Davis Abstract Background CD163 is a monocyte/macrophage-restricted receptor involved in the clearance of hemoglobin,haptoglobin complexes and regulation of inflammatory processes. CD163 is shed from the cell surface and exists as a soluble form in plasma (sCD163). Monocyte CD163 and sCD163 are potential diagnostic tools in variety of disease states. Methods We determined the relation between plasma sCD163 levels by enzyme-linked immunosorbent assay, membrane expressions of CD163, CD64, and CD14 on blood monocytes by flow cytometry, and monocyte counts in 129 random blood samples. Results A strong inverse correlation was found between membrane CD163 expression and sCD163 levels (r = ,0.65, P < 0.001). Monocyte CD163 expression and SCD163 levels did not correlate with the monocyte absolute count. Conclusions The inverse relation between monocyte surface CD163 expression and sCD163 levels in human blood suggests that plasma sCD163 is derived from circulating monocytes, in addition to an unknown component from tissue macrophages. The lack of correlation with the absolute monocyte number suggests that such a balance is driven by the functional state of monocytes, rather than simply by numerical changes in circulating cells. We propose that further clinical evaluations of CD163 as a diagnostic parameter should include simultaneous measurements of soluble and cell-bound forms of this antigen. © 2004 Wiley-Liss, Inc. [source] Reliability of a machine service life prediction in thermal diagnostic testLUBRICATION SCIENCE, Issue 1 2007L. Burstein Abstract Reliability of the machine service life determined in a thermal diagnostic test is estimated in this investigation. The time dependence of a thermal diagnostic parameter, and the residual service life (RSL) predicted on its basis, are examined here with the aid of the Monte-Carlo simulation procedure. The relationships involved are derived from the machine heat balance under two approaches , short term (starting stage) and long term (service time). The diagnostic parameter considered is the temperature change rate during the short-term period. The constants in its expression are determined from experimental data obtained as an example on the gearboxes of three heavy portal cranes with different service times. The diagnostic parameter,time graph (long-term approach) derived from the data is used as reference for prediction of the RSL. The reference dependence obtained from theoretical values of the diagnostic parameter, and the RSL calculated from this dependence, were repeatedly varied, with a view to estimating the reliability of the prediction. The uniform and Weibull distributions were used for generating, respectively, the random fluctuations of the reference gearbox temperature and of the diagnostic parameter of the tested one. It is shown that in 95% of the cases the thermal method entails a two-sided error of at most 3.6%, and for the tested gearbox the discrepancy between the deterministic and simulated averages does not exceed 1.1%. Abstract Background:, Epidemiologic studies suggest that elderly people are more prone to develop severe anaphylactic reactions. However, the exact cause for this phenomenon remains unclear. Aims of the study:, To study the role of the serum tryptase as a diagnostic parameter for individual risk evaluation and its impact on the severity of allergic reactions in elderly people. Methods:, Two hundred and seventy-four consecutive patients visiting the Department of Dermatology, Tübingen, Germany, who were diagnosed with honeybee or wasp venom allergy, were included in the study. Results:, Sting reaction severity increased with increased age and tryptase levels (P = 0.001 and P = 0.0003, respectively). Furthermore, we find not only a general increment in tryptase levels in elderly people (P = 0.0001) but also a continuous increase in tryptase concentrations even below the cut-off (11.4 ,g/l) with increasing age (P = 0.0026). Conclusions:, Our data confirm serum tryptase as a risk factor for severe anaphylactic reaction to hymenoptera stings. Furthermore, we give first evidence that basal serum tryptase levels increase continuously with age and being an indicator for either increased mast cell load or reactivity this can at least partly be responsible for the observed aggravated allergic reactions in elderly people. As those patients are at increased risk for life-threatening anaphylactic reactions, it should be considered to adjust VIT especially in elderly patients with elevated tryptase levels as recommended for patients with mastocytosis by increasing venom doses during VIT and by considering its life-long continuation. [source] Onset and Offset Phonation Threshold Flow in Excised Canine Larynges,THE LARYNGOSCOPE, Issue 7 2008Michael F. Regner Abstract Objectives/Hypothesis: In this study, we examined the hypothesis that the minimum flow required for phonation onset, the onset phonation threshold flow (PTF), is greater than the minimum flow to sustain phonation, the offset PTF. We also explored the hypothesis that the ratio of these two (PTF offset divided by PTF onset) falls within [0.707, 1.0]. Study Design: This was a methodology study to measure onset and offset PTFs in 10 excised canine larynges; their ratio (PTF offset divided by PTF onset) was predicted to fall in a specific domain. Methods: The onset and offset PTF and phonation threshold pressure (PTP) values were observed using 10 excised canine larynges mounted on a bench apparatus. The subglottal flow was increased until phonation was observed, and then decreased until phonation ceased; airflow and pressure measurements at critical conditions of phonation were observed as the PTF and PTP. Larynges with elongated vocal folds were then tested to observe PTF and determine if the hypothesis was observed in pathologic-like larynges. Results: The offset PTF was always less than the onset PTF (P , .0001) and 80.0% of the observed onset-offset PTF ratios were bound by [0.707, 1.0]. Conclusions: PTF onset or offset could be a useful diagnostic parameter of the voice, particularly when used in conjunction with PTP to describe laryngeal resistance and aerodynamic power. Further exploration of the relationship between onset and offset PTF values could augment clinical diagnostic ability and advance current theories on the physics of phonation. [source] Depression in dementia: a comparative and validation study of four brief scales in the elderly ChineseINTERNATIONAL JOURNAL OF GERIATRIC PSYCHIATRY, Issue 5 2004Chee Kum Lam Abstract Aim The study aimed to determine: (i) the diagnostic accuracy of four brief depression scales, the Geriatric Depression Scale (GDS), Even Briefer Assessment Scale for Depression (EBAS DEP), Single Question and Cornell Scale for Depression in Dementia (Cornell) in an elderly Chinese population with varying dementia severity; and (ii) which scale had the best diagnostic performance. Method All four scales were administered to 88 elderly outpatients with dementia: 66 without and 22 with depression. Receiver Operating Characteristic (ROC) analysis was used to establish the optimal cut-off scores of the GDS, EBAS DEP and Cornell scales. The patients' dementia-severity was dichotomously categorized into mild and moderate-severe dementia, and the above analysis was repeated in both these groups to look at changes in the scales' diagnostic performance as dementia advances. Results The best diagnostic scale for detecting depression in dementia was the Cornell scale. Its optimal cut-off score was 6/7 (sensitivity 91.7%, specificity 80.0%) in the mild dementia group and 12/13 (sensitivity 70.0%, specificity 87.0%) in the more advanced dementia group. The optimal cut-off scores of the GDS and EBAS DEP also shifted to higher values when moving from the mild to the more advanced dementia groups, indicating the increasing difficulty on all these scales to detect depression with worsening cognitive impairment. The Single Question, however, was more robust with much less changes in its diagnostic parameters in both dementia cohorts: sensitivity 58.3%, specificity 90.0% for mild dementia, and 60.0 and 84.8%, respectively, for more advanced dementia. Conclusion An efficient strategy to diagnose depression in dementia amongst elderly Chinese patients is to administer the Single Question followed by, when necessary, the Cornell scale. Schäfer Background:, Little is known on the predictive value of sensitization to specific aeroallergens in children with respect to asthma and hay fever incidence in young adulthood. We followed the incidence of asthma and hay fever in children (mean age 11 years) over 9 years, and analyzed the predictive value of sensitization to five common aeroallergens. Methods:, Three consecutive surveys were conducted in East German school children. Specific IgE antibodies to birch and timothy grass pollen, house dust mite, cat, and cladosporium were measured. In 1207 out of the 2453 children, the 9-year incidence of asthma and hay fever was assessed by reported doctors' diagnoses. For sensitization, diagnostic parameters were determined and logistic regression analyses controlled for relevant confounders. Results:, A total of 176/78 incident hay fever/asthma cases occurred equaling a cumulative incidence of 1.93/0.86% per year. Incident asthma was associated with previous sensitization to cat [risk ratio (RR) 3.49, 1.57,7.74] and grass pollen (RR 1.79, 1.01,3.19), whereas incident hay fever was associated with each allergen, with grass pollen (RR 6.00, 4.04,8.90) and cat (RR 5.36, 2.87,9.99) exhibiting the strongest associations. When mutually adjusting for all allergens, sensitization to cat remained significantly associated with asthma and hay fever. The latter was also associated with sensitization to grass pollen. The highest positive predictive values for asthma and hay fever were obtained for cat sensitization (10/49 = 20.4% and 23/49 = 46.9%). Conclusions:, Childhood sensitization to cat and grass pollen predicts the incidence of asthma and hay fever in young adulthood. The predictive capacity differs by allergen and manifestation of atopy. [source] Potential role of the cellular allergen stimulation test (CAST) in diagnosis of allergic bronchopulmonary aspergillosis (ABPA) in cystic fibrosisPEDIATRIC PULMONOLOGY, Issue 4 2007Stephanie Ringer MD Abstract Allergic bronchopulmonary aspergillosis (ABPA) is a severe complication in cystic fibrosis (CF), which is difficult to identify because of overlapping unspecific diagnostic features with common CF-manifestations. The cellular allergen stimulation test (CAST) is used in diagnosis of allergic and pseudoallergic reactions. This assay is based on the determination of sulfidoleukotrienes, which are produced by allergen-stimulated basophils in vitro. The potential role of CAST in diagnosis of ABPA was evaluated in this study. The CAST assay was applied in 27 CF-patients including eight subjects with positive clinical and serological signs of ABPA. Additional to the Nelson-criteria for diagnosis of ABPA specific IgE against recombinant Aspergillus antigens (rAsp f 1, 2, 3, 4, and 6) were assessed. The CAST results were positive in all ABPA-patients and in five controls without any sign of ABPA except positive specific IgE against Aspergillus fumigatus (sensitivity of 100%, specificity of 74%). Specific IgE against rAsp f 4 and/or f 6 were positive in six of the eight ABPA-patients, but not in the controls. Positive CAST results, total serum IgE,>,500 U/ml and positive IgE antibodies against rAsp f 4 and/or f 6 were only found in ABPA-patients (specificity of 100%). The CAST assay on its own includes high sensitivity with lower specificity. For the discrimination of ABPA from sensitization to Aspergillus, the CAST, the highly elevated total serum IgE and rAsp in combination are potential auxiliary diagnostic parameters. [source] A simple pharmacokinetics subroutine for modeling double peak phenomenonBIOPHARMACEUTICS AND DRUG DISPOSITION, Issue 3 2006Ahmad Mirfazaelian Abstract Double peak absorption has been described with several orally administered drugs. Numerous reasons have been implicated in causing the double peak. DRUG-KNT,a pharmacokinetic software developed previously for fitting one and two compartment kinetics using the iterative curve stripping method,was modified and a revised subroutine was incorporated to solve double-peak models. This subroutine considers the double peak as two hypothetical doses administered with a time gap. The fitting capability of the presented model was verified using four sets of data showing double peak profiles extracted from the literature (piroxicam, ranitidine, phenazopyridine and talinolol). Visual inspection and statistical diagnostics showed that the present algorithm provided adequate curve fit disregarding the mechanism involved in the emergence of the secondary peaks. Statistical diagnostic parameters (RSS, AIC and R2) generally showed good fitness in the plasma profile prediction by this model.
