Effectiveness of limited cone-beam computed tomography in the detection of horizontal root fracture

Root fractures were created in the horizontal plane in 18 teeth by a mechanical force and fragments were relocated. Another 18 intact teeth with no horizontal root fracture served as a control group. Thirty-six teeth were placed in the respective empty maxillary anterior sockets of a human dry skull in groups three by three. Intraoral radiographs were obtained in three different vertical views by utilizing Eastman Kodak E-speed film, CCD sensor, RVG 5.0 Trophy and a PSP sensor Digora, Optime. Cone beam CT images were taken with a unit (3D Accuitomo; J Morita MFG. Corp, Kyoto, Japan). Three dental radiologists separately examined the intraoral film, PSP, CCD and cone beam CT images for the presence of horizontal root fracture. Specificity and sensitivity for each radiographic technique were calculated. Kappa statistics was used for assessing the agreement between observers. Chi-square statistics was used to determine whether there were differences between the systems. Results were considered significant at P < 0.05. Cone beam CT images revealed significantly higher sensitivities (P < 0.05) than the intraoral systems between which no significant differences were found. Specificities did not show any statistically significant differences between any of the four systems. The kappa values for inter-observer agreement between observers (four pairs) ranged between 0.82,0.90 for the 3DX evaluations and between 0.63,0.71 for the different types of intraoral images. Limited cone beam CT, outperformed the two-dimensional intraoral, conventional as well as digital, radiographic methods in detecting simulated horizontal root fracture. [source]


Spontaneously healed horizontal root fracture in maxillary first premolar: report of a case

Abstract,,, Root fractures of the posterior teeth are rare and occur as a result of severe trauma. This study describes the horizontal root fracture of a maxillary first premolar. The fractured roots were discovered during a routine radiographic examination. The tooth was asymptomatic and responded positively to electric pulp testing. The patient reported accidental trauma, which occurred 14 years before. Our case is an example of spontaneously healed fractured roots. The interesting findings were that the healing was observed even in the presence of two roots, including preservation of the vitality of the pulp. [source]


Repair of untreated horizontal root fracture: a case report

Abstract ,,,We report a clinical case of horizontal root fracture in the mandibular first premolar induced by a traffic accident. The fractured tooth spontaneously healed and diagnosed radiographically after 2 years. Fractured fragments were separated by a narrow radiolucent line and the edges were rounded, and healing of the tooth is likely to be related to interproximal connective tissue healing. Our case is an example of spontaneously healed fracture roots. An interesting finding is that the healing was observed even in the presence of two root canals, the vitality of the pulp was preserved and displacement of fragments were prevented. [source]


Repair characteristics of horizontal root fracture: a case report

Abstract ,,,Radicular fractures in permanent teeth are uncommon injuries among dental traumas, comprising 0.5,7% of the cases. Fracture occurs most often in the middle-third of the root and rarely at the apical-third. The present paper reports a clinical case of a horizontal radicular fracture located between the middle- and apical-third of a upper left-central incisor followed-up for over 3 years. The tooth was extracted owing to periodontal reasons. Histomorphologically, it showed pulp-vitality preservation and root healing by hard-tissue deposition. [source]


Fracture Resistance of Endodontically Treated Teeth: Three Walls versus Four Walls of Remaining Coronal Tooth Structure

Abstract Purpose: The purpose of this study was to evaluate the fracture resistance of endodontically treated teeth between those with four walls and those with three walls of remaining coronal tooth structure and the effect of the site of the missing coronal wall. Materials and Methods: Thirty-two endodontically treated second mandibular premolars were decoronated, leaving 3 mm above the cementoenamel junction (CEJ). A 0.5-mm-wide chamfer was prepared 1 mm above the CEJ. The teeth were randomly divided into four groups. Group 1 had four walls of coronal tooth structure, whereas groups 2, 3, and 4 had only three walls, missing the buccal, lingual, and mesial wall, respectively. The cast dowel and cores and crowns (Ni,Cr alloy) were cemented with zinc phosphate cement. A compressive load was applied 45° to the long axis, 2 mm below the buccal cusp, with an Instron machine until failure at a crosshead speed of 5 mm/min. Failure load (kg) and mode of failure were recorded. Data were analyzed with one-way ANOVA and Scheffé tests (p < 0.05). Results: Group 1 had the highest fracture resistance (1190.3 ± 110.5 kg), significantly different from the other groups (p < 0.05) (group 2: 578.5 ± 197.4 kg; group 3: 786.6 ± 132.8 kg; group 4: 785.4 ± 289.9 kg). There were no significant differences among the test groups. The mode of failure in group 1 was a horizontal root fracture, whereas that of the other groups was either vertical or oblique fracture. Conclusions: Teeth with four walls of remaining coronal dentine had significantly higher fracture resistance than teeth with only three walls. The site of the missing coronal wall did not affect the fracture resistance of endodontically treated teeth. [source]


Conservative treatment of a cervical horizontal root fracture and a complicated crown fracture: a case report

Abstract This case report describes successful long-term conservative management of a cervical root fracture and a complicated crown fracture of the maxillary central incisors in a 12-year-old patient. A mineral trioxide aggregate partial pulpotomy was performed on the maxillary right central incisor, while the maxillary left central incisor was splinted to the neighbouring lateral incisor using an acid-etch technique. Both teeth remained asymptomatic throughout the 3.5 years of a review period, with the cervical root fracture having mostly healed with the formation of a calcified tissue between the fragments. Two different treatment methods were used for two different injuries that resulted in pulp preservation in both cases. This in turn has provided for normal root development to occur while also allowing for preservation of bone. [source]
