Population-based Triage Management in Response to Surge-capacity Requirements during a Large-scale Bioevent Disaster

Both the naturally occurring and deliberate release of a biological agent in a population can bring catastrophic consequences. Although these bioevents have similarities with other disasters, there also are major differences, especially in the approach to triage management of surge capacity resources. Conventional mass-casualty events use uniform methods for triage on the basis of severity of presentation and do not consider exposure, duration, or infectiousness, thereby impeding control of transmission and delaying recognition of victims requiring immediate care. Bioevent triage management must be population based, with the goal of preventing secondary transmission, beginning at the point of contact, to control the epidemic outbreak. Whatever triage system is used, it must first recognize the requirements of those Susceptible but not exposed, those Exposed but not yet infectious, those Infectious, those Removed by death or recovery, and those protected by Vaccination or prophylactic medication (SEIRV methodology). Everyone in the population falls into one of these five categories. This article addresses a population approach to SEIRV-based triage in which decision making falls under a two-phase system with specific measures of effectiveness to increase likelihood of medical success, epidemic control, and conservation of scarce resources. Stable implantation of orthogonal sensor circuits in Gram-negative bacteria for environmental release

Summary A broad host range, orthogonal genetic platform has been developed to format sensor circuits in the chromosome of Gram-negative microorganisms destined for environmental release as bioindicators of toxic or perilous compounds (e.g. explosives) in soil. The genetic scheme includes the generation of a genomic landing pad for the sensor module with a Tn5 -mini-transposon bearing an optimal attTn7 sequence and a choice of reporter systems with optical and enzymatic outputs. The array of functional elements thereby inserted in the chromosome match that of a cognate plasmid vector which delivers the transcription factors and the promoters to a frame that places the regulatory parts in front of the reporters. Site-specific recombination sites allow the deletion of antibiotic resistances and enables reporter output prior to deliberate release. The system thus allows the production and maintenance of cells in a pre-release state and its intentional conversion in deliverable strains that fulfil all safety, stability and performance criteria. The combination of such a genetic platform with a variant of the transcriptional regulator XylR of Pseudomonas putida that responds to 2,4-dinitrotoluene has been the basis for the production of strains that emit light upon exposure to residues of explosives in a soil microcosm. Summary The European Union (EU) legislation regarding sustainable development moves along two distinct lines: the impact of industries (food industries included) on the environment (release of gases and green house effect and the effect of cultivating genetically modified organisms (GMOs) on the environment. EU voted three communications [COM(2002)524, COM(2003)301, COM(2004)38] in an attempt to set an action plan based on technologies to manage pollution, by promoting less polluting and less resource-intensive products and services and ways to manage resources more efficiently. As such environment-friendly technologies pervade practically all economic activities and sectors it is anticipated that they will reduce effectively energy and resource consumption thereby creating fewer emissions and less waste. As regards the cultivation and/or importing of GMOs, EU legislation was based on two directives (E.U. 90/219/EEC, E.U. 2001/18/EC) and four regulations [Regulation (EC) No. 258/97, Regulation (EC) No. 1830/2003, Regulation (EC) No. 1830/2003 and Regulation (EC) No. 1946/2003]. The directives aimed at adopting measures for limited use of GM micro-organisms, making the procedure for granting consent to the deliberate release and placing on the market of GMOs more efficient and more transparent, making GMO labelling compulsory and thereby enhancing GMOs traceability along the entire food chain. Grasping at the routes of biological invasions: a framework for integrating pathways into policy

Summary 1Pathways describe the processes that result in the introduction of alien species from one location to another. A framework is proposed to facilitate the comparative analysis of invasion pathways by a wide range of taxa in both terrestrial and aquatic ecosystems. Comparisons with a range of data helped identify existing gaps in current knowledge of pathways and highlight the limitations of existing legislation to manage introductions of alien species. The scheme aims for universality but uses the European Union as a case study for the regulatory perspectives. 2Alien species may arrive and enter a new region through three broad mechanisms: importation of a commodity, arrival of a transport vector, and/or natural spread from a neighbouring region where the species is itself alien. These three mechanisms result in six principal pathways: release, escape, contaminant, stowaway, corridor and unaided. 3Alien species transported as commodities may be introduced as a deliberate release or as an escape from captivity. Many species are not intentionally transported but arrive as a contaminant of a commodity, for example pathogens and pests. Stowaways are directly associated with human transport but arrive independently of a specific commodity, for example organisms transported in ballast water, cargo and airfreight. The corridor pathway highlights the role transport infrastructures play in the introduction of alien species. The unaided pathway describes situations where natural spread results in alien species arriving into a new region from a donor region where it is also alien. 4Vertebrate pathways tend to be characterized as deliberate releases, invertebrates as contaminants and plants as escapes. Pathogenic micro-organisms and fungi are generally introduced as contaminants of their hosts. The corridor and unaided pathways are often ignored in pathway assessments but warrant further detailed consideration. 5Synthesis and applications. Intentional releases and escapes should be straightforward to monitor and regulate but, in practice, developing legislation has proved difficult. New introductions continue to occur through contaminant, stowaway, corridor and unaided pathways. These pathways represent special challenges for management and legislation. The present framework should enable these trends to be monitored more clearly and hopefully lead to the development of appropriate regulations or codes of practice to stem the number of future introductions. Anthrax: the challenges for decontamination

Abstract Anthrax remains endemic in many parts of the world with regular infections of livestock presenting a consequent risk to public health. In the United Kingdom anthrax has diminished as a significant threat to human health with only sporadic outbreaks in farm animals derived from ingestion of spores from soil at sites associated with previous outbreaks and the burial of carcasses. Occupationally-derived anthrax, associated with industries involved in the processing of animal products, has historically had an impact on the occurrence of outbreaks of infection. The introduction, in 1965, of vaccination for workers in high-risk occupations contributed significantly to the eradication of the disease from the UK. During 2001 the deliberate release of anthrax spores in the USA, disseminated through the postal system, resulted in the infection of 22 people, five of which resulted in death through inhalational anthrax. At that time anthrax was unheard of in many clinical practices and there was a lack of training and preparedness to handle such incidents; the emergency resulted in medical and public health personnel across the world having a significantly raised awareness of both the organism and the clinical symptoms of infection, and the new threat posed by bioterrorism. In the USA, the immediate public health emergency was followed by the legacy of contaminated buildings and facilities. There had been little previous systematic study of the issues surrounding sampling and decontamination of areas contaminated with Bacillus anthracis. The decontamination of large complex buildings and the equipment they contained required the urgent development and validation of new procedures for both sampling and decontamination. Mathematically Assessing the Consequences of Food Terrorism Scenarios

ABSTRACT:, We derive mathematical expressions for the mean number of casualties resulting from a deliberate release of a biological or chemical agent into a food supply chain. Our analysis first computes the amount of contaminated food as a function of the network topology and vessel sizes in the food processing plant. A probabilistic analysis, in which each potential consumer of contaminated food has his own random purchase time, infectious dose, and incubation period, determines the number of people who consume enough tainted food to get infected or poisoned before the attack is detected and food consumption is halted. These simple formulas can be used by the U.S. government and the food industry to develop a rough-cut prioritization of the threats from food terrorism, which would be a 1st step toward the allocation of appropriate prevention and mitigation resources.
