    Mapping Novel Nouns and Verbs Onto Dynamic Action Events: Are Verb Meanings Easier to Learn Than Noun Meanings for Japanese Children?

    The present research examined how 3- and 5-year-old Japanese children map novel nouns and verbs onto dynamic action events and generalize them to new instances. Studies 1 to 3 demonstrated that although both 3- and 5-year-olds were able to map novel nouns onto novel objects, only 5-year-olds could generalize verbs solely on the basis of the sameness of the action. Study 4 showed that the difficulty young children experience in learning verbs lies mainly in mapping the appropriate element to a verb rather than in encoding and remembering an action itself. The results of this research are related to a long-debated issue of whether noun learning is privileged over verb learning. [source]


    On Resolving Conflicts Between Arguments

    Argument systems are based on the idea that one can construct arguments for propositions,structured reasons justifying the belief in a proposition. Using defeasible rules, arguments need not be valid in all circumstances, therefore, it might be possible to construct an argument for a proposition as well as its negation. When arguments support conflicting propositions, one of the arguments must be defeated, which raises the question of which (sub-) arguments can be subject to defeat. In legal argumentation, metarules determine the valid arguments by considering the last defeasible rule of each argument involved in a conflict. Since it is easier to evaluate arguments using their last rules, can a conflict be resolved by considering only the last defeasible rules of the arguments involved? We propose a new argument system where, instead of deriving a defeat relation between arguments, arguments for the defeat of defeasible rules are constructed. This system allows us to determine a set of valid (undefeated) arguments in linear time using an algorithm based on a JTMS, allows conflicts to be resolved using only the last rules of the arguments, allows us to establish a relation with Default Logic, and allows closure properties such as cumulativity to be proved. We propose an extension of the argument system based on a proposal for reasoning by cases in default logic. [source]

In this paper, we provide a survey of different mobile and wireless technologies and how they have impact AR.     A pedagogical Web service-based interactive learning environment for a digital filter design course: An evolutionary approach

    Abstract The course of digital filter design in electronic/electrical engineering involves complicated mathematical equations and dynamic waveform variations. It is a consensus among educators that using simulation tools assist in improving students' learning experiences. Previous studies on system simulation seemed to lack an appropriate approach to design such a course. Few emphasized the design of an interactive learning environment by using an evolutionary approach. This study integrated the design concept of an evolutionary approach and Web service-based technology into a simulation system entitled Pedagogical Web Service-Based Interactive Learning Environment (PEWSILE) was introduced. The PEWSILE system contained two interactive learning environments,a simple system and an advanced system. It offered a total of six pedagogical Web services. The simple interactive learning environment included text/color-based services, and text/color/diagram-based services. The advanced interactive learning environment included batch-based, interval change-based, comparison-based, and scroll bar-based services. The study also assessed the students' performance in six pedagogical Web services covering interaction and overall use, usefulness, and intention to use through a questionnaire survey and subsequent interviews. Three significant findings were reported. For example, in the advanced interactive learning environment, the designs of interval change-based and comparison-based services make it easier to observe differences in the outcome of parameter change, while batch-based services lacks the element of waveform comparison. In sum, the findings in this study provide helpful implications in designing engineering educational software. © 2010 Wiley Periodicals, Inc.     The solution of fluid mechanics problems by probability methods

    Abstract This study shows how fluid mechanics problems may be solved using probability methods. Problems solved include velocity distributions in steady and unsteady flow between parallel plates as well as steady flow through a constant diameter three-dimensional duct. We find that the results approach exact solutions as the number of random walks increases, and that the complexity of the flow equations precludes extension of the technique to three dimensions.     Adding Depth to Cartoons Using Sparse Depth (In)equalities

    Abstract This paper presents a novel interactive approach for adding depth information into hand-drawn cartoon images and animations. In comparison to previous depth assignment techniques our solution requires minimal user effort and enables creation of consistent pop-ups in a matter of seconds. Inspired by perceptual studies we formulate a custom tailored optimization framework that tries to mimic the way that a human reconstructs depth information from a single image. Its key advantage is that it completely avoids inputs requiring knowledge of absolute depth and instead uses a set of sparse depth (in)equalities that are much easier to specify. Since these constraints lead to a solution based on quadratic programming that is time consuming to evaluate we propose a simple approximative algorithm yielding similar results with much lower computational overhead. We demonstrate its usefulness in the context of a cartoon animation production pipeline including applications such as enhancement, registration, composition, 3D modelling and stereoscopic display.     CCLRC Portal infrastructure to support research facilities

    Abstract The emergence of portal technology is providing benefits in developing portlet interfaces to applications to meet the current and future requirements of CCLRC facilities support. Portlets can be reused by different projects, e.g. the high-profile Integrative Biology project (with the University of Oxford), and in different Java Specification Request 168 Portlet Specification (JSR 168) compliant portal frameworks. Deployment and maintenance of applications developed as portlets becomes easier and manageable. A community process is already beginning and many portal frameworks come with free-to-use useful portlets. As rendering is carried out in the framework, applications can be easily accessible and internationalized. Portlets are compatible with J2EE, thus providing additional capabilities required in the service-oriented architecture (SOA). We also describe how Web service gateways can be used to provide many of the functionalities encapsulated in a portal server in a way to support Grid applications. Portals used as a rich client can allow users to customize or personalize their user interfaces and even their workflow and application access. CCLRC facilities will be able to leverage the work so far carried out on the National Grid Service (NGS) and e-HTPX portals, as they are fully functional and have received detailed user feedback. This demonstrates the usefulness of providing advanced capabilities for e-Research and having the associated business logic in a SOA loosely coupled from the presentation layer for an Integrated e-Science Environment.     The cause and prevention of human birth defects: What have we learned in the past 50 years?

    ABSTRACT This review article dealig with the subject of "The Cause and Prevention of Human Birth Defects" was prepared in celebration of the 40th anniversary of the Japanese Teratology Society. It begins with recollections of some of the important contributions of Japanese scientists in the fields of teratology and embryology and a summary of the many scientific and medical accomplishments of the past 50 years in the fields of teratology, genetics, developmental biology, epidemiology and genetics. The review includes a summary of the drugs, chemicals and physical agents that have been documented to result in congenital malformations and reproductive effects when pregnant women are exposed during pregnancy. The principles of teratology were also summarized and emphasize that 1) no teratogenic agent can be described qualitatively as a teratogen, since a teratogenic exposure must include not only the agent, but also the dose and the time in pregnancy when the exposure occurs. 2) Even agents that have been demonstrated to result in malformatins cannot produce every type of malformation. 3) Known teratogens can be presumptively identified by the spectrum of malformations they produce. 4) It is easier to exclude an agent as a cause of birth defects than to definitively conclude that it was responsible for birth defects. 5) When evaluating the risk of exposures, the dose is a crucial component in determining the risk. 6) Teratogenic agents follow a toxicological dose response curve. This means that each teratogen has a threshold dose, below which, there is no risk of teratogenensis, no matter when in pregnancy the exposure occurred. 7) The evaluation of a child with congenital malformations connot be adequately performed unless it is approached with the same scholarship and detail, as is any other complicated medical problem. 8) Each physician must recognize the consequences of providing erroneous reproductive risks to pregnant women exposed to drugs and chemicals during pregnancy or alleging that a child's malformations are due to an environmental agent without performing a complete and scholarly evaluation.     ESTIMATING INTERVENTION EFFECTS IN VARYING RISK SETTINGS: DO POLICE RAIDS REDUCE ILLEGAL DRUG DEALING AT NUISANCE BARS?,

    This paper investigates the effectiveness of police raids in reducing drug dealing in and around nuisance bars. We examine effects of both dosage (number of raids) and duration (months) of the intervention, as well as the conditioning effects of land use and population characteristics in shaping the underlying risk levels of drug dealing in the target and surrounding areas. Results indicate that the police intervention suppresses levels of drug dealing during periods of active enforcement, but the effects largely disappear when the intervention is withdrawn. Also, the effects of the intervention are mediated by risk characteristics in target and surrounding areas. Target areas characterized by higher levels of risk are more resistant to intervention effects than those with lower levels of risk. Risk factors in nearby areas are also significant. Bars with high levels of risk arising from land uses in surrounding areas are easier to treat, while bars with high levels of population-based risk in surrounding areas are harder to treat.     Hand augmentation with Radiesse® (Calcium hydroxylapatite)

    ABSTRACT:, The hand has remained a considerable treatment challenge, as new soft tissue fillers have arrived in the esthetic marketplace. The challenge has been the result of both the multiple visits required for treatment in, for example, autologous fat grafting and the simple management of pain in the innervated areas of the hand between the bones. This paper introduces a novel, noticeably less painful approach to treatment of the hand with calcium hydroxylapatite (CaHA; Radiesse®, BioForm Medical, San Mateo, CA). Anesthetic is added to the compound prior to injection, resulting in a homogenous admixture of CaHA and anesthetic. A bolus of the mixture is injected into the skin, using tenting, and then spread throughout the hand. The result of this approach , mixing anesthetic with CaHA , is treatment that is easier to massage and disseminate, less painful to the patient than conventional hand injection, and characterized by less swelling and bruising, with minimal post-treatment downtime.     An Appeal to Aid Specialists

    The appeal I am making is (roughly speaking) for aid specialists to do more to help those of us who are not aid specialists to arrive at judgments about the effects of the work of (voluntary) aid agencies that we have at least some reason to think accurate. I argue that the fact that it is so difficult for us to arrive at such judgments at present has certain negative consequences, and that this gives those who are in a position to make it easier for us to arrive at such judgments strong reasons to do so. And I argue that (certain) aid specialists are in such a position. Hence my appeal to them to do so. [source]


    Rescuer Fatigue: Standard versus Continuous Chest-Compression Cardiopulmonary Resuscitation

    Abstract Objectives Continuous chest-compression cardiopulmonary resuscitation (CCC-CPR) has been advocated as an alternative to standard CPR (STD-CPR). Studies have shown that CCC-CPR delivers substantially more chest compressions per minute and is easier to remember and perform than STD-CPR. One concern regarding CCC-CPR is that the rescuer may fatigue and be unable to maintain adequate compression rate or depth throughout an average emergency medical services response time. The specific aim of this study was to compare the effects of fatigue on the performance of CCC-CPR and STD-CPR on a manikin model. Methods This was a prospective, randomized crossover study involving 53 medical students performing CCC-CPR and STD-CPR on a manikin model. Students were randomized to their initial CPR group and then performed the other type of CPR after a period of at least two days. Students were evaluated on their performance of 9 minutes of CPR for each method. The primary endpoint was the number of adequate chest compressions (at least 38 mm of compression depth) delivered per minute during each of the 9 minutes. The secondary endpoints were total compressions, compression rate, and the number of breaks taken for rest. The students' performance was evaluated on the basis of Skillreporter Resusci Anne (Laerdal, Wappingers Falls, NY) recordings. Primary and secondary endpoints were analyzed by using the generalized linear mixed model for counting data. Results In the first 2 minutes, participants delivered significantly more adequate compressions per minute with CCC-CPR than STD-CPR, (47 vs. 32, p = 0.004 in the 1st minute and 39 vs. 29, p = 0.04 in the 2nd minute). For minutes 3 through 9, the differences in number of adequate compressions between groups were not significant. Evaluating the 9 minutes of CPR as a whole, there were significantly more adequate compressions in CCC-CPR vs. STD-CPR (p = 0.0003). Although the number of adequate compressions per minute declined over time in both groups, the rate of decline was significantly greater in CCC-CPR compared with STD-CPR (p = 0.0003). The mean number of total compressions delivered in the first minute was significantly greater with CCC-CPR than STD-CPR (105 per minute vs. 58 per minute, p < 0.001) and did not change over 9 minutes in either group. There were no differences in compression rates or number of breaks between groups. Conclusions CCC-CPR resulted in more adequate compressions per minute than STD-CPR for the first 2 minutes of CPR. However, the difference diminished after 3 minutes, presumably as a result of greater rescuer fatigue with CCC-CPR. Overall, CCC-CPR resulted in more total compressions per minute than STD-CPR during the entire 9 minutes of resuscitation. [source]

    Abstract Recent scientific discoveries have made it much easier to test prenatally for various genetic disabilities, such as Down syndrome. However, while many observers have heralded such "advances" for their effectiveness in detecting certain conditions, others have argued that they perpetuate discrimination by preventing the birth of children with disabilities. This article examines the ethical and social implications of the Human Genome Project for individuals with intellectual disabilities and their families. It details the critique of prenatal testing articulated by many disability rights activists as well as scholarly and professional responses to that critique. A review of the pertinent research literature includes perspectives of genetic professionals, ethicists, disability studies scholars, parents of children with disabilities, and disabled individuals themselves.     Assessment of gestational age and neuromaturation

    Abstract Neuromaturation is the functional development of the central nervous system (CNS). It is by its very nature a dynamic process, a continuous interaction between the genome and first the intrauterine environment, then the extrauterine environment. Understanding neuromaturation and being able to measure it is fundamental to infant neurodevelopmental assessment. Fetal and preterm neuromaturation has become easier to observe with the advent of prenatal ultrasonography and neonatal intensive care units. A number of measures of degree of fetal maturation have been developed and used to estimate gestational age (GA) at birth. The most reliable measures of GA are prenatal measures, especially from the first trimester. Postnatal GA measurements tend to be least accurate at the extremes of gestation, that is, in extremely preterm and post-term infants. Observations of measures of neuromaturation in infants born to mothers with pregnancy complications, including intrauterine growth restriction, multiple gestation, and chronic hypertension, have led to the discovery that stressed pregnancies may accelerate fetal pulmonary and CNS maturation. This acceleration of neuromaturation does not occur before 30 weeks' gestation and has a cost with respect to cognitive limitations manifested in childhood. The ability to measure fetal and preterm neuromaturation provides an assessment of neurodevelopmental progress that can be used to reassure parents or identify at risk infants who would benefit from limited comprehensive follow-up and early intervention services. In addition, measures of neuromaturation have the potential to provide insight into mechanisms of CNS injury and recovery, much-needed early feedback in intervention or treatment trials and a measure of early CNS function for research into the relationships between CNS structure and function. © 2005 Wiley-Liss, Inc.     Models of white matter injury: Comparison of infectious, hypoxic-ischemic, and excitotoxic insults

    Abstract White matter damage (WMD) in preterm neonates is strongly associated with adverse outcome. The etiology of white matter injury is not known but clinical data suggest that ischemia-reperfusion and/or infection-inflammation are important factors. Furthermore, antenatal infection seems to be an important risk factor for brain injury in term infants. In order to explore the pathophysiological mechanisms of WMD and to better understand how infectious agents may affect the vulnerability of the immature brain to injury, numerous novel animal models have been developed over the past decade. WMD can be induced by antenatal or postnatal administration of microbes (E. coli or Gardnerella vaginalis), virus (border disease virus) or bacterial products (lipopolysaccharide, LPS). Alternatively, various hypoperfusion paradigms or administration of excitatory amino acid receptor agonists (excitotoxicity models) can be used. Irrespective of which insult is utilized, the maturational age of the CNS and choice of species seem critical. Generally, lesions with similarity to human WMD, with respect to distribution and morphological characteristics, are easier to induce in gyrencephalic species (rabbits, dogs, cats and sheep) than in rodents. Recently, however, models have been developed in rats (PND 1,7), using either bilateral carotid occlusion or combined hypoxia-ischemia, that produce predominantly white matter lesions. LPS is the infectious agent most often used to produce WMD in immature dogs, cats, or fetal sheep. The mechanism whereby LPS induces brain injury is not completely understood but involves activation of toll-like receptor 4 on immune cells with initiation of a generalized inflammatory response resulting in systemic hypoglycemia, perturbation of coagulation, cerebral hypoperfusion, and activation of inflammatory cells in the CNS. LPS and umbilical cord occlusion both produce WMD with quite similar distribution in 65% gestational sheep. The morphological appearance is different, however, with a more pronounced infiltration of inflammatory cells into the brain and focal microglia/macrophage ("inflammatory WMD") in response to LPS compared to hypoperfusion evoking a more diffuse microglial response usually devoid of cellular infiltrates ("ischemic WMD"). Furthermore, low doses of LPS that by themselves have no adverse effects in 7-day-old rats (maturation corresponding to the near term human fetus), dramatically increase brain injury to a subsequent hypoxic-ischemic challenge, implicating that bacterial products can sensitize the immature CNS. Contrary to this finding, other bacterial agents like lipoteichoic acid were recently shown to induce tolerance of the immature brain suggesting that the innate immune system may respond differently to various ligands, which needs to be further explored.     The first steps in word learning are easier when the shoes fit: comparing monolingual and bilingual infants

    English, French, and bilingual English-French 17-month-old infants were compared for their performance on a word learning task using the Switch task. Object names presented a /b/ vs. /g/ contrast that is phonemic in both English and French, and auditory strings comprised English and French pronunciations by an adult bilingual. Infants were habituated to two novel objects labeled ,bowce' or ,gowce' and were then presented with a switch trial where a familiar word and familiar object were paired in a novel combination, and a same trial with a familiar word,object pairing. Bilingual infants looked significantly longer to switch vs. same trials, but English and French monolinguals did not, suggesting that bilingual infants can learn word,object associations when the phonetic conditions favor their input. Monolingual infants likely failed because the bilingual mode of presentation increased phonetic variability and did not match their real-world input. Experiment 2 tested this hypothesis by presenting monolingual infants with nonce word tokens restricted to native language pronunciations. Monolinguals succeeded in this case. Experiment 3 revealed that the presence of unfamiliar pronunciations in Experiment 2, rather than a reduction in overall phonetic variability was the key factor to success, as French infants failed when tested with English pronunciations of the nonce words. Thus phonetic variability impacts how infants perform in the switch task in ways that contribute to differences in monolingual and bilingual performance. Moreover, both monolinguals and bilinguals are developing adaptive speech processing skills that are specific to the language(s) they are learning. [source]


    Insulin mimetic effects of macrocyclic binuclear oxovanadium complexes on streptozotocin-induced experimental diabetes in rats

    Aim:, The vanadium complexes so far tested for their insulin mimetic effects are either mono- or binuclear and contain only acyclic ligands. The leaching or hydrolysis of vanadyl ions from these complexes is much easier, and hence they elicit side effects. In the present study, a new binuclear macrocyclic oxovanadium complex was synthesized, and its efficacy was studied on streptozotocin (STZ)-induced diabetic rats over a period of 30 days. Methods:, The insulin mimetic effect of the complex was tested on the blood sugar level in the STZ-diabetic rats and on the activities of the carbohydrate-metabolizing enzymes present in the liver. Results:, Administration of vanadium complex to STZ-induced diabetic rats decreased blood glucose levels from hyperglycaemic to normoglycaemic when compared to diabetic rats. The activity of carbohydrate-metabolizing enzymes such as hexokinase, glucose-6-phosphate dehydrogenase, glycogen synthase and glycogen content were increased to near normal in vanadium complex-administered diabetic rats. The biochemical studies such as assay of blood urea and glutamate oxaloacetate transaminases revealed that the complex is not toxic to the system. Conclusion:, The nontoxic nature of this complex may be due to the presence of the vanadyl ions in an intact macrocyclic form. Further, the vanadyl ions present in the macrocyclic binuclear oxovanadium complex are very close to each other, and this may enhance the insulin mimetic activity by synergic effect. [source]


    Impact of therapeutic advances on hypoglycaemia in type 2 diabetes

    Abstract Patients with type 2 diabetes experience hypoglycaemia less frequently than those with type 1 diabetes. Some protection against hypoglycaemia is afforded by the relatively intact glucose counter-regulatory pathways that characterize the pathophysiology of early type 2 diabetes. To some extent, this protection explains why hypoglycaemic episodes in intensively treated individuals with type 2 diabetes, when they occur, are rarely severe. As diabetes progresses and therapy intensifies to achieve recommended glycaemic goals, hypoglycaemia frequency and severity increase. Thus, when it comes to instituting intensive therapy, fear of hypoglycaemia may contribute to health-care providers' ,clinical inertia'. Because maintaining glycaemic control is so important to both public and individual health, many new therapies and technologies have been developed. This manuscript reviews and considers whether these advancements in therapy make glycaemic goals easier to achieve by minimizing hypoglycaemia. Putting the hypoglycaemia experienced by type 2 diabetes patients into appropriate clinical perspective, the impact of recent progress made in pharmacotherapy, drug delivery systems, and BG monitoring on hypoglycaemia incidence is largely positive. The extent to which this progress can effect improvement over traditional therapies will, however, depend upon patient (and provider) education, motivation and behaviour change.     Insulin glargine and receptor-mediated signalling: clinical implications in treating type 2 diabetes

    Abstract Most patients with type 2 diabetes mellitus will eventually require insulin therapy to achieve or maintain adequate glycaemic control. The introduction of insulin analogues, with pharmacokinetics that more closely mimic endogenous insulin secretion, has made physiologic insulin replacement easier to achieve for many patients. However, there are also concerns regarding alteration of binding affinities for the insulin receptor (IR) or insulin-like growth factor-1 receptor (IGF-1R) may increase the mitogenic potential of some analogues. Therefore, this article will review the relevant preclinical and clinical data to assess the mitogenic potential of insulin glargine, a basal insulin analogue, compared with regular human insulin (RHI). Searches of the PubMed database were performed using terms that included ,IR,' ,insulin-like growth factor-1,' ,IGF-1R,' ,type 2 diabetes mellitus,' and ,insulin glargine.' Original articles and reviews of published literature were retrieved and reviewed. Although one study reported increased binding affinity of insulin glargine for the IGF-1R and increased mitogenic potential in cells with excess IGF-1Rs (Saos/B10 osteosarcoma cells), most in vitro binding-affinity and cell-culture studies have demonstrated behaviour of insulin glargine comparable to that of RHI for both IR and IGF-1R binding, insulin signalling, and metabolic and mitogenic potential. Currently published in vivo carcinogenic studies and human clinical trial data have shown that insulin glargine is not associated with increased risk for either cancer or the development or progression of diabetic retinopathy.     A public-health perspective on violence

    Objective:, To describe how specific theories and methods used in public health, especially regarding injuries, are related to violence. Method:, Theories and preventive work in injury research (above accidents) are presented and related to violence. Results: Registration of injuries and an interest in the environment are seen as specific for injuries. In prevention there is a focus on community work and the concept of ,a safe community' has been developed. Haddon's matrix offers a foundation for theoretical injury research. It is formed by cross-tabulating the trichotomy of host-agent-environment against a time dimension. Conclusion:, One practical and one theoretical model on how violence might be seen as an injury have been demonstrated. No clear evaluation has been made of the practical model up to now. The theoretical model has the advantage that the model makes it easier to get a more comprehensive picture of how different factors influence violence. [source]


    Nidotherapy: a new approach to the treatment of personality disorder

    Objective:,To develop a treatment, nidotherapy, or nest therapy, so named because it aims to alter the sufferer's personal environment rather than symptoms or behaviour, in the management of personality disorders. Method:,Case studies, in which analysis of the environmental circumstances associated with the problems of personality disorder is followed by planned adjustment to that environment so that it makes a more appropriate fit for the personality. Results:,Sustained improvement was found in two individuals with personality disorder after nidotherapy. Those with persistent and predictable personality attributes are easier in principle to treat than those with episodic or variable problems. Conclusion:,Nidotherapy deserves further consideration in the management of personality disorders. [source]


    Hodgkin's lymphoma: Diagnostic difficulties in fine-needle aspiration cytology

    Abstract It is commonly believed that cytodiagnosis of Hodgkin's lymphoma (HL) is much easier than that of non-Hodgkin lymphoma (NHL). However, recognition of certain NHL subtypes with Reed-Sternberg (R-S)-like cells and results of immunohistochemical studies point to the contrary. To study the limitations of cytology in diagnosis of HL, fine-needle aspiration (FNA) smears of 130 lymphoma or suspected lymphoma cases were reviewed. Initial and reviewed cytodiagnoses were compared with histopathology in 89 cases. Immunocytochemical and immunohistochemical studies were performed in 56 and 59 cases, respectively. Among histologically diagnosed HL cases, definitive cytodiagnosis of HL (initial as well as reviewed) was significantly less frequent than cytodiagnosis of NHL among histologically diagnosed NHL cases (P = 0.0328 and = 0.0001, respectively). On the other hand, cytologically diagnosed HL/NHL cases were significantly more frequent in the former group (P = 0.0001 and = 0.0018, respectively). ALCL and TCRBCL were the two NHL subtypes which created confusion with HL in FNA smears. Twenty-one cytohistological concordant HL cases and equal number of discordant cases were compared. When compared with discordant group, the patients in concordant group were significantly younger (P = 0.045). Hodgkin/Hodgkin-like cells and typical R-S cells were significantly more frequent in FNA smears of the concordant group (P = 0.0478 and = 0.0431, respectively). Immunocytochemical and immunohistochemical studies showed good correlation with histological diagnosis of HL. It is suggested that proper interpretation of cytologic features, together with use of immunocytochemical parameters can help in reducing the margin of error in cytodiagnois of HL. Diagn.     ENDOSCOPIC SUBMUCOSAL DISSECTION IN THE UPPER GASTROINTESTINAL TRACT: PRESENT AND FUTURE VIEW OF EUROPE

    In Western countries endoscopic mucosal resection (EMR) has been widely accepted for treatment of early Barrett`s neoplasia and flat or depressed colorectal adenomas. In contrast endoscopic submucosal dissection (ESD) is infrequently performed for several reasons. It seems to be difficult to overcome the learning curve of this difficult technique because of the low case volume of early gastric cancer. On the other hand ESD of esophageal or colorectal lesions is even more challenging and is considered to be inappropriate for learning. In addition the indication for esophageal or colorectal ESD is controversial in view of excellent results of the well established EMR technique which is less time-consuming and safer than ESD. A recent survey of leading Western endoscopy centers indicated the limited experience with ESD with a low number of cases for all potential indications. Only a few training courses have been established and the number of ongoing clinical studies is limited. Only 12 out of 340 published articles on "endoscopic mucosal dissection" were reported from Western countries. A better acceptance of ESD requires improvement of the technique to allow an easier, faster and safer approach. There is a strong demand for structured training courses and limitations of human cases to selected centers which participate in prospective trials. A close collaboration between Western and Asian centers is recommended for improvement of the ESD technique and its clinical application. [source]


    ENDOSCOPIC TRANSPAPILLARY CATHETERIZATION INTO THE GALLBLADDER FOR DIAGNOSIS OF GALLBLADDER CARCINOMA

    It is often difficult to determine the precise nature of lesions in the gallbladder by radiographic, endoscopic and ultrasonographic methods. The approach to the gallbladder by a percutaneous transhepatic route has been reported. However, there is a possibility of seeding tumor cells into the peritoneal cavity and liver in a percutaneous procedure. On the contrary, transpapillary route can be performed without a possibility of seeding. The double-contrast cholecystography, intragallbladder sonography, direct biopsy of gallbladder lesions and cytology using gallbladder bile have been performed by the procedure of the transpapillary catheterization into the gallbladder. Confirming malignancy by histopathological diagnosis is desirous for determining therapeutic strategy in gallbladder carcinoma. Gathering gallbladder bile is comparatively easier than biopsy of the lesion using the transpapillary catheterization into the gallbladder. Examination of telomerase-related molecules is useful for diagnosis of pancreatic carcinoma. Usefulness of combination assay of human telomerase reverse transcriptase mRNA (hTERT mRNA) and cytology using gallbladder bile obtained by transpapillary catheterization is reported here. However, it would appear that hTERT mRNA is less important in the diagnosis of gallbladder carcinoma than in that of pancreatic carcinoma. When the molecular biological substances with higher sensitivity are found, the reliance of the combination assay of the molecular biological substances and cytology will be established. [source]


    Destocking as a Drought,mitigation Strategy: Clarifying Rationales and Answering Critiques

    The idea of externally assisted emergency destocking of pastoralists has gained currency in recent years: increasing the incentives for pastoralists to sell animals, or removing the constraints to selling animals in the early stages of drought. We identify two separate rationales put forward by proponents of destocking: environmental benefits and purchasing power/welfare benefits. We consider whether specific recent critiques of ,new range ecology' and specifically of ,tracking policies' do in fact provide arguments against emergency destocking in pastoralist areas. We illustrate some of these themes with a case study of a successful destocking exercise in northern Kenya where a very specific form of support was requested and received by pastoralists themselves. The sorts of destocking that work are likely to have significant effects on pastoralist purchasing power at key points of the drought cycle, but minimal effects on the environment. Clarifying these points will make it easier to promote destocking as a drought,mitigation policy. [source]


    Conifers as invasive aliens: a global survey and predictive framework

    ABSTRACT We summarize information on naturalized and invasive conifers (class Pinopsida) worldwide (data from 40 countries, some with remote states/territories), and contrast these findings with patterns for other gymnosperms (classes Cycadopsida, Gnetopsida and Ginkgoopsida) and for woody angiosperms. Eighty conifer taxa (79 species and one hybrid; 13% of species) are known to be naturalized, and 36 species (6%) are ,invasive'. This categorization is based on objective and conservative criteria relating to consistency of reproduction, distance of spread from founders, and degree of reliance on propagules from the founder population for persistence in areas well outside the natural range of species. Twenty-eight of the known invasive conifers belong to one family (Pinaceae) and 21 of these are in one genus (Pinus). The Cupressaceae (including Taxodiaceae) has six known invasive species (4%) in four genera, but the other four conifer families have none. There are also no known invasive species in classes Cycadopsida, Gnetopsida or Ginkgoopsida. No angiosperm family comprising predominantly trees and shrubs has proportionally as many invasive species as the Pinaceae. Besides the marked taxonomic bias in favour of Pinaceae, and Pinus in particular, invasiveness in conifers is associated with a syndrome of life-history traits: small seed mass (< 50 mg), short juvenile period (< 10 year), and short intervals between large seed crops. Cryptomeria japonica, Larix decidua, Picea sitchensis, Pinus contorta, Pinus strobus, and Pseudotsuga menziesii exemplify this syndrome. Many rare and endangered conifer species exhibit opposite characters. These results are consistent with earlier predictions made using a discriminant function derived from attributes of invasive and noninvasive Pinus species. Informative exceptions are species with small seeds (< 4 mg, e.g. Chamaecyparis spp., Pinus banksiana, Tsuga spp. , mostly limited to wet/mineral substrates) or otherwise ,non-invasive' characters (e.g. large seeds, fleshy fruits, e.g. Araucaria araucana, Pinus pinea, Taxus baccata that are dependent on vertebrates for seed dispersal). Most conifers do not require coevolved mutualists for pollination and seed dispersal. Also, many species can persist in small populations but have the genetic and reproductive capacity to colonize and increase population size rapidly. The underlying mechanisms mediating conifer invasions are thus easier to discern than is the case for most angiosperms. Further information is needed to determine the extent to which propagule pressure (widespread dissemination, abundant plantings, long history of cultivation) can compensate for low ,inherent invasiveness'.     Clinical issues in using buprenorphine in the treatment of opiate dependence

    Abstract This paper looks at the current role of buprenorphine in the treatment of opiate dependence. It suggests that buprenorphine is a useful alternative to methadone and that in at least some cases it may be the preferred option. Buprenorphineis a partial agonist and a partial antagonist with a ceiling of opiate activity probably approximately equal to 30mg methadone. It achieves this at a dose of 10-12mg, although there is considerable individual variation. Because of its ceiling effect it has a good safety profile compared to full agonists such as methadone although some overdose deaths, particularly in conjunction with benzodiazepine abuse, have been reported in France. Induction of buprenorphine may take slightly longer than for methadone and there is a higher dropout rate compared to methadone in the first 2 weeks. This is probably due to the antagonist action of buprenorphine causing more withdrawal symptoms in comparison to methadone. Also, the ceiling effect for buprenorphine means that some clients do not experience sufficient opiate activity to satisfy them. Buprenorphine has a long half-life and dissociates slowly from opiate receptors. Most clients can be dosed second-daily but some find this unacceptable due to mood swings and/or withdrawal symptomson the second day. For these clients daily dosing is required. Transferring from buprenorphine to methadone is straightforward and well tolerated by clients. Transferring from methadone to buprenorphine, however, is more difficult because of the partial antagonist action of buprenorphine. Clients experience withdrawal symptoms that can take up to 2 weeks to settle. Most clients find these symptoms unacceptable when transferring from doses of over 30mg of methadone. The optimum method for transferring from methadone to buprenorphine is still to be determined. Withdrawal from buprenorphine appears to be relatively easier than from methadone. This is presumably due to buprenorphine's partial agonist effect at mureceptors. It is expected that during 2000 buprenorphine will be approved for use in Australia for the treatment of opiate dependence. It may well becomea first-line choice for opiate replacement in heroin dependence. It is also likely to be useful in assisting detoxification fromboth methadone and heroin. [source]


    Response analysis of rigid structures rocking on viscoelastic foundation

    Abstract In this paper the rocking response of slender/rigid structures stepping on a viscoelastic foundation is revisited. The study examines in depth the motion of the system with a non-linear analysis that complements the linear analysis presented in the past by other investigators. The non-linear formulation combines the fully non-linear equations of motion together with the impulse-momentum equations during impacts. The study shows that the response of the rocking block depends on the size, shape and slenderness of the block, the stiffness and damping of the foundation and the energy loss during impact. The effect of the stiffness and damping of the foundation system along with the influence of the coefficient of restitution during impact is presented in rocking spectra in which the peak values of the response are compared with those of the rigid block rocking on a monolithic base. Various trends of the response are identified. For instance, less slender and smaller blocks have a tendency to separate easier, whereas the smaller the angle of slenderness, the less sensitive the response to the flexibility, damping and coefficient of restitution of the foundation.
