Complex nature of the human antisperm antibody response in SCID mice

Summary. Human peripheral blood mononuclear (PBMs) cells were introduced into the peritoneal cavity of severely-combined immunodeficient (SCID) mice in concentrations of 2.5,4.0 × 107 cells per mouse. Whole mononuclear cell suspensions were used either unstimulated or following primary in vitro culture with human spermatozoa. In some experiments, immunodepletion of CD8+ cells was carried out prior to grafting. Lymphocytes were obtained from nonsensitized (to antigen) human subjects or from individuals who were primed in vivo (vasectomized individuals in case of sperm antigens). An enzyme-linked immunosorbent assay was employed to assess total human immunoglobulin (G or M) levels as well as the specificity of the antibodies generated. We have been successful by generating primary and secondary immune responses with ,naďve' human lymphocytes, challenged with chlamydia or ovalbumin but without adjuvant or CD8+ immunodepletion; however, we were unable to induce specific antibodies to spermatozoa under this regime in SCID male mice. We then employed female SCID mice, treated with sperm antigen extracts (glycosylated or deglycosylated) encapsulated in liposomes and human lymphocytes obtained from ,naďve' or pre-sensitized in vivo subjects. It was found that the most pronounced humoral response to sperm antigens was obtained with deglycosylated antigens and PBMs from vasectomized (in vivo pre-primed to spermatozoa) individuals. A presented SCID mice model can be helpful at understanding of antisperm antibody development and the molecular nature of generated antibodies to modified sperm antigenic entities. [source]


Illustrative White Matter Fiber Bundles

Abstract Diffusion Tensor Imaging (DTI) has made feasible the visualization of the fibrous structure of the brain white matter. In the last decades, several fiber-tracking methods have been developed to reconstruct the fiber tracts from DTI data. Usually these fiber tracts are shown individually based on some selection criteria like region of interest. However, if the white matter as a whole is being visualized clutter is generated by directly rendering the individual fiber tracts. Often users are actually interested in fiber bundles, anatomically meaningful entities that abstract from the fibers they contain. Several clustering techniques have been developed that try to group the fiber tracts in fiber bundles. However, even if clustering succeeds, the complex nature of white matter still makes it difficult to investigate. In this paper, we propose the use of illustration techniques to ease the exploration of white matter clusters. We create a technique to visualize an individual cluster as a whole. The amount of fibers visualized for the cluster is reduced to just a few hint lines, and silhouette and contours are used to improve the definition of the cluster borders. Multiple clusters can be easily visualized by a combination of the single cluster visualizations. Focus+context concepts are used to extend the multiple-cluster renderings. Exploded views ease the exploration of the focus cluster while keeping the context clusters in an abstract form. Real-time results are achieved by the GPU implementation of the presented techniques. Corporate social responsibility and the mining industry: conflicts and constructs

In response to widespread and increasing criticism, the mining industry has started to pay serious attention to its environmental and social impacts. This has recently manifested itself in the formulation of corporate social responsibility (CSR) policies and strategies and a proliferation of CSR, environmental, sustainability and community reporting. Several brief case studies are used to illustrate the issues and conflicts that arise between mining company operations and the environment and community, and how these have led to the development of corporate strategies to deal with environment and community issues. The paper then examines mining company reports, revealing the language and constructs used by the mining industry to frame its responsibility to the environment and community and role in possible conflicts. Companies need to better understand the complex nature of the communities in which they operate in order that suitably tailored strategies are developed. Tanning and Cutaneous Malignancy

BACKGROUND Exposure to ultraviolet radiation (UVR) results in a darkening of the skin known as tanning. Recently, it has been shown that tanning is a response to UVR-induced DNA damage and represents the skin's efforts to protect itself against further injury. Despite the link between UVR and cutaneous malignancy, people continue to pursue tanning from natural and artificial sources. This trend is reflected in the exponential rise in skin cancer incidence. OBJECTIVE The objective of this study was to review our current understanding of the factors controlling the tanning response and the relationship to cutaneous carcinogenesis, as well as the impact that the multibillion dollar tanning industry has had on the practice of dermatology. MATERIALS AND METHODS Extensive literature review was conducted in subjects related to tanning and the relationship to cutaneous malignancy. RESULTS Our knowledge of tanning and its effects on the skin has increased tremendously. It is clear that tanning contributes to the development of skin cancer. Despite this information, the incidence of skin cancer continues to increase exponentially. CONCLUSIONS Skin cancer poses a major public health concern and tanning remains the most modifiable risk factor in its etiology. Social, economic, and legislative issues have become tightly intertwined with the complex nature of human behavior in the continued pursuit of an activity that clearly has detrimental effects on one's health. Multiple sensitive periods in human visual development: Evidence from visually deprived children

Abstract Psychophysical studies of children deprived of early visual experience by dense cataracts indicate that there are multiple sensitive periods during which experience can influence visual development. We note three sensitive periods within acuity, each with different developmental time courses: the period of visually-driven normal development, the sensitive period for damage, and the sensitive period for recovery. Moreover, there are different sensitive periods for different aspects of vision. Relative to the period of visually driven normal development, the sensitive period for damage is surprisingly long for acuity, peripheral vision, and asymmetry of optokinetic nystagmus, but surprisingly short for global motion. A comparison of results from unilaterally versus bilaterally deprived children provides insights into the complex nature of interactions between the eyes during normal visual development. © 2005 Wiley Periodicals, Inc. Ammonium perchlorate effects on thyroid function and growth in bobwhite quail chicks

Abstract Bobwhite quail chicks were used to investigate ammonium perchlorate (AP; NH4ClO4) effects on thyroid function and growth. Beginning at 3 to 4 d posthatch, we evaluated organismal thyroid status (circulating hormones), activation of the hypo-thalamic-pituitary-thyroid axis (thyroid wt) and thyroidal hormone content over a wide range of AP concentrations (50 ,g/L , 4,000 mg/L) in drinking water, for relatively short (2-week) and longer (8-week) exposures. Thyroidal thyroxine (T4) content, the most sensitive index of decreased thyroid function, decreased markedly in response to increasing perchlorate exposure. Thyroid weight and plasma T4 were less sensitive indicators and similar in their ability to detect thyroid changes. Growth measurements (body wt and skeletal growth) were very insensitive indices. Because thyroids contain large hormone stores, with low exposures or short time periods, these stores can be used to maintain circulating hormones, at least temporarily. Most depletion of thyroidal T4 occurred during the first two weeks of AP exposure. Subsequent decreases were at a slower rate presumably because thyrotropin stimulation of the thyroids at least partially compensated for some of the perchlorate effect. Additional studies of the interactions between AP concentration and exposure time are needed for understanding the complex nature of thyroid responses to perchlorate. [source]


The concept and status of trait in research on temperament

The aim of the paper is to show that research on temperament is inescapably bound with the concept of trait as applied in personality research. It is the individual differences approach on which temperament studies are based, and traits are the basic units by means of which these differences are described. Taking as a point of departure the definition of trait understood as a relatively stable and individual-specific generalized tendency to behave or react in a certain way expressed in a variety of situations, the hypothetical status of temperament traits is discussed. Special attention is paid to states and behaviour by means of which temperament traits are inferred as well as to the biological and environmental determinants of these traits. Temperamental traits constitute only a part of the personality structure viewed from the perspective of individual differences and this perspective is only one of the many from which the complex nature of personality should be viewed. ,Somatic Styles' examines how classical modes of gender played significant roles in carving out competitive arenas between clerical and lay elites, c.600,900 CE. The paper explores the hermeneutical obstacles standing between the contemporary theorist of gender and the complex nature of the early medieval texts under scrutiny. The analysis reconstructs classicising techniques of gender deployed by early medieval churchmen, and it does so in a way that both challenges the stranglehold of the ,one-sex' model on pre-modern understandings of gender and heals the ,rupture' between the ,Ancient' and the ,Dark Age'. Finally, the essay maps early medieval somatic and gendered styles onto an architectural space where lay and consecrated bodies met , a ninth-century monastic basilica. [source]


Large-scale climatic signatures in lakes across Europe: a meta-analysis

Abstract Recent studies have highlighted the impact of the winter North Atlantic Oscillation (NAO) on water temperature, ice conditions, and spring plankton phenology in specific lakes and regions in Europe. Here, we use meta-analysis techniques to test whether 18 lakes in northern, western, and central Europe respond coherently to winter climate forcing, and to assess the persistence of the winter climate signal in physical, chemical, and biological variables during the year. A meta-analysis approach was chosen because we wished to emphasize the overall coherence pattern rather than individual lake responses. A particular strength of our approach is that time-series from each of the 18 lakes were subjected to the same robust statistical analysis covering the same 23-year period. Although the strongest overall coherence in response to the winter NAO was exhibited by lake water temperatures, a strong, coherent response was also exhibited by concentrations of soluble reactive phosphorus and soluble reactive silicate, most likely as a result of the coherent response exhibited by the spring phytoplankton bloom. Lake nitrate concentrations showed significant coherence in winter. With the exception of the cyanobacterial biomass in summer, phytoplankton biomass in all seasons was unrelated to the winter NAO. A strong coherence in the abundance of daphnids during spring can most likely be attributed to coherence in daphnid phenology. A strong coherence in the summer abundance of the cyclopoid copepods may have been related to a coherent change in their emergence from resting stages. We discuss the complex nature of the potential mechanisms that drive the observed changes. [source]


Increased rainfall variability and reduced rainfall amount decreases soil CO2 flux in a grassland ecosystem

Abstract Predicted climate changes in the US Central Plains include altered precipitation regimes with increased occurrence of growing season droughts and higher frequencies of extreme rainfall events. Changes in the amounts and timing of rainfall events will likely affect ecosystem processes, including those that control C cycling and storage. Soil carbon dioxide (CO2) flux is an important component of C cycling in terrestrial ecosystems, and is strongly influenced by climate. While many studies have assessed the influence of soil water content on soil CO2 flux, few have included experimental manipulation of rainfall amounts in intact ecosystems, and we know of no studies that have explicitly addressed the influence of the timing of rainfall events. In order to determine the responses of soil CO2 flux to altered rainfall timing and amounts, we manipulated rainfall inputs to plots of native tallgrass prairie (Konza Prairie, Kansas, USA) over four growing seasons (1998,2001). Specifically, we altered the amounts and/or timing of growing season rainfall in a factorial combination that included two levels of rainfall amount (100% or 70% of naturally occurring rainfall quantity) and two temporal patterns of rain events (ambient timing or a 50% increase in length of dry intervals between events). The size of individual rain events in the altered timing treatment was adjusted so that the quantity of total growing season rainfall in the ambient and altered timing treatments was the same (i.e. fewer, but larger rainfall events characterized the altered timing treatment). Seasonal mean soil CO2 flux decreased by 8% under reduced rainfall amounts, by 13% under altered rainfall timing, and by 20% when both were combined (P<0.01). These changes in soil CO2 flux were consistent with observed changes in plant productivity, which was also reduced by both reduced rainfall quantity and altered rainfall timing. Soil CO2 flux was related to both soil temperature and soil water content in regression analyses; together they explained as much as 64% of the variability in CO2 flux across dates under ambient rainfall timing, but only 38,48% of the variability under altered rainfall timing, suggesting that other factors (e.g. substrate availability, plant or microbial stress) may limit CO2 flux under a climate regime that includes fewer, larger rainfall events. An analysis of the temperature sensitivity of soil CO2 flux indicated that temperature had a reduced effect (lower correlation and lower Q10 values) under the reduced quantity and altered timing treatments. Recognition that changes in the timing of rainfall events may be as, or more, important than changes in rainfall amount in affecting soil CO2 flux and other components of the carbon cycle highlights the complex nature of ecosystem responses to climate change in North American grasslands. Making schools and young people responsible: a critical analysis of Ireland's obesity strategy

Abstract Worldwide reports of an obesity ,epidemic' prompted the Irish government to appoint a multidisciplinary Taskforce, whose report was published in May 2005. This paper critically analyses the report and its recommendations for reducing health risks among families, children and young people. Using a Foucauldian perspective, we question the report's individualizing focus and support for a strategy which responsibilises schools, families and young people and relies on individuals to do ,the right thing'. Specifically, we examine the Taskforce recommendations for the education sector, and identify their dependence upon a discourse of governmentality, under categories of individualization, responsibilization and freedom of choice, participation and techniques for management of the self. We conclude that the report fails to address the multi-faceted and complex nature of obesity, and obscures the social, economic and material realities of the lives of pupils and schools. [source]


FROM IDEAS TO CONCEPTS TO METAPHORS: THE GERMAN TRADITION OF INTELLECTUAL HISTORY AND THE COMPLEX FABRIC OF LANGUAGE

ABSTRACT Recently, the diffusion of the so-called "new intellectual history" led to the dismissal of the old school of the "history of ideas" on the basis of its ahistorical nature (the view of ideas as eternal entities). This formulation is actually misleading, missing the core of the transformation produced in the field. It is not true that the history of ideas simply ignored the fact that the meaning of ideas changes over time. The issue at stake here is really not how ideas changed (the mere description of the semantic transformation they underwent historically), but rather why they do. The study of the German tradition of intellectual history serves in this essay as a basis to illustrate the meaning and significance of the recent turn from ideas as its object. In the process of trying to account for the source of contingency of conceptual formations, it will open our horizon to the complex nature of the ways by which we invest the world with meaning. That is, it will disclose the presence of different layers of symbolic reality lying beneath the surface level of "ideas," and analyze their differential nature and functions. It will also show the reasons for the ultimate failure of the "history of ideas" approach, why discourses can never achieve their vocation to constitute themselves as self-enclosed, rationally integrated systems, thereby expelling contingency from their realm. In sum, it will show why historicity is not merely something that comes to intellectual history from without (as a by-product of social history or as the result of the action of an external agent), as the history of ideas assumed, but is a constitutive dimension of it. Laparoscopic pancreatic surgery: a review of present results and future prospects

Abstract Pancreatic surgery is still associated with a relatively high morbidity and mortality compared with other specialties. This is a result of the complex nature of the organ, the difficult access as a result of the retroperitoneal position and the number of technically challenging anastomoses required. Nevertheless, the past two decades have witnessed a steady improvement in morbidity and a decrease in mortality achieved through alterations of technique (particularly relating to the pancreatic anastomoses) together with hormonal manipulation to decrease pancreatic secretions. Recently minimally invasive pancreatic surgery has been attempted by several centres around the world which has stimulated considerable interest in this approach. The majority of the cases attempted have been distal pancreatectomies, because of the more straightforward nature of the resection and the lack of a pancreatic ductal anastomosis, but more recently reports of laparoscopic pancreaticoduodenectomy have started to appear. The reports of the series to date have been difficult to interpret and although the results are claimed to be equivalent or better than those associated with a traditional approach a careful examination of the literature and comparison with the best results previously reported does not presently support this. In the present review we examined all the reports of pancreatic procedures performed laparoscopically and compared the results with those previously achieved at open surgery. [source]


Characterizing instantaneous phase relationships in whole-brain fMRI activation data

Abstract Typically, fMRI data is processed in the time domain with linear methods such as regression and correlation analysis. We propose that the theory of phase synchronization may be used to more completely understand the dynamics of interacting systems, and can be applied to fMRI data as a novel method of detecting activation. Generalized synchronization is a phenomenon that occurs when there is a nonlinear functional relationship present between two or more coupled, oscillatory systems, whereas phase synchronization is defined as the locking of the phases while the amplitudes may vary. In this study, we developed an application of phase synchronization analysis that is appropriate for fMRI data, in which the phase locking condition is investigated between a voxel time series and the reference function of the task performed. A synchronization index is calculated to quantify the level of phase locking, and a nonparametric permutation test is used to determine the statistical significance of the results. We performed the phase synchronization analysis on the data from five volunteers for an event-related finger-tapping task. Functional maps were created that provide information on the interrelations between the instantaneous phases of the reference function and the voxel time series in a whole-brain fMRI activation data set. Major affective disorders and schizophrenia: a common molecular signature?,

Abstract Psychiatric disorders, including affective disorders (AD) and schizophrenia (SZ) are among the most common disabling brain diseases in Western populations and result in high costs in terms of morbidity as well as mortality. Although their etiology and pathophysiology is largely unknown, family-, twin-, and adoption studies argue for a strong genetic determination of these disorders. These studies indicate that there is between 40 and 85% heritability for these disorders but point also to the importance of environmental factors. Therefore, any research strategy aiming at the identification of genes involved in the development of AD and SZ should account for the complex nature (multifactorial) of these disorders. During the last decade, molecular genetic studies have contributed a great deal to the identification of genetic factors involved in complex disorders. Here we provide a comprehensive review of the most promising genes for AD and SZ, and the methods and approaches that were used for their identification. Also, we discuss the current knowledge and hypotheses that have been formulated regarding the effect of variations on protein functioning as well as recent observations that point to common molecular mechanisms. The genetic and immunopathological processes underlying collagen-induced arthritis

Summary Animal models of rheumatoid arthritis (RA) have provided substantial insights into basic pathogenic mechanisms of chronic inflammatory arthritis and autoimmune disease in general. Of the variety of models reported, collagen-induced arthritis (CIA) has been the most characterized in terms of both its pathogenesis and its underlying immunological basis. Collagen-induced arthritis has also been the model of choice in terms of testing potential new therapeutic agents for the treatment of human RA. Nevertheless, the complex nature of the balance between T-cell cytokines and the chronic inflammatory processes is only recently becoming clear. This review focuses on these developments, highlighting their implications for our understanding of RA and for the use of CIA as a suitable animal model. [source]


Optimum design of stochastically excited non-linear dynamic systems without geometric constraints

Abstract This paper presents an efficient procedure for min,max dynamic response optimization of stochastically excited non-linear systems with multiple time-delayed inputs. This procedure employs a stochastic linearization technique to overcome system non-linearity and an auto-covariance analysis technique to represent the original stochastic mechanical model in a suitable form for optimization. Special attention is given to the sensitivity analysis, due to the complex nature of the problem. Therefore, exact expressions are obtained in a simple form for the evaluation of the required gradients, which greatly improve the stability and efficiency of the optimization algorithm. Clinical value of p53, c-erbB-2, CEA and CA125 regarding relapse, metastasis and death in resectable non-small cell lung cancer

Abstract The prognostic value of p53 and c-erbB-2 immunostaining and preoperative serum levels of CEA and CA125 was investigated in a prospective multicentric study including 465 consecutive non-small cell lung cancer (NSCLC) patients with resectable tumors. Four end-points were used: lung cancer death, first relapse (either locoregional or metastasis), loco-regional recurrence and metastasis development. Standard statistical survival methods (Kaplan-Meier and Cox regression) were used. The specificity of the prognostic effect across different types of tumors was also explored, as had been planned in advance. Our results showed, once again, that pathological T and N classifications continue to be the strongest predictors regarding either relapse or mortality. Three of the studied markers seemed to add further useful information, however, but in a more specific context. For example, increased CEA concentration defined a higher risk population among adenocarcinomas but not among people with squamous tumors; and p53 overexpression implied a worse prognosis mainly in patients with well differentiated tumors. The analysis of type of relapse proved to be very informative. Thus, CA125 level was associated with a worse prognosis mainly related with metastasis development. Another interesting result was the influence of smoking, which showed a clear dose-response relationship with the probability of metastasis. For future studies, we recommend the inclusion of different endpoints, namely considering the relationship of markers with the type of relapse involved in lung-cancer recurrence. Assessing spatial probabilistic distributional differences in the common space between schizophrenics and normal controls based on a novel automated probabilistic pattern analysis method

Abstract Because of the complex nature of the human brain, a full understanding of its various group specific variation factors such as volume, shape, and location related to age, gender, ethnic, and disease might be provided in both structural and functional neuroimaging studies. To serve this purpose, a novel approach for characterizing the group variability information using group specific labeled probabilistic maps was introduced in this article. An automatic labeling technique was applied to encode group specific probabilistic information for each region of interests (ROIs) covering the overall cortical region and a probabilistic pattern analytic method was proposed to assess the difference in the spatial extent between 70 schizophrenics and 70 controls in the common space. From our proposed method, we found major differences in 17 ROIs that had shown large variation in schizophrenics. Most of these ROIs were in the frontal and the temporal lobe and only three ROIs were in the parietal and the occipital lobe. The ROIs highlighted through our proposed method could be connected with previous morphological findings on schizophrenia and it also might be considered in functional analysis. As a result, our method could provide intuitive information on group difference relevant to the overall anatomical variability in the substructural level. Thus, it could be used as a prompting system to search and examine the regions of the brain that are worthy of further precise analysis by various sub-cortical region based group studies in assessing specific patterns related to diseases. © 2008 Wiley Periodicals, Inc. A generalized homogeneous domination approach for global stabilization of inherently nonlinear systems via output feedback

Abstract In this paper, we introduce a generalized framework for global output feedback stabilization of a class of uncertain, inherently nonlinear systems of a particularly complex nature since their linearization around the equilibrium is not guaranteed to be either controllable or observable. Based on a new observer/controller construction and a homogeneous domination design, this framework not only unifies the existing output feedback stabilization results, but also leads to more general results which have been never achieved before, establishing this methodology as a universal tool for the global output feedback stabilization of inherently nonlinear systems. Regarding the types of preventative treatments available, variations exist across all clinical studies, including differences in target populations (including the type of cerebral ischaemia), risk factors, length of follow-up, drop-out rates and outcomes, which makes translating the results of clinical trials to individual patients difficult. However, with such limitations in mind, this critical albeit nonsystematic review, which compared aspirin with other antiplatelets and in combination with other drugs, showed that the benefit from aspirin treatment is consistently shown in ischaemic stroke, while harms are limited. Furthermore, no definite superiority is apparent across different antiplatelet therapies. Dual antiplatelet regimens may expose to a slight but measurable higher risk of haemorrhagic complications, perhaps in selective groups of patients (i.e. those with severe small-vessel disease or in selective racial groups). Based on our analysis, the indication of aspirin as the first-line choice, also recommended by several acknowledged international or national guidelines, may be confirmed. However, the complex nature of patients at risk of recurrent ischaemic stroke necessitates a comprehensive approach, which should be driven by the primary care physician, whose role is central to successful actions for secondary stroke prevention. Space, sanctity and service; the English Cathedral as heterotopia

Abstract The 43 Anglican Diocesan cathedrals in England attract in excess of 30 million tourist visitors per year, but also function as museums, centres for pilgrimage and foci for the performing arts. This paper examines the complex nature of the experience offered by cathedrals to their visitors, which often generates difficulties associated with sites that may be viewed as interfaces between the sacred and the profane. It also identifies the problems presented by the requirement to earn revenue from visitors (especially by charging admission) when still offering facilities for worship, prayer or meditation. The model adopted is Foucault's concept of sacred space as heterotopia (a ritual space that exists out of time). It is argued that difficulties over admissions charges are not simply straightforward reluctance to pay up, but intrinsically related to the nature of the spiritual experience expected and received by visitors to cathedrals, whether consciously or not. How do Smuggling and Trafficking Operate via Irregular Border Crossings in the Middle East?

This article summarizes main trends, issues, actors, and activities regarding the operation and extension of human trafficking and smuggling via irregular border crossings in the Middle East. Its premise is that rather than the obvious involvement of hierarchical mafia-type organized crime groups, globally articulated networks of locally operating independent, individual groups comprise the essential foundation for human trafficking and smuggling in the region. The available empirical evidence first suggests that elaborating on various aspects of human trafficking and smuggling is a delicate task and any consideration of priorities for data collection and analysis on these activities must start with a clear idea of the information needed and how to obtain that information. Given the highly sensitive nature of trafficking and smuggling issues, there is no simple research practice that can satisfy all these concerns. It is within this context that our analysis here only offers some partial explanation of the complex nature of human trafficking and smuggling in the Middle East. The data used here provide, to the best of our knowledge, the first primary, reliable, and representative information on traffickers and smugglers as they come directly from the narratives of the traffickers and smugglers interviewed. A comprehensive 3-D analysis of polymer melt flow in slit extrusion dies

Abstract An understanding of flow behaviour of polymer melts through a slit die is extremely important for optimizing die design and, consequently, for die performance in processing polymer sheets and films. In view of the complex nature and the physical properties of polymer melts as well as of die geometries, such as coat-hanger dies, no simple mathematical formulae can be used to compute the flow regimes within dies. This paper illustrates the development of a three-dimensional (3-D) computer model of an example of a coat-hanger die design using the computational fluid dynamics package, FIDAP, based on the finite element method. A difference of only 3.7% was found when comparing the velocity distribution at the die exit obtained from the 3-D simulation with that calculated using a two-dimensional analytical design procedure, indicating that full 3-D analysis seems to be unnecessary. However it has been shown that unwanted flow phenomena and production problems can be ameliorated by means of visualization and the detailed information obtained from computer simulations. Comparative simulation results with polymers of different rheological properties in the same die are also described. The comprehensive analyses provide a means of interpretation for flow behavior, which allows modification of the die geometry for optimal design. © 2004 Wiley Periodicals, Inc. Reliability and validity of the CCNCS: a dependency workload measurement system

Aim., To refine, test and evaluate the Community Client Need Classification System (CCNCS). Background., Workload assessment in community nursing is complicated by the range of services that may be delivered in one patient interaction. The CCNCS is a workload measurement system designed to capture the direct and indirect elements of community nursing work and is suitable for use with all care groups in the community. Design., Survey. Method., Forty-four community nurses implemented the CCNCS with all clients in their caseload for four weeks. Community nursing in the Irish Republic is known as public health nursing. The Public Health Nurses (PHNs) recorded the total time in minutes that was spent on each client each week. The satisfaction with and experiences of PHNs using the CCNCS during the study period was also recorded. Results., Participants endorsed the utility of the CCNCS for use in community nursing. Inter-rater and intra-rater reliability results were positive with high level of agreement between raters in relation to scoring community clients. The amount of time the PHNs spent with clients correlated with ascending level of client need. Conclusions., The CCNCS affords insight into the complex nature of community nursing. It discriminates between levels of need and has potential to provide a standardised assessment of need in all community-nursing clients. Adequate resources are required to conduct further testing of the reliability and predictive validity of this system. Relevance to clinical practice., The CCNCS can provide objective evidence of community nursing workload and thus facilitate workforce planning. [source]


,Real Italians and wogs': The discursive construction of Italian identity among first generation Italian immigrants in Western Australia

Abstract We explore the discursive construction of Italian identity among a bilingual sample of Italian-born Western Australians. Focus groups were held with two groups: Italians who had migrated to Australia as children and a group who had migrated as adults. We found intra- and inter-individual differences in identity construction, with much discourse devoted to demonstrating Italian authenticity and negotiating ethnic category boundaries. Shared markers of authenticity included language, heritage and food. The groups varied in their selection of referent groups to make authenticity claims, with the child migrants drawing upon the shared Australian stereotype of ,wogs' to construct and authenticate their Italian-ness. In contrast, adult migrants constructed Italian identity through comparisons with the dominant Australian ethnic group and in relation to a broader ,migrant' identity. Skin hydration: a review on its molecular mechanisms

Summary Water is absolutely essential for the normal functioning of the skin and especially its outer layer, the stratum corneum (SC). Loss of water from the skin must be carefully regulated, a function dependent on the complex nature of the SC. The retention of water in the SC is dependent on two major components: (1) the presence of natural hygroscopic agents within the corneocytes (collectively referred to as natural moisturizing factor) and (2) the SC intercellular lipids orderly arranged to form a barrier to transepidermal water loss (TEWL). The water content of the SC is necessary for proper SC maturation and skin desquamation. Increased TEWL impairs enzymatic functions required for normal desquamation resulting in the visible appearance of dry, flaky skin. There have been recent discoveries regarding the complex mechanisms of skin hydration. In particular, it has been discovered that glycerol, a well-known cosmetic ingredient, exists in the SC as a natural endogenous humectant. Hyaluronan, which has been regarded mainly as dermal component, is found in the epidermis and is important for maintaining normal SC structure and epidermal barrier function. More importantly, the discovery of the existence of the water-transporting protein aquaporin-3 in the viable epidermis and the presence of tight junction structures at the junction between the stratum granulosum and SC have brought new insights into the mechanisms of skin water distribution and barrier function. As rural peoples of Central America and beyond struggle to create and access new forms of market participation and means of survival under the conditions generated by structural adjustment policies, significant social and cultural shifts are taking place at the local level. This paper analyzes on three levels the impact of maquiladora industries: the region and communities, sending households, and individuals. In particular, I address the implications of migration and urbanization for the new communities, the complex nature of diversified households, and attitudes toward industrial and agricultural work. The Couple That Prays Together: Race and Ethnicity, Religion, and Relationship Quality Among Working-Age Adults

A substantial body of research has shown that relationship quality tends to be (a) lower among racial and ethnic minorities and (b) higher among more religious persons and among couples in which partners share common religious affiliations, practices, and beliefs. However, few studies have examined the interplay of race or ethnicity and religion in shaping relationship quality. Our study addresses this gap in the literature using data from the National Survey of Religion and Family Life (NSRFL), a 2006 telephone survey of 2,400 working-age adults (ages 18,59), which contains oversamples of African Americans and Latinos. Results underscore the complex nature of the effects of race and ethnicity, as well as religious variables. In particular, we found that couples' in-home family devotional activities and shared religious beliefs are positively linked with reports of relationship quality. [source]


A Review of Nursing Research on Blood Pressure

Purpose: To provide this second 10-year review of nursing research on blood pressure (BP) and to focus attention on incorporating biopsychosocial factors affecting BP in nursing research. Organizing Construct: Blood pressure is a dynamic, multidimensional, cardiovascular indicator of a person's state rather than a one-dimensional static measurement. Methods: This 10-year literature review 1990,1999 included 54 nursing research articles with BP as an outcome measure. Four nursing research journals were reviewed to identify all nurse-authored articles investigating BP as an outcome variable in adult populations. Inclusion of individual characteristics, environmental factors, dynamic nature of blood pressure, and interpersonal aspects of blood pressure were assessed for each article. Findings: Age, gender, and health status were mentioned consistently in both decades. Reporting of socioeconomic, occupational, educational, activity, and martial status remained low. Descriptions of environments increased, and automated devices were the most common method for BP assessment. Less than half of the articles included a description of the person measuring the BP. Measurement of BP under multiple conditions increased, but measurement within conditions did not. Conclusions: Advances in technology and data analysis have increased knowledge of the dynamic nature of BP, but recognition of the complex nature of BP has not progressed rapidly over the last 2 decades.
