Geller MEd ABSTRACT BACKGROUND:, Social cognitive theory describes self-efficacy and proxy efficacy as influences on fruit and vegetable consumption (FVC). Proxy efficacy was defined as a child's confidence in his or her skills and abilities to get others to act in one's interests to provide fruit and vegetable (FV) opportunities. The purpose of this study was to develop a scale assessing children's self-efficacy and proxy efficacy for FVC at after-school programs and at home. METHODS:, Elementary-aged children (n = 184) attending 7 after-school programs completed a self-efficacy questionnaire relevant to FVC. Questionnaire validity was investigated with exploratory factor analysis and mixed-model analysis of covariance. Internal consistency reliability and readability were also assessed. RESULTS:, The questionnaire assessed 4 constructs: self-efficacy expectations for fruit consumption, self-efficacy expectations for vegetable consumption, proxy efficacy to influence parents to make FV available, and proxy efficacy to influence after-school staff to make FV available. Children perceiving FV opportunities in after-school had greater self-efficacy expectations for FVC and greater proxy efficacy to influence after-school staff compared to students who did not perceive FV opportunities. Children attending schools of higher socioeconomic status (SES) and less diversity were more confident they could influence their parents to make FV available than students attending lower SES and less diverse schools. Adequate internal consistency and test-retest reliabilities were established. CONCLUSIONS:, Self-efficacy is a multicomponent construct that can be assessed in children using the reliable and valid instrument evaluated by the current study. We examined the associations of the consumption of fruits and vegetables and their main subgroups with pancreatic cancer risk within the European Prospective Investigation into Cancer and Nutrition (EPIC). EPIC is comprised of over 520,000 subjects recruited from 10 European countries. The present study included 555 exocrine pancreatic cancer cases after an average follow-up of 8.9 years. Estimates of risk were obtained by Cox proportional hazard models, stratified by age at recruitment, gender, and study center, and adjusted for total energy intake, weight, height, history of diabetes mellitus, and smoking status. Total consumption of fruit and vegetables, combined or separately, as well as subgroups of vegetables and fruits were unrelated to risk of pancreatic cancer. Hazard ratios (95% CI) for the highest versus the lowest quartile were 0.92 (0.68,1.25) for total fruit and vegetables combined, 0.99 (0.73,1.33) for total vegetables, and 1.02 (0.77,1.36) for total fruits. Stratification by gender or smoking status, restriction to microscopically verified cases, and exclusion of the first 2 years of follow-up did not materially change the results. These results from a large European prospective cohort suggest that higher consumption of fruit and vegetables is not associated with decreased risk of pancreatic cancer. © 2008 Wiley-Liss, Inc. [source] Fruit and vegetable consumption and lung cancer risk: Updated information from the European Prospective Investigation into Cancer and Nutrition (EPIC)INTERNATIONAL JOURNAL OF CANCER, Issue 5 2007Jakob Linseisen Abstract The association of fruit and vegetable consumption and lung cancer incidence was evaluated using the most recent data from the European Prospective Investigation into Cancer and Nutrition (EPIC), applying a refined statistical approach (calibration) to account for measurement error potentially introduced by using food frequency questionnaire data. Between 1992 and 2000, detailed information on diet and life-style of 478,590 individuals participating in EPIC was collected. During a median follow-up of 6.4 years, 1,126 lung cancer cases were observed. Multivariate Cox proportional hazard models were applied for statistical evaluation. In the whole study population, fruit consumption was significantly inversely associated with lung cancer risk while no association was found for vegetable consumption. In current smokers, however, lung cancer risk significantly decreased with higher vegetable consumption; this association became more pronounced after calibration, the hazard ratio (HR) being 0.78 (95% CI 0.62,0.98) per 100 g increase in daily vegetable consumption. In comparison, the HR per 100 g fruit was 0.92 (0.85,0.99) in the entire cohort and 0.90 (0.81,0.99) in smokers. Exclusion of cases diagnosed during the first 2 years of follow-up strengthened these associations, the HR being 0.71 (0.55,0.94) for vegetables (smokers) and 0.86 (0.78,0.95) for fruit (entire cohort). Cancer incidence decreased with higher consumption of apples and pears (entire cohort) as well as root vegetables (smokers). In addition to an overall inverse association with fruit intake, the results of this evaluation add evidence for a significant inverse association of vegetable consumption and lung cancer incidence in smokers. © 2007 Wiley-Liss, Inc. [source] Fruits and vegetables and renal cell carcinoma: Findings from the European prospective investigation into cancer and nutrition (EPIC)INTERNATIONAL JOURNAL OF CANCER, Issue 12 2006Steffen Weikert Abstract We examined the association between fruits and vegetables and risk of renal cell carcinoma (RCC) in the European Prospective Investigation into Cancer and Nutrition (EPIC). Dietary intake data and complete follow-up information on cancer incidence were available for 375,851 participants recruited in EPIC centers of 8 countries. During an average follow-up of 6.2 years, 306 incident cases of RCC were identified. The associations of consumption of total vegetables, total fruits, combined total fruits and vegetables and specific subtypes of vegetables with RCC risk were analyzed using Cox proportional hazards, stratified by centre and adjusted for potential confounders. No significant associations between fruit and vegetable consumption and RCC risk were observed despite a wide range of intake. The estimated relative risks (95% confidence intervals [CI]) in men and women combined were 0.97 (0.85,1.11) per 40 g increase in vegetable intake, 1.03 (0.97,1.08) per 40 g increase in fruit intake and 1.02 (0.93,1.11) per 80 g increase in fruit and vegetable intake combined. Among the vegetable subtypes, an inverse association was observed for root vegetables (RR per 8 g increase: 0.88; 95% CI: 0.78,0.99). These results suggest that total consumption of fruits and vegetables is not related to risk of RCC, although we cannot exclude the possibility that very low consumption is related to higher risk. The relationship of specific fruit and vegetable subgroups with RCC risk warrant further investigation. © 2006 Wiley-Liss, Inc. [source] Distribution of 127 edible plant species for antioxidant activities by two assaysJOURNAL OF THE SCIENCE OF FOOD AND AGRICULTURE, Issue 14 2006Ray-Yu Yang Abstract Epidemiological studies have demonstrated the protective effects of fruit and vegetable consumption on human health. A total of 150 edible plants representing 127 species were evaluated for Trolox equivalent antioxidant capacity (TEAC) and superoxide scavenging (SOS) activity. Distributions of vegetables for TEAC and SOS were not continuous but highly skewed. TEAC values on a dry weight basis ranged from 0 to 2105 µmol TE g,1 and SOS values ranged from 0 to 6206 µmol ascorbate equivalent (AE) g,1. The majority of vegetables clustered within 200 µmol TE g,1 and 400 µmol AE g,1. Vegetable species were grouped into six classes after natural logarithm transformation of TEAC and SOS values. Most highly consumed vegetables fell in the very low to medium classes. Species in the very high class for both TEAC and SOS included leaves of Cedrela sinensis (Chinese cedar), Oroxylum indicum (Damocles tree), Cassia siamea (cassod tree) and Cassia tora (sickle senna). Edible plants high in antioxidant activity (AOA) were mostly perennial and underutilised crops, while commonly consumed vegetables were generally low in AOA. There is great potential to increase dietary antioxidants and improve human health through consumption of diverse vegetable species, especially those high in AOA. Low levels of fruit and vegetable consumption, and obesity, especially among children, have potentially severe consequences for the future health of the nation. From a public health perspective, the UK government's role is to help poorer families make informed choices within healthy frameworks for living. However, the question is , to what extent are such policies in accordance with lay experiences of managing diet and nutrition on a low-income? This paper critically examines contemporary public health policies aimed at improving diet and nutrition, identifying the underlying theories about the influences on healthy eating in poor families, and exploring the extent to which these assumptions are based on experiential accounts. It draws on two qualitative systematic reviews , one prioritizing low-income mothers' accounts of ,managing' in poverty; and the other focusing on children's perspectives. The paper finds some common ground between policies and lay experiences, but also key divergencies. Arguably, the emphasis of public health policy on individual behaviour, coupled with an ethos of empowered consumerism, underplays material limitations on ,healthy eating' for low-income mothers and children. Health policies fail to take into account the full impact of structural influences on food choices, or recognize the social and emotional factors that influence diet and nutrition. In conclusion, it is argued that while health promotion campaigns to improve low-income families' diets do have advantages, these are insufficient to outweigh the negative effects of poverty on nutrition. Methods:, A quasi-experimental study was conducted. A Bangkok public primary school was randomly selected and one of the kindergarten levels was purposively chosen. Program implementation consisted of 11 activities over an eight-week period from July to September, 2003. Data on demographic variables, and types and amounts of fruit and vegetables consumed and frequency of fruit and vegetables served were collected before and after the intervention. Program evaluation consisted of an analysis of the pre- and post-test data. Results:, After the intervention, fruit and vegetable eating behaviour scores (median ± interquartile range) revealed significant changes from 3 ± 8 to 7 ± 8 for vegetables and 6 ± 8 to 9 ± 8 for fruit (P -value < 0.001); the different types of consumed vegetables were increased from two to four (P -value , 0.001); and the fruit and vegetable intake was significantly increased from 53 g to 77 g and from 11 g to 23 g respectively (P -value < 0.005). Conclusions:, Results of this pilot study were effective in increasing fruit and vegetable consumption in the kindergarten, however, longer-term evaluation and assessment of the impact on the home environment are required. The inclusion of nutrition education and instruction on healthy eating habits in the course curriculum combined with social support from teachers and families may improve and sustain fruit and vegetable eating behaviours. Alinia Summary Both national and international bodies recommend an increased intake of fruits and vegetables in order to decrease the risk of overweight and obesity. However, there is a rationale to investigate the separate role of fruits. The aim of this paper was to systematically review and analyse published human intervention, prospective observational and cross-sectional studies on fruit intake and body weight in adults. We identified three intervention, eight prospective observational and five cross-sectional studies that explored this relationship. Two of the intervention studies showed that fruit intake reduced body weight, five of the prospective observational studies showed that fruit consumption reduced the risk of developing overweight and obesity, and four of the cross-sectional studies found an inverse association between fruit intake and body weight. Important methodological differences and limitations in the studies make it difficult to compare results. However, the majority of the evidence points towards a possible inverse association between fruit intake and overweight. Future intervention and prospective observational studies examining the direct and independent role of fruit in body-weight management in free-living individuals are needed. Moreover, important determinants such as energy density, energy content, fruit and vegetable consumption, physical form of fruit and preparation methods need to be included in future studies. [source] School-based promotion of fruit and vegetable consumption in multiculturally diverse, urban schoolsPSYCHOLOGY IN THE SCHOOLS, Issue 1 2008Jessica Blom-Hoffman Rates of childhood overweight, have reached epidemic proportions (U.S. Department of Health and Human Services, 2001), and schools have been called on to play a role in the prevention of this medical condition. This article describes a multiyear health promotion effort,the Athletes in Service fruit and vegetable (F&V) promotion program,which is based on social learning theory for urban, elementary school children in kindergarten through third grade. Children participate in the program for a period of 3 years. The goals of the program are to increase opportunities for children to be more physically active during the school day and to help students increase their F&V consumption. This article describes the F&V promotion components of the program that were implemented in year 1, including implementation integrity and treatment acceptability data. Year 1 evaluation data demonstrated that the program is acceptable from the perspective of school staff and was implemented by school staff with high levels of integrity. Hallmarks of the program's successful implementation and high acceptability include (a) having a school-based program champion; (b) designing the program to include low-cost, attractive, interactive materials; (c) including many school staff members to facilitate a culture of healthy eating in the school; and (d) spreading out implementation responsibilities among the multiple staff members so that each individual's involvement is time efficient. © 2007 Wiley Periodicals, Inc. Methods:Secondary data analysis of the 2002 National Children's Nutrition Survey. Results:Few ethnic differences were significant for fruit and vegetable consumption and indicators of physical activity. Where ethnic differences in physical activity were significant, Mäori children and Pacific children were more active than New Zealand European/Other (NZEO) children. Pacific children and Mäori children were significantly more likely to skip meals than NZEO children. Pacific children and Mäori children were significantly more likely to buy some/most of the food they consumed at school from the tuckshop or dairy while NZEO children were more likely to bring their school food only from home. Likewise, Pacific children and Mäori children were significantly more likely to be high consumers of some fatty and sugary foods than NZEO children. Conclusions and Implications:Meal skipping and purchasing food away from home were common for Mäori children and Pacific children; school-based programs that aim to improve access to and subsidise the price of healthy foods, including breakfast, could greatly benefit Mäori and Pacific students. Efforts to improve fruit and vegetable consumption and physical activity should be more universally applied and made culturally appropriate for all children. Methods:, Data were collected in 2004,06, using a three-stage, stratified clustered sample, involving a computer-assisted telephone interview (CATI), oral examination and mailed questionnaire followed by a food frequency questionnaire. Results:, A total of 14 123 adults responded to the CATI (49% response) of whom 5505 (44% of those interviewed) agreed to undergo an oral epidemiological examination. In the nutrition sub-study, a total of n = 1218 persons were approached in New South Wales and Queensland, with n = 1129 responding (92.7% response rate). Among respondents aged 55 years or more 34.5% had <21 teeth. Adjusting for income the prevalence of infrequent consumption (,never or less than once a month') was associated with [PR = prevalence ratio (95% CI)] fewer teeth for the fruits, ,peach, nectarine, plum, apricot' PR = 1.91 (1.12, 3.25) and ,grapes or berries' PR = 1.69 (1.03, 2.76), and for the vegetables ,stir-fried or mixed' PR = 2.34 (1.14, 4.78), ,sweetcorn' PR = 1.45 (1.001, 2.10), ,mushrooms' PR = 1.62 (1.05, 2.50), ,lettuce' PR = 3.99 (1.31, 12.17) and ,soy beans' PR = 1.11 (1.01, 1.21). Conclusions:, An inadequate dentition was associated with lower consumption of a range of fruits and vegetables indicating that dentition-related impairment of chewing ability could have adverse consequences on nutritional intake among Australian adults.
