Selected AbstractsChildhood autism spectrum disorder in the Barwon region: A community based studyJOURNAL OF PAEDIATRICS AND CHILD HEALTH, Issue 12 2004F Icasiano Objective: To establish a community database for children with autism spectrum disorder (ASD) to determine prevalence and identify subgroups based on key intellectual, clinical and family parameters. Methods: Data were collected for children previously diagnosed with an ASD in the Barwon region using parental interview and review of the child's paediatric and psychological records. Preschool diagnoses were typically made by specialist psychologists and school-age diagnoses made by a multidisciplinary team. Results: One hundred and seventy-seven children in the Barwon region were identified as having ASD (82% response rate). The prevalence of ASD was one per 255 children aged two to 17 years. The prevalence increased 10 fold over a 16-year period and this increase was relatively even across all levels of child intellectual functioning. Forty-two percent of children were intellectually disabled (IQ < 70) and performance IQ was significantly higher than verbal IQ but typical block design-comprehension subtest patterns were not common. Nine sibling pair families were identified, 24% were labelled as having attention deficit hyperactivity disorder (ADHD), 6% had epilepsy, and 18.3% were macrocephalic where data were available. Family difficulty was associated with the degree of obsessions/rituals, frequency and intensity of anger/aggression, and ADHD but overall was not associated with the child's intellectual status. Conclusions: The relatively rapid increase in prevalence is consistent with overseas studies and suggests significant changes in diagnostic criteria, increasing community awareness and the need for support at all levels of intellectual functioning. Increased occurrence in siblings and relatives gives further evidence for a genetic cause. Wieringa Background Many studies have reported an increase in the occurrence of asthma and respiratory allergies in recent decades, but this increase is mostly based on studies using rather subjective measurements of asthma and allergies, such as questionnaires and doctor's diagnosis. None of the reviews specifically focused on studies using more ,objective' measurements, such as sensitization (specific IgE or skin prick testing (SPT)), bronchial hyper-responsiveness (BHR) or lung function (LF). Objective To review articles studying a time trend of occurrence of these ,more objective' measurements. Methods A MEDLINE-search (1966,February 2000) was performed. The following criteria were used: population-based, using IgE, SPT, BHR or LF measurements in the same age-group at least twice, with at least 2 years between and using similar methods. Results The MEDLINE-search resulted in only 16 articles, performed in 13 populations in seven different countries. Nine articles used the same objective measurements twice in the whole population. Three of these reported a non-significant increase or decrease. The other six articles found a significant increase in at least one objective measurement and of these only three reported a consistent significant increase. Conclusions The increase in the occurrence of reported asthma and allergy is supported by only a few articles confirming these results with ,more objective measurements'. [source] Are active fund managers collectors of private information or fast interpreters of public information?ACCOUNTING & FINANCE, Issue 3 2010David R. Gallagher G23 Abstract Recent studies of fund manager performance find evidence of outperformance. However limited research exists as to whether such outperformance is because of privately collected information, or merely expedient interpretation of publicly released information. In this study, we examine the trade sequences of active Australian equity fund managers around earnings announcements to provide insights into the source of fund managers' superior information. We document an increased occurrence of buy-sell trade sequences around good-news earnings announcements. The evidence is consistent with fund managers having both private information about forthcoming good-news earnings announcements and being ,short-term profiteers'. We find no evidence that fund managers have private information about forthcoming bad-news earnings announcements. However, we do find an increase in the frequency of fund managers not trading before bad-news earnings announcements only to subsequently sell during announcements. [source] Increased rainfall variability and reduced rainfall amount decreases soil CO2 flux in a grassland ecosystemGLOBAL CHANGE BIOLOGY, Issue 2 2005Christopher W. Harper Abstract Predicted climate changes in the US Central Plains include altered precipitation regimes with increased occurrence of growing season droughts and higher frequencies of extreme rainfall events. Changes in the amounts and timing of rainfall events will likely affect ecosystem processes, including those that control C cycling and storage. Soil carbon dioxide (CO2) flux is an important component of C cycling in terrestrial ecosystems, and is strongly influenced by climate. While many studies have assessed the influence of soil water content on soil CO2 flux, few have included experimental manipulation of rainfall amounts in intact ecosystems, and we know of no studies that have explicitly addressed the influence of the timing of rainfall events. In order to determine the responses of soil CO2 flux to altered rainfall timing and amounts, we manipulated rainfall inputs to plots of native tallgrass prairie (Konza Prairie, Kansas, USA) over four growing seasons (1998,2001). Specifically, we altered the amounts and/or timing of growing season rainfall in a factorial combination that included two levels of rainfall amount (100% or 70% of naturally occurring rainfall quantity) and two temporal patterns of rain events (ambient timing or a 50% increase in length of dry intervals between events). The size of individual rain events in the altered timing treatment was adjusted so that the quantity of total growing season rainfall in the ambient and altered timing treatments was the same (i.e. fewer, but larger rainfall events characterized the altered timing treatment). Seasonal mean soil CO2 flux decreased by 8% under reduced rainfall amounts, by 13% under altered rainfall timing, and by 20% when both were combined (P<0.01). These changes in soil CO2 flux were consistent with observed changes in plant productivity, which was also reduced by both reduced rainfall quantity and altered rainfall timing. Soil CO2 flux was related to both soil temperature and soil water content in regression analyses; together they explained as much as 64% of the variability in CO2 flux across dates under ambient rainfall timing, but only 38,48% of the variability under altered rainfall timing, suggesting that other factors (e.g. substrate availability, plant or microbial stress) may limit CO2 flux under a climate regime that includes fewer, larger rainfall events. An analysis of the temperature sensitivity of soil CO2 flux indicated that temperature had a reduced effect (lower correlation and lower Q10 values) under the reduced quantity and altered timing treatments. Recognition that changes in the timing of rainfall events may be as, or more, important than changes in rainfall amount in affecting soil CO2 flux and other components of the carbon cycle highlights the complex nature of ecosystem responses to climate change in North American grasslands. [source] MC1R common variants, CDKN2A and their association with melanoma and breast cancer riskINTERNATIONAL JOURNAL OF CANCER, Issue 11 2006Tadeusz D, bniak Abstract We sought to examine the association between MC1R variants and the risk of melanoma and breast cancer in Polish population. We also determined the prevalence of compound heterozygous carriers of MC1R and CDKN2A (A148T) variants. We examined 500 unselected melanoma cases, 511 consecutive invasive breast cancer patients, 800 newborns, 421 healthy adults matched for sex and age with the melanoma cases and 511 healthy women matched for sex and age with the breast cancer cases. A statistically significant association of all 4 MC1R variants with the melanoma risk was found. For the R151C variant p value was 0.000008 and odds ratio 2.9; for the V60L variant p value was 0.007 and OR 1.78; for the R160C p was 0.006 and OR 1.76; for the R163Q p was 0.015 and odds ratio 2.1. None of the compound heterozygotes were significantly over-represented among any of the melanoma cases, the highest OR (4.2) observed in patients harbouring the A148T variant in CDKN2A and the R151C variant in MC1R. Positive association was found between carrying any of the MC1R variants and (i) increased occurrence of melanoma among I degree relatives of the carriers; (ii) increased occurrence of melanoma on UV-non-exposed skin areas. We also observed a tendency of increased risk of multiple melanomas among carriers of MC1R variants. The haplotype analysis demonstrates that MC1R variants do not co-occur in cis, compound carriers have both alleles affected. We found no association with the MC1R variants and breast cancer risk. In conclusion, the results of this population-based study show herein that MC1R variants are associated with increased melanoma risk in the Polish population. [source] The surface winds of Sweden during 1999,2000INTERNATIONAL JOURNAL OF CLIMATOLOGY, Issue 2 2006Christine Achberger Abstract This study aims at increasing our understanding of the regional wind climate in Sweden. Spatial and temporal patterns of the surface winds are presented for the years 1999,2000. Annual mean wind speeds range between 2 and 5 m/s with high values at exposed mountainous sites and on islands off the coast. Combining wind speed and direction into mean wind velocities shows that flow conditions are stronger and more coherent in space in southern Sweden than in central and northern Sweden. The spatial scale, defined as the distance between stations when the correlation for wind speed drops to ,0.37, was determined by pairwise correlations between all possible station pairs. Scales range from 38 to 530 km for wind speed and from 40 to 830 km for wind direction depending on the region. They tend to be smaller in central and northern Sweden, where the more pronounced relief has a larger influence on the local wind conditions. The strength and the timing of the annual and diurnal wind speed cycle have been estimated for each station. Amplitudes of the annual cycle are greater at exposed sites and correlate generally well with annual mean wind speeds. Monthly mean wind speeds peak in winter in southern Sweden, but peak in other seasons in the remaining regions. In winter, weaker pressure gradients over northern Sweden and surface-near temperature inversions contribute to weaker surface winds. Diurnal cycles vary in strength between summer and winter. Compared to the last normal climate period (1961,1990), 1999,2000 is characterized by the increased occurrence of westerly and southerly geostrophic flow. [source] Cadmium accumulation and binding characteristics in intact Sertoli/germ cell units, and associated effects on stage-specific functions in vitro: insights from a shark testis modelJOURNAL OF APPLIED TOXICOLOGY, Issue 2 2008Leon M. McClusky Abstract The increased human use of cadmium (Cd) and its increased occurrence in the environment is of concern. The testis is sensitive to Cd because of the steroid-mediated regulation of spermatogenesis, high levels of DNA synthesis and gene transcription, all of which varies in a stage-related manner. Validated techniques (acridine orange vital staining to detect apoptosis and dextran-rhodamine exclusion to assess blood,testis barrier function) were recently developed and the shark testis was proposed as an alternative model for assessing stage-specific functions in living testicular tissue and to study toxicant actions on spermatogenesis. The present paper shows that 109Cd accumulation and binding in vitro was stage-dependent (premeiotic, PrM > meiotic, M > postmeiotic, PoM), rapid and persisted in spermatocysts (intact germ cell/Sertoli cell units) 49 h after washout. In competitive binding analyses of all three spermatocyst stages, Hg, but not Zn, could replace bound 109Cd, suggesting that Cd binding was specific. These findings were associated with a biphasic apoptotic response in the PrM spermatocysts, which was maximal at 10 µm CdCl2 and 1 µm CdCl2 after 2 and 4 days in culture, respectively. Although Cd uptake in PoM cysts was more than 2-fold less than uptake in PrM cysts, the percentage dextran-rhodamine permeant PoM cysts was ,8-fold greater than in controls in the presence of both 10 µm CdCl2 and 30 µm CdCl2 after 4 days culture, indicating that blood,testis barrier function in PoM spermatocysts was compromised. Milewski Abstract Some plants respond to browsing with compensatory regrowth of plant tissues and with increased thorn growth. Associations between browsers and their preferred forage were examined through wandering quarter vegetation sampling and observational studies in an effort to understand how some plants respond to browsing by large African herbivores. Acacia seyal (n = 2680) A. drepanolobium (n = 1850), and Balanites glabra (n = 960) were three species of frequently browsed indigenous plants examined on Game Ranching Ltd. in Kenya. There were several statistically significant associations revealed. Individual A. seyal exposed to intensive browser utilization were observed to lose shoot tips, produce long thorns, and have relatively few flowers and fruits. Browser utilization was associated with increased lateral branching in A. drepanolobium and with an increased occurrence of short, thickened spines in B. glabra. Thorns, spines and flowers were measurable indicators of relative browser utilization, and may be useful features to monitor in the management of large African mammals and their prickly forage Résumé Certaines plantes réagissent au pâturage par une repousse du tissu végétal et une croissance accrue d'épines. [source] B-type natriuretic peptide is related to cardiac function and prognosis in hospitalized patients with decompensated cirrhosisLIVER INTERNATIONAL, Issue 7 2010Joana Pimenta Abstract Background: B-type natriuretic peptide (BNP) concentrations are high in cirrhosis, possibly related to volume status and cirrhotic cardiomyopathy. The prognostic significance of BNP in cirrhosis is unknown. Aims: We aimed to evaluate (i) the influence of haemodynamic parameters and volaemia, assessed by impedance cardiography (ICG), in BNP levels, (ii) the performance of BNP as a prognostic marker, in a cohort of cirrhotic patients. Methods: Patients consecutively hospitalized with decompensated cirrhosis during 1 year were evaluated. At admission, ICG and BNP measurements were performed in 83 patients (median age 56 years; median Child,Pugh score=10). The 70 patients discharged were followed for the occurrence of death within 6 months. Results: Median BNP levels were 130.3 (65.2,363.3) pg/ml. Independent BNP predictors in multivariate linear regression analysis were cardiac output, age and haemoglobin (R2=36.7%). The 24 patients with cardiac systolic dysfunction, defined by low cardiac output, had higher BNP concentrations than the other patients (230.8 vs 98.5 pg/ml, P=0.003). BNP levels above median were associated with an increased occurrence of death within 6 months of discharge (log rank P=0.023). Cardiac output and BNP were predictors of survival in univariate Cox regression analysis. Only BNP remained independently related to the outcome in multivariate analysis [hazard ratio=2.86 (1.11,7.38), P=0.03]. Conclusions: BNP levels in cirrhosis reflect cardiac systolic function and non-cardiac variables that should be considered in their interpretation. BNP is an independent predictor of medium-term survival in advanced cirrhosis, suggesting its utility in risk stratification of decompensated cirrhotic patients. [source] Risk factors for cholelithiasis in pregnancyRESEARCH IN NURSING & HEALTH, Issue 6 2004Glenda Lindseth Abstract This study was designed to examine the relationships of demographics, anthropometrics, prenatal physical activity, serum cholesterol, and nutrient intakes to symptomatic cholelithiasis (gallstone) occurrence in 128 northern plains pregnant women. Data collected at 14 and 26 weeks of pregnancy and 4 weeks after delivery, indicated 25.8% of the Native American and 8.3% of the Caucasian pregnant women experienced symptoms of cholelithiasis. Body mass indices (BMIs) were significantly higher in the pregnant women who experienced cholelithiasis than in those who did not have cholelithiasis, and prenatal physical activity was significantly lower in those same women. History of gallbladder disease (,,=,.73; p,=,.001), BMI (,,=,.33; p,=,.01), and prenatal physical activity (,,=,,.20; p,=,.04) were predictive of increased occurrence of cholelithiasis during pregnancy. © 2004 Wiley Periodicals, Inc. Holmqvist Objective To investigate the relative importance of shared etiologies for rheumatoid arthritis (RA) and ischemic heart disease (IHD) in terms of the well-known increased risk of IHD in patients with RA, by assessing the occurrence of IHD up until the time of the onset of the first symptoms of RA. Methods We assessed the prevalence of a history of IHD, myocardial infarction (MI), and angina pectoris before the onset of RA symptoms in 2 large population-based case,control studies. Patients with newly diagnosed RA according to the criteria of the American College of Rheumatology were included as cases. We used data from the Swedish Early Arthritis Register study and the Swedish Epidemiologic Investigation of Rheumatoid Arthritis case,control study and from general population controls. Information on IHD, MI, and angina pectoris was obtained from the nationwide Hospital Discharge Register and from self reports. We calculated odds ratios (ORs) and 95% confidence intervals (95% CIs) to compare the prevalence of a history of IHD/MI/angina pectoris among patients with RA with that among population controls. Results We could not detect any increased occurrence of IHD, MI, or angina pectoris before the onset of symptoms of RA, regardless of whether data on IHD were obtained from the Hospital Discharge Register or were self reported. As detected in the Hospital Discharge Register, the OR for IHD overall was 1.0 (95% CI 0.9,1.1), the OR for MI was 1.0 (95% CI 0.9,1.1), and the OR for angina pectoris was 1.0 (95% CI 0.9,1.2). Conclusion Shared risk factors or susceptibilities for RA and IHD are likely to contribute less than RA-related factors to the increased occurrence of IHD in patients with manifest RA. Nonetheless, the existence of shared factors associated with longer latency until the occurrence of IHD cannot be excluded. Methods, We studied gestational duration, lowest birthweight and birthweight difference in the twin pair, presence of one or two twins with a respiratory complication, and with jaundice in one or both twins. Risk estimates were calculated as odds ratios with adjustments for year of birth, maternal age, parity and smoking in pregnancy. Main outcome measures, Gestational duration, birth weight, respiratory complications, jaundice. Results, We studied 1545 pairs of dizygotic twins born after IVF, and 8675 pairs of dizygotic twins where IVF was not known to have occurred. The risk for preterm delivery before 32 weeks of gestation was significantly increased among dizygotic twin pairs born after IVF compared with non-IVF dizygotic twin pairs. No significant difference in low birthweight or birthweight difference within twin pairs was seen. There was an increased occurrence of twin pairs with respiratory problems or jaundice, but only the latter diagnosis occurred in a statistically significant excess. Conclusions, The study confirms recent findings that IVF is associated with an increased risk for some neonatal complications, not only among singletons but also among twins. [source] Comparison of self-administered vaginal misoprostol versus placebo for cervical ripening prior to operative hysteroscopy using a sequential trial design,BJOG : AN INTERNATIONAL JOURNAL OF OBSTETRICS & GYNAECOLOGY, Issue 5 2008KS Oppegaard Objective, To compare the impact of 1000 micrograms of self-administered vaginal misoprostol versus self-administered vaginal placebo at home on preoperative cervical ripening in both premenopausal and postmenopausal women before operative hysteroscopy. Design, Two separate but identical parallel, randomised, double-blind, placebo-controlled sequential trials, one in premenopausal women and one in postmenopausal women. The boundaries for the sequential trials were calculated on the primary outcomes of a difference of cervical dilatation ,1 mm, with the assumption of a type 1 error of 0.05 and a power of 0.95. Setting, Norwegian university teaching hospital. Sample, Eighty-six women referred to outpatient operative hysteroscopy. Methods, The women were randomised to either 1000 micrograms of self-administered vaginal misoprostol or self-administered vaginal placebo the evening before outpatient operative hysteroscopy. Main outcome measures, Preoperative cervical dilatation (primary outcome), number of women who achieve a preoperative cervical dilatation ,5 mm, acceptability, complications and adverse effects (secondary outcomes). Results, In premenopausal women, the mean cervical dilatation was 6.4 mm (SD 2.4) in the misoprostol group and 4.8 mm (SD 2.0) in the placebo group, the mean difference in cervical dilatation being 1.6 mm (95% CI 0.5,2.7). Among the premenopausal women receiving misoprostol, 88% achieved a cervical dilatation of ,5 mm compared with 65% in the placebo group. Twelve percent of the women who received misoprostol were difficult to dilate compared with 32% who received placebo. Dilatation was also quicker in the misoprostol group. Misoprostol had no effect on cervical ripening in postmenopausal women compared with placebo, and 43% of the women were difficult to dilate. The trials were terminated after analysis of 21 postmenopausal women and 65 premenopausal women after reaching a conclusion on the primary outcome with only 28% of the number of women needed in a fixed sample size trial. Three of 45 women who received misoprostol experienced severe lower abdominal pain, and there was an increased occurrence of light preoperative bleeding in the misoprostol group. Most women did not experience misoprostol-related adverse effects. The majority (83% of premenopausal and 76% of postmenopausal women) found self-administered vaginal misoprostol at home to be acceptable. There were two serious complications in the premenopausal misoprostol group: uterine perforation with subsequent peritonitis and heavy postoperative bleeding requiring blood transfusion, but these were not judged to be misoprostol related. Complications were otherwise comparatively minor and distributed equally between the two dosage groups. Conclusions, One thousand micrograms of self-administered vaginal misoprostol 12 hours prior to operative hysteroscopy has a significant cervical ripening effect compared with placebo in premenopausal but not in postmenopausal women. Self-administered vaginal misoprostol of 1000 micrograms at home the evening before operative hysteroscopy is safe and highly acceptable, although a small proportion of women experienced severe lower abdominal pain. There is a risk of lower abdominal pain and light preoperative bleeding with this regimen, which is very cheap and easy to use. [source] |
