Selected AbstractsCLINICAL STUDY: Very low dose naltrexone addition in opioid detoxification: a randomized, controlled trialADDICTION BIOLOGY, Issue 2 2009Paolo Mannelli ABSTRACT Although current treatments for opioid detoxification are not always effective, medical detoxification remains a required step before long-term interventions. The use of opioid antagonist medications to improve detoxification has produced inconsistent results. Very low dose naltrexone (VLNTX) was recently found to reduce opioid tolerance and dependence in animal and clinical studies. We decided to evaluate safety and efficacy of VLNTX adjunct to methadone in reducing withdrawal during detoxification. In a multi-center, double-blind, randomized study at community treatment programs, where most detoxifications are performed, 174 opioid-dependent subjects received NTX 0.125 mg, 0.250 mg or placebo daily for 6 days, together with methadone in tapering doses. VLNTX-treated individuals reported attenuated withdrawal symptoms [F = 7.24 (2,170); P = 0.001] and reduced craving [F = 3.73 (2,107); P = 0.03]. Treatment effects were more pronounced at discharge and were not accompanied by a significantly higher retention rate. There were no group differences in use of adjuvant medications and no treatment-related adverse events. Further studies should explore the use of VLNTX, combined with full and partial opioid agonist medications, in detoxification and long-term treatment of opioid dependence. [source] Understanding future ecosystem changes in Lake Victoria basin using participatory local scenariosAFRICAN JOURNAL OF ECOLOGY, Issue 2009Eric O. Odada Abstract Understanding future ecosystem changes is central to sustainable natural resource management especially when coupled with in-depth understanding of impacts of drivers, such as governance, demographic, economic and climate variations and land use policy. This offers comprehensive information for sustainable ecosystem services provision. A foresight process of systematic and presumptive assessment of future state and ecosystem integrity of Lake Victoria basin, as participatory scenario building technique, is presented. Four scenarios have been illustrated as possible future states of the basin over the next twenty years. Using a scenario building model developed in Ventana Simulation (VENSIM®) platform, the paper presents a scenario methodology for tracking changes in lake basin ecosystem status. Plausible trends in land use change, changes in lake levels and contribution of fisheries are presented. This is part of an initial attempt to setup long-term environmental policy planning strategies for Lake Victoria basin. The assumptions, driving forces, impacts and opportunities under each scenario depict major departure and convergence points for an integrated transboundary diagnosis and analysis of regional issues in the basin as well as strategic action planning for long-term interventions. The findings have been presented in terms of temporal, spatial, biophysical and human well-being dimensions. The attempts in this study can be embedded in a policy framework for basin management priority setting and may guide partnerships for environmental management. Smith ABSTRACT Objectives:, To examine physiologic and psychological outcomes associated with maternal obesity in pregnancy and patterns of pregnancy weight gain. To identify effective interventions for maternal obesity. Data sources and study selection:, Search of obesity and pregnancy research conducted over the past 10 years using CIHAHL, Medline ERIC, and PyscInfo databases. Studies including the following keywords were included in the review: obesity, weight gain, body image, pregnancy weight gain, pregnancy obesity complications, preeclampsia and gestational diabetes. Articles were included based on scientific merit and research outcomes. Data synthesis and conclusions:, Maternal obesity is a serious condition that significantly impacts not only mothers' health but also the health and future of their children. It is paramount that all levels of health care providers be aware of consequences of obesity and be knowledgeable of effective interventions. No effective long-term interventions have been demonstrated to prevent or control obesity during pregnancy. The paucity of published results of pregnancy and postpartum interventions to address weight gain in pregnancy suggests the need for more community and individualized based intervention studies, especially focusing on long-term effects. [source] Antioxidant Requirements of Endurance Athletes: Implications for HealthNUTRITION REVIEWS, Issue 3 2006Sarah L. Williams BSc (Hons) This review discusses studies investigating the effects of antioxidant supplementation on exercise-induced oxidative stress with a focus on the health implications. The aim is to determine antioxidant requirements for endurance athletes. Overall, differences in methodology make it difficult to compare the relatively small number of published studies on this topic. The types of studies needed to more adequately assess the health effects of antioxidant supplements in athletes (long-term interventions with hard end points) have not been done. Therefore, there is currently insufficient evidence to recommend antioxidant supplements for endurance athletes. [source] |
