Selected AbstractsThe Village in a Game Park: Local Response to the Demise of Coal Mining in KwaZulu-Natal, South AfricaECONOMIC GEOGRAPHY, Issue 1 2003Tony Binns Abstract: Changing economic circumstances as a result of deindustrialization and market forces dramatically affect local areas and lead to a variety of local-level responses. Economic change and the reaction to this process have received much attention in the context of the decline of old heavy industrial regions in Western Europe and North America. But deindustrialization is also occurring elsewhere, for example, in countries such as South Africa, where the decline of mining and related industries is having a severe impact on the livelihoods of individuals, households, and communities. Considerations of institutional thickness, resourcefulness, and capacities inherent within host communities contribute to an understanding of the likely potential of the development response undertaken. This article considers the situation in a once important coal-mining region in northern KwaZulu-Natal, South Africa, focusing particularly on the community of Utrecht. In the postapartheid period, unemployment in the area has escalated, at a time when greater empowerment of the historically disadvantaged black population is urgently needed. Through cooperation within the community, together with the injection of external funding and collaboration in a series of joint ventures, Utrecht is progressively rebranding itself as a center for tourism. A number of community-initiated projects are discussed, and the dynamics of the formulation and implementation of the projects are evaluated in the context of the capabilities of individuals and institutions. [source] A sustainability assessment of a health equity fund initiative in CambodiaINTERNATIONAL JOURNAL OF HEALTH PLANNING AND MANAGEMENT, Issue 3 2007Bart Jacobs Abstract All but one of the health equity funds (HEFs) currently operating in Cambodia, introduced to address the adverse effects of low user fee exemption rates, rely heavily on external funding and have high administrative overheads. This article reports on a study of one type of HEF, based in Kirivong Operational Health District (KOD) and operated through local pagoda structures, which demonstrates minimal reliance on external funding and low administrative overheads. We utilize an adapted sustainability assessment framework to assess the ability of pagoda structures to enable financial access for the poorest to public sector health services. We further analyse the strengths and limitations of the pagoda-managed equity fund initiative, with a view to assessing not only its sustainability but its potential for replication in other settings. Our study shows that, against key sustainability indicators (health service utilization and health outcomes; management capacity and financial viability; community mobilization and government support), the pagoda-managed equity fund initiative scores well. However, it is evident that some external financial support is needed to allow the HEFs to function effectively. We conclude with recommendations for replicating the initiative, which include working innovatively with indigenous grassroots organizations to enhance community HEF ownership and to keep administrative overheads low. Hawley In order to assess trends in family therapy research, empirical articles (N = 195) from three family therapy journals over a 5-year period were coded for several variables: authorship, external funding, methodology, sample, purpose, cost effectiveness, use of therapeutic model, and topic. Results indicated that a large pecentage of research in these journals focused on nonclinical issues and used nonclinical samples. Authors were affiliated with a wide variety of disciplines and reported low levels of external funding for their research. While a majority of the studies used quantitative methods, there appeared to be a growing number of studies using qualitative methods. Implications of these findings are discussed in light of research reviews over the past two decades. [source] Titrated low-dose vaginal and/or oral misoprostol to induce labour for prelabour membrane rupture: a randomised trialBJOG : AN INTERNATIONAL JOURNAL OF OBSTETRICS & GYNAECOLOGY, Issue 12 2008L Bricker Objective, To evaluate the clinical effectiveness and safety of titrated low-dose misoprostol for induction of labour (IOL) in the presence of prelabour rupture of membranes (PROM). Design, Randomised controlled trial. Setting, Maternity units in the UK (9) and Egypt (1). Population, Women >34 weeks of gestation with PROM, singleton viable fetus and no previous caesarean section. Methods, Subjects randomised to IOL with a titrated low-dose misoprostol regimen (oral except if unfavourable cervix, where initial dose vaginal) or a standard induction method, namely vaginal dinoprostone followed by intravenous oxytocin if the cervix was unfavourable or intravenous oxytocin alone if the cervix was favourable. Main outcome measures, Primary outcome measures were caesarean section and failure to achieve vaginal delivery within 24 hours. Analysis was by intention to treat. Results, The trial did not achieve the planned sample size of 1890 due to failure in obtaining external funding. Seven hundred and fifty-eight women were randomised (375 misoprostol and 383 standard). There were less caesarean section (14 versus 18%, relative risk [RR] 0.79; 95% CI 0.57,1.09) and less women who failed to achieve vaginal delivery within 24 hours in the misoprostol group (24 versus 31%, RR 0.79; 95% CI 0.63,1.00), but the differences were not statistically significant. Subgroup analysis showed that with unfavourable cervix, misoprostol may be more effective than vaginal dinoprostone. There was no difference in hyperstimulation syndrome. There were more maternal adverse effects with misoprostol, but no significant differences in maternal and neonatal complications. Conclusions, Titrated low-dose misoprostol may be a reasonable alternative for IOL in the presence of PROM, particularly in women with an unfavourable cervix. Safety and rare serious adverse events could not be evaluated in a trial of this size. Kukucha At the domestic level, the BC government maintains significant control over softwood lumber as a result of provincial land ownership, executive dominance, and the role of the Ministry of Forests. This is reinforced by a close relationship between the bureaucracy and key industry associations. Under the New Democratic Party government of the 1990s, however, the policy relevance of environment, labour, and aboriginal groups increased. The election of the BC Liberals, however, diminished these interests, with the possible exception of First Nations groups. It is also important, however, to acknowledge the significance of international developments. Institutionally, these pressures contributed to bureaucratic restructuring and a changing "culture" within the Ministry of Forests. Industry relations have also been influenced by consolidation of ownership, bilateral lumber disputes with the United States, and the increasing role of non-elected legal representatives. In addition, environment and labour interests were empowered by external funding, high export demands, and a low Canadian dollar. Although these developments have created an increasingly complex policy process, it is apparent that domestic considerations remain dominant, especially traditional participants, such as the Ministry of Forests and other specific setoral interests.
