DEFINING STANDARD OF CARE IN THE DEVELOPING WORLD: THE INTERSECTION OF INTERNATIONAL RESEARCH ETHICS AND HEALTH SYSTEMS ANALYSIS

ABSTRACT In recent years there has been intense debate regarding the level of medical care provided to ,standard care' control groups in clinical trials in developing countries, particularly when the research sponsors come from wealthier countries. The debate revolves around the issue of how to define a standard of medical care in a country in which many people are not receiving the best methods of medical care available in other settings. In this paper, we argue that additional dimensions of the standard of care have been hitherto neglected, namely, the structure and efficiency of the national health system. The health system affects locally available medical care in two important ways: first, the system may be structured to provide different levels of care at different sites with referral mechanisms to direct patients to the appropriate level of care. Second, inefficiencies in this system may influence what care is available in a particular locale. As a result of these two factors locally available care cannot be equated with a national ,standard'. A reasonable approach is to define the national standard of care as the level of care that ought to be delivered under conditions of appropriate and efficient referral in a national system. This standard is the minimum level of care that ought to be provided to a control group. There may be additional moral arguments for higher levels of care in some circumstances. This health system analysis may be helpful to researchers and ethics committees in designing and reviewing research involving standard care control groups in developing country research. [source]

Water-Yield Reduction After Afforestation and Related Processes in the Semiarid Liupan Mountains, Northwest China,

Abstract:, The increase of coverage of forest/vegetation is imperative to improve the environment in dry-land areas of China, especially for protecting soil against serious erosion and sandstorms. However, inherent severe water shortages, drought stresses, and increasing water use competition greatly restrict the reforestation. Notably, the water-yield reduction after afforestation generates intense debate about the correct approach to afforestation and forest management in dry-land areas. However, most studies on water-yield reduction of forests have been at catchment scales, and there are few studies of the response of total evapotranspiration (ET) and its partitioning to vegetation structure change. This motivates us to learn the linkage between hydrological processes and vegetation structure in slope ecosystems. Therefore, an ecohydrological study was carried out by measuring the individual items of water balance on sloping plots covered by different vegetation types in the semiarid Liupan Mountains of northwest China. The ratio of precipitation consumed as ET was about 60% for grassland, 93% for shrubs, and >95% for forestland. Thus, the water yield was very low, site-specific, and sensitive to vegetation change. Conversion of grassland to forest decreased the annual water yield from slope by 50-100 mm. In certain periods, the plantations at lower slopes even consumed the runon from upper slopes. Reducing the density of forests and shrubs by thinning was not an efficient approach to minimize water use. Leaf area index was a better indicator than plant density to relate ET to vegetation structure and to evaluate the soil water carrying capacity for vegetation (i.e., the maximum amount of vegetation that can be supported by the available soil water for an extended time). Selecting proper vegetation types and plant species, based on site soil water condition, may be more effective than the forest density regulation to minimize water-yield reduction by vegetation coverage increase and notably by reforestation. Finally, the focuses in future research to improve the forest-water relations in dry-land areas are recommended as follows: vegetation growth dynamics driven by environment especially water conditions, coupling of ecological and hydrological processes, further development of distributed ecohydrological models, quantitative relation of eco-water quota of ecosystems with vegetation structures, multi-scaled evaluation of soil water carrying capacity for vegetation, and the development of widely applicable decision support tools. The phylogenetic relationships of Silesaurus opolensis have been the subject of intense debate since its discovery. Silesaurus possesses some features characteristic of ornithischian dinosaurs, such as the presence of a beak at the front of the lower jaw, yet it lacks a number of important femoral and dental synapomorphies of Dinosauria. The microstructure of the long bones (femur, tibia and metatarsal) and ribs of this species reveals a relatively intensive rate of growth, comparable with that seen in small dinosaurs and the gracile crocodylomorph Terrestrisuchus. Cortical bone formed mainly by periosteal tissue with fibro-lamellar matrix (in older specimens parallel fibred) shows very little secondary remodelling and only in one specimen (large tibia ZPAL Ab III/1885) few lines of arrested growth are present in the outermost cortex. The vascularization is relatively dense, mainly longitudinal and ceases towards the periphery, forming almost avascular parallel fibred bone at the bone surface. This indicates maturation and significant decrease in the growth ratio in mature specimens of S. opolensis. The delicate trabeculae exhibit cores formed by the primary cancellous tissue lined with lamellar endosteal bone. The rather intense growth of S. opolensis implies a relatively high metabolic rate. Moreover, evidence from the fibro-lamellar tissue, predominant in the cortex, suggests that this kind of rapid bone deposition could be more typical of Archosauria than previously assumed, a prerequisite for the evolution of the very fast growth rates observed in large ornithischians, sauropods and large theropods. ,Archosauria, Bone histology, Dinosauriformes, Late Triassic, Silesaurus opolensis. Manus-only titanosaurid trackway from Fumanya (Maastrichtian, Pyrenees): further evidence for an underprint origin

The origin of manus-only and manus dominated sauropod trackways has been a matter of intense debate since two hyphothesis exist: (a) manus-only and manus-dominated trackways result from a ,swimming' sauropod, and (b) they result from a selective underprint phenomenon that only leaves the manus recorded. Several new sauropod trackways are reported in the Fumanya tracksite area (Maastrichtian), in SE Pyrenees, where both tracks and undertracks are found on the same stratigraphic bedding surface. In one of the trackways, footprint morphology together with the trackway pattern displays a clear succession of manus-only impressions attributed to a sauropod dinosaur in a walking gait. The ichnological comparison between the manus-only trackway with the other complete trackway (manus-pes) display an identical distribution of the manus pattern. This fact clearly points towards an underprint phenomenon as the origin for manus-only trackways, since it is rather unlikely that the same pattern would completely match different locomotion behaviours such as walking and swimming. Therefore, we suggest an interpretation based on the differential loading between the hindfoot and the forefoot on an upper stratigraphic track-level, for the studied manus-only trackway. , Fumanya tracksite, manus-only trackways, titanosaurs, trackway pattern, underprint, Upper Cretaceous. Comparing health care delivery systems , initiating a student exchange project between Europe and the United States

Background Cross-cultural contact among different health care systems can provide a framework for identifying the strengths and weaknesses of one's own healthcare system. However, such contact has rarely had much impact upon medical education curricula. Despite intense debate on reforming the healthcare delivery systems (HCDS) in Europe and the United States, there is very little formal representation of this interdisciplinary field in our educational programs. Description To address this problem, a medical student exchange program was conducted in which students developed case studies that produced comparative analyses of HCDS in Germany, Sweden, Denmark and the United States. Each case is intended to highlight critical differences among the systems. Evaluation Students and their faculty preceptors completed pre- and post-exchange questionnaires to assess perceived knowledge of the HCDS and the adequacy of time devoted to it in their curricula. Both perceived that too little attention was devoted to this content in their programs. Following the exchange, students described clear increases in perceived knowledge. Discussion Our common interest in curriculum reform was key to implementing the exchange. The written cases generated by the students are being developed as course material in some of the schools and a conference is planned to disseminate the cases and the implementation strategies for their inclusion in medical curricula. The evolution of secondary metabolism , a unifying model

Why do microbes make secondary products? That question has been the subject of intense debate for many decades. There are two extreme opinions. Some argue that most secondary metabolites play no role in increasing the fitness of an organism. The opposite view, now widely held, is that every secondary metabolite is made because it possesses (or did possess at some stage in evolution) a biological activity that endows the producer with increased fitness. These opposing views can be reconciled by recognizing that, because of the principles governing molecular interactions, potent biological activity is a rare property for any molecule to possess. Consequently, in order for an organism to evolve the rare potent, biologically active molecule, a great many chemical structures have to be generated, most of which will possess no useful biological activity. Thus, the two sides of the debate about the role and evolution of secondary metabolism can be accommodated within the view that the possession of secondary metabolism can enhance fitness, but that many products of secondary metabolism will not enhance the fitness of the producer. It is proposed that secondary metabolism will have evolved such that traits that optimize the production and retention of chemical diversity at minimum cost will have been selected. Evidence exists for some of these predicted traits. Opportunities now exist to exploit these unique properties of secondary metabolism to enhance secondary product diversity and to devise new strategies for biotransformation and bioremediation. Time to Keep Going: The Role and Structure of U.S. Forces in a Unified Korea

This year Korea and the U.S. celebrate 50 years of their alliance, which has seen many ups and downs since it came into existence. Today a very intense debate is going on in the USA and Korea about the future role of the U.S. in both the re-unification process and a post-unified Korea. Anti-Americanism is on the rise in South Korea, and demand for withdrawal of American forces is gaining ground in Korean society. An American withdrawal from Korea, however, would be very destabilizing for Korea and the whole of the East Asian region. Since the Korean war, the factors that have made it possible for South Korea, and other countries in the region, to economically prosper are the combination of sound economic polices and hard work by the peoples of these countries, and of the U.S. policies of reopening international markets to the countries of the region. While the presence of the U.S. forces in a post-unified Korea would be a positive factor, the actual structure of these forces would depend on the ground realities and threat perceptions at that time. It would be determined by complex issues of peace and stability inside Korea, its economic situation, and the external situation outside Korea's borders, including Korea's threat perceptions from China. Despite great improvements in technology in the Naval and Air forces, almost all military contingencies still require the use of ground forces to fight or to deter wars. Thus even if U.S. air and naval forces remained stationed in Korea, the absence of the U.S. ground forces would seriously undermine the deterrent and fighting power of the United States in the country and the region as whole. Given the terrain of the Korean peninsula, any possible future military conflict involving Korea would almost certainly be won or lost on land. Accordingly, infantrymen and tanks must remain an essential component of the American forces in Korea. What is more, dependence on air and naval forces for the protection of Korea would weaken traditional alliances and deterrence as well as American support for the very values and political principles that make other countries respect and trust the United States. Localised attitudes matter: a study of sickness absence in Sweden

Abstract The central issues explored in this article are the importance of geographical location in explaining the attitudes of individuals, and the interplay between these attitudes and overt behaviour. The context is as follows: In the late 1990's, sick-listings in Sweden underwent a substantial increase, causing the public expenses for the general sickness insurance to soar. Moreover, the extent of the usage of the insurance was found to vary significantly across different regions within the country. This development of the sickness insurance generally, and the regional differences specifically, have since been the subject of an intense debate. Differences and/or changes in attitudes toward sick leave within the population have been proposed as possible reasons for the regional variations. Much of the discussion has, however, been based on speculative arguments rather than empirical studies. Using data from a survey conducted in 2005, this research explores whether geographical location influences individual attitudes toward sick leave, and whether these attitudes in turn influence the sickness absence of individuals. The data are analysed using factor analysis, ordinary linear regressions and logistic regressions. The results provide some support for the idea that geographical factors matter to individual attitudes, and that variation in these attitudes is in turn associated with propensity for sickness absence. Does CTCF mediate between nuclear organization and gene expression?

Abstract The multifunctional zinc-finger protein CCCTC-binding factor (CTCF) is a very strong candidate for the role of coordinating the expression level of coding sequences with their three-dimensional position in the nucleus, apparently responding to a "code" in the DNA itself. Dynamic interactions between chromatin fibers in the context of nuclear architecture have been implicated in various aspects of genome functions. However, the molecular basis of these interactions still remains elusive and is a subject of intense debate. Here we discuss the nature of CTCF-DNA interactions, the CTCF-binding specificity to its binding sites and the relationship between CTCF and chromatin, and we examine data linking CTCF with gene regulation in the three-dimensional nuclear space. We discuss why these features render CTCF a very strong candidate for the role and propose a unifying model, the "CTCF code," explaining the mechanistic basis of how the information encrypted in DNA may be interpreted by CTCF into diverse nuclear functions. Sexual selection: an evolutionary force in plants?

ABSTRACT Sexual selection has traditionally been used to explain exaggerated sexual traits in male animals. Today the concept has been developed and various other sexually related traits have been suggested to evolve in the same manner. In nearly all new areas where the theory of sexual selection has been applied, there has been an intense debate as to whether the application is justified. Is it the case that some scientists are all too ready to employ fashionable ideas? Or are there too many dogmatic researchers refusing to accept that science develops and old ideas are transformed? Maybe the controversies are simply a reflection of the difficulty of defining a theory under constant re-evaluation. Thus, we begin by summarizing the theory of sexual selection in order to assess the influence of sexual selection on the evolution of plant morphology. We discuss empirical findings concerning potentially affected traits. Although we have tried to address criticisms fairly, we still conclude that sexual selection can be a useful tool when studying the evolution of reproductive traits in plants. Furthermore, by including the evidence from an additional kingdom, a fuller understanding of the processes involved in sexual selection can be gained. [source]


Improving the prognosis of patients after radical cystectomy.

Part I: the role of lymph node dissection
The first two reviews are from the same unit in Germany and describe the well-known but still much discussed ways of improving the prognosis of patients undergoing cystectomy for bladder cancer. The authors review the roles of lymph node dissection and perioperative chemotherapy, and draw conclusions which will be of help for patients having this form of therapy. In a further review, authors from Egypt debate the requirement for a refluxing or non-refluxing uretero-ileal anastomosis in low-pressure reservoirs, drawing on their extensive experience in this field. The optimum treatment for patients with muscle-invasive bladder cancer remains a matter of intense debate; some authors still question the role of radical cystectomy (RC) per se, as it can be a potentially mutilating procedure with subsequent impairment in quality of life. However, the impairment has not been investigated using validated quality-of-life studies. By contrast, it is commonly accepted that no alternative treatment yields similar long-term survival data to RC. However, survival rates after RC are far from satisfying, particularly for patients with ,,pT3 and/or pN+ disease. Therefore, various strategies were introduced to improve survival in these patients, i.e. extension of lymph node dissection during radical surgery and perioperative chemotherapy. Both strategies are analysed and discussed in two mini-reviews, based on data from current publications and from theoretical considerations. [source]


Airway eosinophilia is not a requirement for allergen-induced airway hyperresponsiveness

Background House dust mites (HDMs) are the major source of perennial allergens causing human allergic asthma. Animal models mimicking as closely as possible the allergic features observed in human asthma are therefore interesting tools for studying the immunological and pathophysiological mechanisms involved. Especially the role of eosinophils and allergen-specific immunoglobulin (Ig) E in the pathophysiology of airway hyperresponsiveness (AHR) remains a subject of intense debate. Objective To develop a mouse model of allergic airway inflammation and hyperresponsiveness based on the use of purified house dust mite allergen (Der p 1) as clinical relevant allergen. Furthermore, we studied the effects of low dose allergen exposure on the airway eosinophilia and AHR. Methods On day 0, C57Bl/6 mice were immunized with purified Der p 1 intraperitoneally. From day 14,20, the mice were exposed daily to a 30-min aerosol of different concentrations of house dust mite extract. Results Mice, actively immunized with Der p 1 and subsequently exposed to HDM aerosols, developed AHR, eosinophil infiltration of the airways and allergen-specific IgE. Moreover, lowering the concentration of the HDM aerosol also induced AHR and IgE without apparent eosinophil influx into the airways. Der p 1-sensitized mice exposed to PBS produced IgE, but did not show AHR or eosinophil influx. Conclusion This in vivo model of HDM-induced allergic airway changes suggests that AHR is not related to either eosinophil influx or allergen-specific serum IgE, thereby reducing the importance of these factors as essential elements for allergic AHR.
