Selected AbstractsApoE allelic variability influences pupil response to cholinergic challenge and cognitive impairmentGENES, BRAIN AND BEHAVIOR, Issue 3 2007L.F.M. Scinto Exaggerated pupil response to dilute tropicamide has been suggested as an early biological marker for Alzheimer's disease. The current study links apolipoprotein E (ApoE) allelic variability to the magnitude of pupil response in a sample of community-dwelling elderly without a diagnosis of Alzheimer's disease or dementia. Possession of an ,4 allele influences both the likelihood of exhibiting an exaggerated pupil response above a predetermined cut-off (13% above baseline diameter) and the absolute overall magnitude of the response. Allelic variability was also shown to correlate with cognitive impairments in memory and attention. The data in this study further elucidate the nature of the biological bond between an exaggerated pupil response and the pathology of Alzheimer's disease. ApoE allelic variability is probably linked to pupil response through its influence on tau hyperphosphorylation. The early Alzheimer's pathology seen in the Edinger,Westphal area of cranial nerve III, a major centre for pupil control, is primarily tau-based with significant cell loss in this nucleus leading to central denervation hypersensitivity even in elderly who are clinically silent but who have early pathology. [source] Childhood cancer,mainly curable so where next?ACTA PAEDIATRICA, Issue 4 2000AW Craft More than 70% of childhood cancer is now curable with best modern therapy. The treatment is expensive but in terms of cost per life year saved, USD 1750, compares very favourably with other major health interventions. The rate of improvement in survival is slowing down. New, "designer", treatments are needed and, better still, prevention. The causes of childhood cancer are beginning to emerge. The origin for many is probably in utero and may be initiated by dietary and other environmental exposures perhaps in susceptible individuals. However, one of the great challenges for the future must be to extend the benefits of modern treatment to the 80% of the world's children who currently have little or no access to it in economically disadvantaged and emerging nations. The International Paediatric Oncology Society (SIOP) is leading the way in bringing hope for children with cancer worldwide. In India, with the support of the WHO, there is a "train the trainers" programme. In Africa, pilot studies of cost-effective treatments for Burkitt's lymphoma are producing gratifying results in Malawi and there are several examples of twinning programmes between major centres in developed and less well-developed countries. Conclusions: The future for children with cancer is bright. Most are curable and prevention may be just over the horizon. THOMPSON Summary 1.,The complex terrain and heterogeneous nature of the mountain environment coupled with remoteness from major centres of human activity makes mountains challenging locations for meteorological investigations. Mountainous areas tend to have more varied and more extreme weather than lowlands. 2.,The EMERGE program has the primary aim of assessing the status of remote mountain and sub-arctic lakes throughout Europe for the first time. In this study, we describe the main features of the climate, ice-cover durations and recent temperature trends of these areas. The main weather characteristics of European mountain and sub-arctic lakes are their cold temperatures and year-round precipitation. Mean annual temperatures are generally close to 0 °C, and maximum summer temperatures reasonably close to 10 °C. 3.,Maritime versus continental settings determine the main differences in annual-temperature range among lake districts (10.5 °C in Scotland to 26.7 °C in Northern Finland), and a similar factor for ice-cover duration. Radiation ranges from low (120 W m,2) in the high latitude sub-arctic and high (237 W m,2) in the southern ranges of the Pyrenees and Rila. Similarly, precipitation is high in the main Alpine chain (250 cm year,1 in the Central Southern Alps) and low in the continental sub-arctic (65 cm year,1 in Northern Finland). 4.,The main temporal patterns in air temperature follow those of the adjacent lowlands. All the lake districts warmed during the last century. Spring temperature trends were highest in Finland; summer trends were weak everywhere; autumn trends were strongest in the west, in the Pyrenees and western Alps; while winter trends varied markedly, being high in the Pyrenees and Alps, low in Scotland and Norway and negative in Finland. 5.,Two new, limnological case studies on Lake Redon, in the Pyrenees, highlight the sensitivity of remote lakes to projected changes in the global climate. [source] Renaissance monuments to favourite sonsRENAISSANCE STUDIES, Issue 4 2005Sarah Blake McHam This essay focuses on a previously unnoticed large group of public monuments that was erected at civic expense in fifteenth-century Italy to honour Roman literary notables like Virgil, Ovid, Livy, Pliny the Elder, Pliny the Younger, Catullus and Virtruvius. All the memorials to Roman authors were civic commissions, prominently located in the public spaces of Italian Renaissance cities, either as freestanding statues in the main square, or installed on the exterior of the town hall, or even on the cathedral. Most of the sculptures were not of high artistic quality, or by famous sculptors, or in major centres of artistic production like Florence or Venice. Some never made it past the design stage. For all these reasons, they have not been integrated into the study of Renaissance art history. Consequently, art historians have not realised that some of these parochial monuments introduce important features associated with the Renaissance revival of ancient art that are usually ascribed to later, more famous sculptures. (pp. 458,486) [source] ROLE OF AXILLARY SURGERY IN EARLY BREAST CANCER: REVIEW OF THE CURRENT EVIDENCEANZ JOURNAL OF SURGERY, Issue 7 2000Andrew J. Spillane Background: Controversy continues to surround the best practice for management of the axilla in patients with early breast cancer (EBC), particularly the clinically negative axilla. The balance between therapeutic and staging roles of axillary surgery (with the consequent morbidity of the procedures utilized) has altered. This is due to the increasing frequency of women presenting with early stage disease, the more widespread utilization of adjuvant chemoendocrine therapy and, more recently, the advent of alternative staging procedures, principally sentinel node biopsy (SNB). The aim of the present review is to critically analyse the current literature concerning the preferred management of the axilla in early breast cancer and make evidence-based recommendations on current management. Methods: A review was undertaken of the English language medical literature, using MEDLINE database software and cross-referencing major articles on the subject, focusing on the last 10 years. The following combinations of key words have been searched: breast neoplasms, axilla, axillary dissection, survival, prognosis, and sentinel node biopsy. Results: Despite the trend to more frequent earlier stage diagnosis, levels I and II axillary dissection remain the treatment of choice in the majority of women with EBC and a clinically negative axilla. Conclusions: Sentinel node biopsy has no proven superiority over axillary dissection because no randomized controlled trials have been completed to date. Despite this, SNB will become increasingly utilized due to encouraging results from major centres responsible for its development, and patient demand. Therefore if patients are not being enrolled in clinical trials strict quality controls need to be established at a local level before SNB is allowed to replace standard treatment of the axilla. Unless this is strictly adhered to there is a significant risk of an increase in the frequency of axillary relapse and possible increased understaging and resultant inadequate treatment of patients.
