Remedies for the Menopausal TransitionJOURNAL OF OBSTETRIC, GYNECOLOGIC & NEONATAL NURSING, Issue 6 2003Rosemary Theroux Objective: To critically review qualitative research on women's decision making about the use of hormonal and nonhormonal remedies for the menopausal transition. Data Sources: Computerized searches in CINAHL, MEDLINE, Medscape, and PsychINFO databases, using the keywords decision making, hormone therapy, herbal remedies, attitude toward hormone therapy, and qualitative research; and ancestral bibliographies. Study Selection: Articles from indexed journals from 1982 to 2001 in the English language relevant to the keywords were evaluated. Sixteen studies met inclusion criteria and were included in the analysis. Data Extraction: Study findings were organized into several categories and compared and contrasted across publications and categories. Data Synthesis: Half of the researchers described decision making as a weighing of benefits and risks. Women's considerations, beliefs, and values, as well as interaction with the environment, were primary influences on the process. Conclusions: Major gaps in care for midlife women were identified. Women need information about the process of menopause and the range of available options for menopause management. Nurses can play a major role in providing information, counseling, and developing decision aids. Women's values and beliefs, cultures, life contexts, and desire for involvement in the decision should guide interventions. [source] Tectono-sedimentary evolution of active extensional basinsBASIN RESEARCH, Issue 3-4 2000R. L. Gawthorpe We present conceptual models for the tectono-sedimentary evolution of rift basins. Basin architecture depends upon a complex interaction between the three-dimensional evolution of basin linkage through fault propagation, the evolution of drainage and drainage catchments and the effects of changes in climate and sea/lake level. In particular, the processes of fault propagation, growth, linkage and death are major tectonic controls on basin architecture. Current theoretical and experimental models of fault linkage and the direction of fault growth can be tested using observational evidence from the earliest stages of rift development. Basin linkage by burial or breaching of crossover basement ridges is the dominant process whereby hydrologically closed rifts evolve into open ones. Nontectonic effects arising from climate, sea or lake level change are responsible for major changes in basin-scale sedimentation patterns. Major gaps in our understanding of rift basins remain because of current inadequacies in sediment, fault and landscape dating. [source] Regional climate modulates the canopy mosaic of favourable and risky microclimates for insectsJOURNAL OF ANIMAL ECOLOGY, Issue 3 2007SYLVAIN PINCEBOURDE Summary 1,One major gap in our ability to predict the impacts of climate change is a quantitative analysis of temperatures experienced by organisms under natural conditions. We developed a framework to describe and quantify the impacts of local climate on the mosaic of microclimates and physiological states of insects within tree canopies. This approach was applied to a leaf mining moth feeding on apple leaf tissues. 2,Canopy geometry was explicitly considered by mapping the 3D position and orientation of more than 26 000 leaves in an apple tree. Four published models for canopy radiation interception, energy budget of leaves and mines, body temperature and developmental rate of the leaf miner were integrated. Model predictions were compared with actual microclimate temperatures. The biophysical model accurately predicted temperature within mines at different positions within the tree crown. 3,Field temperature measurements indicated that leaf and mine temperature patterns differ according to the regional climatic conditions (cloudy or sunny) and depending on their location within the canopy. Mines in the sun can be warmer than those in the shade by several degrees and the heterogeneity of mine temperature was incremented by 120%, compared with that of leaf temperature. 4.,The integrated model was used to explore the impact of both warm and exceptionally hot climatic conditions recorded during a heat wave on the microclimate heterogeneity at canopy scale. During warm conditions, larvae in sunlight-exposed mines experienced nearly optimal growth conditions compared with those within shaded mines. The developmental rate was increased by almost 50% in the sunny microhabitat compared with the shaded location. Larvae, however, experienced optimal temperatures for their development inside shaded mines during extreme climatic conditions, whereas larvae in exposed mines were overheating, leading to major risks of mortality. 5,Tree canopies act as both magnifiers and reducers of the climatic regime experienced in open air outside canopies. Favourable and risky spots within the canopy do change as a function of the climatic conditions at the regional scale. The shifting nature of the mosaic of suitable and risky habitats may explain the observed uniform distribution of leaf miners within tree canopies. Chayen Protein crystallization has gained a new strategic and commercial relevance in the next phase of the genome projects, in which X-ray crystallography will play a major role. Considerable advances have been made in the automation of protein preparation and also in the X-ray analysis and bioinformatics stages once diffraction-quality crystals are available. These advances have not yet been matched by equally good methods for the crystallization process itself. In the area of crystallization, the main effort and resources are currently being invested into the automation of screening procedures to identify potential crystallization conditions. However, in spite of the ability to generate numerous trials, so far only a small percentage of the proteins produced have led to structure determinations. This is because screening in itself is not usually enough; it has to be complemented by an equally important procedure in crystal production, namely crystal optimization. In the rush towards structural genomics, optimization techniques have been somewhat neglected, mainly because it was hoped that large-scale screening alone would produce the desired results. In addition, optimization has relied on particular individual methods that are often difficult to automate and to adapt to high throughput. This article addresses a major gap in the field of structural genomics by describing practical ways of automating individual optimization methods in order to adapt them to high-throughput techniques. [source] Long-term Callovian,Oxfordian sea-level changes and sedimentation in the Iberian carbonate platform (Jurassic, Spain): possible eustatic implicationsBASIN RESEARCH, Issue 2 2008Javier Ramajo ABSTRACT Facies analysis across the carbonate platform developed during the Callovian,Oxfordian in the northern Iberian basin (Jurassic, Northeast Spain) is used to characterize successive stages of sedimentary evolution, including palaeoenvironmental reconstructions showing the distribution of a wide spectrum of facies, from ferruginous oolitic, peloidal, spongiolithic to intraclastic. The studied successions consist of two long-term transgressive,regressive cycles bounded by a major unconformity with a major gap, comprising at least the upper Lamberti (Callovian) and Mariae (Oxfordian) Zones. Major transgressive peaks of these two cycles occurred at the end of the Early Callovian (late Gracilis Zone) and at the end of the Middle Oxfordian. The Callovian and Oxfordian successions were further divided into three and seven higher frequency cycles, respectively. The modelling of two sections (i.e. Ricla and Tosos) located 40 km apart in the more subsident open platform areas, allows the reconstruction of two curves showing a similar evolution of long-term sea-level changes that are in theory eustatic, though subject to uncertainties derived form the assumptions required for their construction. The changes affecting the northern Iberian basin seem to reflect nearly homogeneous subsidence (rates around 2 cm kyr,1) combined with possible eustatic changes including an Early Callovian rise, a fall at the middle Callovian,earliest Oxfordian (i.e. the Anceps,Mariae Zones), with average long-term rates around 2 cm kyr,1 (total fall of 40,60 m), a period of lowstand at the Early,Middle Oxfordian transition and a long-term rise at the Middle,Late Oxfordian transition (Transversarium and Bifurcatus Zones). Facies distribution across the Iberian platform indicates a progressive Middle,Late Callovian relative sea-level fall rather than a rapid relative sea-level fall at the end of the Callovian. After this falling episode, the progressive onlap over the swell areas during the Early Oxfordian and at the beginning of the Middle Oxfordian indicates a period of accommodation gain, which is explained by the combined effects of continuous subsidence across the platform and reduced sedimentation rates in spite of the possible eustatic lowstand. Eustatic lowstand, combined with other factors (ocean water circulation, volcanism) could help to explain the loss of carbonate production during the latest Callovian,Early Oxfordian, previous to the widespread eustatic rise and warning recorded at the onset of the Transversarium Zone (Middle Oxfordian). Culley Summary The lungs are a major site of entry of pathogens into the body and thus require rapid and effective innate responses to prevent pathogens establishing infection and to limit their spread. Additionally, the immune response in the lung must be tightly regulated such that pathogens are cleared, but immunopathology and chronic inflammation are prevented. In this review, I consider the role of natural killer (NK) cells in pulmonary infection and inflammation, specifically their contributions to influenza, tuberculosis, asthma and chronic obstructive pulmonary disease (COPD), which are major causes of morbidity and mortality world-wide. Despite evidence of the importance of NK cells in these diseases, there are still major gaps in our understanding of how their function is regulated in this unique tissue environment. Understanding how different beneficial and detrimental effector functions of NK cells are triggered will be crucial if NK cells are to be exploited therapeutically in respiratory disease. [source] Religious Attendance and Happiness: Examining Gaps in the Current Literature,A Research NoteJOURNAL FOR THE SCIENTIFIC STUDY OF RELIGION, Issue 3 2010Ellen Childs Two major gaps exist in research examining the positive association between religious attendance and happiness. First, scholars have argued that this association may be because of people's perceived relationship with God or the social cohesion one experiences with other parishioners, but scholars have not studied both pathways simultaneously. Using nationally representative cross-sectional data (General Social Survey), I first examine the influence of one's perceived relationship with God and one's perceived social cohesion within the religious community on happiness. One's perceived relationship with God is more strongly associated with happiness than is social cohesion. Second, scholars overwhelmingly use religious attendance as the independent variable, with happiness as the dependent variable. Using two waves of data from a nationally representative panel study (National Survey of Families and Households), I then examine the relationship between religious attendance and happiness over time, finding that religious attendance has a greater effect on happiness than happiness has on religious attendance. [source] Assessment of hypotheses about dispersal in a long-lived seabird using multistate capture,recapture modelsJOURNAL OF ANIMAL ECOLOGY, Issue 4 2004Emmanuelle Cam Summary 1Dispersal contributes to spatio-temporal variation in population size and is a key process in studies of life history evolution and studies with conservation implications. However, dispersal is still one of the major gaps in our knowledge of ecological dynamics. The very large literature on metapopulation dynamics lacks empirical bases on dispersal and relevant behavioural parameters. We used multistate capture,recapture models (data from 1988 to 2001) to address hypotheses about movement probability and habitat selection within a system of two breeding colonies in Audouin's gulls (Larus audouinii), an endemic species breeding in the Mediterranean and considered as threatened. 2Movement probability varied over time, and differed greatly between the colonies. 3We did not find evidence of an influence of colony size or density of predators on movement probability. 4In dispersers, our results did not support the hypotheses that movement probability between year t and t+ 1 was influenced by mean breeding success in the colony of origin (i.e. an indicator of habitat quality) or the destination colony in year t or t+ 1, or by the ratio of breeding success in these colonies in year t or t + 1 (i.e. quality gradient). 5Overall, movement probability was higher from the smaller colony to the larger, and from the colony with lower breeding success in year t to the more productive one. This provides slight support for two nonexclusive hypotheses about habitat selection (conspecific attraction and conspecific success attraction). 6Movement probability from the smaller, less productive colony was very high in some years, suggesting that the dynamics of both colonies were strongly influenced by adult dispersal. However, in absolute numbers, more individuals moved from the larger, more productive colony to the smaller one. This suggests that the system may function as a source,sink system. 7Use of multistate models to re-assess local survival showed that survival was lower in the less productive colony with higher emigration probability. This may reflect genuine differences in mortality between colonies, or more probably differences in permanent emigration from the study area. Pollock MPH Abstract Cancer registries are an important research resource that facilitate the study of etiology, tumor biology, patterns of delayed diagnosis and health planning needs. When outcome data are included, registries can track secular changes in survival related to improvements in early detection or treatment. The surveillance, epidemiology, and end results (SEER) registry has been used to identify major gaps in survival for older adolescent and young adult (AYA) patients compared with younger children and older adults. In order to determine the reasons for this gap, the complete registration and accurate classification of AYA malignancies is necessary. There are inconsistencies in defining the age limits for AYAs although the Adolescent and Young Adult Oncology Progress Review Group proposed a definition of ages 15 through 39 years. The central registration and classification issues for AYAs are case-finding, defining common data elements (CDE) collected across different registries and the diagnostic classification of these malignancies. Goals to achieve by 2010 include extending and validating current diagnostic classification schemes and expanding the CDE to support AYA oncology research, including the collection of tracking information to assess long-term outcomes. Indeed, not only do IS professionals account for a large proportion of the PMI constituency, but PMI is also influential in the training of IS project managers. This study explores further the contribution of PMI to IS project management by means of its main publication outlet, the Project Management Journal (PMJ). To do so, the contents of the 39 IS project management articles published in PMJ during 1988,2005 were analyzed. The article focuses on the following dimensions: the relative importance of IS project management articles published by PMJ; the profile of the authors of IS project management articles in PMJ; the main issues, in terms of IS project management, covered by PMJ; and the major gaps, in terms of IS project management, in the coverage of this domain by PMJ. [source] Personality in nonhuman primates: a review and evaluation of past researchAMERICAN JOURNAL OF PRIMATOLOGY, Issue 8 2010Hani D. Freeman Abstract Scientific reports of personality in nonhuman primates are now appearing with increasing frequency across a wide range of disciplines, including psychology, anthropology, endocrinology, and zoo management. To identify general patterns of research and summarize the major findings to date, we present a comprehensive review of the literature, allowing us to pinpoint the major gaps in knowledge and determine what research challenges lay ahead. An exhaustive search of five scientific databases identified 210 relevant research reports. These articles began to appear in the 1930s, but it was not until the 1980s that research on primate personality began to gather pace, with more than 100 articles published in the last decade. Our analyses of the literature indicate that some domains (e.g., sex, age, rearing conditions) are more evenly represented in the literature than are others (e.g., species, research location). Studies examining personality structure (e.g., with factor analysis) have identified personality dimensions that can be divided into 14 broad categories, with Sociability, Confidence/Aggression, and Fearfulness receiving the most research attention. Analyses of the findings pertaining to inter-rater agreement, internal consistency, test,retest reliability, generally support not only the reliability of primate personality ratings scales but also point to the need for more psychometric studies and greater consistency in how the analyses are reported. When measured at the level of broad dimensions, Extraversion and Dominance generally demonstrated the highest levels of inter-rater reliability, with weaker findings for the dimensions of Agreeableness, Emotionality, and Conscientiousness. Few studies provided data with regard to convergent and discriminant validity; Excitability and Dominance demonstrated the strongest validity coefficients when validated against relevant behavioral criterion measures. Overall, the validity data present a somewhat mixed picture, suggesting that high levels of validity are attainable, but by no means guaranteed. To reach a comprehensive deal, major gaps between developing and developed countries must be narrowed. The gaps include the character of common but differentiated responsibilities, financial support, technology transfer, and trade subsidies and sanctions. The paper concludes with some options and recommendations. [source] A critical analysis of the role of gut Oxalobacter formigenes in oxalate stone diseaseBJU INTERNATIONAL, Issue 1 2009Siddharth Siva Hyperoxaluria is a major risk factor for the formation of calcium oxalate stones, but dietary restriction of oxalate intake might not be a reliable approach to prevent recurrence of stones. Hence, other approaches to reduce urinary oxalate to manage stone disease have been explored. The gut-dwelling obligate anaerobe Oxalobacter formigenes (OF) has attracted attention for its oxalate-degrading property. In this review we critically evaluate published studies and identify major gaps in knowledge. Recurrent stone-formers are significantly less likely to be colonized with OF than controls, but this appears to be due to antibiotic use. Studies in animals and human subjects show that colonization of the gut with OF can decrease urinary oxalate levels. However, it remains to be determined whether colonization with OF can affect stone disease. Reliable methods are needed to detect and quantify colonization status and to achieve durable colonization. New information about oxalate transport mechanisms raises hope for pharmacological manipulation to decrease urinary oxalate levels. In addition, probiotic use of lactic acid bacteria that metabolize oxalate might provide a valid alternative to OF. [source] Breast carcinoma in menCANCER, Issue 1 2004A population-based study Abstract BACKGROUND Male breast carcinoma is an uncommon disease, and most previous studies have been single-institution series that were limited by extremely small sample sizes. The goals of the current study were to fill in the major gaps in knowledge regarding the incidence, presenting characteristics, prognostic factors, and survival rates of male breast carcinoma and to determine how breast carcinoma differs between men and women. METHODS Data from the National Cancer Institute Surveillance, Epidemiology, and End Results 1973,1998 database were used. Age-adjusted incidence rates were calculated. Characteristics of the patients and presenting tumors were compared between men and women. Univariate and multivariate analyses were performed to determine the effect of each variable on overall survival. Survival rates by disease stage were compared for men and women. RESULTS Over the years of the study, the incidence of male breast carcinoma increased significantly from 0.86 to 1.08 per 100,000 population (P < 0.001). Men had a higher median age at diagnosis (P < 0.001) and were more likely to have lymph node involvement (P < 0.001), a more advanced stage at diagnosis (P < 0.001), and tumors that were positive for estrogen receptor (ER) (P < 0.001) and progesterone receptor (PR) (P < 0.001). In multivariate analysis, larger tumor size and lymph node involvement were associated with shortened survival. Tumor grade and ER/PR status did not appear to independently influence survival. Relative survival rates by stage of disease for men and women were similar. CONCLUSIONS Although it remains a rare disease, the incidence of male breast carcinoma is increasing. Breast carcinoma in men has some epidemiologic and biologic differences from breast carcinoma in women. Cancer 2004. © 2004 American Cancer Society. [source] |
