Longterm results of deep lamellar keratoplasty using grafts with endothelium

Abstract. Purpose:, To report the longterm results of deep lamellar keratoplasty (DLK) using grafts with their own endothelia. Methods:, Fourteen eyes of 14 patients underwent DLK using grafts with endothelium. The average follow-up was approximately 80.0 months. Preoperative diagnoses included: corneal leukoma (five eyes); gelatinous drop-like corneal dystrophy (three eyes); Avellino corneal dystrophy (two eyes); corneal perforation (two eyes); corneal mucopolysaccharidosis (one eye), and keratoconus (one eye). Results:, Corrected visual acuity was improved in 13 eyes (93%), but ruptures of Descemet's membrane occurred in six eyes (43%) and a double anterior chamber was found in five eyes (36%) postoperatively. Despite this, all grafts remained clear as a result of their functioning endothelia. Conclusions:, Deep lamellar keratoplasty using a graft with its own endothelium is a safe and valuable procedure with flexibility and feasibility that should suit corneal surgeons of all levels. Case of recurrent macular haemorrhage after removal of a sub-internal limiting membrane haematoma secondary to retinal artery macroaneurysm

Abstract The authors report the case of an 80-year-old man with sub-internal limiting membrane haematoma secondary to retinal artery macroaneurysm in the right eye. Corrected visual acuity was 6/60 in the right eye. Vitreous surgery was performed. The internal limiting membrane over the haematoma was removed by pulling with a soft-tipped extrusion cannula, and then the haematoma was removed with a vitreous cutter. One month post surgery vision had improved to 6/9. Two months post surgery, however, the same macroaneurysm ruptured again, and vision decreased to 6/60. Clinicians should be aware that recurrent macular haemorrhage may occur after removal of sub-internal limiting membrane haematoma secondary to macroaneurysm. [source]


Aetiology of childhood vision impairment, metropolitan Atlanta, 1991,93

Data from the population-based Metropolitan Atlanta Developmental Disabilities Surveillance Program (MADDSP) were used to describe the underlying causes of vision impairment (VI; corrected visual acuity in the better eye of 20/70 or worse) in young children (n = 228) in metropolitan Atlanta in 1991,93. Children with VI were identified through record review at multiple educational and medical sources. Children were categorised as having isolated VI or multiple disabilities (i.e. VI plus one or more of four additional developmental disabilities) and as having low vision (visual acuity 20/70,20/400) or blindness (visual acuity worse than 20/400). Medical conditions abstracted from MADDSP sources were reviewed to determine the probable aetiology of a child's VI. Aetiologies were assigned to one of three developmental time periods: prenatal, perinatal, or postnatal. Prenatal aetiologies were identified in 43% of the children; 38% of the prenatal aetiologies were genetic. Perinatal aetiologies were found in 27% of the children. Postnatal aetiologies were rare. Prenatal aetiologies were more common in children with isolated VI; perinatal and postnatal aetiologies were more common in children with multiple disabilities. Children with prenatal aetiologies tended to have less severe vision loss than did children with perinatal or postnatal aetiologies. The distribution varied by birthweight, but did not differ significantly by sex or race. [source]


Predictable signs of benign course of polypoidal choroidal vasculopathy: based upon the long-term observation of non-treated eyes

Abstract. Purpose:, To find predictable signs of benign polypoidal choroidal vasculopathy (PCV). Methods:, Medical records of 13 eyes from 12 patients who were followed up for 5 years or longer without treatment among 258 consecutive patients with PCV were reviewed retrospectively. The main outcomes measured were best corrected visual acuity (BCVA) and fundus findings during the follow-up period. Results:, The average age at presentation was 68 years, and the average follow-up period after diagnosis was 80 months (range, 62,119 months). The initial mean logarithmic value of the minimal angle of resolution (logMAR) BCVA was 0.28 ± 0.26, and the final mean logMAR BCVA was 0.62 ± 0.72. The difference in the logMAR BCVA values between the two points was not statistically significant (p > 0.05). The trend of change from baseline at 2-year follow-up was consistent with those at 5-year follow-up in nine eyes. Fundus findings at the initial examination were classified into two patterns: (i) reddish-orange nodules and detachment of the retinal pigment epithelium with/without detachment of the neurosensory retina (nine eyes); (ii) reddish-orange nodules alone, or nodules and small subretinal haemorrhage (four eyes). In the eyes with the first pattern, clinical course and visual prognosis were variable. An absence of hard exudates could be a sign to maintain a benign clinical course or stable vision with this pattern. The eyes with the second pattern took a benign clinical course with stable vision. Conclusions:, There is certainly a group of PCV eyes with a benign prognosis. Considering the huge cost and risk of current therapies, the initial ocular findings could be deciding factors that determine the necessity for further treatment. [source]
