    SENTIMENT CLASSIFICATION of MOVIE REVIEWS USING CONTEXTUAL VALENCE SHIFTERS

    We present two methods for determining the sentiment expressed by a movie review. The semantic orientation of a review can be positive, negative, or neutral. We examine the effect of valence shifters on classifying the reviews. We examine three types of valence shifters: negations, intensifiers, and diminishers. Negations are used to reverse the semantic polarity of a particular term, while intensifiers and diminishers are used to increase and decrease, respectively, the degree to which a term is positive or negative. The first method classifies reviews based on the number of positive and negative terms they contain. We use the General Inquirer to identify positive and negative terms, as well as negation terms, intensifiers, and diminishers. We also use positive and negative terms from other sources, including a dictionary of synonym differences and a very large Web corpus. To compute corpus-based semantic orientation values of terms, we use their association scores with a small group of positive and negative terms. We show that extending the term-counting method with contextual valence shifters improves the accuracy of the classification. The second method uses a Machine Learning algorithm, Support Vector Machines. We start with unigram features and then add bigrams that consist of a valence shifter and another word. The accuracy of classification is very high, and the valence shifter bigrams slightly improve it. The features that contribute to the high accuracy are the words in the lists of positive and negative terms. Previous work focused on either the term-counting method or the Machine Learning method. We show that combining the two methods achieves better results than either method alone. [source]


    DEFINING DYSKARYOSIS , THE BSCC CLASSIFICATION IN 2006

    The terminology used for reporting cervical samples in the UK is the BSCC classification, which has evolved over many years. In 2002 the BSCC held a consensus conference to review the BSCC classification, with the intention of providing clearer results for women, improving concordance with other terminologies and facilitating consistency with new scientific developments and technologies. The consensus conference was well attended and robust. In the intervening years there have also been other further advances on morphometry, data on outcomes, data from EQA and other sources. Liquid Based cytology (LBC) has been implemented by the NHS CSP. All of these developments have impacted on the proposed classification, which will be presented in full. The term ,dyskaryosis' will be retained and several of the current reporting categories will be relatively unchanged, though additional information on LBC will be provided. The major proposed changes are: (1) A move to a single category of ,High Grade Dyskaryosis' to replace the existing categories of moderate and severe dyskaryosis. (2) Sub-division of Borderline change into three categories. Borderline change in glandular cells Borderline change ?high grade Borderline change, NOS (3) The current grades of Mild Dyskaryosis and Borderline change with Koilocytosis to be merged. Details of these proposals, together with illustrations and the evidence base for change will be presented. [source]


    MANAGEMENT OF OBSCURE GASTROINTESTINAL BLEEDING BASED ON THE CLASSIFICATION OF CAPSULE ENDOSCOPIC BLEEDING FINDINGS

    Background:, Double-balloon endoscopy (DBE) and capsule endoscopy (CE) have been useful in managing obscure gastrointestinal bleeding (OGIB). However, DBE is invasive, complex and time-consuming, therefore indications should probably be selective. The aim of this study was to evaluate the usefulness of the classification of the CE bleeding findings for determining the indications and timing of DBE in patients with OGIB. Methods:, From February 2003 to January 2009, 123 patients with OGIB who underwent CE were included in this study. These CE findings were classified based on the bleeding source. Type CE-I, II, III, IV and 0 indicate active bleeding, previous bleeding, lesions without active bleeding, a lesion outside of the small bowel, and no findings, respectively. We compared diagnostic yield and outcome between the classification and the findings of DBE or enteroclysis. Results:, Comparisons of the positive findings rate with DBE or enteroclysis, the treatment rate and the rebleeding rate with the classification showed: CE-Ia, 100% (6/6), 50% (3/6), 33.3% (2/6); Ib, 66.7% (4/6), 0% (0/6), 16.7% (1/6); IIa, 33.3% (1/3), 33.3% (1/3), 33.3% (1/3); IIb, 53.8% (7/13),15.4% (2/13), 30.8% (4/13); III, 100% (84/84), 9.5% (8/84), 8.3% (7/84); IV, 100% (2/2), 50% (1/2), 0% (0/2); and 0, 0% (0/9), 0% (0/9), 0% (0/9), respectively. Conclusions:, The proportion of patients requiring treatment, the positive findings rate with DBE or enteroclysis and the rebleeding rates tended to be higher in the higher ranked classification types (CE-I > II > III > IV > 0). These findings suggest that the classification can provide useful information on determining the indications and timing of DBE. [source]

    SEISMIC FACIES ANALYSIS BASED ON 3D MULTI-ATTRIBUTE VOLUME CLASSIFICATION, DARIYAN FORMATION, SE PERSIAN GULF

    Interpretation of recently acquired 3D seismic data from the adjacent Sirri C and D oilfields in the SE Persian Gulf indicates that a 3D interpretation of seismic facies is crucial to resolve the internal stratal geometries of the Aptian Dariyan Formation. This carbonate formation passes southward into the Shu'aiba Formation, a prolific reservoir rock of similar facies in the UAE. Lack of exposures and limited cored intervals have forced reliance on the seismic data for evidence of the depositional environment and the internal architecture of potential reservoir rocks. The progradational nature of the Dariyan Formation and the occurrence of carbonate build-ups within it make this stratal geometry complex. The complex internal heterogeneity of the build-ups and presence of seismic noise make mapping of the build-ups in 3D space using conventional seismic interpretation tools difficult, despite the availability of high-quality 3D seismic data covering the area. The high quality seismic and limited well data from this field is one of the few datasets of this kind presented in the literature. A procedure for the hierarchical multi-attribute analysis of seismic facies using Paradigm's Seis Facies software is used in this study to provide a 3D interpretation of the stratal patterns. Principal component analysis reduces the noise and redundant data by representing the main data variances as a few vector components in a transformed coordinate system. Cluster analysis is performed using those components which have the greatest contribution to the maximum spread of the data variability. Six seismic attribute volumes are used in this study and the result is a single 3D classified volume. Important new information obtained from within the Dariyan Formation gives new insights into its stratigraphic distribution and internal variability. This method of processing seismic data is a step towards exploring for subtle stratigraphic traps in the study area, and may help to identify exploration targets. [source]


    A NEW PARADIGM FOR FRESHWATER FRAGILARIOID DIATOM CLASSIFICATION?

This new system is antagonistic to that presented by Williams and Round (1987) because Lange-Bertalot recognizes a marked variability in the characters chosen and a supposed overall continuity of morphological features among the genera created by his English counterparts. Lange-Bertalot then proposes a partitioning of Fragilaria into two genera: Fragilaria and Staurosira mainly based on the presence/absence of rimoportulae and areolate girdle bands. The newly defined Fragilaria includes relatively large phytoplankters such as F. capucina and F. crotonensis. In turn, Staurosira includes, for the most part, small periphytic organisms, and contains several new species that were based on varieties of old Fragilaria taxa. This fragmentation of species and their varieties is based on a supposed morphological discontinuity. As a consequence an apparent increase in species diversity has occurred within the fragilarioid group. The present work analyzes Lange-Bertalot's new paradigm and confronts it with recent LM and SEM evidence. The incorporation of concepts such as plasticity, polymorphism, and parallel evolution in current classification systems is also discussed. It is concluded that Lange-Bertalot's system represents a step backward from that of Williams and Round. Some adjustments in the latter scheme could be sufficient to accommodate the diversity of fragilarioids known at present. [source]

    AMERICAN SOCIETY OF ANESTHESIOLOGISTS CLASSIFICATION OF PHYSICAL STATUS AS A PREDICTOR OF WOUND INFECTION

    Background: Wound infection occurs when bacterial contamination overcomes the hosts' defences against bacterial growth. Wound categories are a measurement of wound contamination. The American Society of Anesthesiologists (ASA) classification of physical status may be an effective indirect measurement of the hosts' defence against infection. This study examines the association between the ASA score of physical status and wound infection. Methods: A retrospective review of a prospective study of antibiotic prophylaxis was carried out. Patients with a documented ASA score who received optimal prophylactic antibiotics were included. The anaesthetist scored the ASA classification of physical status in theatre. Other risk factors for wound infection were also documented. Patients were assessed up to 30 days postoperatively. Results: Of 1013 patients there were 483 with a documented ASA score. One hundred and one may not have received optimal prophylaxis, leaving a database of 382 patients. There were 36 wound infections (9.4%). Both the ASA classification of physical status (P = 0.002) and the wound categories (P = 0.034) significantly predicted wound infection. The duration of surgery, patient's age, acuteness of surgery and the organ system being operated on did not predict wound infection. On logistic regression analysis the ASA score was the strongest predictor of wound infection. Conclusion: When effective prophylactic antibiotics were used the ASA classification of physical status was the most significant predictor of wound infection. [source]


    PRIMARY PERITONITIS IN ADULTS: IS IT TIME TO LOOK FOR A BETTER DIAGNOSTIC CLASSIFICATION?

    Background: The occurrence of primary peritonitis is rare. The experience of a single institution and the definition of this disease are reviewed. Methods: A retrospective audit of primary peritonitis at a single institution was undertaken. Age, sex, clinical presentation, initial diagnosis, imaging and blood tests, comorbidities, recent medical intervention, surgical treatment and operative findings, antibiosis, bacterial isolates, the course of hospital treatment and follow up, including later diagnoses, were reviewed. Results: The audit was carried out at Christchurch Hospital between January 1998 and March 2005. A total of 115 patients were identified with a diagnosis of non-specific peritonitis, of which five patients were identified with a diagnosis of primary peritonitis. All were women aged 22,38 years. Three had undergone a caesarean section in the last 3 months. The details of these cases are presented in detail. Conclusion: Five cases of primary peritonitis are reported, and from a review of published work, a classification of primary peritonitis is suggested, which more clearly identifies the cause of the disease.     A PETROARCHAEOLOGICAL ANALYSIS AND CHARACTERIZATION OF FLINT RAW MATERIALS FROM THE IBERIAN NEOLITHIC MINE OF CASA MONTERO
    Casa Montero is a mining complex located outside Madrid (Spain), dated from the Early Neolithic (c. 5400,5000 cal bc). An area of some 4 ha has been investigated and some 4000 shafts recorded, of which 324 have been excavated. The characterization of its raw flint materials and the establishment of its diagnostic features are indispensable in the reconstruction of the distribution of the mine's products beyond the immediate site. This work reports the geological study of the mine's Miocene flint layers and their petrological characterization. Archaeological samples from the mine's shafts were classified according to macroscopic features and petrological characteristics. [source]

    Feature selection is an essential data processing step to remove irrelevant and redundant attributes for shorter learning time, better accuracy, and better comprehensibility. A number of algorithms have been proposed in both data mining and machine learning areas. These algorithms are usually used in a single table environment, where data are stored in one relational table or one flat file. They are not suitable for a multi-relational environment, where data are stored in multiple tables joined to one another by semantic relationships. To address this problem, in this article, we propose a novel approach called,FARS,to conduct both,Feature,And,Relation,Selection for efficient multi-relational classification. Through this approach, we not only extend the traditional feature selection method to select relevant features from multi-relations, but also develop a new method to reconstruct the multi-relational database schema and eliminate irrelevant tables to improve classification performance further. The results of the experiments conducted on both real and synthetic databases show that,FARS,can effectively choose a small set of relevant features, thereby enhancing classification efficiency and prediction accuracy significantly.     Evaluation Metrics in Classification: A Quantification of Distance-Bias

    This article provides a characterization of bias for evaluation metrics in classification (e.g., Information Gain, Gini, ,2, etc.). Our characterization provides a uniform representation for all traditional evaluation metrics. Such representation leads naturally to a measure for the distance between the bias of two evaluation metrics. We give a practical value to our measure by observing the distance between the bias of two evaluation metrics and its correlation with differences in predictive accuracy when we compare two versions of the same learning algorithm that differ in the evaluation metric only. Experiments on real-world domains show how the expectations on accuracy differences generated by the distance-bias measure correlate with actual differences when the learning algorithm is simple (e.g., search for the best single feature or the best single rule). The correlation, however, weakens with more complex algorithms (e.g., learning decision trees). Our results show how interaction among learning components is a key factor to understand learning performance. [source]


    Fast Volume Rendering and Data Classification Using Multiresolution in Min-Max Octrees

    Large-sized volume datasets have recently become commonplace and users are now demanding that volume-rendering techniques to visualise such data provide acceptable results on relatively modest computing platforms. The widespread use of the Internet for the transmission and/or rendering of volume data is also exerting increasing demands on software providers. Multiresolution can address these issues in an elegant way. One of the fastest volume-rendering alrogithms is that proposed by Lacroute & Levoy 1 , which is based on shear-warp factorisation and min-max octrees (MMOs). Unfortunately, since an MMO captures only a single resolution of a volume dataset, this method is unsuitable for rendering datasets in a multiresolution form. This paper adapts the above algorithm to multiresolution volume rendering to enable near-real-time interaction to take place on a standard PC. It also permits the user to modify classification functions and/or resolution during rendering with no significant loss of rendering speed. A newly-developed data structure based on the MMO is employed, the multiresolution min-max octree, M 3 O, which captures the spatial coherence for datasets at all resolutions. Speed is enhanced by the use of multiresolution opacity transfer functions for rapidly determining and discarding transparent dataset regions. Some experimental results on sample volume datasets are presented. [source]


    Reference-Free Damage Classification Based on Cluster Analysis

    The ultimate goal of this study was to develop an in-site non-destructive testing (NDT) technique that can continuously and autonomously inspect the bonding condition between a carbon FRP (CFRP) layer and a host reinforced concrete (RC) structure, when the CRFP layer is used for strengthening the RC structure. The uniqueness of this reference-free NDT is two-fold: First, features, which are sensitive to CFRP debonding but insensitive to operational and environmental variations of the structure, have been extracted only from current data without direct comparison with previously obtained baseline data. Second, damage classification is performed instantaneously without relying on predetermined decision boundaries. The extraction of the reference-free features is accomplished based on the concept of time reversal acoustics, and the instantaneous decision-making is achieved using cluster analysis. Monotonic and fatigue load tests of large-scale CFRP-strengthened RC beams are conducted to demonstrate the potential of the proposed reference-free debonding monitoring technique. Based on the experimental studies, it has been shown that the proposed reference-free NDT technique may minimize false alarms of debonding and unnecessary data interpretation by end users. [source]


    Spatial Information: Classification and Applications in Building Design

    Physical properties of building components are usually represented in building data models by their three-dimensional geometry and topology,also called spatial information. While geometric data of building components can be manipulated and managed by a computer-aided design (CAD) interface, their spatial relations (or topological information) are conventionally represented in a manual fashion into data models. The manual data representation, however, is inherently a complex and challenging task due to the wide variety of spatial relationships. Thus, topological information should be classified and modeled in such a way that the required spatial data for a particular design task can be automatically retrieved. This paper attempts to identify and classify various topological information commonly used in building design and construction into more specific categories (e.g., adjacency, connection, containment, separation, and intersection) to support automatic deduction of the spatial information in a computer-based building design system. The paper also discusses typical applications of the topological relations to different design activities. Finally, the development of deduction algorithms and the proposed building design system are briefly described. [source]


    Evaluation of 6 Prognostic Models Used to Calculate Mortality Rates in Elderly Heart Failure Patients With a Fatal Heart Failure Admission

    The objective was to evaluate 6 commonly used heart failure (HF) prognostic models in an elderly, fatal HF population. Predictive models have been established to quantify risk among HF patients. The validation of these models has not been adequately studied, especially in an elderly cohort. Applying a single-center, retrospective study of serially admitted HF patients who died while in the hospital or within 30 days of discharge, the authors evaluated 6 prognostic models: the Seattle Heart Failure Model (SHFM), Heywood's model, Classification and Regression Tree (CART) Analysis, the Heart Failure Survival Score (HFSS), Heart Failure Risk Scoring System, and Pocock's score. Eighty patients were included (mean age, 82.7 ± 8.2 years). Twenty-three patients (28.75%) died in the hospital. The remainder died within 30 days of discharge. The models' predictions varied considerably from one another and underestimated the patients' actual mortality. This study demonstrates that these models underestimate the mortality risk in an elderly cohort at or approaching the end of life. Moreover, the predictions made by each model vary greatly from one another. Many of the models used were not intended for calculation during hospitalization.     Anemia and Cost in Medicare Patients With Congestive Heart Failure

    The objective of this study was to examine the total cost to Medicare associated with the presence of anemia in congestive heart failure (CHF) patients. International Classification of Diseases, Ninth Revision, Clinical Modification codes were used to identify anemia, CHF, and comorbid conditions in 2002, and total Medicare costs were calculated for 2003. The mean patient age was 77.8 years. Anemia, present in 32% of CHF patients, was associated with older age, female sex, non-white race, and increasing burden of comorbidity. The total per-member-per-month cost in 2003 was $1781.01 among CHF patients with anemia in the preceding year compared with $1142.38 for CHF patients without anemia, a ratio of 1.56 (95% confidence interval, 1.5589,1.5592). When adjustment was made for baseline demographic factors and comorbid conditions, the corresponding ratio was 1.25 (95% confidence interval, 1.2546,1.2548). Anemia, a common association of CHF in elderly patients, is an antecedent association of increased societal medical expenditure. [source]


    Classification of atopic hand eczema and the filaggrin mutations

    Hand eczema is a common disease with various risk factors of which atopic dermatitis is known to be one of the most important. Recently, two mutations in the gene coding for filaggrin, a protein important for the skin barrier, have repeatedly been shown to be associated with atopic dermatitis. Moreover, one study point towards an association between the filaggrin null alleles and the subgroup of patients having both hand eczema and atopic dermatitis. For the remainder of hand eczema patients, still unknown genetic risk factors exist. We propose that in future, classification of atopic hand eczema should distinguish between patients with and without the filaggrin null alleles and to further differentiate between associations with type I allergy, type IV allergy and exposure to irritants, respectively. Furthermore, we suggest future studies of atopic hand eczema to analyse for the filaggrin mutations. We believe this will increase the possibility of subgrouping this otherwise heterogenic disease and thereby enable a better phenotype,genotype characterization of hand eczema. This could improve the preventive initiatives, secure better information of patients about the prognosis for their disease, and possibly enable targeted treatment. [source]


    Evaluation of the skin sensitizing potency of chemicals by using the existing methods and considerations of relevance for elicitation

    The Technical Committee of Classification and Labelling dealing with harmonized classification of substances and classification criteria under Directive 67/548/EEC on behalf of the European Commission nominated an expert group on skin sensitization in order to investigate further the possibility for potency consideration of skin sensitizers for future development of the classification criteria. All substances and preparations should be classified on the basis of their intrinsic properties and should be labelled accordingly with the rules set up in the Directive 67/548/EEC. The classification should be the same under their full life cycle and in the case that there is no harmonized classification the substance or preparation should be self-classified by the manufacturer in accordance with the same criteria. The Directive does not apply to certain preparations in the finished state, such as medical products, cosmetics, food and feeding stuffs, which are subject to specific community legislation. The main questions that are answered in this report are whether it would be possible to give detailed guidance on how to grade allergen potency based on the existing methods, whether such grading could be translated into practical thresholds and whether these could be set for both induction and elicitation. Examples are given for substances falling into various potency groups for skin sensitization relating to results from the local lymph node assay, the guinea pig maximization test, the Buehler method and human experience.     The North American Industry Classification System and Its Implications for Accounting Research,

    Abstract Industry classification is an important component of the methodological infrastructure of accounting research. Researchers have generally used the Standard Industrial Classification (SIC) system for assigning firms to industries. In 1999, the major statistical agencies of Canada, Mexico, and the United States began implementing the North American Industry Classification System (NAICS). The new scheme changes industry classification by introducing production as the basis for grouping firms, creating 358 new industries, extensively rearranging SIC categories, and establishing uniformity across all NAFTA nations. We examine the implications of the change for accounting research. We first assess NAICS's effectiveness in forming industry groups. Following Guenther and Rosman 1994, we use financial ratio variances to measure intra-industry homogeneity and find that NAICS offers some improvement over the SIC system in defining manufacturing, transportation, and service industries. We also evaluate whether NAICS might have an impact on empirical research by reproducing part of Lang and Lundholm's 1996 study of information-transfer and industry effects. Using SIC delineations, they focus on whether industry conditions or the level of competition is the main source of uncertainty resolved by earnings announcements. Across all levels of aggregation, we find inferences are similar using either SIC or NAICS. How-ever, we also observe that the regression coefficients in Lang and Lundholm's model show smaller intra-industry dispersion for NAICS, relative to SIC, definitions. Overall, the results suggest that NAICS definitions lead to more cohesive industries. Because of this, researchers may encounter some differences in using NAICS-industry definitions, rather than SIC, but these will depend on research design and industry composition of the sample.     Classification of Anxiety and Depressive disorders: problems and solutions

    Abstract The American Psychiatric Association and the World Health Organization have begun to revise their classifications of mental disorders. Four issues related to these revisions are discussed in this study: the structure of the classifications, the relationship between categories and dimensions, the sensitivity of categorical thresholds to definitions, and maximizing the utility and validity of the diagnostic process. There is now sufficient evidence to consider replacing the present groupings of disorders with an empirically based structure that reflects the actual similarities among disorders. For example, perhaps the present depression and anxiety disorders would be best grouped as internalizing disorders. Most mental disorders exist on a severity dimension. The reliability and validity of the classification might be improved if we accepted the dimensional nature of disorders while retaining the use of categorical diagnoses to enhance clinical utility. Definitions of the thresholds that define categories are very susceptible to detail. In International Classification of Diseases-11(ICD-11) and Diagnostic and Statistical Manual of Mental Disorders-V (DSM-V), disorders about which there is agreement should be identically defined, and disorders in which there is disagreement should be defined differently, so that research can identify which definition is more valid. The present diagnostic criteria are too complex to have acceptable clinical utility. We propose a reduced criterion set that can be remembered by clinicians and an enhanced criterion set for use with decision support tools.     Classification of Compression Bandages: Practical Aspects

    BACKGROUND Compression bandages appear to be simple medical devices. However, there is a lack of agreement over their classification and confusion over the use of important terms such as elastic, inelastic, and stiffness. OBJECTIVES The objectives were to propose terms to describe both simple and complex compression bandage systems and to offer classification based on in vivo measurements of subbandage pressure and stiffness. METHODS A consensus meeting of experts including members from medical professions and from companies producing compression products discussed a proposal that was sent out beforehand and agreed on by the authors after correction. RESULTS Pressure, layers, components, and elastic properties (P-LA-C-E) are the important characteristics of compression bandages. Based on simple in vivo measurements, pressure ranges and elastic properties of different bandage systems can be described. Descriptions of composite bandages should also report the number of layers of bandage material applied to the leg and the components that have been used to create the final bandage system. CONCLUSION Future descriptions of compression bandages should include the subbandage pressure range measured in the medial gaiter area, the number of layers, and a specification of the bandage components and of the elastic property (stiffness) of the final bandage.     Comparison of the Melbourne Assessment of Unilateral Upper Limb Function and the Quality of Upper Extremity Skills Test in hemiplegic CP

    This study investigated interrater reliability and measurement error of the Melbourne Assessment of Unilateral Upper Limb Function (Melbourne Assessment) and the Quality of Upper Extremity Skills Test (QUEST), and assessed the relationship between both scales in 21 children (15 females, six males; mean age 6y 4mo [SD 1y 3mo], range 5,8y) with hemiplegic CP. Two raters scored the videotapes of the assessments independently in a randomized order. According to the House Classification, three participants were classified as level 1, one participant as level 3, eight as level 4, three as level 5, one participant as level 6, and five as level 7. The Melbourne Assessment and the QUEST showed high interrater reliability (intraclass correlation 0.97 for Melbourne Assessment; 0.96 for QUEST total score; 0.96 for QUEST hemiplegic side). The standard error of measurement and the smallest detectable difference was 3.2% and 8.9% for the Melbourne Assessment and 5.0% and 13.8% for the QUEST score on the hemiplegic side. Correlation analysis indicated that different dimensions of upper limb function are addressed in both scales. [source]


    Quantification of upper extremity function and range of motion in children with cerebral palsy

    This study evaluated the hypothesis that upper extremity function and range of motion can be quantified reliably in children with cerebral palsy (CP) in a busy clinical setting. The specific aim was to determine the inter- and intrarater reliability of a modified House Functional Classification (MHC) system to evaluate upper extremity function and a standardized instrument to document upper extremity range of motion (Upper Extremity Rating Scale [UERS]). Sixty-five children with CP (43 males, 22 females, mean age 9y 2mo, SD 4y 1mo) with spasticity involving the upper extremity (quadriplegia n=22; hemiplegia n=36; diplegia n=7; Gross Motor Functional Classification System Levels I n=41, II n=6, III n=3, IV n=5, V n=10) were evaluated independently by occupational therapists and orthopedic surgeons using both instruments at several visits. Inter- and intrarater reliability were determined for both instruments by calculating measures of agreement (weighted kappa values and intraclass correlation coefficients [ICCs]). Interrater agreement (ICC=0.94) and intrarater agreement (ICC=0.96) on the MHC were good to excellent. Similarly, inter-rater agreement (kappa 0.66,0.81) and intrarater agreement (kappa 0.64,0.88) on the UERS was either good or excellent. The MHC and the UERS provide standardized, reliable, reproducible, and efficient instruments that can be used by occupational therapists and orthopedic surgeons to evaluate the upper extremities of children with CP.     Validity and reliability of the guidelines of the Surveillance of Cerebral Palsy in Europe for the classification of cerebral palsy

    The validity and reliability of the guidelines of the Surveillance of Cerebral Palsy in Europe (SCPE) for the classification of cerebral palsy (CP) were tested by administering 10 written case vignettes via an interactive web-based link to 30 SCPE partners. There was a moderately good level of agreement (,=0.59) about inclusion as a CP case on the SCPE database. Classification by CP subtype differed in two main areas: assigning spastic versus dyskinetic and judgement of distribution of spastic involvement. Agreement on Gross Motor Function Classification System (GMFCS) level was less good than reported in previous studies. Twenty respondents repeated the test 5 months later and there was good repeatability for case inclusion (,= 0.72) but considerable variation in assignment of CP subtype and GMFCS level. There is a need for further collaborative work and training to improve harmonization of the classification of CP, including examination, application of SCPE guidelines, and register coding.     Intrapartum fever and chorioamnionitis as risks for encephalopathy in term newborns: a case,control study

    In this study we examined the relationship between diagnoses of isolated intrapartum fever or chorioamnionitis and the risk of encephalopathy in term newborns. We conducted a population-based, case,control study in Washington State using 1994 to 2002 linked data from the Washington State Birth Registry and the Comprehensive Hospital Abstract Reporting System (CHARS). We identified 1060 singleton, term newborns (602 males, 458 females) with International Classification of Diseases (ICD-9) diagnoses consistent with encephalopathy, and 5330 unaffected control newborns (2756 males, 2574 females). Intrapartum fever was defined by a diagnosis of intrapartum temperature of >38°C in the birth registry or CHARS databases. Chorioamnionitis was defined using ICD-9 diagnoses recorded in CHARS. We identified 2.2 cases of encephalopathy per 1000 births. Isolated intrapartum fever was associated with a 3.1-fold (95% confidence interval [CI] 2.3-4.2) increased risk of newborn encephalopathy. Chorioamnionitis was associated with a 5.4-fold (95% CI 3.6-7.8) increased risk of encephalopathy. We found that isolated intrapartum fever and chorioamnionitis were independently associated with an increased risk of encephalopathy in term infants. Our data also indicate that there is a spectrum of risk for encephalopathy in term infants exposed to intrapartum fever. Infants born to women with signs of chorioamnionitis other than isolated intrapartum fever may be at higher risk of encephalopathy than those exposed only to isolated intrapartum fever. [source]
