Research, policy and practice: why developing countries are different

Better utilization of research and evidence in development policy and practice can help save lives, reduce poverty and improve the quality of life. However, there is limited systematic understanding of the links between research and policy in international development. The paper reviews existing literature and proposes an analytical framework with four key arenas: external influences, political context, evidence and links. Based on the findings of stakeholder workshops in developing countries around the world, the paper identifies four key issues that characterize many developing countries. These are: (i) troubled political contexts; (ii) problems of research supply; (iii) external interference; and (iv) the emergence of civil society as a key player. Despite these challenges, two institutional models seem to be particularly effective: (i) think tanks and (ii) regional networks. Copyright © 2005 John Wiley & Sons, Ltd. [source]


A reconstructed digit by transplantation of a second toe for control of an electromechanical prosthetic hand

The treatment options for the loss of an entire human hand and part of the forearm are currently limited to the transplantation of toe(s) to the amputation stump or a Krukenberg's bifurcation hand, and using a cosmetic or functional prosthesis. The functional prosthetic hand, such as the prevailing myoelectrically controlled prosthetic hand, has an action accuracy that is affected by many factors. The acceptance rate of the three planes freedom myoelectronic hand by the patients was 46,90% because of poor function caused by the weakness of signal and strong external interference. In this report, the left second toe was transplanted to the patient's forearm amputation stump. Mandates from the brain are relayed by the action of this reconstructed digit, to control a special designed multidimension freedom electronic prosthetic hand. After rehabilitation and adaptation training, the correct recognition rate of the electronic prosthetic hand controlled by this reconstructed digit is a remarkable 100%. © 2002 Wiley-Liss, Inc. Induction Ovens and Electromagnetic Interference:

What is the Risk for Patients with Implanted Pacemakers?
Electromagnetic fields may interfere with normal pacemaker function. Despite the introduction of modern pacemakers and bipolar lead systems, electromagnetic interference (EMI) still remains to be a concern during daily lives when patients are exposed to cellular phones, electronic security systems, and several household appliances. The aim of this study was to evaluate potential EMI risk of induction ovens, which are increasingly used in private households. The study included 40 consecutive patients (22 men, 18 women; age73 ± 11 years) with implanted DDD, VVI, VDD, and AAI pacemaker systems. The pacemakers were programmed to unipolar sensing and pacing. Sensitivity remained unchanged, if the measured sensing threshold was more than twice the programmed value; otherwise, it was set at half of the measured sensing threshold. Patients were placed in a sitting position at the closest possible distance of about 20 cm between two cooking pots and pacemaker bending the upper part of the body slightly over the induction oven. The energy was increased stepwise to the maximum. One pot was removed and placed again at the highest oven level. Potential interference was monitored continuously. The study showed no incidence of pacemaker malfunction during the entire test while the patients with intrinsic cardiac rhythms were exposed to the induction oven at varying energy strengths. Likewise, there was no external interference when the patients were paced at heart rates of 10,15 beats/min above their heart rates. The programmed parameters remained unchanged after the study. Indigenous School Policies and Politics: The Sociopolitical Relationship of Wayãpi Amerindians to Brazilian and French Guianan Schooling

In this article, I aim to demonstrate the relationships between the educational policies of the Brazilian and French Guyana governments and the sociopolitical structure of the Wayãpi in respect to these educational practices. My main objective is to go beyond the normal concept that the school is an external interference that catalyzes processes of "Indigenous acculturation," to make clear that the Wayãpi sociopolitical forms of interaction that govern their relationships with alterity also govern their relationships with the state and its representatives. Early homing of adult mesenchymal stem cells in normal and infarcted isolated beating hearts

Abstract Little is known on the early homing features of transplanted mesenchymal stem cells (MSCs). We used the isolated rat heart model to study the homing of MSCs injected in the ventricular wall of a beating heart. In this model all types of cells and matrix elements with their interactions are represented, while external interferences by endothelial/neutrophil interaction and neurohormonal factors are excluded. We studied the morphology and marker expression of MSCs implanted in normal hearts and in the border-zone of infarcted myocardium. Early morphological adaptation of MSC homing differs between normal and infarcted hearts over the first 6 hrs after transplantation. In normal hearts, MSCs migrate very early through the interstitial milieu and begin to show morphological changes. Yet, in infarcted hearts MSCs remain in the site of injection forming clusters of round-shaped cells in the border-zone of the infarcted area. Both in normal and infarcted hearts, immuno-histochemistry and confocal imaging showed that, besides the proliferative marker proliferating cell nuclear agent (PCNA), some transplanted cells early express myoblastic maker GATA-4, and some of them show a VWF immunopositivity. Moreover, a few hours after injection connexin-43 is well evident between cardiomy-ocytes and injected cells. This study indicates for the first time that the isolated beating heart is a good model to study early features of MSC homing without external interferences. The results show (i) that MSCs start to change marker expression few hours after injection into a beating heart and (ii) that infarcted myocardium influences transplanted MSC morphology and mobility within the heart.
