Impact of Age at First Drink on Stress-Reactive Drinking

Background: Although recent data from animal models indicate that adolescent ethanol exposure increases self-administered ethanol intake in adult rats, the impact of age at first drink on the association between stress and drinking has not been studied in humans. Methods: Data collected in the 2001 to 2002 National Epidemiologic Survey on Alcohol and Related Conditions (NESARC) were used to estimate the extent to which age at first drink modified the association between stress and average daily volume (ADV) of ethanol intake in a sample of 26,946 past-year drinkers. Successive models estimated the magnitude and significance of the interaction between age at first drink (ages 14 or younger, 15,17, and 18 or older) and number of stressors (out of 12 past-year negative life events) after (1) adjusting for sociodemographic characteristics, (2) additionally adjusting for family history of alcoholism, comorbid psychopathology, adolescent, and past-year tobacco and illicit drug use, and (3) additionally adjusting for all other significant interactions with number of stressors. Results: Even after adjusting for a wide range of confounders and their interactions with stress, initiation of drinking at ages 14 and younger increased the association between the number of stressors and ADV of ethanol consumption by 8% (p=0.014), when considering the full range of 12 potential stressors. In fact, the positive association between stress and consumption was significant only for this group of drinkers with early adolescent exposure to ethanol. Within this group, ADV of consumption increased by an average of 7% with each additional stressor experienced, although the exact percentage increase varied as a function of other covariates that had significant interactions with stress. When a reduced set of 4 stressors was considered, the magnitudes of the associations were mostly unchanged, but the modifying effect of age at first drink fell short of statistical significance (p=0.309) in the fully adjusted model. Conclusions: The findings of this study are consistent with the argument that early-onset drinking may increase stress-reactive ethanol consumption; however, these findings need to be replicated in an experimental human study in order to control fully the direction of the relationship between stress and consumption. Enhanced Prepulse Inhibition Following Adolescent Ethanol Exposure in Sprague-Dawley Rats

Abstract: Objectives: Recent studies have demonstrated that ethanol exposure differentially affects adolescents and adults. The current studies were designed to compare the effects of 2-week exposure to ethanol during adolescence or adulthood on the acoustic startle response (ASR) and prepulse inhibition (PPI) Methods: Male Sprague-Dawley rats were exposed to ethanol vapor 12 hr/d (on from 6 pm to 6 am) for 14 days during adolescence or adulthood. Six days after the cessation of ethanol vapor exposure, the ASR and PPI were assessed. Results: During ethanol treatment, overall blood alcohol levels averaged 230 to 250 mg/dl in the adolescent and adult treatment groups. Assessment of the ASR revealed that latency to startle was more rapid in adolescents than in adults, but ASR latency was not altered by ethanol exposure. In addition, ASR magnitude was lower in adolescents and was decreased in ethanol-exposed rats on startle trials. Ethanol exposure significantly enhanced PPI, but only after adolescent exposure Conclusions: These data further demonstrate a differential sensitivity of adolescents and adults to the effects of ethanol exposure. Specifically, a 2-week period of ethanol exposure during adolescence selectively enhanced PPI, a neurobehavioral index of sensorimotor gating. However, ASR magnitude was decreased by ethanol exposure regardless of age. On the basis of previous studies, the effects of ethanol exposure on PPI data could indicate that adolescent rats exposed to ethanol are more likely to exhibit behavioral inflexibility and that ethanol exposure acts as a more potent physical stressor in adolescent rats. Refining the measurement of exposure to violence (ETV) in urban youth

Correlational analysis, classical test theory, confirmatory factor analysis, and multilevel Rasch modeling were used to refine a measure of adolescents' exposure to violence (ETV). Interpersonal violence could be distinguished from other potentially traumatic events; it was also possible to distinguish three routes of exposure (victimization, witnessing, and learning of). Correlations confirmed that ETV subscales are related to measures of aggression, delinquency, and depression/anxiety. Reliability was improved by combining ETV subscales and/or caregiver and youth reports. Valid and reliable measures of ETV are critical to future research in associating violence exposure with common mental health and behavioral outcomes and disorders, and tracking how early violence exposure may affect future outcomes for adolescents. © 2007 Wiley Periodicals, Inc. Self-reported violence amongst adolescents in Denmark: is alcohol a serious risk factor?

Abstract Aim: To describe the prevalence of adolescents' exposure to different types of violence (at home and outside the home) and associations between severe violence and alcohol consumption, taking account of gender and the influence of other factors. Methods: A multimedia computer-based survey amongst a nationally representative sample of 9th grade pupils included comprehensive data on exposure to physical violence, use of alcohol and additional socio-demographic and behavioural factors. Analyses included gender disaggregated cross tabulations and logistic regressions. Results: Around 9.3% of girls and 12.3% of boys reported being victims of physical violence within the last 12 months. Also, 2.7% of girls and 3.5% of boys reported severe violence. Gender differences existed in place of occurrence; girls were more frequently assaulted at home, boys outside the home. Alcohol was only associated with violence exposure outside the home for boys, not girls. Conclusions: About one-tenth of adolescents in Denmark reported exposure to any physical violence and 3% to severe violence during the last year. Alcohol consumption was not a risk factor for girls and only a risk factor for boys regarding violence occurring outside the home. [source]
