Severe hemophilia with mild bleeding phenotype: molecular characterization and global coagulation profile

Summary.,Background: Patients with severe hemophilia may show very varied bleeding tendencies, and the reasons for this heterogeneous clinical expression are unclear. The factor VIII/FIX genotype is the main determinant of the residual factor activity; however, different bleeding phenotypes have also been reported in patients sharing the same mutation. Such global coagulation tests as thrombin generation assays are tools with which to investigate different coagulation profiles among severe hemophiliacs. Objectives, patients and methods: This case,control study was aimed at comprehensively evaluating the role of genotype and endogenous thrombin potential (ETP) as predictors of the clinical phenotype in severe hemophiliacs with an extremely mild bleeding tendency (cases, n = 22), in comparison with those showing a typical bleeding tendency (controls, n = 50). Results: Cases were more frequently affected by hemophilia B than by hemophilia A, and showed a lower incidence of severe FVIII/FIX gene defects (referred to as null mutations), higher FVIII and FIX antigen levels and higher ETP values in platelet-rich plasma than controls (P < 0.05). By multivariate logistic regression, only non-null mutations were confirmed as an independent predictor of a mild clinical phenotype. Conclusions: These results indicate that non-null mutations represent the main determinant of the bleeding tendency, and that ETP measurement in platelet-rich plasma is able to identify severe hemophiliacs with a mild clinical phenotype. Ulcerative Dermatitis, Thrombocytopenia, and Neutropenia in Neonatal Foals

This report describes transient ulcerative dermatitis, severe thrombocytopenia, and mild neutropenia in 6 foals from 4 mares from geographically diverse regions of the United States. The foals presented at <4 days of age with oral and lingual ulcers, and crusting and erythema around the eyes, muzzle, and perineal, inguinal, axillary, trunk, and neck regions. There was a severe thrombocytopenia (0,30,000 platelets/,L), leukopenia (1,900-3,200 white blood cells/,L), and mild neutropenia (500-1,800 neutrophils/,L). Four of the 6 foals had petechiae and ecchymotic hemorrhages and 3 had bleeding tendencies. Results of examination of a bone marrow biopsy from 1 foal were normal and results of a platelet surface immunoglobulin test in another were negative. Histopathology of the skin in all foals showed subepidermal clefting with subjacent vascular dilation, dermal hemorrhage, and superficial papillary necrosis. The foals were treated supportively with broad-spectrum antibiotics (5/6), corticosteroids (3/6), gastric ulcer prophylaxis (6/6), whole-blood transfusion (4/6), and platelet-rich plasma (1/6). The skin lesions and thrombocytopenia (>50,000 platelets/,L) improved in 2 weeks (4/6). Two foals had a decline in their platelet counts when the steroids were decreased and needed protracted treatment. All foals survived and were healthy as yearlings. Two mares that had 2 affected foals each, upon subsequent pregnancies to different stallions, had healthy foals when an alternate source of colostrum was given. The findings in the cases in this report suggest a possible relationship between colostral antibodies or some other factor in the colostrum and the thrombocytopenia and skin lesions, although further investigation is warranted to confirm or refute this hypothesis. Cellular microparticles: new players in the field of vascular disease?

Abstract Microparticles are small membrane vesicles that are released from cells upon activation or during apoptosis. Cellular microparticles in body fluids constitute a heterogeneous population, differing in cellular origin, numbers, size, antigenic composition and functional properties. Microparticles support coagulation by exposure of negatively charged phospholipids and sometimes tissue factor, the initiator of coagulation in vivo. Microparticles may transfer bioactive molecules to other cells or microparticles, thereby stimulating cells to produce cytokines, cell-adhesion molecules, growth factors and tissue factor, and modulate endothelial functions. Microparticles derived from various cells, most notably platelets but also leucocytes, lymphocytes, erythrocytes and endothelial cells, are present in the circulation of healthy subjects. Rare hereditary syndromes with disturbances in membrane vesiculation leading to a decreased numbers of microparticles clinically present with a bleeding tendency. In contrast, elevated numbers of microparticles are encountered in patients with a great variety of diseases with vascular involvement and hypercoagulability, including disseminated intravascular coagulation, acute coronary syndromes, peripheral arterial disease, diabetes mellitus and systemic inflammatory disease. Finally, microparticles are a major component of human atherosclerotic plaques. In view of their functional properties, cell-derived microparticles may be an important intermediate in the cascade of cellular and plasmatic dysfunctions underlying the process of atherogenesis. Dental procedures in adult patients with hereditary bleeding disorders: 10 years experience in three Italian Hemophilia Centers

Summary., Excessive bleeding after dental procedures are one of the most frequent complications occurring in patients with hereditary bleeding disorders. In this retrospective study we collected data from 10 years of experience in the oral care of patients with congenital haemorrhagic disorders in three Italian Hemophilia Centers. Between 1993 and 2003, 247 patients with inherited bleeding disorders underwent 534 dental procedures including 133 periodontal treatments, 41 conservative dentistry procedures, 72 endodontic treatments and 288 oral surgery procedures. We recorded 10 bleeding complications (1.9%), most of which occurred in patients with severe/moderate haemophilia A undergoing multiple dental extractions. Thus, our protocol of management of patients with hereditary bleeding tendency undergoing oral treatment or surgery has been shown to be effective in preventing haemorrhagic complications. [source]


Central venous lines in haemophilia

Summary., Infections and technical problems are the most frequent complications when using implantable central venous access devices in patients with haemophilia. There are two major experiences reported concerning infections in noninhibitor patients: one is approximately 0.2 infections per 1000 days and the other approximately 1.0 (0.7,1.6) per 1000 days. Infections are more frequent in inhibitor patients and approximately one infection per 6,12 months of use can be expected. The figures are low for clinically apparent thrombosis in the larger series on record, but routine venograms were not carried out in most of these series. In studies where this has been done, a high frequency of abnormalities on venograms has been seen in some but not in others. The final decision to use a central line has to take into account the medical goal, the patient's bleeding tendency, the social situation and the expected risk of complications at the particular haemophilia centre. Some of the complications may be reduced by adequate aseptic measures both during implantation and in subsequent use, and by clear basic routines for surveillance of the systems and repeated education of the users. Factor XI deficiency and its management

Factor XI deficiency has a more variable bleeding tendency than haemophilia A or B. Individuals with severe deficiency have only a mild bleeding tendency, which is typically provoked by surgery, but the risk of bleeding is not restricted to individuals with severe deficiency. The bleeding tendency varies between individuals with similar factor XI levels, and sometimes the bleeding tendency of an individual may vary. The reasons for this are not fully understood, although in cases of severe deficiency there is some correlation between phenotype and genotype. Factor XI is activated by thrombin. The role of factor XI in physiological processes has become clearer since this fact was discovered, and the discovery has contributed to a revised model of blood coagulation. Factor XI deficiency occurs in all racial groups, but is particularly common in Ashkenazi Jews. The factor XI gene is 23 kilobases long. Two mutations are responsible for most factor XI deficiency in the Ashkenazi population, but a number of other mutations have now been reported in other racial groups. Individuals with factor XI deficiency may need specific therapy for surgery, accidents, and dental extractions. Several therapies are available which include fresh frozen plasma, factor XI concentrates, fibrin glue, antifibrinolytic drugs, and desmopressin. Each has advantages and risks to be considered. Factor XI concentrate may be indicated for procedures with a significant risk of bleeding especially in younger patients with severe deficiency, but its use in older patients has been associated with thrombotic phenomena. If fresh frozen plasma is to be used it is preferable to obtain one of the virally inactivated products. Fibrin glue is a useful treatment which deserves further study. [source]


Predictive value of clinical and microbiological parameters for the treatment outcome of scaling and root planing

Abstract Objectives: To compare the clinical and microbiological outcome of non-surgical periodontal therapy after 6 months with data obtained after hygienic phase or 6 weeks after completion of non-surgical therapy, in order to evaluate the value of clinical and microbiological parameters to predict treatment success. Material and Methods: Clinical and microbiological data were available from 271 sites in 10 systemically healthy non-smokers with moderate-to-advanced chronic periodontal disease (24,32 sites per individual). Subgingival plaque samples were tested for the presence of Porphyromonas gingivalis, Actinobacillus actinomycetemcomitans, Tannerella forsythensis and Treponema denticola using RNA probes. Results: Stepwise multiple linear regression analysis revealed a significant impact of the number of sites with visible plaque index >1 after hygienic phase on the bleeding tendency of a subject at month 6 (p<0.01). Furthermore, an association could be demonstrated between the number of residual pockets (PD>3 mm) 6 months after therapy and the number of bleeding sites and suppurating sites after hygienic phase (p=0.016). Six weeks after therapy, the mean total bacterial loads had a significant impact on the bleeding tendency of a subject at month 6 (p<0.01). Although the average numbers of sites with persisting P. gingivalis, A. actinomycetemcomitans, T. forsythensis and T. denticola seemed to be very similar 6 weeks and 6 months after therapy, large variations were noted between subjects, and therefore the microbiological status of a subject at week 6 could not predict the status at month 6. Conclusions: The present study showed a limited potential of microbiological tests, performed after hygienic phase or shortly after non-surgical periodontal therapy, to predict the clinical outcome 6 months later, but confirmed the importance of an establishment of perfect oral hygiene before non-surgical therapy. Coagulation dynamics and platelet functions in obstructive jaundiced patients

Abstract Background:, All of the body systems are affected by increased levels of bilirubin. The aim of this study is to investigate the function of platelets and clotting dynamics in patients with obstructive jaundice. Methods:, Liver function tests, serum CRP, PT, PTT and hemogram were measured in 23 patients with obstructive jaundice. Thromboelastography (TEG) was done for the evaluation of coagulation dynamics, while platelet function assay (PFA 100) was used to evaluate platelet functions. Blood samples were obtained at two occasions, before the drainage and 3 weeks after the relief of the obstruction. Results:, Hypercoagulation was detected in 80% of patients. Maximum strength, elasticity, coagulation indices of the clot were correlated with increased concentrations of direct bilirubin. Although maximum strength of coagulum usually represents increased activity of platelet function, membrane closure times with PFA 100 were found to be prolonged in 30% of patients, reduced values were determined in 17% of patients. No demonstrable effect on coagulation parameters and platelet function were detected after drainage procedures regardless of modality. Conclusions:, Even though there is a general assumption about the increased bleeding tendency in obstructive jaundiced patients, we could not demonstrate reduced clotting activity by measuring with either PFA or TEG. On the contrary we observed tendency for hypercoagulation independent of increased prothrombin times. The most probable cause of this effect is the increased activity of fibrin polymers on platelet membrane. [source]

Ex vivo inhibition of thrombus formation by an anti-glycoprotein VI Fab fragment in non-human primates without modification of glycoprotein VI expression

Summary.,Objectives:,Glycoprotein (GP)VI is an attractive target for the development of new antithrombotic drugs. Its deficiency protects animals in several models of thrombosis, arterial stenosis and ischemia-,reperfusion while inducing no major bleeding tendency. The Fab fragment of one anti-GPVI monoclonal antibody (9O12.2) inhibits all GPVI functions in vitro. The aim of this study was to determine the ex vivo effects of 9O12.2 Fab on hemostasis, coagulation and thrombosis in non-human primates. Methods and results:,Blood samples were collected from cynomolgus monkeys before and after (30, 90 and 150 min, 1 and 7 days) a bolus injection of 9O12.2 Fab (4 mg kg,1) or vehicle. Platelet counts and coagulation tests (prothrombin time, activated partial thromboplastin time) were not modified following Fab injection. The PFA-100 closure time increased during the first hours and returned to initial values on day + 1. Platelet-bound Fab was detected from 30 min to 24 h after Fab injection without GPVI depletion at any time. Collagen-induced platelet aggregation was selectively and fully inhibited at 30 min. Thrombus formation on collagen in flowing whole blood (1500 s,1) was delayed and decreased, and collagen-induced or tissue factor-induced thrombin generation in platelet-rich plasma was profoundly inhibited. Conclusion:,The anti-GPVI 9O12.2 Fab inhibits thrombus formation ex vivo in non-human primates with a composite effect on platelet activation and thrombin generation in the absence of GPVI depletion. Evaluation of low PAI-1 activity as a risk factor for hemorrhagic diathesis

Summary.,Background: Prospective studies of the epidemiology and clinical significance of low plasminogen activator inhibitor type 1 (PAI-1) activity are lacking. Objective: To evaluate the prevalence of low PAI-1 activity in patients with a bleeding tendency in comparison with a normal population. Methods: In 586 consecutive patients, referred because of bleeding symptoms, we added analyses of PAI-1 activity and tissue plasminogen activator complex with PAI-1 (t-PA,PAI-1) to the routine investigation, consisting of platelet count, bleeding time, prothrombin time, activated partial thromboplastin time, fibrinogen, factor VIII, von Willebrand factor activity, and antigen. Controls were 100 blood donors and 100 age- and sex-matched healthy individuals. The latter were also evaluated regarding the previous bleeding episodes. The bleeding history was classified as clinically significant or not, and the criteria were fulfilled in 75% of the patients and 18% of the healthy controls. Results: The routine laboratory investigation of the patients was negative in 57%. Low PAI-1 activity, defined as <1.0 U mL,1, was found in 23% of the patients and in 13% and 10% of the blood donors and healthy controls, respectively (odds ratio and 95% CI, 2.04; 1.11,3.77 and 2.75; 1.39,5.42, respectively). The difference remained statistically significant after the adjustment for body mass index, use of estrogens, sex and age (odds ratio for patients vs. healthy controls 3.23; 95% CI, 1.22,8.56, P = 0.019). The distribution of the 4G/5G genotypes in the patients was not different from that of two control populations. No specific symptom predicted for low PAI-1, which did not aggravate the clinical picture in association with the other hemostatic defects. Low tPA,PAI-1 was not associated with the increased bleeding tendency. Conclusion: Low PAI-1 activity is common in patients with a bleeding diathesis, but it is a risk factor of minor clinical importance and not associated with specific bleeding manifestations. Real-Time quantitative PCR analysis of factor XI mRNA variants in human platelets

Summary., Coagulation factor XI (FXI) plays an essential role in blood coagulation. A deficiency of FXI is an unusual hemorrhagic diathesis in that the bleeding tendency can be highly variable, ranging from severe deficiencies with no symptoms to mild and moderate deficiencies requiring multiple blood transfusions for hemorrhages. This variability in bleeding has been attributed to a number of factors including the presence of a novel form of FXI associated with platelets, which ameliorates the bleeding in some cases of FXI deficiency. However, the nature of this platelet FXI molecule is controversial. Hsu et al. (J Biol Chem 1998; 273: 13787,93) suggest that it is a product of normal FXI , but lacking exon V whilst Martincic et al. (Blood 1999; 94: 3397,404) were unable to detect this alternatively spliced variant using RT-PCR. In order to resolve this controversy, we have employed the highly sensitive technique of real-time quantitative RT-PCR using RNA isolated from FXI-deficient patients. Our results indicate that the platelets of both normal and FXI deficient individuals contain FXI mRNA that is identical to the mRNA found in liver. An exon V deleted splice variant was not detected. Thus the FXI message is not alternatively spliced in platelets and therefore would not be able to produce an unusual FXI protein. [source]


Cappuccino mutation in an autoimmune-prone strain of mice suggests a role of platelet function in the progression of immune complex crescentic glomerulonephritis

Objective Crescent formation in the renal glomerulus is a typical manifestation of progressive glomerulopathy associated with fatal renal failure; therefore, its prevention is of clinical importance. Little is known about the pathogenic mechanism for crescent formation. This study was undertaken in an attempt to identify the events that are critical for crescent formation in immune complex crescentic glomerulonephritis (CGN) by analyzing a novel mutant strain of mice. Methods A spontaneous mutant strain of mice was isolated from the autoimmune-prone strain EOD, which stably develops fatal CGN. The mutant phenotypes were assessed histopathologically, hematologically, and immunologically. The mutation was searched for with positional cloning using microsatellite markers. Results Compared with wild-type EOD (WT-EOD) mice, mutant EOD (mut-EOD) mice showed marked improvement in CGN in conjunction with an improvement in spontaneous mortality. In WT-EOD mice, an inverse correlation between blood urea nitrogen concentration and blood platelet count and massive accumulation of platelets in the glomerulus were evident, suggesting that an accumulation of platelets in the glomerulus contributes to the progression of CGN. The mutant platelets showed an abnormal aggregation in response to collagen and thrombin, associated with a bleeding tendency in mut-EOD mice. Genetic analysis revealed a deleterious mutation in the cappuccino gene (cno), which encodes a protein that belongs to a complex called the biogenesis of lysosome-related organelle complex 1 and is profoundly involved in platelet function. Morphologic examination revealed a partial defect in dense body formation in the ,-granule of platelets. Conclusion The present findings suggest that platelet functions have a critical role in crescent formation in autoimmune GN. [source]

Molecular pathology of haemophilia B in Turkish patients: identification of a large deletion and 33 independent point mutations

Summary. Heterogeneous mutations in the coagulation factor IX (FIX) gene result in a bleeding tendency known as haemophilia B. The haemophilia B mutation database has a total of 2353 patient entries, including 10 of the estimated 1000 Turkish patients. In this study, a more comprehensive analysis of the molecular pathology of haemophilia B in Turkey revealed one large deletion and 33 point mutations in the FIX gene of 34 unrelated patients. Haplotype analysis using six polymorphic sites showed that the mutations identified in a total of 45 patients occurred on 13 different haplotypes and that each mutation was family specific. Microbiological and clinical outcomes and patient satisfaction for two treatment options in the edentulous lower jaw after 10 years of function

Abstract Background: Long-term data on microbiological and clinical outcome as well as on patient satisfaction after implant therapy in the edentulous mandible are limited. Especially comparisons between fixed full prostheses (FFPs) and overdentures (ODs), or between anchoring systems for the latter are scarce. Aim: This study aimed to evaluate both of these parameters at the 10-year follow-up in a group of fully edentulous patients rehabilitated via an OD or a FFP (the latter to allow inter-group comparison). Material and methods: A total of 37 fully edentulous patients (25 ODs, 12 FFPs, age at implant installation ranged from 36 to 85 years) participated in this study. All subjects received their implants (Brånemark System®, Nobel Biocare AB, Gothenburg, Sweden) 10 years previously. For the ODs different attachment systems (bar, magnets, ball) had been applied that allowed a further intra-group comparison. At the follow-up visit, 10 years after the abutment insertion, a series of periodontal parameters were recorded, long-cone radiographs were taken and subgingival plaque samples were collected for analysis using checkerboard DNA,DNA hybridization. The clinical and radiographic data were recorded at abutment connection and after 1 and 10 years. Results: After 10 years of loading, mean plaque and bleeding indices and changes in attachment or marginal bone level were not significantly different, neither between the OD and FFP group, nor within the OD group. The marginal bone loss between abutment connection and year 10 was 0.86 and 0.73 mm for OD and FFP groups, respectively. The subgingival microbiota at implant sites from all (sub)-groups was comparable, with low numbers of DNA counts (±10 × 105) but high detection frequencies of Actinobacillus actinomycetemcomitans (>90%), Porphyromonas gingivalis (>85%) and Tannerella forsythensis (30%). The composition of the subgingival microbiota was influenced by probing depth and bleeding tendency. Patient satisfaction was very high for both types of prosthetic rehabilitation. The FFP group scored only slightly better for chewing comfort and general satisfaction. Conclusion: These data indicate that from the clinical and microbiological standpoint, as well as patient satisfaction, both an OD and a FFP offer a favourable long-term outcome. Résumé Les données à long terme de la guérison clinique et microbiologique ainsi que la satisfaction du patient après un traitement par implants dans la mandibule édentée sont limitées. Essentiellement des comparaisons entre les prothèses fixées complètes et les prothèses fixées amovibles ou entre les systèmes d'ancrage pour ces derniers sont rares. Cette étude a eu pour but d'évaluer ces paramètres après dix ans dans un groupe de patients complètement édentés qui avaient été soignés par une prothèse amovible ou fixée (cette dernière pour permettre la comparaison intergroupes). Trente-sept patients édentés [25 prothèses amovibles (OD), douze prothèses fixées (FFP), l'âge au moment du placement des implants était de 36 à 85 ans] ont participéà cette étude. Tous les sujets avaient reçu des implants Brånemark® 10 ans auparavant. Pour les prothèses amovibles, différents types d'attache (balle, aimant) avaient été utilisés, ce qui permettaient une comparaison intragroupe supplémentaire. Lors de la visite du suivi, dix ans après l'insertion des implants, une série de paramètres parodontaux ont été enregistrés, des radiographies par long cône prises et des échantillons de plaque sous-gingivale prélevés pour l'analyse utilisant l'hybridisation ADN-ADN échiquier. Les données cliniques et radiographiques ont été enregistrées au moment de la connexion de l'implant et après une et dix années. Dix années après la mise en charge, les indices de plaque et de saignement et les variations dans l'attache ou les niveaux osseux marginaux n'étaient pas significativement différents ni entre les groupes OD et FFP ni à l'intérieur du groupe OD. La perte osseuse marginale entre les connexions des piliers et dix ans après étaient respectivement de 0,86 et 0,73 mm pour les groupes OD et FFP. La flore sous-gingivale au niveau des implants pour tous les groupes et sous-groupes étaient semblables avec de faibles comptages ADN (±10 × 105) mais des fréquences de détection importantes de A. actinomycetemcomitans (>90%), P. gingivalis (>85%) et T.forsythensis (30%). La composition de la flore sous-gingivale a été influencée par la profondeur de la poche et la tendance au saignement. La satisfaction du patient était très élevée pour les deux types de prothèses. Le groupe FFP n'était qu'un petit peu meilleur pour le confort à la mastication et la satisfaction générale. Ces données indiquent que d'un point de vue clinique et microbiologique autant que d'un point de vue de satisfaction du patient les prothèses amovibles et fixées sont favorables à long terme. Zusammenfassung Hintergrund: Die Langzeitresultate über den mikrobiologischen und klinischen Verlauf und über die Zufriedenheit der Patienten nach Implantatversorgung im zahnlosen Unterkiefer sind limitiert. Im Speziellen sind Vergleiche zwischen festsitzenden totalen Brücken und Hybridprothesen und den verschiedenen Befestigungssystemen für Hybridprothesen selten. Ziel: Das Ziel der Studie war, beide Parameter anlässlich der Nachuntersuchung nach 10 Jahren bei einer Gruppe von zahnlosen Patienten, welche mit einer Hybridprothese oder mit einer festsitzenden Brücke wiederhergestellt worden waren, auszuwerten (bei den Hybridprothesen sollten auch Vergleiche innerhalb der Gruppe durchgeführt werden). Material und Methoden: Insgesamt nahmen 37 zahnlose Patienten an der Studie teil (25 Hybridprothesen (OD), 12 festsitzende Prothesen (FFP), Alter zum Zeitpunkt der Implantation 36 bis 85 Jahre). Alle Subjekte hatte vor 10 Jahren ihre Implantate erhalten (Brånemark System®, Nobel Biocare, Schweden). Bei den Hybridprothesen waren verschiedene Befestigungssysteme verwendet worden (Steg, Magnet, Kugeln). Dies erlaubte Vergleiche innerhalb der Gruppe. Bei der Nachuntersuchung 10 Jahre nach Einsetzten der Prothetikteile wurden parodontale Parameter aufgenommen, Röntgenbilder mit der Langkonustechnik angefertigt und subgingivale Plaqueproben zur Analyse mittels Ceckerboard DNA,DNA Hybridisierung entnommen. Die klinischen und radiologischen Daten wurden beim Einsetzten der Prothetikteile und nach 1 und 10 Jahren aufgenommen. Resultate: Nach 10 Jahren Belastung bestanden weder zwischen der OD und FFP Gruppe, noch innerhalb der OD Gruppe statistisch signifikante Unterschiede im mittleren Plaque- und Blutungsindex und in der Attachment- und marginalen Knochenhöhe. Der Verlust an marginalem Knochen zwischen der Montage der Prothetikteile und nach 10 Jahren betrug 0.86 mm für die OD Gruppe und 0.73 mm für die FFP Gruppe. Die subgingivale Flora war bei allen Implantatstellen der (Sub-) Gruppen vergleichbar. Es bestand eine geringe Anzahl an DNA Zählungen (±10 × 105) aber eine hohe Entdeckungsfrequenz für A. actinomycetemcomitans (>90%), P. gingivalis (>85%) und T. forsythensis (30%). Die Zusammensetzung der subgingivalen Mirkoflora wurde durch die Sondierungstiefe und die Blutungstendenz beeinflusst. Die Zufriedenheit der Patienten war für beide Arten der prothetischen Wiederherstellung sehr hoch. Die FFP Gruppe erreichte nur geringfügig bessere Werte bezüglich Kaukomfort und genereller Zufriedenheit. Schlussfolgerung: Diese Daten zeigen, dass sowohl vom klinischen und mikrobiologischen Standpunkt aus als auch seitens der Patientenzufriedenheit die Hybridprothese und die festsitzende totale Brücke gute Langzeitresultate zeigen. Resumen Antecedentes: Los datos a largo plazo sobre los resultados microbiológicos y clínicos al igual que la satisfacción del paciente tras la terapia de implantes en la mandíbula edéntula son limitados. Son especialmente escasas las comparaciones entre prótesis fija completa y sobredentaduras, o entre sistemas de anclaje. Intención: Este estudio se intentó para evaluar ambos parámetros en el control de seguimiento de los 10 años en un grupo de pacientes totalmente edéntulos rehabilitados por medio de una sobredentadura o una prótesis completa fija (la última para permitir comparaciones intergrupo). Material y métodos: Un total de 37 pacientes totalmente edéntulos (25 sobredentaduras (OD), 12 prótesis completas fijas (FFP), la edad en el momento de la implantación varió entre 36 a 85 años) participaron en este estudio. Todos los sujetos recibieron sus implantes (Brånemark System®, Nobel Biocare, Suecia) 10 años antes. Para las sobredentaduras se aplicaron diferentes sistemas de anclaje (barras, imanes, bolas) lo que permitieron una ulterior comparación intragrupo. En la visita de seguimiento, 10 años tras la colocación de los pilares, se recogieron una serie de parámetros periodontales, se tomaron radiografías de cono largo y se recogieron muestras de la placa subgingival para análisis usando la cuadrícula de DNA-DNA hibridación. Los datos clínicos y radiográficos se recogieron al conectar los pilares y tras 1 y 10 años. Resultados: Tras 10 años de carga, los índices medios de placa y sangrado y los cambios en el nivel óseo marginal y de inserción no fueron significativos, ni entre los grupos OD y FFP, ni dentro del grupo OD. La pérdida de hueso marginal entre la conexión de los pilares y el año 10 fue de 0.86 y 0.73 mm para los grupos OD y FFP, respectivamente. La microflora subgingival en los lugares de implantes fue comparable entre todos los (sub)-grupos, con un bajo recuento de DNA (±10 × 105) pero una alta detección de A. actinomycetemcomitans (>90%), P. gingivalis (>85%) y T. forsythensis (30%). La composición de la microflora subgingival fue influida por la profundidad de sondaje y la tendencia al sangrado. La satisfacción de los pacientes fue muy alta para ambos grupos de rehabilitación protésica. El grupo FFP puntuó solo un poco mejor para la satisfacción masticatoria y satisfacción general. Conclusión: Estos datos indican que desde el punto de vista clínico y microbiológico al igual que la satisfacción del paciente, tanto la sobredentadura como la prótesis completa fija ofrecen unos resultados favorables a largo plazo.
