Renal silica calculi in an infant

Abstract, We report on a rare case of urinary silica calculi in a 10-month-old boy. The boy showed acute pyelonephritis with left hydronephrosis. Ultrasonography and computed tomography revealed a calculus at the left ureteropelvic junction and three additional calculi in the left renal pelvis. Because his acute pyelonephritis was refractory to conventional chemotherapy, the patient underwent successful left percutaneous nephrostomy followed by percutaneous nephrolithotripsy for the renal calculi. All stones disappeared and his postoperative course was uneventful. On infrared spectrophotometry, the wavelength pattern of the stones exhibited two peaks at 1100 and 1650 cm,1, consistent with the determination that the calculi consisted of a mixture of silicate (78%) and calcium oxalate (22%). We consider that the etiology of the calculi in this child can be ascribed to the silicate-rich water used to dilute milk. In Japan, 46 adult patients with urinary silicate calculi have been reported in the literature; however, there is no report of the disease in an infant in Japan. [source]


Preparation of sheath,core bicomponent composite ion-exchange fibers and their properties

Abstract Based on sheath,core bicomponent composite ion-exchange fibers with modified polystyrene and modified polypropylene, composite fibers were further crosslinked and sulfonated with chlorosulfonic acid to produce strongly acidic cation-exchange fibers. The optimal technology of the obtained fibers was examined, and the structures of the fibers were investigated with Fourier transform infrared spectrophotometry, differential scanning calorimetry, and so forth. The properties were measured with chemical titration, filament strength instrumentation, scanning electron microscopy, and so forth. The results showed that cation-exchange fibers with better mechanical properties and a higher exchange capacity were obtained, and they had higher practicability. © 2008 Wiley Periodicals, Inc. Plate-out in PVC extrusion.

Samples of extruder plate-out from industrial rigid PVC production lines were investigated by using a number of analytical techniques. The combined use of SEM-EDX (scanning electron microscopy , energy dispersive X-ray analysis), thermal analysis, FTIR (Fourier transform infrared spectrophotometry), and LIMA (laser-induced mass analysis) enabled most plate-out components to be identified and linked to likely formulation ingredients. The FTIR and thermal analyses were used to identify organic components. The FTIR analysis was also useful for identifying some inorganic compounds present in sufficient quantities, while EDX detected the elements present. The LIMA was the most sensitive technique, detecting trace quantities of both cations and anions. Calcium carbonate, titanium dioxide, and lead stabilizers were found in all die plate-out samples studied, together with small amounts of lubricants. J. VINYL ADDIT. Cerebral near infrared spectroscopy for the measurement of indocyanine green elimination in cirrhosis

Background: Indocyanine green (ICG) clearance is a useful indicator of hepatic function but most measurement methods are invasive. Aim: To validate a less invasive technique using cerebral near infrared spectrophotometry (NIRS) to measure ICG elimination, and to compare it with the established methods for the determination of ICG clearance in a group of normal controls and patients with cirrhosis. Method: NIRS was used to measure ICG elimination in 41 cirrhotic patients and nine healthy volunteers. The first 13 of the cirrhotic patients also had their ICG clearance measured by the conventional spectrophotometric technique. Results: NIRS ICG elimination rate (ICG-k) and spectrophotometry ICG-k values correlated strongly (r= 0.828, P < 0.001, n=13). There was a significant reduction in the mean NIRS-k in cirrhotic patients and within Child,Pugh classes A, B, and C (P < 0.001). Conclusion: Measurement of ICG elimination by the NIRS method is at least as reliable as the conventional spectrophotometric technique in normals and in patients with cirrhosis. This technique merits further development for use as a bedside, less invasive liver function test. [source]


Phosphoglyceride crystal deposition disease

An extremely rare phosphoglyceride deposition disease is reported. A healthy 62-year-old Japanese woman suffered from tumors that repeatedly appeared in injured soft tissues for more than 20 years. No immunologic disorders or abnormal laboratory data were found. Histology showed foreign body granulomas consisting of macrophages surrounding yellowish-white crystals. The crystals were weakly positive by von Kossa's method, were dissolved in 30% acetic acid with gas, and were easily dissolved in 0.1 N NaOH or potassium hydroxide, losing their crystal structure. Using a scanning electron microscopy X-ray microanalyzer, phosphorus and calcium peaks were detected. Phosphoglycerides were detected by microscopic infrared spectrophotometry and microsampling mass spectrometry. The gold hydroxamic acid method for detecting phosphoglyceride showed strong positive staining in the crystals. Based on the above analyses, the deposited crystals were regarded as phosphoglyceride, which bound calcium as a counter ion. The crystals tended to be deposited at sites of injury, where macrophages had accumulated. The patient had received many injections of a medicine made from alcohol extract from bovine liver. We suspect that this medicine was related to the cause of the deposition as the deposition repeatedly appeared at the site of the injections. [source]
