Selected AbstractsAmbient temperature and risk of death from accidental drug overdose in New York City, 1990,2006ADDICTION, Issue 6 2010Amy S. B. Bohnert ABSTRACT Background Mortality increases as ambient temperature increases. Because cocaine affects core body temperature, ambient temperature may play a role in cocaine-related mortality in particular. The present study examined the association between ambient temperature and fatal overdoses over time in New York City. Methods Mortality data were obtained from the Office of the Chief Medical Examiner for 1990 to 2006, and temperature data from the National Oceanic and Atmospheric Association. We used generalized additive models to test the relationship between weekly average temperatures and counts of accidental overdose deaths in New York City, controlling for year and average length of daylight hours. Results We found a significant relation between ambient temperature and accidental overdose fatality for all models where the overdoses were due in whole or in part to cocaine (all P < 0.05), but not for non-cocaine overdoses. Risk of accidental overdose deaths increased for weeks when the average temperature was above 24°Celsius. Conclusions These results suggest a strong relation between temperature and accidental overdose mortality that is driven by cocaine-related overdoses rising at temperatures above 24°Celsius; this is a substantially lower temperature than prior estimates. To put this into perspective, approximately 7 weeks a year between 1990 and 2006 had an average weekly temperature of 24 or above in New York City. Heat-related mortality presents a considerable public health concern, and cocaine users constitute a high-risk group. [source] DOES VIVIPARITY EVOLVE IN COLD CLIMATE REPTILES BECAUSE PREGNANT FEMALES MAINTAIN STABLE (NOT HIGH) BODY TEMPERATURES?EVOLUTION, Issue 8 2004Richard Shine Abstract Viviparity (live bearing) has evolved from egg laying (oviparity) in many lineages of lizards and snakes, apparently in response to occupancy of cold climates. Explanations for this pattern have focused on the idea that behaviorally thermoregulating (sun-basking) pregnant female reptiles can maintain higher incubation temperatures for their embryos than would be available in nests under the soil surface. This is certainly true at very high elevations, where only viviparous species occur. However, comparisons of nest and lizard temperatures at sites close to the upper elevational limit for oviparous reptiles (presumably, the selective environment where the transition from oviparity to viviparity actually occurs) suggest that reproductive mode has less effect on mean incubation temperatures than on the diel distribution of those temperatures. Nests of the oviparous scincid lizard Bassiana duperreyi showed smooth diel cycles of heating and cooling. In contrast, body temperatures of the viviparous scincid Eulamprus heatwolei rose abruptly in the morning, were high and stable during daylight hours, and fell abruptly at night. Laboratory incubation experiments mimicking these patterns showed that developmental rates of eggs and phenotypic traits of hatchling B. duperreyi were sensitive to this type of thermal variance as well as to mean temperature. Hence, diel distributions as well as mean incubation temperatures may have played an important role in the selective forces for viviparity. More generally, variances as well as mean values of abiotic factors may constitute significant selective forces on life-history evolution. [source] Night stocking facilitates nocturnal migration of hatchery-reared Atlantic salmon, Salmo salar, smoltsFISHERIES MANAGEMENT & ECOLOGY, Issue 1 2009L. J. ROBERTS Abstract, Hatchery-reared salmon, Salmo salar L., smolts are generally stocked during daylight hours, but the natural migration of smolts tends to occur at night. Recapture rates and timing of migration were compared between Atlantic salmon smolts stocked during the day and during the evening. Timing of release had no significant effect on the number of smolts recaptured, but had a strong effect on nocturnal behaviour. When stocked in the evening (but not during the day) hatchery-reared smolts moved almost exclusively during the night. This study suggests that timing the release to coincide with the natural time of smolt migration may provide valuable acclimatisation and facilitate nocturnal smolt passage. [source] Tropospheric O3 moderates responses of temperate hardwood forests to elevated CO2: a synthesis of molecular to ecosystem results from the Aspen FACE projectFUNCTIONAL ECOLOGY, Issue 3 2003D. F. Karnosky Summary 1The impacts of elevated atmospheric CO2 and/or O3 have been examined over 4 years using an open-air exposure system in an aggrading northern temperate forest containing two different functional groups (the indeterminate, pioneer, O3 -sensitive species Trembling Aspen, Populus tremuloides and Paper Birch, Betula papyrifera, and the determinate, late successional, O3 -tolerant species Sugar Maple, Acer saccharum). 2The responses to these interacting greenhouse gases have been remarkably consistent in pure Aspen stands and in mixed Aspen/Birch and Aspen/Maple stands, from leaf to ecosystem level, for O3 -tolerant as well as O3 -sensitive genotypes and across various trophic levels. These two gases act in opposing ways, and even at low concentrations (1·5 × ambient, with ambient averaging 34,36 nL L,1 during the summer daylight hours), O3 offsets or moderates the responses induced by elevated CO2. 3After 3 years of exposure to 560 µmol mol,1 CO2, the above-ground volume of Aspen stands was 40% above those grown at ambient CO2, and there was no indication of a diminishing growth trend. In contrast, O3 at 1·5 × ambient completely offset the growth enhancement by CO2, both for O3 -sensitive and O3 -tolerant clones. Implications of this finding for carbon sequestration, plantations to reduce excess CO2, and global models of forest productivity and climate change are presented. [source] Water savings in mature deciduous forest trees under elevated CO2GLOBAL CHANGE BIOLOGY, Issue 12 2007SEBASTIAN LEUZINGER Abstract Stomatal conductance of plants exposed to elevated CO2 is often reduced. Whether this leads to water savings in tall forest-trees under future CO2 concentrations is largely unknown but could have significant implications for climate and hydrology. We used three different sets of measurements (sap flow, soil moisture and canopy temperature) to quantify potential water savings under elevated CO2 in a ca. 35 m tall, ca. 100 years old mixed deciduous forest. Part of the forest canopy was exposed to 540 ppm CO2 during daylight hours using free air CO2 enrichment (FACE) and the Swiss Canopy Crane (SCC). Across species and a wide range of weather conditions, sap flow was reduced by 14% in trees subjected to elevated CO2, yielding ca. 10% reduction in evapotranspiration. This signal is likely to diminish as atmospheric feedback through reduced moistening of the air comes into play at landscape scale. Vapour pressure deficit (VPD)-sap flow response curves show that the CO2 effect is greatest at low VPD, and that sap flow saturation tends to occur at lower VPD in CO2 -treated trees. Matching stomatal response data, the CO2 effect was largely produced by Carpinus and Fagus, with Quercus contributing little. In line with these findings, soil moisture at 10 cm depth decreased at a slower rate under high-CO2 trees than under control trees during rainless periods, with a reversal of this trend during prolonged drought when CO2 -treated trees take advantage from initial water savings. High-resolution thermal images taken at different heights above the forest canopy did detect reduced water loss through altered energy balance only at <5 m distance (0.44 K leaf warming of CO2 -treated Fagus trees). Short discontinuations of CO2 supply during morning hours had no measurable canopy temperature effects, most likely because the stomatal effects were small compared with the aerodynamic constraints in these dense, broad-leaved canopies. Hence, on a seasonal basis, these data suggest a <10% reduction in water consumption in this type of forest when the atmosphere reaches 540% ppm CO2. [source] Interannual climatic variation mediates elevated CO2 and O3 effects on forest growthGLOBAL CHANGE BIOLOGY, Issue 6 2006MARK E. KUBISKE Abstract We analyzed growth data from model aspen (Populus tremuloides Michx.) forest ecosystems grown in elevated atmospheric carbon dioxide ([CO2]; 518 ,L L,1) and ozone concentrations ([O3]; 1.5 × background of 30,40 nL L,1 during daylight hours) for 7 years using free-air CO2 enrichment technology to determine how interannual variability in present-day climate might affect growth responses to either gas. We also tested whether growth effects of those gasses were sustained over time. Elevated [CO2] increased tree heights, diameters, and main stem volumes by 11%, 16%, and 20%, respectively, whereas elevated ozone [O3] decreased them by 11%, 8%, and 29%, respectively. Responses similar to these were found for stand volume and basal area. There were no growth responses to the combination of elevated [CO2+O3]. The elevated [CO2] growth stimulation was found to be decreasing, but relative growth rates varied considerably from year to year. Neither the variation in annual relative growth rates nor the apparent decline in CO2 growth response could be explained in terms of nitrogen or water limitations. Instead, growth responses to elevated [CO2] and [O3] interacted strongly with present-day interannual variability in climatic conditions. The amount of photosynthetically active radiation and temperature during specific times of the year coinciding with growth phenology explained 20,63% of the annual variation in growth response to elevated [CO2] and [O3]. Years with higher photosynthetic photon flux (PPF) during the month of July resulted in more positive growth responses to elevated [CO2] and more negative growth responses to elevated [O3]. Mean daily temperatures during the month of October affected growth in a similar fashion the following year. These results indicate that a several-year trend of increasingly cloudy summers and cool autumns were responsible for the decrease in CO2 growth response. [source] Foraging behaviour and feeding locations of Rock Shags Phalacrocorax magellanicus from a colony in Patagonia, ArgentinaIBIS, Issue 3 2001FLAVIO QUINTANA During 1996 and 1997, foraging Rock Shags Phalacrocorax magellanicus were studied at Punta Loma, Argentina using radio-transmitters deployed on ten adult shags during the chick-rearing period. Rock Shags undertook 2.6 ± 0.6 sd trips per day. The mean duration of a feeding trip was 2.6 ± 0.7 hours. A bird spent 36% of daylight hours away from the colony on feeding trips, diving for 92% of the foraging trip, and made a mean of 106 dives per foraging trip. Foraging trip duration was strongly correlated with the total number of dives made in one foraging trip. Rock Shags fed mainly in water less than 10m deep with a gravelly sand bottom and within 5 km of shore. Mean foraging range was 3.8 ± 2.6 km and 2.6 ± 2.3 km for 1996 and 1997, respectively. These results suggest a high foraging effort (diving time per foraging trip) for Rock Shags, presumably associated with poor food conditions close to the colony. Comparison is made with other Phalacrocorax species. [source] Army ants in four forests: geographic variation in raid rates and species compositionJOURNAL OF ANIMAL ECOLOGY, Issue 3 2007SEAN O'DONNELL Summary 1The New World army ants are top predators in the litter of tropical forest, but no comprehensive studies exist on variation in assemblage-wide activity and species composition. We used standardized protocols to estimate foraging raid rates and species composition of army ant communities in four Neotropical forests. The study sites spanned approximately 10° latitude, with two sites each in Central and South America. 2We recorded a total of 22 species of army ants. The four sites varied in observed and estimated species richness. Species overlap was highest between the Central American sites, and lowest between the South American sites. 3Raid activity varied significantly among sites. Raid activity per kilometre of trail walks was over four times higher at the most active site (Sta. Maria, Venezuela) than at the least active site (Barro Colorado Island, Panama). Furthermore, each site showed a different diel pattern of activity. For example, raid activity was higher during daylight hours in Costa Rica, and higher at night in Venezuela. Raid activity relationships with ambient temperature also varied significantly among sites. 4The overall rate of army ant raids passing through 1 m2 plots was 0·73 raids per day, but varied among sites, from 0 raids per day (Panama) to 1·2 raids per day (Venezuela). 5Primarily subterranean species were significantly more abundant in Venezuela, and above-ground foragers that form large swarm fronts were least abundant in Panama. The site heterogeneity in species abundance and diel activity patterns has implications for army ant symbionts, including ant-following birds, and for the animals hunted by these top predators. [source] A novel artificial habitat collection device for studying resettlement patterns in anguillid glass eelsJOURNAL OF FISH BIOLOGY, Issue 5 2001V. Silberschneider The number of glass eels Anguilla australis and A. reinhardtii caught in artificial habitat collectors, made from a PVC base and polyethylene split rope fibres, was related to the number of rope fibre tufts attached to each collector rather than collector area directly. Ageing of collectors in situ to promote algal growth enhanced the catch of glass eels. Glass eels entered the collectors at night primarily during the flood tide, and did not move into the collectors during daylight hours. Glass eel abundance increased with increasing distance from the freshwater drain located in the causeway. The artificial habitat collectors are effective for assessing relative numbers of resettling glass eels and may be useful for studying recruitment and settlement patterns of other anguillid eel species, as well as identifying areas and habitats within a catchment that provide important shelter for glass eels. Sampling glass eels can be carried out with maximum effect and minimum effort using compact, aged artificial habitat collectors on the night time flood tide when low tide coincides with dusk. [source] The contribution of the swimbladder to buoyancy in the adult zebrafish (Danio rerio): A morphometric analysisJOURNAL OF MORPHOLOGY, Issue 6 2008George N. Robertson Abstract Many teleost fishes use a swimbladder, a gas-filled organ in the coelomic cavity, to reduce body density toward neutral buoyancy, thus minimizing the locomotory cost of maintaining a constant depth in the water column. However, for most swimbladder-bearing teleosts, the contribution of this organ to the attainment of neutral buoyancy has not been quantified. Here, we examined the quantitative contribution of the swimbladder to buoyancy and three-dimensional stability in a small cyprinid, the zebrafish (Danio rerio). In aquaria during daylight hours, adult animals were observed at mean depths from 10.1 ± 6.0 to 14.2 ± 5.6 cm below the surface. Fish mass and whole-body volume were linearly correlated (r2 = 0.96) over a wide range of body size (0.16,0.73 g); mean whole-body density was 1.01 ± 0.09 g cm,3. Stereological estimations of swimbladder volume from linear dimensions of lateral X-ray images and direct measurements of gas volumes recovered by puncture from the same swimbladders showed that results from these two methods were highly correlated (r2 = 0.85). The geometric regularity of the swimbladder thus permitted its volume to be accurately estimated from a single lateral image. Mean body density in the absence of the swimbladder was 1.05 ± 0.04 g cm,3. The swimbladder occupied 5.1 ± 1.4% of total body volume, thus reducing whole-body density significantly. The location of the centers of mass and buoyancy along rostro-caudal and dorso-ventral axes overlapped near the ductus communicans, a constriction between the anterior and posterior swimbladder chambers. Our work demonstrates that the swimbladder of the adult zebrafish contributes significantly to buoyancy and attitude stability. [source] EFFECTS OF WATERCRAFT NOISE ON THE ACOUSTIC BEHAVIOR OF BOTTLENOSE DOLPHINS, TURSIOPS TRUNCATUS, IN SARASOTA BAY, FLORIDAMARINE MAMMAL SCIENCE, Issue 4 2004Kara C. Buckstaff Abstract Watercraft may provide the greatest source of anthropogenic noise for bottlenose dolphins living in coastal waters. A resident community of about 140 individuals near Sarasota, Florida, are exposed to a vessel passing within 100 m approximately every six minutes during daylight hours. I investigated the circumstances under which watercraft traffic may impact the acoustic behavior of this community, specifically looking for short-term changes in whistle frequency range, duration, and rate of production. To analyze whistles and received watercraft noise levels, acoustic recordings were made using two hydrophones towed from an observation vessel during focal animal follows of 14 individual dolphins. The duration and frequency range of signature whistles did not change significantly relative to vessel approaches. However, dolphins whistled significantly more often at the onset of approaches compared to during and after vessel approaches. Whistle rate was also significantly greater at the onset of a vessel approach than when no vessels were present. Increased whistle repetition as watercraft approach may simply reflect heightened arousal, an increased motivation for animals to come closer together, with whistles functioning to promote reunions. It may also be an effective way to compensate for signal masking, maintaining communication in a noisy environment. Cayabyab Summary ,,Here, the leaf photoacclimatory plasticity and efficiency of the tropical seagrass Thalassia testudinum were examined. ,,Mesocosms were used to compare the variability induced by three light conditions, two leaf sections and the variability observed at the collection site. ,,The study revealed an efficient photosynthetic light use at low irradiances, but limited photoacclimatory plasticity to increase maximum photosynthetic rates (Pmax) and saturation (Ek) and compensation (Ec) irradiances under high light irradiance. A strong, positive and linear association between the percentage of daylight hours above saturation and the relative maximum photochemical efficiency (FV/FM) reduction observed between basal and apical leaf sections was also found. ,,The results indicate that T. testudinum leaves have a shade-adapted physiology. However, the large amount of heterotrophic biomass that this seagrass maintains may considerably increase plant respiratory demands and their minimum quantum requirements for growth (MQR). Although the MQR still needs to be quantified, it is hypothesized that the ecological success of this climax species in the oligotrophic and highly illuminated waters of the Caribbean may rely on the ability of the canopy to regulate the optimal leaf light environment and the morphological plasticity of the whole plant to enhance total leaf area and to reduce carbon respiratory losses. [source] Risk factors for low birthweight in north-east Brazil: the role of caesarean sectionPAEDIATRIC & PERINATAL EPIDEMIOLOGY, Issue 3 2001Antônio A.M. Silva Summary Caesarean section (CS) delivery is associated with low birthweight (LBW) in south-east Brazil. A hospital-based study was conducted on singleton infants from mothers residing in São Luís, to assess if an association between CS and LBW was found in the northern part of the country, where the CS rate is lower than in the south-east. A standardised questionnaire was administered to a sample of 2541 mothers in 10 hospitals, representing 94% of all deliveries, from March 1997 to February 1998. In a logistic model, type of delivery was the independent variable, the other variables were treated as confounders, and interaction terms were added between type of delivery and all other factors. LBW was associated with low maternal height, maternal smoking, primiparity, previous LBW, public insurance, preterm birth and CS. The CS rate was 33.7%. The risk of CS was higher for primiparous and married mothers, those with high level of schooling and attended by the same physician during prenatal and delivery care, deliveries held in private hospitals, daylight hours or evenings, and for those mothers who had adequate prenatal care. Because it appears unlikely that only medical reasons are operative, it is a possibility that CS could cause LBW, reflecting abusive indications for elective CS. [source] Changes in soil C-isotopic composition in an agroecosystem under Free Air Carbon dioxide Enrichment (FACE) treatment during a crop rotation period,RAPID COMMUNICATIONS IN MASS SPECTROMETRY, Issue 11 2005Anette Giesemann FACE (Free Air Carbon dioxide Enrichment) has been used since 1999 to evaluate the effects of future atmospheric CO2 concentrations on an arable crop agroecosystem. The experiment conducted at the Institute of Agroecology at the Federal Research Centre in Braunschweig consists of a typical local crop rotation of winter barley, a cover crop, sugar beet and winter wheat. The atmospheric CO2 concentration of ambient air is about 375 ppm with a ,13C value of ,7 to ,9,, and 550,ppm (,13C value,=,,20.2,) during daylight hours in the rings fumigated with additional CO2. Thus, the surplus C can be traced in the agricultural system. Over the course of the first experimental period (3-year crop rotation period), the C-isotopic composition and the C concentration in soil were monitored monthly. Plant samples were analysed according to the relevant developmental stages of the crop under cultivation. A 13C depletion was observed in plant parts, as well as in soil samples from the FACE rings under CO2 enrichment, indicating that labelled C has reached both respective ecosystem compartments. Albeit farming management practice (especially ploughing) leads to a mixing of ,old' and ,new' C compounds throughout all soil horizons down to the end of the ploughing layer and resulted in a heterogeneous distribution of newly formed C compounds in the soil, isotope analysis of soil C reflected where the surplus C went. [source] The effects of extreme seasonality of climate and day length on the activity budget and diet of semi-commensal chacma baboons (Papio ursinus) in the Cape Peninsula of South AfricaAMERICAN JOURNAL OF PRIMATOLOGY, Issue 2 2010A.C. van Doorn Abstract We examined the effects of extreme seasonality on the activity budget and diet of wild chacma baboons with access to a high-quality, human-derived food source. The Cape Peninsula of South Africa is unusual among nonhuman primate habitats due to its seasonal extremes in day length and climate. Winter days are markedly shorter and colder than summer days but have higher rainfall and higher primary production of annually flowering plants. This combination of fewer daylight hours but higher rainfall is substantially different from the ecological constraints faced by both equatorial baboon populations and those living in temperate climates with summer rainfall. We sought to understand how these seasonal differences affect time budgets of food-enhanced troops in comparison to both other food-enhanced troops and wild foraging troops at similar latitudes. Our results revealed significant seasonal differences in activity budget and diet, a finding that contrasts with other baboon populations with access to high-return anthropogenic foods. Similar to nonprovisioned troops at similar latitudes, troop members spent more time feeding, socializing, and traveling during the long summer days compared to the short winter days, and proportionately more time feeding and less time resting in summer compared to winter. Summer diets consisted mainly of fynbos and nonindigenous foods, whereas winter diets were dominated by annually flowering plants (mainly grasses) and ostrich pellets raided from a nearby ostrich farm. In this case, food enhancement may have effectively exaggerated seasonal differences in activity budgets by providing access to a high-return food (ostrich pellets) that was spatially and temporally coincident with abundant winter fallback foods (grasses). The frequent use of both alien vegetation and high-return, human-derived foods highlights the dietary flexibility of baboons as a key element of their overall success in rapidly transforming environments such as the South African Cape Peninsula. [source] Mutational changes in S-cone opsin genes common to both nocturnal and cathemeral Aotus monkeysAMERICAN JOURNAL OF PRIMATOLOGY, Issue 7 2007David H. Levenson Abstract Aotus is a platyrrhine primate that has been classically considered to be nocturnal. Earlier research revealed that this animal lacks a color vision capacity because, unlike all other platyrrhine monkeys, Aotus has a defect in the opsin gene that is required to produce short-wavelength sensitive (S) cone photopigment. Consequently, Aotus retains only a single type of cone photopigment. Other mammals have since been found to show similar losses and it has often been speculated that such change is in some fashion tied to nocturnality. Although most species of Aotus are indeed nocturnal, recent observations show that Aotus azarai, an owl monkey species native to portions of Argentina and Paraguay, displays a cathemeral activity pattern being active during daylight hours as frequently as during nighttime hours. We have sequenced portions of the S-cone opsin gene in A. azarai and Aotus nancymaae, the latter a typically nocturnal species. The S-cone opsin genes in both species contain the same fatal defects earlier detected for Aotus trivirgatus. On the basis of the phylogenetic relationships of these three species these results imply that Aotus must have lost a capacity for color vision early in its history and they also suggest that the absence of color vision is not compulsively linked to a nocturnal lifestyle. Campbell Abstract Flight activity of Phorodon humuli was monitored using suction traps, laboratory studies and mark and recapture experiments. Emigrants were trapped as they flew from a Myrobalan (Prunus cerasifera) hedge and among dwarf hops (Humulus lupulus). Daily flight curves were bimodal with 69% and 38% of emigrants caught in the morning peak near Myrobalan and among hops, respectively. The median period of flight activity was from 2 h after sunrise until 30 min before sunset. The lower temperature for flight was 13.5°C in the field and 14.9°C for take off in the laboratory. Variations in wind speed had little effect on flight activity explaining <2.5% of the total variance among insect counts. The percentage of emigrants on hop declined exponentially with time. The relationship, y= 10.9(±2.0) + 64.3(±2.3) × 0.92(±0.01)t where t = daylight hours (standard error in parentheses), explained 98.3% of the variance. Hence, 62% of new arrivals flew within 1 day of arrival and 79% within 2 days. Similar numbers arrived as departed at 08:30, 10:30 and 12:30 h, but at 14:30 h twice as many arrived than departed and at 16:30 h, the accumulation was threefold. Daily flight curves of return migrants and males leaving hop were bimodal with 70% and 80%, respectively, trapped in the earlier peak. In the field, the median lower temperature for flight was 13.2°C for return migrants and a nonsignificantly different 12.8°C for males. The mean temperature for take off by return migrants was 15.7°C in the laboratory. Leaf development was partitioned into seven arbitrary development stages; the time needed for leaves to progress through each developmental stage was used to measure days required to reach maturity. Measurements were made twice each week throughout 1997. Means and variances with in emergence rates of new leaves and time needed for new leaves to pass through six leaf development stages were calculated for 26 14-day periods throughout the year and compared with degree days, solar radiation, soil temperature, and daylight hours accumulated during the periods. Leaf production rates ranged from 0.15,0.73 leaves day,1 plant,1. The average time from emergence to maturity was 22.6 days. Both leaf production and development rates varied greatly with the seasons, and were strongly associated with the weather variables measured, but significant cyclic rate fluctuations unrelated to weather were also detected. Leaf emergence rates were more strongly related to average soil temperature, whereas leaf development rates to maturity were more strongly related to solar radiation and degree days. Cox Abstract The traditional on-call system for the management of acute general surgical admissions is inefficient and outdated. A new model, Acute Surgical Unit (ASU), was developed at Nepean Hospital in 2006. The ASU is a consultant-driven, independent unit that manages all acute general surgical admissions. The team has the same make up 7 days a week and functions the same every day, including weekends and public holidays. The consultant does a 24-h period of on-call, from 7 pm to 7 pm. They are on remote call from 7 pm to 7 am and are in the hospital from 7 am to 7 pm with their sole responsibility being to the ASU. The ASU has a day team with two registrars, two residents and a nurse practitioner. All patients are admitted and stay in the ASU until discharge or transfer to other units. Handover of the patients at the end of each day is facilitated by a comprehensive ASU database. The implementation of the ASU at Nepean Hospital has improved the timing of assessment by the surgical unit. There has been significant improvement in the timing of operative management, with an increased number and proportion of cases being done during daylight hours, with an associated reduction in the proportion of cases performed afterhours. There is greater trainee supervision with regard to patient assessment, management and operative procedures. There has been an improvement in the consultants' work conditions. The ASU provides an excellent training opportunity for surgical trainees, residents and interns in the assessment and management of acute surgical conditions. [source] |
