Preventive Care in the Emergency Department: Should Emergency Departments Conduct Routine HIV Screening?

Abstract Objective: To perform a systematic review of the emergency medicine literature to assess the appropriateness of offering routine HIV screening to patients in the emergency department (ED). Methods: The systematic review was conducted with the aid of a structured template, a companion explanatory guide, and a grading and methodological scoring system based on published criteria for critical appraisal. Two reviewers conducted independent searches using OvidR, PubMed, MD Consult, and Grateful Med. Relevant abstracts were reviewed; those most pertinent to the stated objective were selected for complete evaluation using the structured template. Results: Fifty-two relevant abstracts were reviewed; of these, nine were selected for detailed evaluation. Seven ED-based prospective cross-sectional seroprevalence studies found HIV rates of 2,17%. Highest rates of infection were seen among patients with behavioral risks such as male homosexual activity and intravenous drug use. Two studies demonstrated feasibility of both standard and rapid HIV testing in the ED, with more than half of the patients approached consenting to testing by either method, consistent with voluntary testing acceptance rates described in other settings. Several cost,benefit analyses lend indirect support for HIV screening in the ED. Conclusions: Multiple ED-based studies meeting the Centers for Disease Control and Prevention Guideline threshold to recommend routine screening, in conjunction with limited feasibility trials and extrapolation from cost,benefit studies, provide evidence to recommend that EDs offer HIV screening to high-risk patients (i.e., those with identifiable risk factors) or high-risk populations (i.e., those where HIV seroprevelance is at least 1%). Carbon monoxide uptake kinetics in unamended and long-term nitrogen-amended temperate forest soils

Abstract The effect of nitrogen (N) additions on the dynamics of carbon monoxide consumption in temperate forest soils is poorly understood. We measured soil CO profiles, potential rates of CO consumption and uptake kinetics in temperate hardwood and pine control plots and plots amended with 50 and 150 kg N ha,1 year,1 for more than 15 years. Soil profiles of CO concentrations were above atmospheric levels in the high-N plots of both stands, suggesting that in these forest soils the balance between consumption and production may be shifted so that either production is increased or consumption decreased. Highest rates of CO consumption were measured in the organic horizon and decreased with soil depth. In the N-amended plots, CO consumption increased in all but one soil depth of the hardwood stand, but decreased in all soil depths of the pine stand. CO enzyme affinities increased with soil depth in the control plots. However, enzyme affinities in the most active soil depths (organic and 0,5 cm mineral) decreased in response to low levels of N in both stands. In the high-N plots, affinities dramatically-increased in the hardwood stand, but decreased in the organic horizon and increased slightly in the 0,5 cm mineral soil in the pine stand. These findings indicate that long-term N addition either by fertilization or deposition may alter the size, composition and/or physiology of the community of CO consumers so that their ability to act as a sink for atmospheric CO has changed. This change could have a substantial effect on the lifetime of greenhouse gases such as CH4 and therefore the future of Earth's climate. [source]


Combining descriptive and functional analyses to assess and treat screaming

This study systematically examined the functional relation between screaming and environmental events in a 13-year-old boy diagnosed with autism and PDD-NOS. A functional analysis of screaming was conducted in study 1, with six conditions (attention, demand, play, alone, tangible,toy, and tangible,edible). Highest rates of screaming were observed during the tangible,toy and tangible,edible conditions. The analog assessment was followed by a descriptive assessment, which revealed the participant was most likely to scream when teacher attention was diverted to other students. Additionally, items found to maintain screaming during the analog assessment were never delivered contingent upon screaming in the participant's natural environment. The results of the descriptive assessment suggested that the analog assessment might have produced a false-positive outcome. A treatment based upon the results of the combined assessments led to a 50% reduction in screaming within four weeks. Two-year follow-up data indicated that screaming occurred at near-zero rates. Environmental Risk Factors Predisposing to the Development of Basal Cell Carcinoma

Background. Basal cell carcinomas (BCCs) are the most common malignancies in white people. The incidence varies depending on the region of the world, with the highest rate of 1% to 2% per year noted in Australia. It is estimated that BCC incidence increases by 5% annually. An increasing incidence of BCC is in line with the changes in the living style and exposure to various environmental factors. Objective. To present the environmental factors that may influence the development of BCCs. The influence of ultraviolet radiation exposure alone and in connection with immunosuppression, smoking, occupational factors, as well as arsenic and ionizing radiation exposure, was described. Conclusion. BCC is a very complex disease, with many factors influencing its development. Environmental factors are very important for the prevalence of BCC, and most of them can be avoided. The exposure to ultraviolet radiation is undoubtedly of great risk; therefore, the national campaigns against aggressive, seasonal sun exposure, especially in children and adolescents, as well as using sunscreens, are of great value in the fight against BCC development. Physical modelling of fault scarp degradation under freeze,thaw cycles

Abstract Physical modelling has been developed in order to simulate the effects of periglacial erosion processes on the degradation of slopes and scarps. Data from 41 experimental freeze,thaw cycles are presented. They attest to the efficiency of periglacial processes that control both erosion and changes in scarp morphology: (i) cryoexpulsion leads to an increase of scarp surface roughness and modifies significantly the internal structure of the active layer; (ii) combined effects of frost creep and gelifluction lead to slow and gradual downslope displacements of the active layer (0·3 cm/cycle); (iii) debris flows are associated with the most significant changes in scarp morphology and are responsible for the highest rate of scarp erosion; (iv) quantification of the erosion rate gives values close to 1 cm3 cm,2 for 41 freeze,thaw cycles. These experimental results are consistent with field data acquired along the La Hague fault scarp (Normandy, France) where an erosion rate of 4·6 ± 1 m3 m,2 per glacial stage has been computed from the volume of natural slope deposits stored during the Weichselian glacial stage. These results show that moist periglacial erosion processes could lead to an underestimation of Plio-Quaternary deformation in the mid-latitudes. Copyright © 2006 John Wiley & Sons, Ltd. [source]


Characteristics and comorbidity of drug and alcohol-related emergency department presentations detected by nursing triage text

ABSTRACT Introduction This study used nursing triage text to detect drug- and alcohol-related emergency department (ED) presentations and describe their patient and service delivery characteristics. Methods Data were reviewed for all ED presentations from 2004 to 2006 (n = 263 937) from two hospitals in Sydney, Australia. Each record included two nursing triage free-text fields, which were searched for more than 100 drug-related and more than 60 alcohol-related terms. Adjusted odds ratios were used to compare the characteristics of drug and alcohol-related ED presentations with all other ED presentation types. Results Just over 5% of ED presentations were identified as alcohol-related and 2% as drug-related. The most prevalent drug-related ED presentations specified were related to amphetamines (18%), heroin (14%), cannabis (14%) and ecstasy (12%), while nearly half (43%) were drug unspecified. Polydrug use was mentioned in 25% of drug-related and 9% of alcohol-related ED presentations, with the highest rate of polydrug use among ecstasy-related (68%) presentations. Drug- and alcohol-related ED presentations were significantly more likely than other ED presentations to have a mental health diagnosis, with the highest rates found among cannabis-related (OR = 7.6) or amphetamine-related (OR = 7.5) presentations. Conclusion The ED provides an opportunity for early intervention for patients presenting with comorbid drug and alcohol and mental health problems. Further research is needed to assess the prevalence of drug and alcohol problems in ED patients with mental health problems and to develop effective interventions in that setting. [source]


Impact of artificial photoperiodism on the colony development of the bumblebee Bombus terrestris

Abstract This study investigated the effect of the photoperiodic regimes 0 h light : 24 h dark (LD 0:24), LD 8:16, LD 16:8 and LD 24:0 at 28°C and 50% Relative Humidity (RH) on the colony development of hibernated (2.0, 2.5, 3.0, 3.5 and 4.0 months) bumblebee queens. The queens which had hibernated for 3.0 months and which were reared in a LD 8:16 photoperiod showed the highest rate of colony initiation (88.2%), colony foundation (67.6%) and progeny queen production (38.2%). The photoperiod LD 8:16 also produced the shortest period of colony initiation and colony foundation. The highest number of sexual males (171.2 ± 12.2) and queens (91.2 ± 9.9) were produced in the colonies when 3.5 and 3.0 month hibernated queens were kept in an LD 8:16 photoperiod. The results show that light regime and hibernation duration affect colony characteristics of Bombus terrestris. [source]


Renal Response to Arginine Vasopressin During the Oestrous Cycle in the Rat: Comparison of Glucose and Saline Infusion Using Physiological Doses of Vasopressin

The renal response to arginine vasopressin in the rat has been shown to depend on reproductive status. However there is no consensus as to when the kidney is most responsive. The varying results could depend on the protocol and the dose of hormone used. A study has been performed, with physiological doses of vasopressin, comparing the responses during infusion of hypotonic saline and glucose. After an equilibration period of 150 min, conscious rats were infused on each of the four days of the oestrous cycle with either isotonic saline (0.077 M) or 0.14 M glucose for a control period of 45 min. Vasopressin was then infused at 10-40 fmol min,1 for 1 h, followed by a recovery period of 90 min. Timed urine samples were collected for determination of volume, sodium concentration and osmolality. During the control period urine flow was greatest at oestrus and dioestrus day 2 and sodium excretion on dioestrus day 2 irrespective of the infusate. Vasopressin concentrations achieved lay within the physiological range and no difference was observed between the different days for a given dose. Infusion of vasopressin in both saline and glucose produced a dose-dependent antidiuresis, the greatest responses being seen of pro-oestrus and dioestrus day 2. It was only with the highest rate of infusion that a significant increase in sodium excretion was seen on each day of the cycle and the greatest responses were seen on pro-oestrus and dioestrus day 1 for both infusates. Thus the kidney shows the greatest response to physiological doses of vasopressin at pro-oestrus and dioestrus day 1 irrespective of the infusate employed. [source]


Testosterone 1,-hydroxylation by human cytochrome P450 3A4

Human cytochrome P450 3A4 forms a series of minor testosterone hydroxylation products in addition to 6,-hydroxytestosterone, the major product. One of these, formed at the next highest rate after the 6,- and 2,-hydroxy products, was identified as 1,-hydroxytestosterone. This product was characterized from a mixture of testosterone oxidation products using an HPLC-solid phase extraction-cryoprobe NMR/time-of-flight mass spectrometry system, with an estimated total of ,,6 µg of this product. Mass spectrometry established the formula as C19H29O3 (MH+ 305.2080). The 1-position of the added hydroxyl group was established by correlated spectroscopy and heteronuclear spin quantum correlation experiments, and the ,-stereochemistry of the added hydroxyl group was assigned with a nuclear Overhauser correlated spectroscopy experiment (1,-H). Of several human P450s examined, only P450 3A4 formed this product. The product was also formed in human liver microsomes. [source]


Quantifying prehistoric soil erosion,A review of soil loss methods and their application to a Celtic square enclosure (Viereckschanze) in Southern Germany

This paper discusses the strengths and weaknesses of three different methods for quantifying prehistoric soil erosion. Method A estimates erosion by determining the amount of colluvium stored downhill. Method B involves reconstructing a former erosion surface using truncated soil horizons. Method C compares the elevation of a paleosol beneath an earthwork with the modern surface in the surrounding area. Each method was applied to a Celtic earthwork (Viereckschanze) at Poign (near Regensburg) in Southern Germany in order to cross-check the different results. For an erosion area of 3.6 ha and during 300 years of agricultural usage, Method A calculates a minimum erosion rate of 20.8 t/ha/a. Method B computes 10 t/ha/a of soil loss. Method C yields the highest rate of erosion with 24.2 t/ha/a. We have confidence in Method C, which implies an underassesment of soil loss in using methods A or B. © 2007 Wiley Periodicals, Inc. Interactive effects of water table and precipitation on net CO2 assimilation of three co-occurring Sphagnum mosses differing in distribution above the water table

Abstract Sphagnum cuspidatum, S. magellanicum and S. rubellum are three co-occurring peat mosses, which naturally have a different distribution along the microtopographical gradient of the surface of peatlands. We set out an experiment to assess the interactive effects of water table (low: ,10 cm and high: ,1 cm) and precipitation (present or absent) on the CO2 assimilation and evaporation of these species over a 23-day period. Additionally, we measured which sections of the moss layer were responsible for light absorption and bulk carbon uptake. Thereafter, we investigated how water content affected carbon uptake by the mosses. Our results show that at high water table, CO2 assimilation of all species gradually increased over time, irrespective of the precipitation. At low water table, net CO2 assimilation of all species declined over time, with the earliest onset and highest rate of decline for S. cuspidatum. Precipitation compensated for reduced water tables and positively affected the carbon uptake of all species. Almost all light absorption occurred in the first centimeter of the Sphagnum vegetation and so did net CO2 assimilation. CO2 assimilation rate showed species-specific relationships with capitulum water content, with narrow but contrasting optima for S. cuspidatum and S. rubellum. Assimilation by S. magellanicum was constant at a relatively low rate over a broad range of capitulum water contents. Our study indicates that prolonged drought may alter the competitive balance between species, favoring hummock species over hollow species. Moreover, this study shows that precipitation is at least equally important as water table drawdown and should be taken into account in predictions about the fate of peatlands with respect to climate change. Prevalence rate of Cryptosporidium infection in hemodialysis patients in Iran

Abstract Cryptosporidium is one of the most common causes of diarrhea in the world, which can be severe and prolonged in immunocompromised patients. We compared the prevalence rate of Cryptosporidium infection in hemodialysis patients and 2 control groups (i.e., their healthy family members and normal population). Stool specimens of 104 adult outpatient chronic hemodialysis patients, their 91 healthy family members, and 140 healthy individuals were examined for the presence of Cryptosporidium oocysts by using a modified acid-fast staining method. Twelve (11.5%) dialysis patients were infected with Cryptosporidium. This was significantly higher than 4 (4.4%), and 5 (3.6%) cases in the 2 control groups, respectively (p<0.05). There was no significant difference between the 2 control groups. The prevalence rate of Cryptosporidium infection did not correlate with patients' sex, age, duration of dialysis, history of kidney transplantation, or history of taking immunosuppressive drugs. However, it was significantly higher in diabetics vs. nondiabetics (19.4% vs. 8.3%, respectively, p<0.05). Our results indicate that the prevalence rate of Cryptosporidium infection is considerably higher in dialysis patients than in the general population. Moreover, dialyzed diabetic patients had the highest rate of infection. As hemodialysis patients are candidates for renal transplantation, general preventive measures against acquiring Cryptosporidium infection must be considered. [source]


Nitrogen fixation and denitrification in a floodplain forest near Manaus, Brazil

Abstract The Amazon floodplain (várzea) is seasonally affected by water level fluctuations of the Solimőes/Amazon River. The drastic environmental changes that occur also include microbiological processes, such as nitrogen (N2) fixation and denitrification. Both processes were measured in the soil by the acetylene reduction assay and the acetylene block method in a várzea forest on Ilha de Marchantaria, Central Amazonia, Brazil. In the surface soil horizon (0,5 cm), N2 fixation was highest during the exposed period (0·04,0·26 nmolN h,1 g,1 dry weight (dw)). In contrast, denitrification varied from 0 to 1·40 nmolN h,1 g,1 dw, with high rates during the submerged and the transition periods. No significant difference between locations with legume trees, with non-legume trees and without trees could be observed. N2 fixation rates of incubations (litter down to 450 cm depth) for samples collected during the exposed period ranged from 0 to 0·11 nmolN h,1 g,1 dw, with highest rates in the surface soil horizon (0,5 cm). Denitrification ranged from 0 to 0·05 nmolN h,1 g,1 dw, with the highest rate at 250,300 cm depth, which was just below the water table. The maximum N2 fixation rate (0·89 nmolN h,1 g,1dw) and denitrification rate (0·09 nmolN h,1 g,1 dw) occurred in the litter layer. On average, at least three times as much N is lost from the surface soil horizon via denitrification than is gained by N2 fixation annually, but the rates are strongly influenced by the flood pulse. Rock magnetism and paleomagnetic stratigraphy of forearc sediments of the Japan Trench, ODP Sites 1150 and 1151

Abstract Magnetic measurements were carried out to investigate rock magnetic properties and paleomagnetic directions of late and middle Miocene sediments recovered from the land side of the Japan Trench during the Ocean Drilling Program Leg 186. Because the low coercive component in natural remanent magnetization (NRM) normalized by anhysteretic remanent magnetization shows that the drilling-induced magnetization is severe in the sections obtained by the advanced hydraulic piston coring method, careful analyses of demagnetization of NRM using the ,demagnetization plane' were carried out to decompose the direction and intensity. Magnetostratigraphic correlation down to the upper Miocene, supplemented by biostratigraphic data, revealed that the sedimentation rates are characterized by drastic changes, with the early Pliocene having the highest rate. This high sedimentation rate is related to the subsidence of the southern deep-sea terrace of the Japan Trench. [source]


Pneumonia and Influenza Hospitalizations in Elderly People with Dementia

OBJECTIVES: To compare the demographic and geographic patterns of pneumonia and influenza (P&I) hospitalizations in older adults with dementia with those of the U.S. population and to examine the relationship between healthcare accessibility and P&I. DESIGN: Observational study using historical medical claims from the Centers for Medicare and Medicaid Services (CMS) and CMS records supplemented with information derived from other large national sources. SETTING: Retrospective analysis of medical records uniformly collected over a 5-year period with comprehensive national coverage. PARTICIPANTS: A study population representative of more than 95% of all people aged 65 and older residing in the continental United States. MEASUREMENTS: Six million two hundred seventy-seven thousand six hundred eighty-four records of P&I between 1998 and 2002 were abstracted, and county-specific outcomes for hospitalization rates of P&I, mean length of hospital stay, and percentage of deaths occurring in a hospital setting were estimated. Associations with county-specific elderly population density, percentage of nursing home residents, median household income per capita, and rurality index were assessed. RESULTS: Rural and poor counties had the highest rate of P&I and percentage of influenza. Patients with dementia had a lower frequency of influenza diagnosis, a shorter length of hospital stay, and 1.5 times as high a rate of death as the national average. CONCLUSION: The results suggest strong disparities in healthcare practices in rural locations and vulnerable populations; infrastructure, proximity, and access to healthcare are significant predictors of influenza morbidity and mortality. These findings have important implications for influenza vaccination, testing, and treatment policies and practices targeting the growing fraction of patients with cognitive impairment. [source]


Are the Northern Andes a species pump for Neotropical birds?

Phylogenetics, biogeography of a clade of Neotropical tanagers (Aves: Thraupini)
Abstract Aim, We used mitochondrial DNA sequence data to reconstruct the phylogeny of a large clade of tanagers (Aves: Thraupini). We used the phylogeny of this Neotropical bird group to identify areas of vicariance, reconstruct ancestral zoogeographical areas and elevational distributions, and to investigate the correspondence of geological events to speciation events. Location, The species investigated are found in 18 of the 22 zoogeographical regions of South America, Central America and the Caribbean islands; therefore, we were able to use the phylogeny to address the biogeographical history of the entire region. Methods, Molecular sequence data were gathered from two mitochondrial markers (cytochrome b and ND2) and analysed using Bayesian and maximum-likelihood approaches. Dispersal,vicariance analysis (DIVA) was used to reconstruct zoogeographical areas and elevational distributions. A Bayesian framework was also used to address changes in elevation during the evolutionary history of the group. Results, Our phylogeny was similar to previous tanager phylogenies constructed using fewer species; however, we identified three genera that are not monophyletic and uncovered high levels of sequence divergence within some species. DIVA identified early diverging nodes as having a Northern Andean distribution, and the most recent common ancestor of the species included in this study occurred at high elevations. Most speciation events occurred either within highland areas or within lowland areas, with few exchanges occurring between the highlands and lowlands. The Northern Andes has been a source for lineages in other regions, with more dispersals out of this area relative to dispersals into this area. Most of the dispersals out of the Northern Andes were dispersals into the Central Andes; however, a few key dispersal events were identified out of the Andes and into other zoogeographical regions. Main conclusions, The timing of diversification of these tanagers correlates well with the main uplift of the Northern Andes, with the highest rate of speciation occurring during this timeframe. Central American tanagers included in this study originated from South American lineages, and the timing of their dispersal into Central America coincides with or post-dates the completion of the Panamanian isthmus. The Wnt antagonist secreted frizzled-related protein-1 controls osteoblast and osteocyte apoptosis

Abstract Mechanisms controlling human bone formation remain to be fully elucidated. We have used differential display-polymerase chain reaction analysis to characterize osteogenic pathways in conditionally immortalized human osteoblasts (HOBs) representing distinct stages of differentiation. We identified 82 differentially expressed messages and found that the Wnt antagonist secreted frizzled-related protein (sFRP)-1 was the most highly regulated of these. Transient transfection of HOBs with sFRP-1 suppressed canonical Wnt signaling by 70% confirming its antagonistic function in these cells. Basal sFRP-1 mRNA levels increased 24-fold during HOB differentiation from pre-osteoblasts to pre-osteocytes, and then declined in mature osteocytes. This expression pattern correlated with levels of cellular viability such that the pre-osteocytes, which had the highest levels of sFRP-1 mRNA, also had the highest rate of cell death. Basal sFRP-1 mRNA levels also increased 29-fold when primary human mesenchymal stem cells were differentiated to osteoblasts supporting the developmental regulation of the gene. Expression of sFRP-1 mRNA was induced 38-fold following prostaglandin E2 (PGE2) treatment of pre-osteoblasts and mature osteoblasts that had low basal message levels. In contrast, sFRP-1 expression was down-regulated by as much as 80% following transforming growth factor (TGF)-,1 treatment of pre-osteocytes that had high basal mRNA levels. Consistent with this, treatment of pre-osteoblasts and mature osteoblasts with PGE2 increased apoptosis threefold, while treatment of pre-osteocytes with TGF-,1 decreased cell death by 50%. Likewise, over-expression of sFRP-1 in HOBs accelerated the rate of cell death threefold. These results establish sFRP-1 as an important negative regulator of human osteoblast and osteocyte survival. © 2005 Wiley-Liss, Inc. [source]


The Intergenerational Cycle of Criminality,Association with Psychopathy,

Abstract:, Preventive interventions early in life are likely to lower the risk of intergenerational transmission of criminal behavior. We investigated if psychopathy among homicidal offenders is associated with criminal offending among the offenders' offspring. The basic sample consisted of consecutive Finnish homicide offenders (during 1995,2004) who had been subjected to a forensic psychiatric examination and rated for a file-based PCL-R, and their offspring. Criminal behavior among both genders of the offspring was more common than in the general population. In general, the offspring's crimes against others (e.g., threat, intimidation, deprivation of freedom, breach of domicile) were associated with their parent's psychopathy. A grandfather's major mental disorder was associated with a high rate of crime committed by the offspring. Especially, the sons of male psychopathic homicidal offenders had the highest rate of committing crimes, which was often expressed as vandalism. Oral cancer over four decades: epidemiology, trends, histology, and survival by anatomical sites

The subtypes and different sites of oral cancer has different etiology epidemiology and survival rate. Prevalence of the various anatomical oral sites provided potential baseline for improvement of clinical approach. Methods:, Incidence and survival rates were derived from the Israel National Cancer Registry and included all registered data between 1970 and 2006. Oral cancer included the lips, tongue, buccal mucosa, gums, vestibulum, floor of the mouth, and palate. Results:, Most prevalent oral cancer subtype was squamous cell carcinoma (SCC) among men above the age of 55 years. Females had a higher incidence of SCC in lateral border of tongue, gums and buccal mucosa. Lymphoma and sarcoma were the most prevalent under the age of 20. Melanomas and metastatic disease revealed the lowest survival rate, while invasive or infiltrating basal cell carcinoma in the lips had the highest rate. The highest oral survival rate was for the lip, and the lowest was for the tongue and gums. Conclusions:, Early detection of oral cancer is important for all the medical health team. Decrease in lip carcinoma may be a result of occupational or awareness changes and should be studied. Non-epithelial tumors under the age of 20 should be considered as a differential diagnosis. A basic oral examination should be included in all routine medical examinations, with emphasis on high-risk patients and high-risk oral sites. [source]


Content analysis of food advertising in Turkish television

Aim: Television (TV) viewing has been implicated in children's weight gain. We aimed to conduct a comprehensive content analysis of TV food advertising in Turkey. Methods: TV advertisements (ads) in the four most popular national free to air Turkish TV channels, were assessed on two weekdays and two weekend days at four time periods of the day; 0800,1200,1200,1600,1600,2000 and 2000,2400 h for each TV channel (64 h assessed for each TV channel), making a total of 256 h. Each ad was analysed for food and drink content, duration and audiovisual properties. Results: There were 8853 TV ads and 2848 of these were related to food (32.1%). A majority of food ads included high-calorie, high-fat, high-sugar food and drink rather than core/healthy foods (81%). Chocolate and chocolate bars were the most frequently advertised food/drink product, followed by cakes, coffee, tea, candies, gum, fast food, chips, juices/carbonated beverages, margarine and ice-cream formed the highest rate of food products advertised in decreasing order. Thirty per cent of all obesogenic/unhealthy ads targeted childhood by using audiovisual techniques. The proportion of total advertisements which were for food or drink, and the proportion of food advertisements that were for unhealthy foods were both much higher at the weekend (33% vs. 30% and 84% vs. 78%, respectively). The time period between 1600 and 2000 h was the most concentrated time slot (33%) for food advertising. Conclusions: This study provides data for the first time on the high levels of obesogenic food advertising on Turkish TV. This should alarm policy-makers to set limits on food advertising targeted towards children especially in countries like Turkey in which childhood obesity is emerging as an important public health issue. ENHANCEMENT OF INTRACELLULAR SAXITOXIN ACCUMULATION BY LIDOCAINE HYDROCHLORIDE IN THE CYANOBACTERIUM CYLINDROSPERMOPSIS RACIBORSKII T3 (NOSTOCALES),

The metabolic effect of three different concentrations of lidocaine hydrochloride (0.01, 0.1, and 1 ,M) on growth and saxitoxin (STX) production of the freshwater cyanobacterium Cylindrospermopsis raciborskii (Wolosznska) T3 was analyzed. Lidocaine hydrochloride increased both the growth rate and the final growth yield in the toxic cyanobacterium, with a maximum of 25% and 18% for a 1-,M dose, respectively. Moreover, C. raciborskii T3 samples harvested at the end of the growth phase and analyzed for STX content by HPLC showed an increase in STX intracellular concentration of 14.3% and 49.3% after exposure to 0.01 and 0.1 ,M lidocaine hydrochloride, respectively, whereas 1 ,M lidocaine hydrochloride resulted in a 114% incremental change in STX content. The time course of the 1-,M lidocaine hydrochloride effect showed the highest rate of increase in mean STX intracellular concentration (298%) within the first 2 h after induction. The increase in STX content induced by lidocaine hydrochloride in C. raciborskii T3 was dependent on the concentration of Na+ ions in the culture medium and alkaline pH. The results suggest a possible action of lidocaine hydrochloride on membrane ion fluxes and the hypothesis of a potential linkage between cyanobacterial homeostasis and STX regulation. [source]


Prospective Follow-Up of Empirically Derived Alcohol Dependence Subtypes in Wave 2 of the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC): Recovery Status, Alcohol Use Disorders and Diagnostic Criteria, Alcohol Consumption Behavior, Health Status, and Treatment Seeking

Background:, We have previously reported on an empirical classification of Alcohol Dependence (AD) individuals into subtypes using nationally representative general population data from the 2001 to 2002 Wave 1 of the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC) and latent class analysis. Our results suggested a typology of 5 separate clusters based upon age of onset of AD, multigenerational familial AD, rates of antisocial personality disorder (ASPD), endorsement of specific AD and Alcohol Abuse (AA) criteria, and the presence of comorbid mood, anxiety, and substance use disorders (SUD). In this report, we focus on the clinical follow-up of these cluster members in Wave 2 of the NESARC (2004 to 2005). Methods:, The mean interval between NESARC Wave 1 and NESARC Wave 2 interviews was 36.6 (SD = 2.6) months. For these analyses, we utilized a Wave 2 NESARC sample that was comprised of a total of 1,172 individuals who were initially ascertained as having past-year AD at NESARC Wave 1 and initially subtyped into one of 5 groupings using latent class analysis. We identified these subtypes as: (i) Young Adult, characterized by very early age of onset, minimal family history, and low rates of psychiatric and SUD comorbidity; (ii) Functional, characterized by older age of onset, higher psychosocial functioning, minimal family history, and low rates of psychiatric and SUD comorbidity; (iii) Intermediate Familial, characterized by older age of onset, significant familial AD, and elevated comorbid rates of mood disorders SUD; (iv) Young Antisocial, characterized by early age of onset and elevated rates of ASPD, significant familial AD, and elevated rates of comorbid mood disorders and SUD; (v) Chronic Severe, characterized by later onset, elevated rates of ASPD, significant familial AD, and elevated rates of comorbid mood disorders and SUD. In this report, we examine Wave 2 recovery status, health status, alcohol consumption behavior, and treatment episodes based upon these subtypes. Results:, Significantly fewer of the Young Adult and Functional subtypes continued to meet full DSM-IV AD criteria in Wave 2 than did the Intermediate Familial, the Young Antisocial, and the Chronic Severe subtypes. However, we did not find that treatment seeking for alcohol problems increased over Wave 1 reports. In Wave 2, Young Antisocial and Chronic Severe subtypes had highest rates of past-year treatment seeking. In terms of health status, the Intermediate Familial, the Young Antisocial, and the Chronic Severe subtypes had significantly worse mental health scores than the Young Adult and Functional subtypes. For physical health status, the Functional, Intermediate Familial, Young Antisocial, and the Chronic Severe subtypes had significantly worse scores than the Young Adult subtype. In terms of alcohol consumption behavior, the Young Adult, Functional, and Young Antisocial subtypes significantly reduced their risk drinking days between Wave 1 and Wave 2, whereas the Intermediate Familial and the Chronic Severe subtypes did not. Discussion:, The results suggest that the empirical AD typology predicts differential clinical outcomes 3 years later. Persistence of full AD, treatment seeking, and worse mental health status were associated most strongly with those subtypes manifesting the greatest degree of psychiatric comorbidity. Reductions in alcohol consumption behavior and good physical health status were seen among the 2 younger subtypes. Overall, the least prevalent subtype, the Chronic Severe, showed the greatest stability in the manifestations of AD, despite having the highest rate of treatment seeking. [source]


The selling of the President 2004: a marketing perspective

George W. Bush won the 2004 US Presidential election despite the facts of one thousand people losing their life in the Iraq war, the highest rate of increase in unemployment in 70 years and a vitriolic propaganda campaign (Michael Moore, etc.) against him. This case study seeks to explain the success through the prism of marketing theory and conceptual structures, that is, that the Bush team had a superior communications strategy and, within those parameters, superior marketing elements. Thus we seek to surface and integrate a number of causal explanations for his victory that arose from a political marketing orientation, specifically the offer of a ,coherent narrative'; the conduct of a ,permanent campaign'; more effective negative advertising (especially by pro-Bush 527 groups), targeting and packaging; the success of the late campaign ,big tent' ploy. None of this however seeks to exclude the more purely political explanations for his success (located in such phenomena as the mobilization of the ,Christian Right' and the continuity to the aura attached to the ,911 President'); nor is the application of marketing thought to political contexts treated uncritically. A further aim is to introduce political marketing modes of analysis to a political science audience,not to present them as a new ,correctness', for they are certainly vulnerable to challenge, but rather to precipitate more of an intellectual exchange between these two disciplines. Recurrent hepatitis C after liver transplantation: On-treatment prediction of response to peginterferon/ribavirin therapy

Sustained virologic response (SVR) in the treatment of recurrent hepatitis C virus (HCV) infection after liver transplantation (LT) remains suboptimal. We evaluated efficacy of pegylated interferon alfa (PEG) and ribavirin (RBV) (PEG/RBV) combination therapy in LT recipients with recurrent HCV and predictive values of rapid virological response (RVR) and early virologic response (EVR). Between January 2001 and October 2005, LT recipients with recurrent HCV were intended to be treated for 48 weeks with PEG/RBV combination therapy independent of genotype or virologic response [53 patients (79% genotype 1)]. On-treatment predictor of response at week 4 (RVR) was defined as undetectable HCV RNA, and at week 12 (EVR) as undetectable HCV RNA or a >2 log10 drop from pretreatment viral load. SVR was seen in 19 (35%) patients. Patients with genotype 2/3 were more likely to achieve SVR than those with genotype 1 (87% versus 23%; P = 0.001). The highest rate of SVR was seen in patients with RVR [specificity and positive predictive value (PPV) = 100%] while the highest rate of treatment failure was seen in those who did not have EVR [sensitivity and negative predictive value (NPV) = 100%]. The NPV of RVR to identify those who will not achieve SVR was also very high (88%). EVR had low PPV (63%) to identify those with SVR. In conclusion, PEG/RBV combination therapy is effective in the treatment of post-LT recurrent HCV. On-treatment virologic monitoring is highly predictive of SVR and may optimize the virologic response and minimize toxicity. Relevance of caspase activity during apoptosis in pubertal rat spermatogenesis

Abstract Caspases are a family of cysteine-proteases, activated upon several different stimuli, which execute apoptosis in many cell death models. Previous work of our group has shown rats have the highest rate of apoptosis during the first wave of spermatogenesis (between 20 and 25 days after birth), as evaluated by TUNEL and caspase activity. However, the hierarchical order of caspase activation and the relevance of each caspase during germ cell apoptosis are not clear. Thus, the goal of this work is to take a pharmacological approach to dissect the apoptosis pathway of caspase activation. Results showed that intratesticular injection of a caspase-8 inhibitor (z-IETD-fmk), or a pan-caspase inhibitor (z-VAD- fmk), significantly decreased the cleavage of p115 and PARP, two endogenous substrates of caspases, in 22-day-old rats. Additionally, these inhibitors promoted a significant reduction in the number of apoptotic germ cells. On the other hand, intratesticular injection of two different inhibitors of the intrinsic pathway (z-LEHD-fmk and minocycline) did not have any effect upon caspase substrates cleavage (p115 and PARP) or the number of apoptotic germ cells. Therefore, we conclude that the extrinsic pathway of apoptosis plays an important role in physiological germ cell apoptosis during the first round of spermatogenesis in the rat. Mol. Reprod. Sites of recurrence in oral and oropharyngeal cancers according to the treatment approach

OBJECTIVE: The purpose of this study is to evaluate the rates and the sites of tumour recurrence in patients with oral and oropharyngeal carcinomas. DESIGN: This is a retrospective study of a series of cases treated in a single institution. PATIENTS AND METHODS: A series of 2067 patients with oral and oropharyngeal squamous carcinoma, treated from 1954 to 1998 were analysed. The treatment approach was: surgery, 624 cases (30.2%); radiotherapy alone, 729 cases (35.3%); radiotherapy and surgery, 552 cases (26.7%) and radiotherapy and chemotherapy, 162 cases (7.8%). MAIN OUTCOME MEASURES: Tumour recurrence was observed in 1079 patients (52.2%): 561 cases of local recurrences (27.1%); 168 neck recurrences (8.1%); 252 locoregional recurrences (12.2%); 59, distant metastasis (2.9%) and 39 (1.9%), combination of distant metastasis with local, neck or locoregional recurrence. RESULTS: The rates of recurrence varied significantly according to the treatment performed. Oral cavity cancer patients undergoing radiotherapy alone or in combination with chemotherapy presented the highest rates of neck recurrences (22.5 and 40.0%, respectively) for clinical stage (CS) I/II and of local (41.2 and 30.1%) and locoregional (21.7 and 31.1%) recurrences for CS III/IV; yet, for CS III/IV, surgery without neck dissection was associated with the highest rates of neck recurrences (20.7%), but no differences were observed in the rates of local or locoregional recurrences for CS I/II patients. For oropharynx cancer patients with CS I/II there was no difference in the rate of locoregional failures according to the treatment. However, patients with CS III/IV undergoing radiotherapy present a highest rate of local (42.3%) and locoregional (28.8%) failures. CONCLUSION: The results suggest that surgery should be the first option for initial clinical stage oral and oropharyngeal cancers. For advanced cases independently of the site of the tumour, surgery and postoperative radiotherapy should be the standard of care because it is associated with the lowest rates of locoregional recurrence. Rates of caesarean section: analysis of global, regional and national estimates

Summary Rates of caesarean section are of concern in both developed and developing countries. We set out to estimate the proportion of births by caesarean section (CS) at national, regional and global levels, describe regional and subregional patterns and correlate rates with other reproductive health indicators. We analysed nationally representative data available from surveys or vital registration systems on the proportion of births by CS. We used local non-parametric regression techniques to correlate CS with maternal mortality ratio, infant and neonatal mortality rates, and the proportion of births attended by skilled health personnel. Although very unevenly distributed, 15% of births worldwide occur by CS. Latin America and the Caribbean show the highest rate (29.2%), and Africa shows the lowest (3.5%). In developed countries, the proportion of caesarean births is 21.1% whereas in least developed countries only 2% of deliveries are by CS. The analysis suggests a strong inverse association between CS rates and maternal, infant and neonatal mortality in countries with high mortality levels. There is some suggestion of a direct positive association at lower levels of mortality. CS levels may respond primarily to economic determinants. [source]

Summary The object of this study is to present descriptive epidemiological characteristics of retinoblastoma (Rb) in children aged 0,14 years, seen at the Mexican Social Security Institute hospitals in Mexico City (MC) from 1990 to 1994. This is a retrospective, observational hospital survey. Clinical records of 52 Rb cases were reviewed; 39 were patients who did not reside in MC (non-residents), and 13 were MC residents. The study period was 1990,94. The male/female ratio (M/F) was 1.6. Average annual incidence (AAI) was estimated by age and sex (rates per 1 000 000). Annual average percentage change (AAPC) in incidence rates was estimated in children from 0 to 14 years. The AAI for MC residents was 3.2; the highest rate being for those <1 year olds (rate of 20.8); AAPC was 6.9% [95% CI ,27.5, 57.4]; the highest incidence was for the south-eastern region of MC residents (rate of 5.9); 13 cases (25%) were diagnosed at stage III or IV, and 11 cases (21.2%) were bilateral. Incidence of Rb is similar to that in developed countries and shows no increasing trend. Patients from communities outside MC are more frequently diagnosed at stages III and IV. [source]


Persistence and efficacy of two diatomaceous earth formulations and a mixture of diatomaceous earth with natural pyrethrum against Tribolium confusum Jacquelin du Val (Coleoptera: Tenebrionidae) on wheat and maize

Abstract Laboratory tests were conducted to assess the insecticidal and residual effects of three diatomaceous earth (DE) formulations, Insecto®, PyriSec® and SilicoSec®, against Tribolium confusum Jacquelin du Val on wheat and maize. Quantities of wheat and maize were treated with the above formulations at 500, 1000 and 1500 mg kg,1 and stored at 25 °C and 55% relative humidity (RH). Samples were taken on the day of storage and every 30 days until completion of a 360 day period of storage. Adults of T. confusum were exposed to these samples at 25 °C and 55% RH and the mortality was measured after 24 and 48 h and 7 and 14 days of exposure. Adult mortality was higher on wheat than on maize. At the beginning of the storage period, mortalities after 14 day exposure on maize treated with the highest rate were 60, 63 and 81% for Insecto®, PyriSec® and SilicoSec® respectively, while on wheat the mortality was 100% for all DEs. On the same commodities 360 days after treatment the respective mortality figures for wheat were 99, 98 and 100%, while in the case of maize they did not exceed 7%. Generally, mortality at exposures ,48 h decreased with increasing storage time. Furthermore, although mortality on wheat increased with dose, the increase in DE dose from 1000 to 1500 mg kg,1 resulted in only a small increase in mortality. Thus a DE treatment of 1000 mg kg,1 was shown to provide long-term protection of wheat against T. confusum, although higher DE application rates and exposure intervals are needed for a satisfactory level of protection of maize against this pest. Estimating ethnic differences in self-reported new use of antidepressant medications: results from the Multi-Ethnic Study of Atherosclerosis

Abstract Introduction There is evidence that the utilization of antidepressant medications (ADM) may vary between different ethnic groups in the United States population. Methods The Multi-Ethnic Study of Atherosclerosis (MESA) is a population-based prospective cohort study of 6814 US adults from 4 different ethnic groups. After excluding baseline users of ADM, we examined the relation between baseline depression and new use of ADM for 4 different ethnicities: African,Americans (n,=,1822), Asians (n,=,784) Caucasians (n,=,2300), and Hispanics (n,=,1405). Estimates of the association of ethnicity and ADM use were adjusted for age, study site, gender, Center for Epidemiologic Studies Depression Scale (CES-D), alcohol use, smoking, blood pressure, diabetes, education, and exercise. Non-random loss to follow-up was present and estimates were adjusted using inverse probability of censoring weighting (IPCW). Results Of the four ethnicities, Caucasian participants had the highest rate of ADM use (12%) compared with African,American (4%), Asian (2%), and Hispanic (6%) participants. After adjustment, non-Caucasian ethnicity was associated with reduced ADM use: African,American (HR: 0.42; 95% Confidence Interval (CI): 0.31,0.58), Asian (HR: 0.14; 95%CI: 0.08,0.26), and Hispanic (HR: 0.47; 95%CI: 0.31,0.65). Applying IPCW to correct for non-random loss to follow-up among the study participants weakened but did not eliminate these associations: African,American (HR: 0.48; 95%CI: 0.30,0.57), Asian (HR: 0.23; 95%CI: 0.13,0.37), and Hispanic (HR: 0.58; 95%CI: 0.47,0.67). Conclusion Non-Caucasian ethnicity is associated with lower rates of new ADM use.
