    Effect of Angular Error on Tissue Doppler Velocities and Strain

    One of the major criticisms of ultrasound Doppler is its angle dependency, that is its ability to measure velocity components directly to or from the transducer only. The present article aims to investigate the impact of this angular error in a clinical setting. Apical two- and four-chamber views were recorded in 43 individuals, and the myocardium was marked by hand in each image. We assume that the main direction of the myocardial velocities is longitudinal and correct for the angular error by backprojecting measured velocities onto the longitudinal direction drawn. Strain was calculated from both corrected and uncorrected velocities in 12 segments for each individual. The results indicate that the difference between strain values calculated from corrected and uncorrected velocities is insignificant in 5 segments and within a decimal range in 11 segments. The biggest difference between measured and corrected strain values was found in the apical segments. Strain is also found to be more robust against angular error than velocities because the difference between corrected and uncorrected values is smaller for strain. Considering that there are multiple sources of noise in ultrasound Doppler measurements, the authors conclude that the angular error has so little impact on longitudinal strain that correction for this error can safely be omitted.     Search for the Optimal Right Ventricular Pacing Site: Design and Implementation of Three Randomized Multicenter Clinical Trials

    Background: The optimal site to permanently pace the right ventricle (RV) has yet to be determined. To address this issue, three randomized prospective multicenter clinical trials are in progress comparing the long-term effects of RV apical versus septal pacing on left ventricular (LV) function. The three trials are Optimize RV Selective Site Pacing Clinical Trial (Optimize RV), Right Ventricular Apical and High Septal Pacing to Preserve Left Ventricular Function (Protect Pace), and Right Ventricular Apical versus Septal Pacing (RASP). Methods: Patients that require frequent or continuous ventricular pacing are randomized to RV apical or septal pacing. Optimize RV excludes patients with LV ejection fraction <40% prior to implantation, whereas the other trials include patients regardless of baseline LV systolic function. The RV septal lead is positioned in the mid-septum in Optimize RV, the high septum in Protect Pace, and the mid-septal inflow tract in RASP. Lead position is confirmed by fluoroscopy in two planes and adjudicated by a blinded panel. The combined trials will follow approximately 800 patients for up to 3 years. Results: The primary outcome in each trial is LV ejection fraction evaluated by radionuclide ventriculography or echocardiography. Secondary outcomes include echo-based measurements of ventricular/atrial remodeling, 6-minute hall walk distance, brain natriuretic peptide levels, and clinical events (atrial tachyarrhythmias, heart failure, stroke, or death). Conclusion: These selective site ventricular pacing trials should provide evidence of the importance of RV pacing site in the long-term preservation of LV function in patients that require ventricular pacing and help to clarify the optimal RV pacing site.     Apical and basal neurones isolated from the mouse vomeronasal organ differ for voltage-dependent currents

    The mammalian vomeronasal organ (VNO) contains specialized neurones that transduce the chemical information related to pheromones into discharge of action potentials to the brain. Molecular and biochemical studies have shown that specific components of the pheromonal transduction systems are segregated into two distinct subsets of vomeronasal neurones: apical neurones and basal neurones. However, it is still unknown whether these neuronal subsets also differ in other functional characteristics, such as their membrane properties. We addressed this issue by studying the electrophysiological properties of vomeronasal neurones isolated from mouse VNO. We used the patch-clamp technique to examine both the passive membrane properties and the voltage-gated Na+, K+ and Ca2+ currents. Apical neurones were distinguished from basal ones by the length of their dendrites and by their distinct immunoreactivity for the putative pheromone receptor V2R2. The analysis of passive properties revealed that there were no significant differences between the two neuronal subsets. Also, apical neurones were similar to basal neurones in their biophysical and pharmacological properties of voltage-gated Na+ and K+ currents. However, we found that the density of Na+ currents was about 2-3 times greater in apical neurones than in basal neurones. Consistently, in situ hybridization analysis revealed a higher expression of the Na+ channel subtype III in apical neurones than in basal ones. In contrast, basal neurones were endowed with Ca2+ currents (T-type) of greater magnitude than apical neurones. Our findings indicate that apical and basal neurones in the VNO exhibit distinct electrical properties. This might have a profound effect on the sensory processes occurring in the VNO during pheromone detection. [source]


    Immediate surgical repositioning following intrusive luxation: a case report and review of the literature

    Abstract,,, This report presents a case of severe intrusive luxation of mature maxillary lateral incisor in a 10-year-old boy. The intruded tooth was immediately repositioned (surgical extrusion) and splinted within 2 h following injury. Tetracycline therapy was initiated at the time of repositioning and maintained for 10 days. Pulp removal and calcium hydroxide treatment of the root canal was carried out after repositioning. Splint was removed 1 month later. Definitive root canal treatment with gutta percha was accomplished at the second month recall. Clinical and radiographic examination 28 months after the surgical extrusion revealed satisfactory apical and periodontal healing. [source]


    Cloning and characterization of cDNA for syndecan core protein in sea urchin embryos

    The cDNA for the core protein of the heparan sulfate proteoglycan, syndecan, of embryos of the sea urchin Anthocidaris crassispina was cloned and characterized. Reverse transcription,polymerase chain reaction (RT-PCR) was used with total ribonucleic acid (RNA) from late gastrula stage embryos and degenerate primers for conserved regions of the core protein, to obtain a 0.1 kb PCR product. A late gastrula stage cDNA library was then screened using the PCR product as a probe. The clones obtained contained an open reading frame of 219 amino acid residues. The predicted product was 41.6% identical to mouse syndecan-1 in the region spanning the cytoplasmic and transmembrane domains. Northern analysis showed that the transcripts were present in unfertilized eggs and maximum expression was detected at the early gastrula stage. Syndecan mRNA was localized around the nuclei at the early cleavage stage, but was then found in the ectodermal cells of the gastrula embryos. Western blotting analysis using the antibody against the recombinant syndecan showed that the proteoglycan was present at a constant level from the unfertilized egg stage through to the pluteus larval stage. Immunostaining revealed that the protein was expressed on apical and basal surfaces of the epithelial wall in blastulae and gastrulae. [source]


    Membrane dynamics of cleavage furrow closure in Xenopus laevis

    Abstract Epithelial membrane polarity develops early in Xenopus development, with membrane inserted along the earliest cleavage furrows by means of localized exocytosis. The added surface constitutes a new basolateral domain important for early morphogenesis. This basolateral surface becomes isolated from the outside by furrow closure, a zippering of adjacent apical,basolateral margins. Time-lapse microscopy of membrane-labeled embryos revealed two distinct kinds of protrusive activity in furrow closure. Early in furrowing, protrusive activity was associated with purse-string contractility along the apical,basolateral margins. Later in furrow progression, a basolateral protrusive zone developed entirely within the new membrane domain, with long motile filopodia extending in contractile bands from the exposed surfaces. Filopodia interacting with opposing cell surfaces across the cleavage furrow appeared to mediate blastomere,blastomere adhesion, contact spreading and lamellipodial protrusion. Interference with these dynamic activities prevented furrow closure, indicating a basic role for both marginal and basolateral protrusive activities in early embryogenesis.     Role of Parasternal Data Acquisition During Contrast Enhanced Real-Time Three-Dimensional Echocardiography

    Background: Recent technical developments have resulted in high-resolution real time three-dimensional echocardiography (RT3DE). The purpose of this study was to investigate the beneficial role of parasternal-acquired images in addition to apical-acquired images during contrast stress RT3DE. Methods: The study comprised 30 consecutive patients (52 ± 11 years, 18 males) with chest pain referred for routine stress testing. The contrast RT3DE images were acquired from the apical and parasternal window with a Sonos 7500 echo system attached to a X4 matrix array transducer. Results: From the apical and parasternal acquisition, 464 segments (91%) and 267 segments (52%) could be analyzed, respectively (P < 0.001). From the apical window, more basal segments were not analyzable (22 of 180, 12% vs. 24 of 330, 7%; P = 0.06). From the parasternal window, more apical segments were not analyzable (117 of 150, 78% vs. 126 of 360, 35%; P < 0.01). The mean image quality index of the 464 analyzable segments from the apical-acquired images was 2.43. Fourteen of 180 basal segments (8%), 12 of 180 midventricular segments (7%) and 2 of 150 apical segment (1%) were only available with parasternal data acquisition. In addition to these 28 segments, 79 segments (15%) already visualized from the apical window improved in quality. The overall mean image quality index, now assessed from 492 (96%) of all segments, using both the apical and parasternal acquired data, improved to 2.74 (P < 0.05). Conclusions: Addition of parasternal to apical acquisition of contrast RT3DE data can decrease the number of nonvisualized segments and improve mean image quality. [source]


    Mid-Ventricular Ballooning Heart Syndrome

    Stress cardiomyopathies have been increasingly reported these last years, especially in women as a transient left ventricular apical ballooning syndrome. We report six cases in whom, in the context of anxious situations, echocardiograms and ventriculographies revealed mid-ventricular akinesis with preservation of apical and basal contractilities with normal coronary arteriography. This "mid-ventricular ballooning heart syndrome " should probably be classified as a new type of heart stress related syndrome. [source]


    Color M-Mode Flow Propagation Velocity: Is It Really Preload Independent?

    Objective: This study investigates the change in flow propagation velocity (FPV) in uremic patients who undergo regular hemodialysis (H/D). Materials and Methods: We studied 93 uremic patients (44 men and 49 women; 59 ± 14-years-old) receiving regular hemodialysis. Patients were separated by baseline left ventricular ejection fraction (LVEF): 71 patients with LVEF > 50% (group 1), 13 patients with LVEF 35,50% (group 2), 7 patients with LVEF < 35% (group 3). All patients were in sinus rhythm before H/D. They received complete transthoracic echocardiographic examinations. Flow propagation velocity was measured by color M-mode echocardiography in apical four chambers view. All these parameters were obtained before and after H/D. Paired data were compared. According to different H/D amounts, we viewed the FPV response after H/D in variant baseline LVEF groups. Result: The baseline FPV became lower in patients with low LVEF. After H/D, obvious decrement of FPV occurred in group 1, but there were no obvious changes in groups 2 and 3. In fact, a slight increment of FPV was found in group 3. In patients with baseline LVEF > 50%, FPV after H/D was almost always lower, regardless of H/D amount. But there was different response in patients with baseline LVEF < 50%. Conclusion: Flow propagation velocity is preload independent in patients with LVEF < 50%, but it is preload-dependent in patients with LVEF > 50%. [source]


    An Echocardiographic Analysis of the Long-Term Effects of Carvedilol on Left Ventricular Remodeling, Systolic Performance, and Ventricular Filling Patterns in Dilated Cardiomyopathy

    Background: The long-term clinical benefit of beta blockade is well recognized, but data quantifying long-term effects of beta blockade on remodeling of the left ventricle (LV) is limited. Methods: This consecutive series evaluates the long-term response of the LV to the addition of carvedilol to conventional therapy for dilated cardiomyopathy. There were 33 patients who had a LV ejection fraction <45%, LV enlargement and symptomatic heart failure. Quantitative Doppler echocardiography was performed at baseline 6, 12, 24, and 36 months after initiation of carvedilol to evaluate LV ejection fraction, LV volume, wall stress, mass, regional function, and diastolic performance. Results: Compared to baseline there was a significant and sustained reduction in end-systolic volume and end-systolic wall stress with a corresponding improvement in LV ejection fraction. The LV mass did not decline but relative wall thickness increased toward normal. An analysis of regional wall motion responses showed an improvement in all areas, particularly the apical, septal, and lateral walls that was significantly more frequent in patients with a nonischemic etiology. Filling patterns of the LV remained abnormal throughout the study but changed with therapy suggesting a decline in filling pressures. These changes were sustained for 3 years. Conclusion: (1) The addition of carvedilol to conventional therapy for a dilated cardiomyopathy significantly improves LV ejection fraction and reduces LV end-systolic volume and wall stress for at least 3 years, (2) the response to 6 months of treatment predicts the long-term response, (3) the typical response is partial improvement of the LV, complete return to normal size, and function is uncommon, and (4) abnormalities of LV filling persist in virtually all patients throughout the course of treatment.     Enhanced Left Ventricular Endocardial Border Delineation with an Intravenous Injection of SonoVue, a New Echocardiography Contrast Agent:

    A European Multicenter Study
    The safety and efficacy of SonoVue (also referred to as BR1), a new contrast agent for delineating endocardial border of the left ventricle after intravenous administration, was assessed. Two hundred and eighteen patients with suspected coronary artery disease undergoing fundamental echocardiography for the assessment of left ventricle were enrolled in a prospective multicenter, single blind, cross-over study with random sequence allocation of four different doses of SonoVue. Endocardial border definition in the apical and parasternal views was scored as O = not visible, 1 = barely visible, and 2 = well visualized before and after contrast enhancement. Analysis was performed by two pairs of off-site observers. Safety of SonoVue was also assessed. Results of our study indicated that the mean improvements in the endocardial border visualization score were as follows: 3.1 ± 7.8 (95% CI, 2.5 and 3.7) for 0.5 ml, 3.4 ± 8.0 (95% CI, 2.8 and 4.0) for 1 ml, 3.4 ± 7.9 (95% CI, 2.8 and 4.0) for 2 ml, and 3.7 ± 8.0 (95% CI, 3.1 and 4.3) for 4 ml (P < 0.05 for all doses from baseline). Changes from baseline in endocardial visualization scores were also seen in the apical views (P < 0.05) and they were dose-dependent (P < 0.001). Similar enhancements of endocardial visualization scores were observed in the apical views in patients with suboptimal baseline echocardiographic images. Diagnostic confidence for assigning a score and image quality also were significantly better following contrast enhancement. No significant changes in the laboratory parameters and vital signs were noted following contrast enhancement, and the side effects were minimal. It was concluded that SonoVue is safe and effective in delineating endocardial border, including in patients with suboptimal baseline images. [source]


    Altered membrane glycoprotein targeting in cholestatic hepatocytes

    Eur J Clin Invest 2010; 40 (5): 393,400 Abstract Background, Hepatocytes are polarized epithelial cells with three morphologically and functionally distinct membrane surfaces: the sinusoidal, lateral and canalicular surface domains. These domains differ from each other in the expression of integral proteins, which concur to their polarized functions. We hypothesize that the cholestasis-induced alterations led to partial loss of hepatocyte polarity. An altered expression of membrane proteins may be indicative of functional disorders. Alkaline liver phosphatase (ALP), one of the most representative plasma membrane glycoproteins in hepatocytes, is expressed at the apical (canalicular) pole of the cell. Because the release of ALP protein in the bloodstream is significantly increased in cholestasis, the enzymatic levels of plasma ALP have major relevance in the diagnosis of cholestatic diseases. Here we assess the cholestasis-induced redistribution of membrane glycoproteins to investigate the ALP release. Materials and methods, We performed enzymatic histochemistry, immunohistochemistry, lectin histochemistry, immunogold and lectin-and immunoblotting studies. Experimental cholestasis was induced in rats by ligation of common bile duct (BDL). Results, The BDL led to altered membrane sialoglycoprotein targeting as well as to ultrastructural and functional disorders. Disarrangement of the microtubular system, thickening of the microfilamentous pericanalicular ectoplasm and disturbance of the vectorial trafficking of membrane glycoprotein containing vesicles were found. Conclusions, Altogether, results indicate that the cholestasis-induced partial loss of hepatocyte cell polarity leads to mistranslocation of ALP to the sinusoidal plasma membrane from where the enzyme is then massively released into the bloodstream.     Azido, Cyanato, and Thiocyanato Coordination Induced Distortions in Pentacoordinated [CoIIA(bip)]2 (A = NCS,, N3,, or NCO,) Complexes

    Abstract A new family of distorted pentacoordinate [Co2] complexes was prepared and structurally characterized. In [CoIIA(bip)]2·S (1,3) [A = NCS,, N3,, NCO,; S = dmf, MeOH, dcm; Hbip = 2,6-bis(phenylmethyliminomethyl)-4-methylphenol], the nonbonded Co···Co separations are in the 3.243 to 3.254 Ĺ range, and the pseudohalide-coordinated CoII ions are asymmetrically doubly bridged by the phenolate oxygen atoms of the ligand. The complementary basal,apical (b,a) and apical,basal (a,b) coordination modes of the phenolate bridges of bip, offer one O and two N donors for metal-ion coordination. In the three dinuclear complexes, the h.s. CoII ions are coupled antiferromagnetically to yield a singlet ground state. The solid-state variable-temperature magnetic susceptibility measurement data on the complexes were fitted to an isotropic Heisenberg dimer model that allowed estimation of the antiferromagnetic interactions for 1,3.     First (Peroxo)vanadium(V) Complex with Heteroligand Formed in Reaction System , Synthesis, Structure and Reactivity of K[VO(O2)(omeida)]·H2O {omeida = N -[2-(2-oxomorpholine-4-yl)ethyl]iminodiacetato(2,)}

    Abstract The crystalline peroxo complex of vanadium(V), K[VO(O2)(omeida)]·H2O, where omeida is a ,-lactone derivative, N -[2-(2-oxomorpholine-4-yl)ethyl]iminodiacetate(2,), has been obtained by reaction of vanadate with H2O2 and N -(2-hydroxyethyl)ethylenediaminetriacetic acid (HEDTA) in acidic aqueous solution at pH = 3 and 278 K. X-ray analysis revealed a distorted pentagonal-bipyramidal coordination around the vanadium atom, with a typical cis arrangement of oxo and peroxo ligands in apical and equatorial positions, respectively. Two amino nitrogen atoms of the tetradentate omeida(2,)-N1,N2,O1,O2 ligand occupy the neighbouring equatorial positions of the pentagonal plane, and two oxygen atoms of carboxymethyl groups bound to the same N1 nitrogen atom are in equatorial and apical positions. The six-membered lactone ring in omeida was formed in the reaction solution from carboxy and hydroxy groups not involved in coordination with the vanadium atom. The 51V NMR spectra of K[VO(O2)(omeida)]·H2O, and of peroxovanadate/HEDTA/H2O and vanadate/HEDTA/H2O solutions, as well as the 1H NMR spectrum of HEDTA, proved that lactone ring closure proceeds only in peroxovanadate but not vanadate solutions. Spectroscopic investigation of the oxygen transfer reaction from the peroxo ligand in [VO(O2)(omeida)], to the thiolato sulfur atom in [Co(en)2{S(CH2)2NH2}]2+ or [Co(en)2(cyst)]+, and of the oxidation of N -acetyl- L -cysteine by K[VO(O2)(omeida)]·H2O, revealed much more complicated reaction mechanisms than those of other (amino-polycarboxylato)monoperoxo complexes of vanadium(V).     Vestigial prototroch in a basal nemertean, Carinoma tremaphoros (Nemertea; Palaeonemertea)

    Summary Nemerteans have been alleged to belong to a protostome clade called the Trochozoa that includes mollusks, annelids, sipunculids, echiurids, and kamptozoans and is characterized by, among other things, the trochophore larva. The trochophore possesses a prototroch, a preoral belt of specialized ciliary cells, derived from the trochoblast cells. Nemertea is the only trochozoan phylum for which presence of the trochophore larva possessing a prototroch had never been shown. However, so little is known about nemertean larval development that comparing it with development of other trochozoans is difficult. Development in the nemertean clade Pilidiophora is via a highly specialized planktonic larva, the pilidium, and most of the larval body is lost during a drastic metamorphosis. Other nemerteans (hoplonemerteans and palaeonemerteans) lack a pilidium, and their development is direct, forming either an encapsulated or planktonic "planuliform" larva, producing a juvenile without a dramatic change in body plan. We show that early in the development of a member of a basal nemertean assemblage, the palaeonemertean Carinoma tremaphoros, large squamous cells cover the entire larval surface except for the apical and posterior regions. Although apical and posterior cells continue to divide, the large surface cells cleavage arrest and form a contorted preoral belt. Based on its position, cell lineage, and fate, we suggest that this belt corresponds to the prototroch of other trochozoans. Lack of differential ciliation obscures the presence of the prototroch in Carinoma, but differentiation of the trochoblasts is clearly manifested in their permanent cleavage arrest and ultimate degenerative fate. Our results allow a meaningful comparison between the development of nemerteans and other trochozoans. We review previous hypotheses of the evolution of nemertean development and suggest that a trochophore-like larva is plesiomorphic for nemerteans while a pilidium type of development with drastic metamorphosis is derived.     Facts, fantasies and fun in epithelial physiology

    The hallmark of epithelial cells is their functional polarization. It is those membrane proteins that are distributed differentially, either to the apical or to the basal surface, that determine epithelial physiology. Such proteins will include ,pumps', ,channels' and ,carriers', and it is the functional interplay between the actions of these molecules that allows the specific properties of the epithelium to emerge. Epithelial properties will additionally depend on: (a) the extent to which there may be a route between adjacent cells (the ,paracellular' route); and (b) the folding of the epithelium (as, for example, in the loop of Henle). As for other transporters, there is polarized distribution of amino-acid carriers; the molecular basis of these is of considerable current interest with regard to function, including ,inborn errors' (amino-acidurias); some of these transporters have additional functions, such as in the regulation of cell fusion, in modulating cell adherence and in activating intracellular signalling pathways. Collaboration of physiologists with fly geneticists has generated new insights into epithelial function.     Redistribution of the sheep neonatal Fc receptor in the mammary gland around the time of parturition in ewes and its localization in the small intestine of neonatal lambs

    Summary Maternal immunity is mediated exclusively by colostral immunoglobulins in ruminants. As the neonatal Fc receptor (FcRn) is suggested to be involved in the transport of immunoglobulin G (IgG) in the mammary gland, we cloned this receptor from sheep and analysed its expression in the mammary gland around the time of parturition and also in the small intestine from the newborn lamb. FcRn heavy-chain mRNA was detected (by using in situ hybridization) exclusively in the acinar and ductal epithelial cells in mammary gland biopsies both before and after parturition. Immunohistochemistry revealed that the cytoplasm of the epithelial cells of the acini and ducts in the mammary gland biopsies stained homogeneously before parturition. A remarkable difference was observed in the pattern after lambing, where the apical side of the cells was strongly stained. The presence of the FcRn in the acinar and ductal epithelial cells of the mammary gland, and the obvious change in distribution before and after parturition, indicate that the FcRn plays an important role in the transport of IgG during colostrum formation in ruminants. Immunohistochemical analysis detected a strong apical and a weak basal FcRn signal in the duodenal crypt cells of a neonatal lamb, which have been previously demonstrated to secrete IgG1 in newborn ruminants. The FcRn was not detected in the duodenal enterocytes, which absorb intact IgG from the colostrum in a non-specific manner. These data suggest that FcRn is involved in IgG1 secretion in ruminant epithelial cells. [source]

    Abstract The present paper describes two new species of Microgaster Latreille, 1804, viz: M. longiterebra sp. nov. (,) and M. longicaudatus sp. nov. (,). M. longiterebra sp. nov. is described from Kunming, Yunnan Prov. in China closely related to M. subcompletus, but differs from the latter in the combination of the following features: 1) APOL as long as OD; 2) Mesonotum strongly punctate on anterior 2/ 3) r shorter than width of stigma; 4) cu-a of hind wing medially curved; 5) Apex of hypopygium ending far beyond apex of abdomen. M. longicaudatus sp. nov. is described from Tianmushan, Zhejiang Prov. in China closely related to M. ductilis, but differs from the latter in the combination of the following features: 1) middle and hind femora reddish yellow only with apical 1/4 blackish; 2) 1-SR 0.5 times as long as 1-M; 3) the hairy part of ovipositor sheath 0.5 times as long as hind tibia; 4) tergite 3 with slightly weaker rugosity.     The effect of ultrasonically activated irrigation on reduction of Enterococcus faecalis in experimentally infected root canals

Abstract Aim, To investigate the ability of an ultrasonically activated irrigating system to eliminate bacteria from the canal wall and dentinal tubules of extracted teeth. Methodology, One hundred and thirty roots of intact human teeth were inoculated with Enterococcus faecalis for 4 weeks. The straight roots were randomly allocated to a baseline group (n = 25) or subjected to routine cleaning and shaping procedures (n = 105). Two sub-groups of prepared canals were then additionally exposed either to ultrasonic irrigation with 1% sodium hypochlorite (NaOCl) for 1 min (n = 35) or to 1 week of intracanal medication with calcium hydroxide [Ca(OH)2] (n = 35). All roots were processed for light microscopy (Brown and Brenn stain) (n = 28) or scanning electron microscopy (n = 7). Triplicate histological sections from each of the apical, middle and coronal thirds were scored for bacterial presence using pre-defined criteria. Results, Baseline bacterial penetration resulted in an average depth of tubule invasion of 151 ,m. Routine canal preparation failed to eliminate bacteria consistently from either the canal wall or within tubules. Ultrasonic irrigation and medication with Ca(OH)2 consistently eliminated bacteria from the canal wall (P < 0.001) compared with baseline and routine treatment, and more frequently from dentinal tubules than routine canal preparation alone (P < 0.01). Ultrasonic irrigation was as effective in bacterial reduction as 1 week of intracanal medication with Ca(OH)2, but neither led to complete bacterial elimination in all roots. Conclusions, Ultrasonically activated irrigation for 1 min with 1% NaOCl after canal preparation in straight root canals is potentially an effective supplementary step in microbial control.     Ex vivo evaluation of the accuracy and coefficient of repeatability of three electronic apex locators using a simple mounting model: a preliminary report

Abstract Aim, To compare ex vivo the accuracy and coefficient of repeatability of three electronic apex locators in locating the apical constriction. Methodology, Thirty-one single-rooted teeth were used. The teeth were sectioned at the cement,-enamel junction. A mounting model was used for the measurement of electronic length (EL). The Root ZX-II, the Mini Apex Locator and the Novapex were used for electronic measurements. Each electronic measurement was obtained and repeated. After the last measurement, the file was cemented in place, and the apical 4 mm of each root canal was exposed. The distance from the tip of the file to the apical constriction was determined by three investigators and compared with the corresponding ELs. Results, The coefficient of repeatability of all devices was acceptable: Root ZX-II, 0.04 mm; Mini Apex Locator, 0.10 mm; and Novapex, 0.08 mm. There was little variation in inter-examiner agreement; the ,C (Lin) correlation coefficient was 0.83 for examiners 1 and 2, 0.88 for examiners 1 and 3 and 0.99 for examiners 2 and 3. Using the Root ZX-II, 13 of 31 electronic measurements were located at the apical constriction (42%). Otherwise, the tip of the file was not located at the apical constriction in any of the electronic measurements with the other two devices. The Wilcoxon signed rank test did not reveal any statistical difference between the Root ZX-II measurements and the actual length (P = 0.628), but there was a statistical difference between the Mini Apex Locator and Novapex measurements and the actual length position (P < 0.05). Conclusions, The devices tested in this study had a high coefficient of repeatability. The Root ZX-II was accurate, but the Mini Apex Locator and Novapex were not accurate in locating the apical constriction. [source]


    Effectiveness of HERO 642 versus Hedström files for removing gutta-percha fillings in curved root canals: an ex vivo study

    Abstract Aim, To compare the effectiveness of gutta-percha removal and the maintenance of canal anatomy when using the HERO 642 system or Hedström files (H-files) in mandibular molar teeth. Methodology, The root canals of 40 mandibular molar teeth were instrumented using H-files and filled with gutta-percha and sealer. After 1 year in storage, the roots were sectioned horizontally to provide apical, middle and coronal root thirds. Sections were photographed, and an individual muffle was produced for each tooth. Teeth were randomly divided into four groups (n = 10) and the gutta-percha removed using either the HERO 642 system or H-files, with or without solvent. Digital images of the root canals were then re-taken. Root thirds were inspected for lateral perforations, and the percentage of the residual canal filling was determined on postoperative images. Transportation and centring ratio were calculated using preoperative and postoperative images of the cross-sections of root thirds. Results, H-files groups were associated with less filling material than the HERO 642 system (H-files,HERO 642 P = 0.056, H-files,HERO 642+solvent P = 0.041, H-files + solvent,HERO 642 P = 0.018, H-files + solvent,HERO 642 + solvent P = 0.016). The percentage of residual filling material was similar in the apical thirds, and the contribution of solvent to canal debridement was not statistically significant (P > 0.05). Perforation occurred mesiobuccally in 48% of specimens in the apical sections of mesial roots. There were no significant differences for centring ratio, transportation and perforation rate between groups. Conclusions, H-files left less gutta-percha overall; however, there was no difference in the apical third. The effect of solvent was not remarkable. Both instrument systems created a large number of perforations. [source]


    Analysis of the gutta-percha filled area in C-shaped mandibular molars obturated with a modified MicroSeal technique

    Abstract Aim, To analyse the gutta-percha filled area of C-shaped molar teeth root filled with the modified MicroSeal technique with reference to the radiographic features and the C-shaped canal configuration. Methodology, Twenty-three mandibular second molar teeth with C-shaped roots were classified according to their radiographic features as: type I , merging, type II , symmetrical and type III , asymmetrical. The canals were root filled using a modified technique of the MicroSeal system. Horizontal sections at intervals of 600 ,m were made 1 mm from the apex to the subpulpal floor level. The percentage of gutta-percha area from the apical, middle and coronal levels of the radiographic types was analysed using the Kruskal,Wallis test. Complementary analysis of the C-shaped canal configurations (C1, C2 and C3) determined from cross-sections from the apical third was performed in a similar way. Results, No significant differences were found between the radiographic types in terms of the percentage of gutta-percha area at any level (P > 0.05): apical third, type I: 77.04%, II: 70.48% and III: 77.13%, middle third, type I: 95.72%, II: 93.17%, III: 91.13% and coronal level, type I: 98.30%, II: 98.25%, III: 97.14%. Overall, the percentage of the filling material was lower in the apical third (P < 0.05). No significant differences were found between the C-shaped canal configurations apically; C1: 72.64%, C2: 79.62%, C3: 73.51% (P > 0.05). Conclusions, The percentage of area filled with gutta-percha was similar in the three radiographic types and canal configuration categories of C-shaped molars. These results show the difficulty of achieving predictable filling of the root canal system when this anatomical variation exists. In general, the apical third was less completely filled. [source]


    Influence of tooth age and root section on root dentine dye penetration

    Abstract Aim, To investigate dye permeability of root dentine according to patients' age, root section and dye penetration time. Methodology, A total of 96 extracted human single-rooted teeth, assigned to four age groups (<30, 30,45, 45,60 and >60 years) were separated at the cemento,enamel junction and root canals were enlarged. The root surfaces were coated with cyanocrylate to prevent external dye penetration and centrifuged in distilled water to eliminate air. For dye penetration the root canals were filled with methylene blue 5%. After 1, 30 and 60 days eight roots per age group were cross-sectioned in 1 mm slices. Dye penetrated areas and the complete dentine areas were digitized and measured. Differences between groups were judged with anova and LSD, P < 0.05 or P < 0.01. Results, The root section, the patients' age and the penetration time influenced significantly the penetrated areas (P < 0.05). After 1 and 30 days significant differences could be found only in the apical root sections between all age groups (P < 0.05). Dye penetration areas systematically decreased with increasing age and also from coronal to apical (P < 0.01). Conclusions, Age influenced dye penetration significantly. Dye penetration also depended on the location (coronal, middle and apical) within the root canal. These findings indicate that there may be a correlation between the tooth age and permeability of root dentine, which may influence the distribution and effectiveness of drugs used for root canal disinfection.     Influence of endodontic sealer cement on fibreglass post bond strength to root dentine

    Abstract Aim, To test the hypothesis that the composition of endodontic sealer cements and the time elapsed between root filling and fibreglass post fixation interferes with adhesion to root canal dentine. Methodology, Sixty bovine incisor roots were divided into five groups (n = 12): CI, unfilled; SI, filled with a calcium hydroxide-based cement-Sealer 26, and immediate post fixation; S7, Sealer 26 and post fixation after 7 days; EI, filled with a zinc oxide and eugenol-based cement-Endofill and immediate fixation; and E7 Endofill and post fixation after 7 days. The posts were cemented with adhesive system and dual resin cement. Ten roots were cross-sectioned to obtain two 1-mm-thick discs for each cervical (TC), middle (TM) and apical (TA) third of the prepared root portion. The posts were submitted to a micropush-out test. The other two teeth were evaluated using scanning electron microscopy to analyse the bond interface. Data were analysed using anova, Tukey and Dunnett tests (P < 0.05). Results, Group EI was associated with a significant reduction in bond strength values irrespective of the root region; TC = 3.50 MPa (P = 0.0001); TM = 2.22 MPa (P = 0.0043) and TA = 1.45 MPa (P = 0.003). Region of canal had an influence on the values for the cement used in group E7, in which only the TA presented differences from the CI. Conclusions, Endofill interfered negatively with the bond to root dentine along its full length and in the TA when post fixation was delayed for 7 days. Bond strength decreased from crown to apex in all groups. [source]


    Development of an ex vivo model for the study of microbial infection in human teeth

    Aims, (1) To infect human teeth artificially to mimic root canal and dentine infection, using the Constant Depth Film Fermenter (CDFF); (2) To verify the similarity of the infections to those found, in vivo, using culture and microscopy (SEM, LM and TEM). Methodology, Human teeth [n = 38 and n = 28, for phases I (preliminary) and II (definitive), respectively] were infected within the CDFF for a period of 28 days and at pre-selected time points were removed, externally decontaminated using validated protocols and subjected to either culture-dependent or microscopy protocols. The condition of the teeth was varied in phase I to establish the feasibility of the approach and identify optimal conditions. This informed the selection of optimal conditions for definitive test in phase II. For culture-dependent analysis in this phase, a dentine filing sample was obtained from the apical 5 mm of the root canal and cultured anaerobically to allow isolation of individual strains. Bacterial DNA was extracted from purified isolates, the 16S rRNA genes amplified by PCR and the amplicons sequenced for identity using sequence databases. Teeth assigned for microscopy were post-fixed in 3% gluteraldehyde after removal from the CDFF and then subjected to appropriate protocols prior to microscopic evaluation of the infection. Results, All three microscopy techniques and culture-dependent analysis confirmed infection of the human teeth using the CDFF, with root canal infections visually resembling closely those seen in vivo. Furthermore, partial 16S rRNA gene sequencing of DNA from cultured isolates confirmed a selective number of 7,9 genera/species in the apical portion of two teeth each at 7 and 28 days; these taxa are also commonly recovered from teeth with apical periodontitis, in vivo. There were no objective measures other than speciation and topographical evaluation to compare the artificial and real (in vivo) infections. Conclusions, The proposed ex vivo model has the potential for development into an investigative tool for studying the dynamics of bacterial ecology in infected root canals, both before and after treatment. Its advantage is the ability to control both the abiotic and biotic factors. There is a need for the development of objective measures to compare artificial and real bacterial biofilms. [source]


    Evaluation of canal filling after using two warm vertical gutta-percha compaction techniques in vivo: a preliminary study

    Abstract Aim, To evaluate the quality of root canal filling when comparing two warm gutta-percha filling techniques in vivo. Methodology, Human teeth were randomly divided into two equal groups, with 30 canals each. The root canals were shaped by hand and ProFile 0.04 rotary instruments to size 20,40 at the end-point and then filled with gutta-percha cones and AH-Plus. In group A, a traditional warm vertical compaction technique was performed using the Touch'n Heat, and back-filling with the Obtura II. In group B, a modified warm vertical compaction technique was used: small amounts of gutta-percha were removed, and the remaining most apical 3 mm were compacted with a 1 mm movement; then thermomechanical back-filling was performed. The teeth were extracted, stored in dye, cleared, and the distance between the apex and apical limit of the filling, linear dye penetration, and voids were measured from the buccal, lingual, mesial and distal perspective. The homogeneity of variance and means was verified using Levene's test and t -test. anova and Dunnett post hoc test were used to establish the significance and to analyse the effects through multiple comparisons. Results, Compared with the specimens of group A, the specimens of group B exhibited less mean linear dye penetration (P < 0.05), smaller void length (P , 0.05) and maximal width (P , 0.05) when examined in all four views, and a more precise filling when viewed from the buccal aspect (P < 0.05). Conclusions, The modified warm vertical compaction technique with apical back-filling produced a more effective and precise three-dimensional filling. [source]


    The influence of root canal shape on the sealing ability of two root canal sealers

    Abstract Aim, To evaluate the influence of root canal form on the sealing ability of two root canal sealers. Methodology, Twenty radiographically confirmed straight and 20 curved root canals were prepared with a stepback hand filing technique. Root canal aberrations created during preparation were determined by the use of double exposure radiographic technique. The prepared canals were filled with lateral condensation of gutta-percha and one or other of two root canal sealers (Pulp Canal Sealer and Sealapex). Leakage along the apical 10 mm of roots was measured with a fluid transport model at 1, 3, 6, 9 and 12-month intervals. Results, There were no statistically significant differences between straight and curved root canals (P > 0.05) for prevalence of root canal transportation. The prevalence of apical transportation was 80% in the straight and 85% in the curved root canals. A complete seal was more frequently observed in straight canals compared with curved canals. Utilizing the ,* index, analysis showed the filling with Sealapex allowed more leakage than Pulp Canal Sealer at 1 year. Conclusion, Under the conditions of the study, root canal form influenced short-term sealing ability. In the long-term the seal was affected by the sealer rather than root canal form. [source]


    Anatomic Endodontic Technology (AET) , a crown-down root canal preparation technique: basic concepts, operative procedure and instruments

    Abstract Aim, To illustrate the conceptual basis and the operative procedure of the Anatomic Endodontic TechnologyTM (AET) technique and to illustrate the specific instruments used in each phase. Summary, The basic characteristics of the AETTM technique are reported. The instruments and procedure are described in three phases: coronal access, coronal-middle preparation and apical preparation. In the first phase, correct cavity design is described. In the coronal-middle phase, the use of four stainless steel shaping instruments, powered by a reciprocating handpiece is described, incorporating a brushing-milling action against canal walls. During the apical phase, dedicated apical stainless steel and NiTi hand instruments are used to complete the preparation. The stainless steel apical files are used with a 1/4 turn and withdrawal movement whilst the NiTi are used in 360° rotary motion. Key learning points ,,Most current canal shaping techniques do not prepare all the canal walls and can result in over-enlargement in some areas. ,,AETTM defines three regions of the canal: coronal (from the cavo-surface of the access cavity), coronal-middle, and apical. ,,Coronal-middle instrumentation is undertaken with four mechanically driven stainless steel shaping instrument used in brushing-milling action against canal walls. ,,Apical preparation is completed by hand and with the formation of a stop.     Comparison of MicroSeal and System B/Obtura II obturation techniques

    Abstract Aim, To compare the proportion of gutta-percha, sealer and voids following the filling of simulated root canals in plastic blocks using two warm gutta-percha techniques. Methodology, Ten simulated root canals in plastic blocks were prepared using System GT Rotary Files® and a .02 taper size 40 Quantec rotary nickel,titanium instrument and then filled with MicroSeal and Kerr EWT sealer. A further 10 simulated canals were prepared solely with System GT Rotary Files® and obturated with System B/Obtura II and Kerr EWT sealer. All blocks were sectioned at 1 mm intervals. The sectioned root fillings were photographed through a stereomicroscope and the resultant slides scanned. The images were analysed using adobe photoshop 6.0 to give the proportions of gutta-percha, sealer or void at each level. The data were analysed using unpaired Student's t -test and Mann,Whitney U -test. Results, The MicroSeal technique produced a dense homogeneous gutta-percha fill at the apical 1 and 2 mm levels similar to the System B/Obtura II technique, but further coronally the sealer became mixed into the MicroFlow gutta-percha producing a heterogeneous mass with significantly less (P < 0.05) solid gutta-percha in the root filling compared to System B/Obtura II. The MicroSeal technique generally had more pooling of cement than the System B technique, however, this was only significantly different (P < 0.05) at the 4 mm level. The System B/Obtura II obturation gave an average of 99.27% ± 1.09 gutta-percha fill at all levels. Conclusions, The MicroSeal technique produced a heterogeneous fill consisting of the MicroFlow Master Cone and gutta-percha from the MicroFlow Cartridge mixed with sealer. The System B/Obtura II technique produced a homogeneous fill at all levels. [source]
