Metacognition as a mediator of the effects of impairments in neurocognition on social function in schizophrenia spectrum disorders

Objective:, This study explored whether Mastery, a domain of metacognition that reflects the ability to use knowledge about mental states to respond to psychological challenges, mediated the effects of neurocognition on the frequency of social contact and persons' capacity for social relatedness. Method:, Participants were 102 adults with schizophrenia or schizoaffective disorder. Neurocognition was represented by a single factor score produced by a principal components analysis of a neurocognitive test battery. Mastery was assessed using the metacognitive assessment scale and social functioning by the quality of life scale. Results:, Using structural equation modeling, specifically measured-variable path analysis, a mediational model consisting of neurocognitive capacity linked to mastery and capacity for social relationships and mastery linked with frequency of social contact and capacity for social relatedness showed acceptable fit to the observed data. This persisted after controlling for negative and cognitive symptoms. Conclusion:, Results suggest that certain forms of metacognition mediate the influence of neurocognition upon function in schizophrenia. [source]


Measurements in the Addictions for Triage and Evaluation (MATE): an instrument based on the World Health Organization family of international classifications

ABSTRACT Aims To present and evaluate a measurement tool for assessing characteristics of people with drug and/or alcohol problems for triage and evaluation in treatment. Measurements in the Addictions for Triage and Evaluation (MATE) is composed of 10 modules, selected on the basis of a detailed set of specifications. Conceptually, the MATE was constructed according to the ICD and International Classification of Functioning (ICF) in the World Health Organization (WHO) classification system. Two of the ICF-related modules were newly designed. Design Monitoring feasibility and field-testing in a treatment-seeking population with researcher and clinician-administered test,retest interviews, construct validation with related instruments and evaluation of the dimensional structure of the ICF-related modules. Setting The research was conducted in a large, regional substance abuse treatment centre in the Netherlands and at the Municipal Health Service of Amsterdam. Participants A total of 945 treatment-seeking patients were recruited during routine intakes, 159 of whom were interviewed twice; 32 problem drug users were also recruited from the Amsterdam cohort studies among problem drug users. Findings Completion time was reasonably short, and there were relatively few missing data. The factor structure of the ICF-related modules revealed a three-factor model with an acceptable fit. Inter-rater reliability ranged between 0.75 and 0.92 and was satisfactory, but interviewer reliability ranged between 0.34 and 0.73, indicating that some of subscales need to be improved. Concurrent validity was indicated by significant correlations (>0.50) between the ICF-related modules and the WHO Disability Assessment Schedule II (WHODAS II) and WHO Quality of Life brief version (WHOQOL-BREF). Conclusions The MATE can be used to allocate patients to substance abuse treatment. Because it is a comprehensive but flexible measurement tool that is also practical to use, the MATE is well suited for use in a heterogeneous population. [source]


Factorial structure and cross-cultural invariance of the Oral Impacts on Daily Performances

The issue of cross-cultural construct validation and measurement invariance of the Oral Impacts on Daily Performances (OIDP) questionnaire is important. Using confirmatory factor analysis (CFA), this study evaluated a proposed three-factor structure of the OIDP questionnaire in Tanzanian adolescents and adults and assessed whether this model would be replicated in Ugandan adolescents. Between 2004 and 2007, OIDP data were collected from 1,601 Tanzanian adolescents, 1,031 Tanzanian adults, and 1,146 Ugandan adolescents. Model generation analysis was restricted to Tanzanian adolescents, and the model achieved was tested, without modification, in Tanzanian adults and in Ugandan adolescents. A modified three-factor solution with cross-loadings improved the fit of the OIDP model to the data compared with a one-factor model and the original three-factor model within the Tanzanian [comparative fit index (CFI) = 0.99] and Ugandan (CFI = 0.98) samples. Cross-validation in Tanzanian adults provided a reasonable fit (CFI = 0.98). Multiple-group CFA demonstrated acceptable fit [,2 = 140.829, degrees of freedom (d.f.) = 24, CFI = 0.98] for the unconstrained model, whereas unconstrained and constrained models were statistically significantly different. Factorial validity was confirmed for the three-factor OIDP model. The results provide evidence for cross-cultural equivalence of the OIDP, suggesting that this measure is comparable, at least to some extent, across Tanzanian and Ugandan adolescents. [source]


Testing of a measurement model for baccalaureate nursing students' self-evaluation of core competencies

Abstract Title.,Testing of a measurement model for baccalaureate nursing students' self-evaluation of core competencies. Aim. This paper is a report of a study to test the psychometric properties of the Self-Evaluated Core Competencies Scale for baccalaureate nursing students. Background. Baccalaureate nursing students receive basic nursing education and continue to build competency in practice settings after graduation. Nursing students today face great challenges. Society demands analytic, critical, reflective and transformative attitudes from graduates. It also demands that institutions of higher education take the responsibility to encourage students, through academic work, to acquire knowledge and skills that meet the needs of the modern workplace, which favours highly skilled and qualified workers. Methods. A survey of 802 senior nursing students in their last semester at college or university was conducted in Taiwan in 2007 using the Self-Evaluated Core Competencies Scale. Half of the participants were randomly assigned either to principal components analysis with varimax rotation or confirmatory factor analysis. Results. Principal components analysis revealed two components of core competencies that were named as humanity/responsibility and cognitive/performance. The initial model of confirmatory factor analysis was then converged to an acceptable solution but did not show a good fit; however, the final model of confirmatory factor analysis was converged to an acceptable solution with acceptable fit. The final model has two components, namely humanity/responsibility and cognitive/performance. Both components have four indicators. In addition, six indicators have their correlated measurement errors. Conclusion. Self-Evaluated Core Competencies Scale could be used to assess the core competencies of undergraduate nursing students. In addition, it should be used as a teaching guide to increase students' competencies to ensure quality patient care in hospitals. [source]


Psychometric Analysis of the Brisbane Practice Environment Measure (B-PEM)

Abstract Purpose: To undertake rigorous psychometric testing of the newly developed contemporary work environment measure (the Brisbane Practice Environment Measure [B-PEM]) using exploratory factor analysis and confirmatory factor analysis. Methods: Content validity of the 33-item measure was established by a panel of experts. Initial testing involved 195 nursing staff using principal component factor analysis with varimax rotation (orthogonal) and Cronbach's alpha coefficients. Confirmatory factor analysis was conducted using data from a further 983 nursing staff. Results: Principal component factor analysis yielded a four-factor solution with eigenvalues greater than 1 that explained 52.53% of the variance. These factors were then verified using confirmatory factor analysis. Goodness-of-fit indices showed an acceptable fit overall with the full model, explaining 21% to 73% of the variance. Deletion of items took place throughout the evolution of the instrument, resulting in a 26-item, four-factor measure called the Brisbane Practice Environment Measure-Tested. Conclusions: The B-PEM has undergone rigorous psychometric testing, providing evidence of internal consistency and goodness-of-fit indices within acceptable ranges. The measure can be utilised as a subscale or total score reflective of a contemporary nursing work environment. Clinical Relevance: An up-to-date instrument to measure practice environment may be useful for nursing leaders to monitor the workplace and to assist in identifying areas for improvement, facilitating greater job satisfaction and retention. Abstract We apply a new algorithm, called the unbiased minimal variance (hereafter UMV) estimator, to reconstruct the cosmic density and peculiar velocity fields in our local Universe from the SEcat catalogue of peculiar velocities comprising both early- (ENEAR) and late-type (SFI) galaxies. The reconstructed fields are compared with those predicted from the IRAS PSCz galaxy redshift survey to constrain the value of ,=,0.6m/b, where ,m and b are the mass density and the bias parameters. The comparison of the density and velocity fields is carried out within the same methodological framework, and leads, for the first time, to consistent values of ,, yielding ,= 0.57+0.11,0.13 and ,= 0.51 ± 0.06, respectively. We find that the distribution of the density and velocity residuals, relative to their respective errors, is consistent with a Gaussian distribution with ,, 1, indicating that the density field predicted from the PSCz is an acceptable fit to that deduced from the peculiar velocities of the SEcat galaxies. Temporal detection in human vision: dependence on eccentricity

Studies of human perception of time-varying luminance often aim to estimate either temporal impulse response shapes or temporal modulation transfer functions (MTFs) of putative temporal processing mechanisms. Previously, temporal masking data have been used to estimate the properties and numbers of these temporal mechanisms in central vision for 1 cycle per degree (cpd) targets [Fredericksen and Hess (1998)]. The same methods have been used to explore how these properties change with stimulus energy [Fredericksen and Hess (1997)] and spatial frequency [Fredericksen and Hess (1999)]. We present here analyses of the properties of temporal mechanisms that detect temporal variations of luminance in peripheral vision. The results indicate that a two-filter model provides the best model for our masking data, but that no multiple filter model provides an acceptable fit across the range of parameters varied in this study. Single-filter modelling shows differences between processing mechanisms at 1 cpd in central vision and those that operate eccentrically. We find evidence that this change is because of differences in relative sensitivities of the mechanisms, and to differences in fundamental mechanism impulse responses. [source]


Discrimination between worry and anxiety among cancer patients: development of a brief cancer-related worry inventory

Abstract Objectives: A psychometric scale for assessing cancer-related worry among cancer patients, called the Brief Cancer-Related Worry Inventory (BCWI), was developed. Methods: A cross-sectional questionnaire survey for item development was conducted of 112 Japanese patients diagnosed with breast cancer, and test,retest validation analysis was conducted using the data from another prospective study of 20 lung cancer patients. The questionnaire contained 15 newly developed items for cancer-related worry, the Hospital Anxiety and Depression Scale, The Impact of Event Scale Revised, and the Medical Outcomes Study Short Form-8. Results: Exploratory factor analysis of the 15 items yielded a 3-factor structure including (1) future prospects, (2) physical and symptomatic problems and (3) social and interpersonal problems. A second-order confirmatory factor analysis identified a second-order factor called cancer-related worry and confirmed the factor structure with an acceptable fit (chi-square (df=87)=160.16, P=0.001; GFI=0.83; CFI=0.92; RMSEA=0.09). The internal consistency and test,retest reliability were confirmed with the lung cancer sample. Multidimensional scaling found that cancer-related worry is separate from anxiety, depression, and posttraumatic stress disorder (PTSD) symptoms. Conclusion: Our study succeeded in developing and confirming the validity and reliability of a BCWI. The study also confirmed the discriminable aspects of cancer-related worry from anxiety, depression, and PTSD symptoms. Copyright © 2008 John Wiley & Sons, Ltd. [source]


The role of impulsivity in actual and problematic use of the mobile phone

Several authors have investigated the risks arising from the growth in mobile phone use (e.g. debts incurred by young people). The aims of the present study are (1) to validate a new questionnaire assessing problematic mobile phone use: the Problematic Mobile Phone Use Questionnaire (PMPUQ), and (2) to investigate the relationships between the PMPUQ and the multi-faceted construct of impulsivity. With these aims, 339 subjects were screened using the PMPUQ and the UPPS Impulsive Behaviour Scale (UPPS) which assesses four distinct components associated with impulsive behaviours (urgency, lack of premeditation, lack of perseverance and sensation seeking). The results showed that the PMPUQ has an acceptable fit and assesses four different dimensions of problematic mobile phone use (prohibited use, dangerous use, dependence, financial problems). While each facet of impulsivity played a specific role in mobile phones use, urgency appeared to be the strongest predictor of problematic use. Copyright © 2008 John Wiley & Sons, Ltd. [source]

Objectives:, To test the factor structure of oral health outcomes within Gilbert's conceptual model among 65-year olds in Sweden and Norway. It was hypothesized that responses to 14 observed indicators could be explained by three correlated factors, symptom status, functional limitations and oral disadvantages, that each observed oral health indicator would associate more strongly with the factor it is supposed to measure than with competing factors and that the proposed 3-factor structure would possess satisfactory cross-national stability with 65-year olds in Norway and Sweden. Methods:, In 2007, 6078 Swedish- and 4062 Norwegian adults borne in 1942 completed mailed questionnaires including oral symptoms, functional limitations and the eight item Oral Impacts on Daily Performances inventory. Results:, Model generation analysis was restricted to the Norwegian study group and the model achieved was tested without modifications in Swedish 65-year olds. A modified 3-factor solution with cross-loadings, improved the fit to the data compared with a 2-factor- and the initially proposed 3-factor model among the Norwegian [comparative fit index (CFI) = 0.97] and Swedish (CFI = 0.98) participants. All factor loadings for the modified 3-factor model were in the expected direction and were statistically significant at CR > 1. Multiple group confirmatory factor analyses, with Norwegian and Swedish data simultaneously revealed acceptable fit for the unconstrained model (CFI = 0.97), whereas unconstrained and constrained models were statistically significant different in nested model comparison. Conclusions:, Within construct validity of Gilbert's model was supported with Norwegian and Swedish 65-year olds, indicating that the 14-item questionnaire reflected three constructs; symptom status, functional limitation and oral disadvantage. Measurement invariance was confirmed at the level of factor structure, suggesting that the 3-factor model is comparable to some extent across 65-year olds in Norway and Sweden. [source]


Single- and dual-porosity modelling of multiple tracer transport through soil columns: effects of initial moisture and mode of application

Summary We investigated the effect of initial moisture contents and mode of application on the displacement of multiple conservative tracers through undisturbed columns of a Humic Gleysol. Bromide was applied at the soil surface and chloride was injected at 5 cm depth. The columns were irrigated with deuterium-enriched water. A dual-porosity model and two single-porosity models were calibrated separately to Br, and Cl, elution curves in the two columns. Elution curves were almost identical for Br, and Cl, under initially wet conditions, whereas the displacement of Br, was faster than that of Cl, in the initially dry column, indicating rapid transport with preferential flow. Only the dual-porosity model described the long-tailing breakthrough of Cl, in the initially dry column adequately. The parameter values giving acceptable fits for ,Br dry' were not compatible with the description of the three other elution curves, which could be adequately modelled with a single set of parameter values. The estimated set of common parameters was validated by comparing with the elution curves of deuterium water, nitrate and sulphate, as well as with resident tracer concentrations at four depths. The results showed that solutes can be displaced much faster when applied at the surface of initially dry soil than when applied to wet soil or when resident in the soil matrix. The simulation results suggest that solute transport under initially dry conditions was governed by preferential flow of infiltration water through macropores by-passing the matrix due to shrinkage cracks and water repellence of matrix surfaces.
