    Detecting New Forms of Network Intrusion Using Genetic Programming

    How to find and detect novel or unknown network attacks is one of the most important objectives in current intrusion detection systems. In this paper, a rule evolution approach based on Genetic Programming (GP) for detecting novel attacks on networks is presented and four genetic operators, namely reproduction, mutation, crossover, and dropping condition operators, are used to evolve new rules. New rules are used to detect novel or known network attacks. A training and testing dataset proposed by DARPA is used to evolve and evaluate these new rules. The proof of concept implementation shows that a rule generated by GP has a low false positive rate (FPR), a low false negative rate and a high rate of detecting unknown attacks. Moreover, the rule base composed of new rules has high detection rate with low FPR. An alternative to the DARPA evaluation approach is also investigated. [source]


    An elaborate education of basic genetic programming using C++

    Abstract Evolutionary search is a global search method based on natural selection. In engineering curriculum, these techniques are taught in courses like Evolutionary Computation, Engineering Optimization, etc. Genetic algorithm (GA) is popular among these algorithms. Genetic programming (GP), developed by John Koza, is a powerful extension of GA where a chromosome/computer program (CP) is coded as a rooted point-labeled tree with ordered branches. The search space is the space of all possible CPs (trees) consisting of functions and terminals appropriate to the problem domain. GP uses, like GA, crossover and mutation for evolution. Due to tree-structured coding of individuals, the initial population generation, genetic operators' use, and tree decoding for fitness evaluations demand careful computer programming. This article describes the programming steps of GP implementation (using C++ language) for students' easy understanding with pseudocodes for each step.     Recognition of depressive symptoms in the elderly: What can help the patient and the doctor

    Abstract The general public heavily underrecognizes depression and depressive symptoms. This underrecognition is more pronounced among elderly people, and this study is an initial attempt to quantify the problem in a Greek elderly sample. Additionally the authors attempt to identify patient-related factors, which can assist a subject to recognize the depressive symptoms and the general practitioner to note their existence. Members of senior citizen centers (n = 682) participated in presentations about "depression in the elderly" and completed a questionnaire including the GDS-4 scale, four questions concerning depression risk factors and a question concerning a recent visit to a physician for depressive symptoms. Amongst those participating, 35.8% presented depressive symptoms (GDS-4 , 2). The calculated rate for recognition of depression in the studied population was very low (17.3%). Patients with depressive symptoms were more often females and had a higher proportion of past history of depression and a lack of social support. Patients with a past history of depression and more severe forms of illness consulted a doctor more frequently. Finally, subjects suffering from depressive symptoms and comorbid medical illness were characterized by a higher proportion of past history, lack of support, and existence of multiple risk factors. The authors propose that the inclusion in public campaigns of activities with an experiential dimension, e.g., patient videos and the use of a very simple screening tool, such as the GDS-4 scale by general practitioners (GP), could be helpful in improving the recognition of depressive symptoms by the patient and his/her relatives and its diagnosis by the doctor.     Experience of trauma and conversion to psychosis in an ultra-high-risk (prodromal) group

Objective:, We aimed to replicate a recent finding of high prevalence of trauma history in patients at ,ultra-high risk' (UHR) of psychotic disorder and to investigate whether trauma predicts conversion to psychosis in this population. Method:, A consecutive sample of UHR patients was assessed. History of trauma was accessed with the General Trauma Questionnaire. Cox regression models were used to explore relationship between conversion to psychosis and trauma. Results:, Of 92 UHR patients nearly 70% had experienced a traumatic event and 21.7% developed psychosis during follow-up (mean 615 days). Patients who had experienced a sexual trauma (36%) were significantly more likely to convert to first-episode psychosis (OR 2.96) after controlling for meeting multiple UHR intake groups. Conclusion:, UHR patients have a high prevalence of history of trauma. Previous sexual trauma may be a predictor of onset of psychotic disorder in this population. [source]


    Risk factors for incident mild cognitive impairment , results from the German Study on Ageing, Cognition and Dementia in Primary Care Patients (AgeCoDe)

Objectives:, To provide age- and gender-specific incidence rates of MCI among elderly general practitioner (GP) patients (75+ years) and to identify risk factors for incident MCI. Method:, Data were derived from the longitudinal German Study on Ageing, Cognition and Dementia in Primary Care Patients (AgeCoDe). Incidence was calculated according to the ,person-years-at-risk' method. Risk factors were analysed using multivariate logistic regression models. Results:, During the 3-year follow-up period, 350 (15.0%) of the 2331 patients whose data were included in the calculation of incidence developed MCI [person-years (PY) = 6198.20]. The overall incidence of MCI was 56.5 (95% confidence interval = 50.7,62.7) per 1000 PY. Older age, vascular diseases, the apoE ,4 allele and subjective memory complaints were identified as significant risk factors for future MCI. Conclusion:, Mild cognitive impairment is frequent in older GP patients. Subjective memory complaints predict incident MCI. Especially vascular risk factors provide the opportunity of preventive approaches. [source]


    Co-morbidity and the utilization of health care for Australian veterans with diabetes

    Diabet. Med. 27, 65,71 (2010) Abstract Objective, To examine the impact of co-morbidity on health service utilization by Australian veterans with diabetes. Methods, A retrospective cohort study was undertaken including veterans aged , 65 years dispensed medicines for diabetes in 2006. Data were sourced from the Australian Department of Veterans' Affairs health claims database. Utilization of preventive health services for diabetes was assessed, including claims for glycated haemoglobin (HbA1c) test, microabuminuria, podiatry services, diabetes care plans, medication reviews, case conferences, general practitioner (GP) management plans and ophthalmology/optometry services. Results, Among the 17 095 veterans dispensed medicines for diabetes, more than 80% had four or more co-morbid conditions. Those with a higher number of co-morbidities were more likely to have had claims for optometry/ophthalmology services and podiatry services, but not for other services. Veterans with at least one diabetes-related hospital admission had no more claims for diabetes health services than those who had no diabetics-related hospital admission, except for endocrinology services (relative risk = 1.26, 95% confidence intervals 1.15,1.37). Veterans with dementia were less likely to have had claims for diabetes health services while patients with renal failure were more likely to have had claims for the services. Conclusions, Low utilization of preventive diabetes care services is apparent in all co-morbidity groups. Patients with renal failure or dementia used more and less health services resources, respectively. Given the high mean age of this population, there may be valid reasons for the low use, such as competing health demands and patients' preferences. [source]


    The association of alcohol dependence with general practice attendance

    Abstract Introduction and Aims. This study was designed to examine the relationship between alcohol dependence and general practitioner (GP) service attendance in Australia. Design and Methods. Data were analysed from the 1997 Australian National Survey of Mental Health and Wellbeing. In this survey, a representative sample of the Australian population was interviewed to ascertain past 12 month psychiatric diagnoses for all major mental disorders as well as the use of primary and other health services (n = 10 641, 79% response rate). Results. People with alcohol dependence comorbid with other psychiatric disorders have higher rates of service usage than those without such disorders. Discussion and Conclusions. Alcohol dependence comorbid with mental disorders has a significant impact on GP service in Australia.     Mailed treatment to augment primary care for alcohol disorders: A randomised controlled trial

    Abstract Introduction and Aims. Remote delivery of interventions is needed to address large numbers of people with alcohol use disorders who are spread over large areas. Previous correspondence trials typically examined its effects as stand-alone treatment. This study aimed to test whether adding postal treatment to general practitioner (GP) support would lower alcohol use more than GP intervention alone. Design and Methods. A single-blind, randomised controlled trial with a crossover design was conducted over 12 months on 204 people with alcohol use disorders. Participants in an immediate correspondence condition received treatment over the first 3 months; those receiving delayed treatment received it in months 3,6. Results. Few participants were referred from GPs, and little intervention was offered by them. At 3 months, 78% of participants remained in the study. Those in immediate treatment showed greater reductions in alcohol per week, drinking days, anxiety, depression and distress than those in the delayed condition. However, post-treatment and follow-up outcomes still showed elevated alcohol use, depression, anxiety and distress. Greater baseline anxiety predicted better alcohol outcomes, although more mental distress at baseline predicted dropout. Discussion and Conclusions. The study gave consistent results with those from previous research on correspondence treatments, and showed that high levels of participant engagement over 3 months can be obtained. Substantial reductions in alcohol use are seen, with indications that they are well maintained.     Role of the general practitioner in smoking cessation

    Abstract This paper reflects on the role of general practitioners in smoking cessation and suggests initiatives to enhance general practice as a setting for effective smoking cessation services. This paper is one of a series of reflections on key issues in smoking cessation. In this article we highlight the extent that general practitioners (GPs) have contact with the population, evidence for effectiveness of GP advice, barriers to greater involvement and suggested future directions. General practice has an extensive population reach, with the majority of smokers seeing a GP at least once per year. Although there is level 1 evidence of the effectiveness of smoking cessation advice from general practitioners, there are substantial barriers to this advice being incorporated routinely into primary care consultations. Initiatives to overcome these barriers are education in smoking cessation for GPs and other key practice staff; teaching of medical students about tobacco and cessation techniques, clinical practice guidelines; support for guideline implementation; access to pharmacotherapies; and development of referral models. We believe the way forward for the role of the GPs is to develop the practice as a primary care service for providing smoking cessation advice. This will require education relevant to the needs of a range of health professionals, provision of and support for the implementation of clinical practice guidelines, access for patients to smoking cessation pharmacotherapies and integration with other cessation services such as quitlines [source]


    Trajectories of resilience over 25 years of individuals who as adolescents consulted for substance misuse and a matched comparison group

    ABSTRACT Aims To examine trajectories of resilience over 25 years among individuals who as adolescents received treatment for substance misuse, the clinical sample (CS) and a matched general population sample (GP). Design Comparison of the CS and GP over 25 years using Swedish national registers of health care and criminality. Setting A substance misuse clinic for adolescents in an urban area in Sweden. Measurements Resilience was defined as the absence of substance misuse, hospitalizations for physical illnesses related to substance misuse, hospitalization for mental illness and law-abiding behaviour from ages 21 to 45 years. Participants The CS included 701 individuals who as adolescents had consulted a clinic for substance misuse. The GP included 731 individuals selected randomly from the Swedish population and matched for age, sex and birthplace. Findings A total of 52.4% of the GP and 24.4% of the CS achieved resilience in all domains through 25 years. Among the CS, another one-third initially displayed moderate levels of resilience that rose to high levels over time, one-quarter displayed decreasing levels of resilience over time, while 9.3% showed little but improving resilience and 8.8% showed no resilience. Levels of resilience were associated with the severity of substance misuse and delinquency in adolescence. Conclusions Individuals who had presented substance misuse problems in adolescence were less likely to achieve resilience over the subsequent 25 years than was a matched general population sample, and among them, four distinct trajectories of resilience were identified. The severity and type of problems presented in adolescence distinguished the four trajectories. [source]


    The effect of time spent in treatment and dropout status on rates of convictions, cautions and imprisonment over 5 years in a primary care-led methadone maintenance service

    ABSTRACT Background Methadone maintenance treatment (MMT) in primary care settings is used increasingly as a standard method of delivering treatment for heroin users. It has been shown to reduce criminal activity and incarceration over periods of periods of 12 months or less; however, little is known about the effect of this treatment over longer durations. Aims To examine the association between treatment status and rates of convictions and cautions (judicial disposals) over a 5-year period in a cohort of heroin users treated in a general practitioner (GP)-led MMT service. Design Cohort study. Setting The primary care clinic for drug dependence, Sheffield, 1999,2005. Participants The cohort comprised 108 consecutive patients who were eligible and entered treatment. Ninety were followed-up for the full 5 years. Intervention The intervention consisted of MMT provided by GPs in a primary care clinic setting. Measurements Criminal conviction and caution rates and time spent in prison, derived from Police National Computer (PNC) criminal records. Findings The overall reduction in the number of convictions and cautions expected for patients entering MMT in similar primary care settings is 10% for each 6 months retained in treatment. Patients in continuous treatment had the greatest reduction in judicial disposal rates, similar to those who were discharged for positive reasons (e.g. drug free). Patients who had more than one treatment episode over the observation period did no better than those who dropped out of treatment. Conclusions MMT delivered in a primary care clinic setting is effective in reducing convictions and cautions and incarceration over an extended period. Continuous treatment is associated with the greatest reductions. [source]


    Determinants of successful chronic hepatitis C case finding among patients receiving opioid maintenance treatment in a primary care setting

    ABSTRACT Aims Injection drug users are at high risk for chronic hepatitis C virus infection (CHC). Opioid maintenance treatment (OMT) offers a unique opportunity to screen for CHC. This study proposed the hypothesis that a general practitioner (GP) with special interest in addiction medicine can achieve CHC screening rates comparable to specialized centres and aimed to investigate determinants for a successful CHC case finding in a primary care setting. Design and participants Retrospective medical record analysis of 387 patients who received opioid maintenance therapy between 1 January 2002 and 31 May 2008 in a general practice in Zurich, Switzerland. Measurements Successful CHC assessment was defined as performance of hepatitis C virus (HCV) serology with consecutive polymerase chain reaction-based RNA and genotype recordings. The association between screening success and patient characteristics was assessed using multiple logistic regression. Findings Median (interquartile range) age and duration of OMT of the 387 (268 males) patients was 38.5 (33.6,44.5) years and 34 (11.3,68.0) months, respectively. Fourteen patients (3.6%) denied HCV testing and informed consent about screening was missing in 13 patients (3.4%). In 327 of 360 patients (90.8%) with informed consent a successful CHC assessment has been performed. Screening for HCV antibodies was positive in 136 cases (41.6%) and in 86 of them (63.2%) a CHC was present. The duration of OMT was an independent determinant of a successful CHC assessment. Conclusions In addicted patients a high CHC assessment rate in a primary care setting in Switzerland is feasible and opioid substitution provides an optimal framework. [source]


    Short-term load forecasting using informative vector machine

    Abstract In this paper, a novel method is proposed for short-term load forecasting, which is one of the important tasks in power system operation and planning. The load behavior is so complicated that it is hard to predict the load. The deregulated power market is faced with the new problem of an increase in the degree of uncertainty. Thus, power system operators are concerned with the significant level of load forecasting. Namely, probabilistic load forecasting is required to smooth power system operation and planning. In this paper, an IVM (Informative Vector Machine) based method is proposed for short-term load forecasting. IVM is one of the kernel machine techniques that are derived from an SVM (Support Vector Machine). The Gaussian process (GP) satisfies the requirements that the prediction results are expressed as a distribution rather than as points. However, it is inclined to be overtrained for noise due to the basis function with N2 elements for N data. To overcome this problem, this paper makes use of IVM that selects necessary data for the model approximation with a posteriori distribution of entropy.     Virus-evolutionary linear genetic programming

    Abstract Many kinds of evolutionary methods have been proposed. GA and GP in particular have demonstrated their effectiveness in various problems recently, and many systems have been proposed. One is Virus-Evolutionary Genetic Algorithm (VE-GA), and the other is Linear Genetic Programming in C (LGPC). The performance of each system has been reported. VE-GA is the coevolution system of host individuals and virus individuals. That can spread schema effectively among the host individuals by using virus infection and virus incorporation. LGPC implements the GP by representing the individuals to one dimension as if GA. LGPC can reduce a search cost of pointer and save machine memory, and can reduce the time to implement GP programs. We have proposed that a system introduce virus individuals in LGPC, and analyzed the performance of the system on two problems. Our system can spread schema among the population, and search solution effectively. The results of computer simulation show that this system can search for solution depending on LGPC applying problem's character compared with LGPC. © 2008 Wiley Periodicals, Inc.     Mortality risk up to 25 years after initiation of treatment among 420 Swedish women with alcohol addiction

    ABSTRACT Aims Women treated for alcohol addiction have mortality rates three to five times those of women from the general population (GP). However, these figures may be inflated because socially disadvantaged women with advanced drinking careers are over-represented in previous studies. Our aim was to study the long-term mortality of socially relatively well-functioning patients coming to their first treatment, compared to matched GP controls. Design The mortality rates and causes of death were compared between patients and their matched GP controls, using data from the Causes of Death Register throughout the follow-up period (0,25 years). Setting A specialized treatment programme for women only, called ,Early treatment for Women with Alcohol Addiction' (EWA) at the Karolinska Hospital, Stockholm, Sweden. Participants Subjects (n = 420) receiving their first treatment at the EWA programme, compared to a group of matched GP women (n = 2037). Findings The women patients had significantly higher mortality than matched GP controls throughout the whole follow-up period, with a relative risk of 2.4. However, the younger women had four times higher mortality than their matched controls. The peak of deaths occurred during the first 5 years, and alcohol-related causes of death were highly over-represented, as were uncertain suicides and accidents. Conclusions First-time-treated women with alcohol addiction have a substantially lower mortality than reported previously from clinical samples, except for the youngest group. Our figures were corrected for confounding factors such as socio-demographic status. We believe our results could apply to broader groups of heavy drinking women, inside or outside specialized treatment settings. [source]


    Association of type of sport and performance level with anatomical site of orthopaedic injury diagnosis

    Summary Reason for performing study: Although anecdotal reports of increased orthopaedic injury risk in equine sports exist, there is little scientific evidence to support this. Objectives: To test whether horses undertaking a single competitive sport have increased risk of specific injuries compared to those used for general purpose riding (GP); and whether injury type varies with sport category and performance level. Methods: Data from 1069 records of horses undergoing orthopaedic evaluation (1998,2003) and meeting inclusion criteria were reviewed. Sport category (GP, showjumping, dressage, eventing, racing), level (nonelite or elite) and diagnosis were recorded. Effects of sport category and level on probability of a specific diagnosis were assessed using chisquared tests. Logistic regression was used to determine which competitive sports and levels increased risk of injury compared with GP. Results: Overall there was a significant effect of sport category and level on diagnosis (P<0.0001). There was significant difference between anatomical site injured and sport category (P<0.0001); a high risk of forelimb superficial digital flexor tendon injury in elite eventing (P<0.0001) and elite showjumping (P=0.02); distal deep digital flexor tendon (DDFT) injury in elite showjumping (P=0.002); and hindlimb suspensory ligament injury in elite (P<0.0001) and nonelite (P=0.001) dressage. There was a low risk of tarsal injury in elite eventing (P=0.01) and proximal DDFT injury in dressage (P = 0.01). Conclusions: Horses competing in different sports are predisposed to specific injuries; particular sports may increase the risk of injury at certain anatomical sites; and the type and site of injury may reflect the type and level of performance. Potential relevance: These findings could guide clinicians in the diagnosis of sport related injuries. [source]


    Nigrostriatal lesion induces D2-modulated phase-locked activity in the basal ganglia of rats

    Abstract There is a debate as to what modifications of neuronal activity underlie the clinical manifestations of Parkinson's disease and the efficacy of antiparkinsonian pharmacotherapy. Previous studies suggest that release of GABAergic striatopallidal neurons from D2 receptor-mediated inhibition allows spreading of cortical rhythms to the globus pallidus (GP) in rats with 6-hydroxydopamine-induced nigrostriatal lesions. Here this abnormal spreading was thoroughly investigated. In control urethane-anaesthetized rats most GP neurons were excited during the active part of cortical slow waves (,direct-phase' neurons). Two neuronal populations having opposite phase relationships with cortical and striatal activity coexisted in the GP of 6-hydroxydopamine-lesioned rats. ,Inverse-phase' GP units exhibited reduced firing coupled to striatal activation during slow waves, suggesting that this GP oscillation was driven by striatopallidal hyperactivity. Half of the pallidonigral neurons identified by antidromic stimulation exhibited inverse-phase activity. Therefore, spreading of inverse-phase oscillations through pallidonigral axons might contribute to the abnormal direct-phase cortical entrainment of basal ganglia output described previously. Systemic administration of the D2 agonist quinpirole to 6-hydroxydopamine-lesioned rats reduced GP inverse-phase coupling with slow waves, and this effect was reversed by the D2 antagonist eticlopride. Because striatopallidal hyperactivity was only slightly reduced by quinpirole, other mechanisms might have contributed to the effect of quinpirole on GP oscillations. These results suggest that antiparkinsonian efficacy may rely on other actions of D2 agonists on basal ganglia activity. However, abnormal slow rhythms may promote enduring changes in functional connectivity along the striatopallidal axis, contributing to D2 agonist-resistant clinical signs of parkinsonism. [source]

In this study, we investigated the ultrastructural localization of the MUC5B and MUC7 mucins in human labial glands by means of a postembedding immunogold technique. Thin sections of normal human labial glands, obtained during surgery, were incubated with polyclonal antibodies to human salivary mucins MUC5B and MUC7, and then with gold-labeled secondary antibodies. Specific MUC5B reactivity was found in the secretory granules of mucous cells of all glands examined, and was associated with the luminal membrane of duct cells. MUC7 labeling was observed in the granules of both mucous and seromucous secretory cells of the glandular parenchyma. Quantitative analyses demonstrated that seromucous granules have higher immunogold labeling densities for MUC7 than mucous granules. Our immunohistochemical data extend the results of previous light microscopic studies of MUC5B and MUC7 localizations, pointing out the significant contribution of human labial glands in the secretion process of these two mucins.     Effect of dentin conditioning on dentin permeability and micro-shear bond strength

    The purpose of this study was to compare fluid flow rates across dentin surfaces treated with four conditioners. The effect of conditioning on the micro-shear bond strengths of glass ionomer cement (Fuji IX GP) and resin-based adhesives (Single Bond 2 or Clearfil SE Bond) were also investigated. Under a simulated pressure of 1.3 kPa, two dentin conditioners, phosphoric acid, and a self-etching primer were applied to the dentin surfaces. Dentinal fluid flows at baseline and after conditioning were recorded for 15 min each. The conditioned surfaces were examined using a scanning electron microscope. The micro-shear bond strengths of the glass ionomer cement and of the resin-based adhesives bonded to conditioned dentin surfaces were evaluated while simulated intrapulpal pressure was maintained at 0 or 1.3 kPa. Only the dentin surface etched with phosphoric acid showed a significant increase in permeability. Micro-shear bond strengths of Fuji IX GP were not affected by conditioning the dentin surfaces or by bonding at different intrapulpal pressures (0 or 1.3 kPa). The effects on bond strengths of resin-based adhesives depended on the system used. The simulated positive intrapulpal pressure during bonding significantly affected the adherence of Single Bond 2, whereas Clearfil SE Bond was unaffected. [source]


    The Acidity of Brønsted CH Acids in DMSO , The Extreme Acidity of Nonacyanocyclononatetraene

    Abstract The gas-phase (GP) acidity of a large variety of CH organic acids is studied by a carefully selected B3LYP/6-311+G(2d,p)//B3LYP/6-31G(d) theoretical model. Acidities are mirrored by the proton affinities of the corresponding conjugate bases. A very good agreement with available experimental data is achieved. It is concluded that the model applied has a high predictive value and that theoretical PA(anion)GP values can be used instead of measured data in cases where experiments are not feasible or not performed. It is shown, employing the isodensity polarized continuum model (IPCM), that there is a good linear relationship between the enthalpies of the proton-transfer reactions in DMSO and the observed pKa(DMSO) values. This relation can be used in predicting the acidity of strong, neutral organic superacids. As an illustrative case, the acidity of cyclononatetraene-1,2,3,4,5,6,7,8,9-nonacarbonitrile ("nonacyanocyclononatetraene") is considered. It is conclusively shown that this compound should be a very potent superacid, as evidenced by its PA(anion)GP (260.0 kcal·mol,1) and pKa (,14.8).     Fabrication of Galactosylated Polyethylenimine and Plasmid DNA Multilayers on poly (D,L -lactic acid) Films for in situ Targeted Gene Transfection,

    This study presents surface-mediated targeted in situ gene delivery from gene-tagged poly(D,L -lactic acid) (PDLLA) films, which were fabricated via a layer-by-layer (LbL) assembly technique with galactosylated polyethylenimine (GP) and plasmid DNA (pDNA, pSV-,-galactosidase). A linear growth of GP/pDNA multilayered films was observed. The pDNA was continuously released from multilayered films for over 32,h. The multilayered structure degraded and simultaneously formed GP/pDNA complexes in situ when exposing to a physiological environment. The pDNA was well protected by GP against DNase I digestion within formed GP/pDNA complexes. Our results demonstrated that GP contributes to receptor-mediated targeting for cell uptake and in situ gene transfection. The results reported here are potentially important for gene therapy, surface engineering of biomaterials, tissue engineering and implant technology. [source]

ROS are considered to play an important role in the progression of allergic contact dermatitis. N -acetylcysteine (NAC) could increase glutathione levels in the skin and act as an antioxidant. Aims:, We attempted to demonstrate that NAC could inhibit chromium hypersensitivity in a coadjuvant chromium-sensitized albino guinea pig model by counteracting the formation of ROS. Methods:, We utilized a coadjuvant chromium-sensitized albino guinea pig model to evaluate both the severity of the skin reaction by intradermal and epicutaneous elicitation tests and the sensitization rate of chromium hypersensitivity in NAC-treated and NAC-untreated albino guinea pigs (GP). Furthermore, three ROS parameters, including H2O2, malondialdehyde (MDA) levels in the skin and the oxygen radical absorbance capacity (ORAC) in plasma, were analyzed in NAC-treated and NAC-untreated coadjuvant chromium-sensitized albino GP. Results:, The severity of the skin reaction in the intradermal and epicutaneous elicitation test significantly diminished when the albino GP were treated with a dose of 1200 mg/kg/day of NAC. This dose also significantly decreased the sensitization rate of chromium hypersensitivity. In addition, treatment with 1200 mg/kg/day of NAC significantly reduced the H2O2 and MDA levels in the skin and significantly increased the ORAC in the plasma of albino GP. Therefore, NAC could be a potential chemopreventative agent to prevent the progression of chromium hypersensitivity. [source]


    Effects of the G376E and G157D mutations on the stability of yeast enolase , a model for human muscle enolase deficiency

    The first known human enolase deficiency was reported in 2001 [Comi GP, Fortunato F, Lucchiari S, Bordoni A, Prelle A, Jann S, Keller A, Ciscato P, Galbiati S, Chiveri L et al. (2001) Ann Neurol50, 202,207]. The subject had inherited two mutated genes for ,-enolase. These mutations changed glycine 156 to aspartate and glycine 374 to glutamate. In order to study the effects of these changes on the structure and stability of enolase, we have introduced the corresponding changes (G157D and G376E) into yeast enolase. The two variants are correctly folded. They are less stable than wild-type enolase with respect to thermal denaturation, and both have increased Kd values for subunit dissociation. At 37 °C, in the presence of salt, both are partially dissociated and are extensively cleaved by trypsin. Under the same conditions, wild-type enolase is fully dimeric and is only slightly cleaved by trypsin. However, wild-type enolase is also extensively cleaved if it is partially dissociated. The identification of the cleavage sites and spectral studies of enolase have revealed some of the structural differences between the dimeric and monomeric forms of this enzyme. [source]


    Winter diatom blooms in a regulated river in South Korea: explanations based on evolutionary computation

    Summary 1. An ecological model was developed using genetic programming (GP) to predict the time-series dynamics of the diatom, Stephanodiscus hantzschii for the lower Nakdong River, South Korea. Eight years of weekly data showed the river to be hypertrophic (chl. a, 45.1 ± 4.19 ,g L,1, mean ± SE, n = 427), and S. hantzschii annually formed blooms during the winter to spring flow period (late November to March). 2. A simple non-linear equation was created to produce a 3-day sequential forecast of the species biovolume, by means of time series optimization genetic programming (TSOGP). Training data were used in conjunction with a GP algorithm utilizing 7 years of limnological variables (1995,2001). The model was validated by comparing its output with measurements for a specific year with severe blooms (1994). The model accurately predicted timing of the blooms although it slightly underestimated biovolume (training r2 = 0.70, test r2 = 0.78). The model consisted of the following variables: dam discharge and storage, water temperature, Secchi transparency, dissolved oxygen (DO), pH, evaporation and silica concentration. 3. The application of a five-way cross-validation test suggested that GP was capable of developing models whose input variables were similar, although the data are randomly used for training. The similarity of input variable selection was approximately 51% between the best model and the top 20 candidate models out of 150 in total (based on both Root Mean Squared Error and the determination coefficients for the test data). 4. Genetic programming was able to determine the ecological importance of different environmental variables affecting the diatoms. A series of sensitivity analyses showed that water temperature was the most sensitive parameter. In addition, the optimal equation was sensitive to DO, Secchi transparency, dam discharge and silica concentration. The analyses thus identified likely causes of the proliferation of diatoms in ,river-reservoir hybrids' (i.e. rivers which have the characteristics of a reservoir during the dry season). This result provides specific information about the bloom of S. hantzschii in river systems, as well as the applicability of inductive methods, such as evolutionary computation to river-reservoir hybrid systems.     Stroke secondary prevention and blood pressure reduction: an observational study of the use of PROGRESS therapy

    Abstract The Perindopril pROtection aGainst REcurrent Stroke Study (PROGRESS) showed the efficacy of blood pressure reduction in secondary stroke prevention. This anti-hypertensive treatment (perindopril 4 mg daily plus indapamide 1.5 mg daily) is now routinely proposed to patients referred to our department for stroke or transient ischaemic attack (TIA). The aim of this study was to evaluate the prescription of PROGRESS therapy during hospitalization and to identify the predictors of therapy discontinuation after discharge. Eligible patients admitted to the Amiens University hospital for acute stroke or TIA from January to April 2003 were included (n = 101). At 1 year, the use of PROGRESS therapy was evaluated by structured phone interviews. In addition, each patient's general practitioner (GP) was also contacted to provide information. PROGRESS therapy was mentioned on the hospital discharge summary significantly less frequently after cardioembolic stroke (OR: 0.15; 95% CI: 0.05,0.5; P = 0.001) and TIA (OR: 0.12; 95% CI: 0.02,0.7; P = 0.02). At 1 year, only 25.7% of patients were treated with optimal PROGRESS therapy (perindopril 4 mg daily plus indapamide 1.5 mg daily). Mention of PROGRESS therapy in the discharge summary was the main predictor of optimal PROGRESS therapy at 1 year (OR: 10.8; 95% CI: 1.3,88.3; P = 0.03). This study shows that mention of PROGRESS therapy in the discharge summary must be improved as it is associated with a higher use of these anti-hypertensive agents 1 year after stroke/TIA. [source]


    How appropriate is asthma therapy in general practice?

    Abstract High association between burden of asthma and inadequate disease control make asthma management a major public health issue. We studied asthma management practices of general practitioners (GPs) in France to describe drug therapy and more specifically, to identify correlates of antibiotic prescriptions, a marker of inappropriate asthma management. Patients with persistent asthma aged 17,50 years were evaluated in a 12-month retrospective study using a computerized GPs database (Thales) and a patient survey, in which patients reported hospital contacts, use of oral corticosteroids and recent asthma symptoms. Therapy was described and the correlates of antibiotic prescriptions in the previous year were identified using multivariate logistic regression. During the study period, 16.4% of 1038 patients received one or more prescriptions of theophylline, 31.3% long-acting beta-agonists and 61.6% inhaled corticosteroids. Rates of prescription of antibiotics, expectorants, antihistamines, antitussives and nasal corticosteroids were 57.6, 42.0, 33.0, 19.9, and 14.4%, respectively. In parallel, 15% of patients reported at least one hospital contact for asthma and 43.1% used oral corticosteroids. Antibiotic prescriptions were more likely co-prescribed in patients using expectorants [odds ratio (OR) = 13.0, 95% confidence interval (CI) = 8.5,19.8] and antitussives (OR = 6.5, 95% CI =3.7,11.6). Moreover, patients using courses of oral corticosteroids, and often visiting their GP (more than four times) during the study period were more likely to receive antibiotics. The results were unchanged when analyses were restricted to non-smokers and younger patients (,40 years). Asthma management was sub-optimal among asthma patients treated by general practitioners in France. Antibiotics, expectorants, antihistamines, antitussives and nasal corticosteroids were commonly prescribed while asthma controllers were under-used. [source]


    The whorl-specific action of a petunia class B floral homeotic gene

    Background GREEN PETAL (GP) is thought to be a petunia class B floral homeotic gene, because the gp mutant flower displays a severe homeotic conversion of petals into sepals in the second whorl. However, since the third whorl stamens remain unaffected in the gp null mutant, gp is different from class B mutants in Arabidopsis and Antirrhinum, which also show a conversion of the third whorl stamens into the carpelloid tissue. BLIND (BL) is thought to be a petunia class A floral homeotic gene, because the bl mutant flower displays homeotic conversions of sepals into the stigmatoid tissue in the first whorl and of the corolla limb into antheroid structures in the second whorl. Results A double mutant line homozygous for both bl and gp mutations was constructed. The bl gp double mutant flower displays homeotic conversions of sepals into the stigmatoid tissue in the first whorl and of the corolla limb into antheroid structures with stigmatoid tips in the second whorl. In the third and fourth whorls of the mutant flower, organs remained unchanged. In the gp flower, a petunia B-type gene FBP1 is expressed strongly in the third whorl organs, but much more weakly in the second whorl organs. In the bl gp flower, FBP1 was found to be expressed strongly in the second whorl organs as well as in the third whorl organs. Conclusions Petunia has a class B gene other than GP that determines organ identities, both in the second and third whorls of the double mutant flower, and the action of the postulated class B gene (here called PhBX) is prevented by the BL gene in the second whorl of the gp flower. PhBX appears to be a gene that specifically interacts with the FBP1 gene, and is involved in the up-regulation of FBP1. [source]


    Electrosprayed Enzyme Coatings as Bioinspired Alternatives to Bioceramic Coatings for Orthopedic and Oral Implants

    Abstract The biological performance of orthopedic and oral implants can be significantly improved by functionalizing the non-physiological metallic implant surface through the application of biologically active coatings. In this paper, a cost-effective alternative to traditional biomedical coatings for bone substitution through exploitation of the specific advantages of the electrospray deposition technique for the immobilization of the enzyme alkaline phosphatase (ALP) onto the implant surface is presented. Since ALP increases the local inorganic phosphate concentration required for physiological mineralization of hard tissues, ALP coatings will enable enzyme-mediated mineralization onto titanium surfaces. To evaluate the bone-bioactive capacity of the ALP-coated titanium surface, soaking experiments are performed. Although the purely inorganic so-called simulated body fluid is the standard in vitro procedure for predictive studies on potential bone bonding in vivo, an alternative testing solution is proposed that also contains organic phosphates (cell culture medium supplemented with the organic ,-b; -glycerophosphate (,-b; -GP) and serum proteins), thereby resembling the in vivo conditions more closely. Under these physiological conditions, the electrosprayed ALP coatings accelerated mineralization onto the titanium surface as compared to noncoated implant material by means of enzymatic pathways. Therefore, this novel approach toward implant fixation holds significant promise. [source]


    The effect of additives in silages of pure timothy and timothy mixed with red clover on chemical composition and in vitro rumen fermentation characteristics

    Abstract The aim was to compare the effects of additives on direct cut silages of pure timothy and timothy mixed with tetraploid red clover. First and second growth cuts were ensiled during three consecutive years, 1994, 1995 and 1996, either without any additive or with the addition of formic acid, or lactic acid bacteria in combination with molasses. Effects of the additives on the degradation characteristics of the herbage and the silages were analysed using an automatic in vitro gas production (GP) technique. At the end of the in vitro procedures, organic matter and neutral-detergent fibre (NDF) degradabilities were determined. The tetraploid red clover persisted in the leys during the 3 years and was the dominant species at the second growth in the mixed leys. The herbage from the mixed crops had lower dry-matter contents, higher crude protein concentrations and higher buffering capacity compared with the pure timothy at both cuts. In general, the additives reduced pH, and the concentrations of ammonium-N and acetic acid in the silages. The treated silages had a more rapid faster GP in both crops. The silages from the mixed crop benefited more from the additives compared with the grass silages. The additives affected the soluble fractions as well as the NDF degradability of the silages of the mixed crop more than those fractions of the grass silages. The addition of molasses in combination with a commercial inocula resulted in increased production of lactic acid and ethanol in silages from both crops. The silages without additives could not meet the requirements for good silages according to the standards of the Swedish dairy industry. [source]


    Bernard Soulier syndrome in pregnancy: a systematic review

    Summary., Bernard Soulier syndrome (BSS) is a rare disorder of platelets, inherited mainly as an autosomal recessive trait. It is characterised by qualitative and quantitative defects of the platelet membrane glycoprotein (GP) Ib-IX-V complex. The main clinical characteristics are thrombocytopenia, prolonged bleeding time and the presence of giant platelets. Data on the clinical course and outcome of pregnancy in women with Bernard Soulier syndrome is scattered in individual case reports. In this paper, we performed a systematic review of literature and identified 16 relevant articles; all case reports that included 30 pregnancies among 18 women. Primary postpartum haemorrhage was reported in 10 (33%) and secondary in 12 (40%) of pregnancies, requiring blood transfusion in 15 pregnancies. Two women had an emergency obstetric hysterectomy. Alloimmune thrombocytopenia was reported in 6 neonates, with one intrauterine death and one neonatal death. Bernard Soulier syndrome in pregnancy is associated with a high risk of serious bleeding for the mother and the neonate. A multidisciplinary team approach and individualised management plan for such women are required to minimise these risks. An international registry is recommended to obtain further knowledge in managing women with this rare disorder. [source]
