Extensive Report (extensive + report)
Selected AbstractsThirty years on from the paper ,Gust Spectrum Fatigue Crack Propagation in Candidate Skin Materials', Fatigue of Engineering Materials and Structures, Vol.FATIGUE & FRACTURE OF ENGINEERING MATERIALS AND STRUCTURES, Issue 1 2009IMPACT OF THE PAPER This paper1 addressed a very specific topic and by itself would not have had much impact in the intervening years. However, most of the paper's content was subsequently included in an extensive report on the Damage Tolerance (DT) properties of aluminium alloys.2 This report enabled guidelines for flight simulation fatigue crack growth testing to be formulated.3,4 [source] A shift to ambulatory medical education in IsraelTHE CLINICAL TEACHER, Issue 2 2010Khaled Karkabi Summary Background:, The Council for Higher Education in Israel published an extensive report in 2007, calling for a significant increase of undergraduate medical education in Israel in ambulatory care settings. The objective of this article is to propose an action plan aimed at shifting undergraduate medical education in Israel towards ambulatory education. Context:, The main barriers to increasing ambulatory education in Israel are lack of academic recognition for teaching and excellence, conflict between patient care, income and teaching, lack of an adequate educational infrastructure and faculty in ambulatory care, and insufficient support and involvement of the health organisations. However, there is great potential for developing ambulatory education in Israel based on existing resources: Israel has a well-established primary care network, has chronic disease management programmes, community-based preventive medicine and health promotion activities, and an emerging structure for home, palliative and terminal care in the community. Innovation:, The proposed action plan presents a framework for enhancing ambulatory education in undergraduate medical education in Israel, and allows site-specific adjustments according to the preferences, resources and capabilities of each of the four medical schools. Implications:, A national shift to ambulatory education in Israel can be implemented through the existing coordination mechanism of the four medical schools. A government funding policy that encourages the collaboration between four medical schools will be beneficial both in terms of resource utilisation and the engagement of other stakeholders. The recognition of community services and educational excellence can be advanced by establishing academies of teaching scholars. Hakim Abstract Aim:, In contrast to extensive reports on the roles of Nav1.5 , -subunits, there have been few studies associating the , -subunits with cardiac arrhythmogenesis. We investigated the sino-atrial and conduction properties in the hearts of Scn3b,/, mice. Methods:, The following properties were compared in the hearts of wild-type (WT) and Scn3b,/, mice: (1) mRNA expression levels of Scn3b, Scn1b and Scn5a in atrial tissue. (2) Expression of the ,3 protein in isolated cardiac myocytes. (3) Electrocardiographic recordings in intact anaesthetized preparations. (4) Bipolar electrogram recordings from the atria of spontaneously beating and electrically stimulated Langendorff-perfused hearts. Results:,Scn3b mRNA was expressed in the atria of WT but not Scn3b,/, hearts. This was in contrast to similar expression levels of Scn1b and Scn5a mRNA. Immunofluorescence experiments confirmed that the ,3 protein was expressed in WT and absent in Scn3b,/, cardiac myocytes. Lead I electrocardiograms from Scn3b,/, mice showed slower heart rates, longer P wave durations and prolonged PR intervals than WT hearts. Spontaneously beating Langendorff-perfused Scn3b,/, hearts demonstrated both abnormal atrial electrophysiological properties and evidence of partial or complete dissociation of atrial and ventricular activity. Atrial burst pacing protocols induced atrial tachycardia and fibrillation in all Scn3b,/, but hardly any WT hearts. Scn3b,/, hearts also demonstrated significantly longer sinus node recovery times than WT hearts. Conclusion:, These findings demonstrate, for the first time, that a deficiency in Scn3b results in significant atrial electrophysiological and intracardiac conduction abnormalities, complementing the changes in ventricular electrophysiology reported on an earlier occasion. Baschetti Abstract Endocrinologists were not included in the multidisciplinary working groups that prepared two recent reports on chronic fatigue syndrome, despite its unequalled clinical overlap with Addison's disease, which is a classic endocrine disorder. The failure to include at least one endocrinologist in those panels may explain why in their extensive reports there is not a single word about the 42 clinical features that chronic fatigue syndrome shares with Addison's disease, including all the signs and symptoms listed in the case definition of this syndrome.
